The current assessment and management of thyroid nodules
Introduction. The widespread use of diagnostic imaging favored the increasing incidence of thyroid nodules. Although
most of nodules are benign, their clinical importance lies in the need to exclude malignancy. In assessing and managing
thyroid nodules may occur the phenomenon of overdiagnosis and overtreatment on one hand and the risk of missing an
aggressive thyroid cancer on the other hand. The equilibrium that has to be reached by health care providers.
Materials and methods. We conducted a PubMed, MEDLINE, ISI Web of Science, Cochrane databases search for the relevant and recent guidelines, meta-analysis, randomized controlled trial, reviews articles related to „thyroid nodules assessment”, „thyroid nodules management”, „thyroid nodules guidelines”, „thyroid nodules surgery”.
Results. The initial assessment of thyroid nodules includes an evaluation of clinical, laboratory and sonographic risk factors.
Due to the sonographic features and size, the nodules are selected for biopsy. Cytologically benign nodules are usually followed-up, minimally invasive techniques may be required in certain cases. In suspected or confirmed malignancy, the treatment options of thyroid nodules include surgery or active surveillance. The main controversies appear in management of nodules with inconclusive cytology, low-risk cancers, multinodular goiters, hyperfunctioning nodules, and thyroid incidentalomas.
Conclusions. Thyroid nodules due to the high incidence and heterogeneity of background diseases cannot be evaluated
and managed in one standardized approach. In the existing literature, there are discussed multiple options for diagnosis
and treatment of thyroid nodules. We have reviewed the guidelines recommendations, novel published data, and controversial questions for health care professionals, to understand and provide efficient, personalized, and cost-effective management of patients with thyroid nodules in order to avoid automatic intensive testing and intervention and balancing each
case from the patient expectations and demands
Surgical treatment of patients with hemorrhoids by using traditional and minimally invasive methods
Background: Hemorrhoids are a common pathology of the ano-rectal region and their treatment remains relevant. New minimally invasive methodsof surgical treatment of hemorrhoids have been developed recently. Transanal Doppler-Guided Hemorrhoidal Artery Ligation (DG-HAL) of internalhemorrhoids is an up-to-date minimally invasive surgical method of hemorrhoid treatment. In the Republic of Moldova, many patients seek medicalhelp only in advanced stages with large prolapse. It is not always possible to resolve the problem with minimally invasive methods. In medical literaturethere is little elucidation of the simultaneous combination of the DG-HAL method with the excision of external hemorrhoidal nodes. In the Republic ofMoldova this study is being carried out for the first time. The authors present the results of combined surgical treatment of patients with hemorrhoids.The combined method of surgical treatment of hemorrhoidal disease has been implemented in our country and the efficacy of the method for individualapproach has been demonstrated.
Material and methods: The results of the surgical treatment of 15 patients with the diagnosis of chronic hemorrhoids III-IV grade were evaluated. Patientswere treated with the combined surgical method (DG-HAL with excision of hypertrophied external hemorrhoids and / or skin tags).
Results: Postoperative pain after “visual analogue scale” ranged from 3 to 6. Postoperative hospitalization constituted from 1 to 6 days. The durationof the surgery during adoption of approach was 30-60 minutes, but in the years 2016-2017 it constitutes 25-40 minutes. Severe complications have notbeen detected.
Conclusions: The individual approach by combined surgical treatment of patients with hemorrhoidal disease improves treatment outcomes
Complications of Meckel's diverticulum. A review

Obiective. Scopul articolului a fost de a analiza datele din sursele existente de literatură, referitor la complicațiile diverticulului Meckel.
Material și metode. Au fost studiate un număr de publicații științifice ce prezintă informații referitoare la epidemiologie, anatomia diverticulului Meckel, complicații, inclusiv cele rare, tactica de tratament chirurgical al diverticulului Meckel, cu sau fără asocierea complicațiilor. În
calitate de surse au fost utilizate publicații internaționale prin intermediul platformelor online Google Scholar și PubMed. Cuvintele cheie folosite
la căutare au fost: “Meckel diverticulum”, “complications of Meckel’s diverticulum”, “treatment of Meckel’s diverticulum”.
Rezultate. Au fost identificate 30 publicații științifice ce relevă informații actuale referitor la tactica de tratament chirurgical al diverticulului Meckel
necomplicat și însoțit de complicații.
Concluzii. Diverticulul Meckel se manifestă clinic la copii prin hemoragii gastro-intestinale, iar la adulți se manifestă prin semne clinice de inflamație
sau ocluzie intestinală. Revizia porțiunii distale a ileonului este o manevră intraoperatorie obligatorie în cazurile când schimbările morfologice ale
apendicelui nu corespund cu tabloul clinic de apendicită acută. Dacă în apendicele inflamat persistă schimbări morfologice certe, atunci diverticulul
Meckel descoperit incidental, dar fără schimbări morfologice, nu trebuie înlăturat. Dacă schimbările morfologice în apendicele vermicular inflamat
stârnesc dubii, diverticulul Meckel incidental depistat trebuie înlăturat obligator. Volumul tratamentului chirurgical al diverticulului Meckel este
în raport direct cu particularitățile anatomice și a schimbărilor patofiziologice ale diverticulului Meckel: diverticulectomia, este indicată când
există o bază îngustă a diverticulului, și se efectuează similar apendicectomiei; rezecția în clin se va efectua în cazul bazei largi a diverticulului și în
absența mucoasei gastrice ectopice în această zonă; rezecția ileonului în bloc cu diverticulul se va efectua în cazurile de ectopie a mucoasei la baza
diverticulului Meckel sau cu trecere a zonei de ectopie pe mucoasa ileonului.Summary
Objectives. The purpose of the article was to analyze data from existing literature sources on the complications of the Meckel diverticulum.
Material and methods. A number of scientific publications have been studied that present information on the epidemiology, anatomy of the
Meckel's diverticulum, its complications, including the rare ones, the tactical treatment of complicated and uncomplicated Meckel's diverticulum.
International data bases have been used such as online sources Google Scholar and PubMed. Keywords used in the search were: "Meckel diverticulum",
"complication of Meckel’s diverticulum", "treatment of Meckel’s diverticulum".
Results. 30 scientific publications show current information on surgical treatment tactics of complicated and uncomplicated Meckel's diverticulum.
Conclusions. Meckel's diverticulum is clinically manifested in children by gastrointestinal bleeding, and in adults by clinical signs of inflammation
or intestinal obstruction. If certain morphological changes persist in the inflamed appendix, but absent inside Meckel's diverticulum discovered
incidentally, the Meckel's diverticulum should not be removed. If the morphological changes in the inflamed vermicular appendix raise doubts,
the incidental detected Meckel's diverticulum must be removed. The volume of surgical treatment of Meckel's diverticulum is directly related to
the anatomical features and pathophysiological changes of Meckel's diverticulum: diverticulectomy is indicated when there is a narrow base of
the diverticulum, and is performed similarly to appendicectomy; the resection in the wedge will be performed in the case of the broad base of the
diverticulum and in the absence of the ectopic gastric mucosa in this area; resection of the ileum in a block with the diverticulum will be performed
in cases of ectopy of the mucosa at the base of the Meckel's diverticulum or passage of the ectopy area on the ileum mucosa

Abordarea chirurgicală actuală a nodulilor tiroidieni. Caz clinic
Department of Surgery no. 5, Nicolae Testemitanu SUMPhBackground. Thyroid nodules are common lesions being highlighted clinically and by ultrasound up to
76 % in the general population. Due to the malignant character of the thyroid nodules is debatable the
duration of their clinical and paraclinical observation and the opportunity of decision-making for
surgery. Objective of the study. Optimization of tactics for diagnosis and surgical treatment of thyroid
nodules. Material and Methods. C.L., a 45 years old female patient is complaining of discomfort in
the anterior cervical region, "globus" sensation, irritability, and fatigue. She was under the
endocrinologist's supervision for 7 years, the patient was diagnosed with a thyroid nodule in the right
lobe, which despite medication with L-thyroxin 75 mg, increased in size. Results. Serum levels of
thyroid hormones and TSH- without deviations. The ultrasound of the right lobe detected a nodular
formation with hyperechoic elements surrounded by hypoechoic margin measuring 2,14x2,44 x3,25 cm,
with active vascularization and mixed type. It confirms scintigraphically the presence of “hot nodule”
at the upper pole of the lobe. Lobectomy was performed with the intraoperative frozen section, where
was detected follicular carcinoma of the right lobe. The intervention ended with a total thyroidectomy.
The diagnosis was confirmed by histopathological intervention, followed by long-term postoperative
supervision without any complication. Conclusion. The evolution of thyroid nodules, without
underestimating their malignant potential, requires a continuous clinical and paraclinical follow-up.
With the appearance of suspicious signs of malignancy, the approach to thyroid nodules will be
multidisciplinary and directed towards radical treatment.Introducere. Nodulii tiroidieni sunt leziuni frecvente, fiind evidențiați clinic și ecografic până la 76 %
în populația generală. Datorită caracterului malign al nodulilor tiroidieni este discutabilă durata
monitorizării acestora și momentul decizional oportun pentru intervenția chirurgicală. Scopul lucrării.
Optimizarea tacticii de diagnostic și tratament chirurgical a nodulilor tiroidieni. Material și Metode.
Pacienta C.L., 45 ani, se adresează cu acuze la disconfort în regiunea cervicală anterioară, senzație de
″nod în gât″, iritabilitate, fatigabilitate. S-a aflat la evidența medicului endocrinolog timp de 7 ani, cu
un nodul tiroidian al lobului drept, care în pofida medicației cu comprimate L-thyroxin 75 mg s-a mărit
în dimensiuni. Rezultate. Valorile serice ale hormonilor glandei tiroide și ale TSH - fără devieri.
Ultrasonografic în lobul drept s-a depistat o formațiune nodulară,cu elemente hiperecogene înconjurată
de o margine hipoecogenă, cu dimensiuni de 2,14x2,44x3,25 cm, cu vascularizare activă, tip mixt,
determinată scintigrafic ca “nodul fierbinteˮ la polul superior al lobului. S-a recurs la lobectomie cu
efectuarea intraoperatorie a investigației histologice extemporanee, unde s-a apreciat Carcinom folicular
pe fondal de adenom al lobului drept, intervenția finalizând cu tiroidectomie totală. La investigația
histopatologică s-a confirmat diagnosticul. Monitorizarea postoperatorie pe termen lung fără
complicații. Concluzii. Evoluția nodulilor tiroidieni, fără a subestima potențialul malign al acestora,
necesită o urmărire continuă clinică și paraclinică. Odată cu apariția semnelor suspecte de malignitate,
abordarea nodulilor tiroidieni va fi multidisciplinară și direcționată spre tratament radical
Paravasal Endoscopic Sclerosation of the Esophageal Varices with 1% Aethoxysklerol – a Prophilactic Method of Haemorrhages in the Esophagean Varices in Patients with Hepatic Cirrhosis and Portal Hypertension
Curs chirurgie, Facultatea stomatologie USMF „Nicolae Testemiţanu”Slerotherapy endoscopic paravasal results with 1% Aethoxysklerol solution are sown in this article in 30 patients with hepatic cirrhosis and portal hypertension.The patients age was between 31 and 68, 14 having HC subcompensation and 16 – HC decompensation stage.The analysis revealed 12 patients having esophagean varices of the 2nd-3rd degree and in 16 patients of the 3rd degree.
Complications were noted only in 5 cases,no deaths.The paravasal slerosation method with 1% Aethoxysklerol is exposed,in the article the data are studied and a sheme for treatment is proposed.
În lucrare sînt redate rezultatele sclerozării endoscopice paravazale cu sol.Aethoxysklerol 1% la 30 pacienţi cu ciroză hepatică şi hipertensiune portală. Vîrsta pacienţilor varia de la 31 pînă la 68 ani, din ei 14 aveau CH stadiul subcompensare şi 16- CH în stadiul de decompensare. La 12 pacienţi s-au depistat varice esofagiene gr.II-III şi la 18 – gr.III.
Complicaţii s-au întîlnit în 5 cazuri, fără cazuri letale.Este expusă metodica sclerozării paravazale cu sol.Aethoxysklerol 1%, sunt studiate rezultatele la distanţă, propusă schema de tratament

Rezumat
Obiective. Scopul articolului a fost de a analiza datele din sursele existente de literatură, cu referință la cazurile clinice rare și extrem de rare ale
apendicitei acute.
Material și metode. Au fost studiate un număr de 66 de publicații științifice ce prezintă informații referitoare la anatomia topografică a apendicelui vermicular și a tabloului clinic și tacticii de tratament chirurgical, în funcție de topografia atipică a apendicelui și cecului. În calitate de surse de
literatură au fost utilizate publicații internaționale prin intermediul platformelor online Google Scholar și PubMed. Cuvintele cheie utilizate la căutare
au fost: “rare atypical clinical cases of acute appendicitis”, “rare atypical topographic cases of the appendix and cecum”, “congenital anomalies”,
“ectopia of appendix”, “treatment features of the rare clinical-anatomical cases of acute appendicitis”, “situs inversus”.
Rezultate. Au fost identificate 66 de publicații științifice ce relevă informații actuale referitor la anatomia topografică rară a apendicelui vermicular
în apendicita acută, particularitățile tabloului clinic și a tacticii de tratament ale cazurilor atipice și rare de apendicită acută.
Concluzii. Nu există o opinie unică în privința incidenței poziției anatomice a apendicelui, unii autori consideră că poziția cea mai frecventă este
cea descendentă, fiind urmată de poziția laterală ori retrocecală, alți autori consideră mai frecventă poziția retrocecală, urmată de cea descendentă.
Atunci când, pe fundalul unui tablou clinic de apendicită acută, apendicile vermicular “lipsește” intraoperator la locul de convergență a teniilor
cecului, este necesar de a exclude: apendicita acută cu apendicele situat complet sub seroasa cecului, apendicita bontului apendicular, apendicita
acută determinată de invaginația completă a apendicelui, poziție complet retrocecală a apendicelui vermicular. Tratamentul apendicitei acute în caz
de hernie Amyand sau De Garengeot constă în apendicectomie cu sau fără hernioplastie cu țesuturi proprii, iar utilizarea hernioplastiilor cu meșe
sintetice este contraindicată. În cazul suspecției apendicitei acute pe stânga este necesar de a confirma clinic și instrumental inversia de organe. În
caz de dezvoltare a apendicitei acute pe un fundal patologic preexistent de cancer al apendicelui, boala Crohn, Yersineoze, etc, diagnosticul clinic
definitiv se bazează pe examenul histologic, care denotă prezența celulelor atipice, sau a inflamării granulomatoase.Summary

Material and methods. A number of 66 scientific publications presenting information about the topographic anatomy of the vermicular appendix,
the clinical picture and surgical treatment tactics based on the atypical topography of the appendix have been studied. International publications
through the online platforms Google Scholar and PubMed have been used as sources. The keywords used in the search were: “rare atypical clinical
cases of acute appendicitis”, “rare atypical topographic forms of the appendix and cecum”, “congenital anomalies of appendix”, “ectopia of appendix”,
“treatment features of the rare clinical-anatomical cases of acute appendicitis”, “situs inversus”.
Results. We identified 66 scientific publications that reveal current information about the rare topographic anatomy of the vermicular appendix in
acute appendicitis, the particularities of the clinical picture and the tactics of treatment of unusual and rare cases of acute appendicitis.
Conclusions. There is no single opinion about the incidence of appendix position, some authors consider that the most common position is the
descending position, followed by the lateral or retrocecal position, other authors consider the retrocecal position more frequent, followed by the
descending position. When on the background of a clinical picture of acute appendicitis the vermicular appendix is "missing" during operation at
the place of convergence of the cecum tenia it is necessary to exclude: acute appendicitis with appendix located completely below the serosa of the
cecum, stump appendicitis, invagination of the appendix, completed retroperitoneal position of the appendix. The treatment of acute appendicitis
in case of Amyand or De Garengeot hernia consists of appendicectomy with or without hernioplasty with own tissues, also the use of hernioplasts
with synthetic meshes is contraindicated. In case of suspicion of acute left appendicitis it is necessary to confirm clinically and instrumentally the
reversal of organs. In case of development of acute appendicitis on a pre-existing pathological background of appendix cancer, Crohn's disease,
Yersineosis, etc, the definitive clinical diagnosis is based on histological examination, which denotes the presence of atypical cells, or granulomatous
inflammation
Modern aspects of classification of groin hernias
Facultatea Stomatologie, USMF „Nicolae Testemiţanu”, Spitalul Clinic Militar CentralClasificarea herniei inghinale are ca scop repartizarea herniilor în tipuri cu deosebiri esenţiale, ce permite chirurgului să individualizeze
strategia şi tactica chirurgicală a fiecărui tip de hernie în parte. Scopul studiului este evaluarea comparativă a clasificărilor herniilor
inghinale, utilizate actualmente în practică chirurgicală. Articolul prezintă o revistă a literaturii ce cuprinde mai multe surse bibliografice
la aceasta temă. Rezultate analizei clasificărilor elaborate evidenţiază că nici una dintre acestea nu corespunde pe deplin tuturor criteriilor
unei clasificări ideale. Clasificarea herniilor inghinale trebuie să reflecte localizarea anatomică a herniei, stadiul de evoluţie a herniei şi
starea afectării structurilor anatomice ale canalului inghinal, să fie universală pentru abordul chirurgical convenţional şi laparoscopic şi
uşoară de memorizat. Cea mai populară la moment este clasificarea Nyhus, bazată pe principiul gradului de afectare a peretelui posterior
şi stării orificiului profund al canalului inghinal. Totodată, ultima clasificare propusă de Societatea Europeană a Herniologilor este bazată
pe criteriul localizării anatomice, oferă o metodă simplă pentru aprecierea dimensiunilor defectului herniar şi este activ promovată pentru
utilizare intraoperatorie şi, cu scop ştiinţific, în analiza statistică a rezultatelor curei chirurgicale a herniilor inghinale.
The primary purpose of the classification system for groin hernia is to divide the groin hernias in clearly defined anatomic and pathogenetic
types, so that reasonable comparisons can be made between various treatment strategies. The aim of the study is to analyze the available
classifications for groin hernias. This article provides the review of different classification systems used in daily surgical practice and for
scientific purpose. Given the multiplicity of operative techniques and approaches for the repair of groin hernias, it appears that none
of existing classifications is proven to satisfy all. The ideal classification system for groin hernias should be based on anatomic location,
anatomic function, be applicable for both anterior and posterior approaches and easy to remember. The Nyhus classification is one of the
most frequently used. However, the European Hernia Society (EHS) suggests an easy and simple classification based on the Aachen classification, with respect to the anatomic localization and the size of the hernia orifice defect. The EHS promotes the general and systematic
use of this classification for intraoperative description of the type of hernia and to increase the comparison of results in the literature

Rezumat.
Polipoza veziculei biliare este o entitate relativ rar întîlnită, cu incidența de 4-7%, elucidată destul de laconic în literatura de
specialitate. Polipoza veziculară pînă în prezent este subiectul discuțiilor în literatura de specialitate din cauza controverselor
referitor la etiologie, patogenie, diagnostic și tratament. Diagnosticul preoperator se efectuează cu ajutorul examenului ultrasonografic de rutină și este în unele cazuri destul de dificil, simulînd alte patologii ale veziculei biliare. La stabilirea definitivă a
diagnosticului de polip sau polipoză veziculară, rolul cel mai important îi revine examenului morfologic a preparatului înlăturat.
E necesar de apreciat rolul metodelor imunohistochimice în stabilirea diagnosticului și a posibilei malignizări. Actualmente,
intervenția chirurgicală este considerată metoda de elecție de tratament și profilaxie a malignizării polipilor veziculari. Scopul
lucrării constă în ameliorarea rezultatelor tratamentului pacienților cu polipoza veziculară în baza optimizării tacticii de
diagnosticare și a intervenției chirurgicale.Summary.
Gallbladder polyposis is a relatively rare entity, with an incidence of 4-7%, elucidated quite succinctly in the specialized literature. Vesicular polyposis is still the subject of discussion in the specialized literature due to controversies regarding etiology,
pathogenesis, diagnosis, and treatment. The preoperative diagnosis is carried out with the help of the routine ultrasonographic
examination and is in some cases quite difficult, to simulate other diseases of the gallbladder. When definitively establishing the
diagnosis of a polyp or vesicular polyposis, the most important role belongs to the morphological examination of the removed
specimen. It is necessary to appreciate the role of immunohistochemical methods in establishing the diagnosis and possible malignancy. Currently, surgical intervention is considered the method of choice for the treatment and prophylaxis of malignancy
of vesicular polyps. The purpose of the review was to improve the results of the treatment of patients with gallbladder polyposis
based on the optimization of diagnostic tactics and surgical intervention.Резюме.
Полипоз желчного пузыря является относительно редким заболеванием с частотой 4-7%, достаточно кратко
описанным в специализированной литературе. Везикулярный полипоз до сих пор является предметом дискуссий в
специализированной литературе из-за разногласий в отношении этиологии, патогенеза, диагностики и лечения.
Предоперационная диагностика проводится с помощью ультразвукового исследования и бывает в ряде случаев
достаточно сложным, симулируя другие патологии желчного пузыря. При окончательном установлении диагноза
полипа или везикулярного полипоза важнейшая роль принадлежит морфологическому исследованию удаленного
препарата. Необходимо оценить роль иммуногистохимических методов в установлении диагноза и возможной
малигнизации. В настоящее время хирургическое вмешательство считается методом выбора для лечения и профилактики малигнизации полипов желчного пузыря. ЦЕЛЬ. Улучшить результаты лечения больных везикулярным

Evaluation of the results of surgical treatment of patients with hemorrhods by classical traditional and minimally invasive methods
