    A Framework for Introducing Program Evaluation to Extension Faculty and Staff

    There is an increasing requirement for Cooperative Extension to demonstrate that programs are making an impact on the populations served, yet many staffs have little background in evaluation. This article presents a framework found to be useful in introducing the basics of evaluation to Extension staff. The authors draw upon Jacobs\u27 (1988) Five-Tiered approach to program evaluation and the Joint Committee on Standards for Educational Evaluation (1994) as a base. This foundation allows staff a better understanding of the processes involved in conducting evaluations

    4-H Youth Development Apprenticeship Program: A Pilot of a New Certification in Youth Development

    The field of youth development is expanding, requiring 4-H Extension staff to gain expertise in the field without leaving their jobs to return to school. To address this need, the 4-H Youth Development Practitioner Apprenticeship (YDPA) program was developed under the guidance of the National Association of Extension 4-H Agents and the National 4-H Council through funding from the U. S. Department of Labor. An evaluation of the pilot of YDPA found that staff gained numerous skills needed to effectively serve youth. Suggestions for program continuation are discussed

    Measuring Impacts with Young Audiences: Adapting a Life-Skills Instrument for Use with Third- to Fifth-Grade Youth

    Capturing the impacts Extension programming has on younger school-age audiences is often difficult, yet staff are increasingly asked to document program effectiveness. The limited literacy skills and concrete reasoning of young school age children make the use of written evaluations challenging, yet observations and interviews are time consuming and costly for programs. This article discusses how an evaluation instrument was adapted for use with third to fifth grade 4-H youth. The system was piloted with 65 youth who attended a 4-H camp. Implications and suggestions for others adapting written evaluation instruments are offered

    Rural After-School Programs: Meeting the Needs of At-Risk Youth and Their Families

    Extant research has focused on out-of-school time among urban and suburban youth, yet there remains scant information on rural low-income youth and their families. This study examines the salience of after-school programs for rural youth. Using an ecological framework, we assess changes in youth behavior and skill acquisition based on reports from 47 youth attending after-school programs in two rural communities, along with their primary caregivers and teachers. Results indicate enhanced life skills and social behaviors among youth attendees. Implications for future research, programming, and policymaking are discussed

    Betaine Treatment Attenuates Chronic Ethanol-Induced Hepatic Steatosis and Alterations to the Mitochondrial Respiratory Chain Proteome

    Introduction. Mitochondrial damage and disruption in oxidative phosphorylation contributes to the pathogenesis of alcoholic liver injury. Herein, we tested the hypothesis that the hepatoprotective actions of betaine against alcoholic liver injury occur at the level of the mitochondrial proteome. Methods. Male Wister rats were pair-fed control or ethanol-containing liquid diets supplemented with or without betaine (10 mg/mL) for 4-5 wks. Liver was examined for triglyceride accumulation, levels of methionine cycle metabolites, and alterations in mitochondrial proteins. Results. Chronic ethanol ingestion resulted in triglyceride accumulation which was attenuated in the ethanol plus betaine group. Blue native gel electrophoresis (BN-PAGE) revealed significant decreases in the content of the intact oxidative phosphorylation complexes in mitochondria from ethanol-fed animals. The alcohol-dependent loss in many of the low molecular weight oxidative phosphorylation proteins was prevented by betaine supplementation. This protection by betaine was associated with normalization of SAM : S-adenosylhomocysteine (SAH) ratios and the attenuation of the ethanol-induced increase in inducible nitric oxide synthase and nitric oxide generation in the liver. Discussion/Conclusion. In summary, betaine attenuates alcoholic steatosis and alterations to the oxidative phosphorylation system. Therefore, preservation of mitochondrial function may be another key molecular mechanism responsible for betaine hepatoprotection

    Exercise-Based Stroke Rehabilitation: Clinical Considerations Following the COVID-19 Pandemic

    Background. The COVID-19 pandemic attributable to the severe acute respiratory syndrome virus (SARS-CoV-2) has had a significant and continuing impact across all areas of healthcare including stroke. Individuals post-stroke are at high risk for infection, disease severity, and mortality after COVID-19 infection. Exercise stroke rehabilitation programs remain critical for individuals recovering from stroke to mitigate risk factors and morbidity associated with the potential long-term consequences of COVID-19. There is currently no exercise rehabilitation guidance for people post-stroke with a history of COVID-19 infection. Purpose. To (1) review the multi-system pathophysiology of COVID-19 related to stroke and exercise; (2) discuss the multi-system benefits of exercise for individuals post-stroke with suspected or confirmed COVID-19 infection; and (3) provide clinical considerations related to COVID-19 for exercise during stroke rehabilitation. This article is intended for healthcare professionals involved in the implementation of exercise rehabilitation for individuals post-stroke who have suspected or confirmed COVID-19 infection and non-infected individuals who want to receive safe exercise rehabilitation. Results. Our clinical considerations integrate pre-COVID-19 stroke (n = 2) and COVID-19 exercise guidelines for non-stroke populations (athletic [n = 6], pulmonary [n = 1], cardiac [n = 2]), COVID-19 pathophysiology literature, considerations of stroke rehabilitation practices, and exercise physiology principles. A clinical decision-making tool for COVID-19 screening and eligibility for stroke exercise rehabilitation is provided, along with key subjective and physiological measures to guide exercise prescription. Conclusion. We propose that this framework promotes safe exercise programming within stroke rehabilitation for COVID-19 and future infectious disease outbreaks

    Barriers and opportunities for evidence-based health service planning: the example of developing a Decision Analytic Model to plan services for sexually transmitted infections in the UK

    Decision Analytic Models (DAMs) are established means of evidence-synthesis to differentiate between health interventions. They have mainly been used to inform clinical decisions and health technology assessment at the national level, yet could also inform local health service planning. For this, a DAM must take into account the needs of the local population, but also the needs of those planning its services. Drawing on our experiences from stakeholder consultations, where we presented the potential utility of a DAM for planning local health services for sexually transmitted infections (STIs) in the UK, and the evidence it could use to inform decisions regarding different combinations of service provision, in terms of their costs, cost-effectiveness, and public health outcomes, we discuss the barriers perceived by stakeholders to the use of DAMs to inform service planning for local populations, including (1) a tension between individual and population perspectives; (2) reductionism; and (3) a lack of transparency regarding models, their assumptions, and the motivations of those generating models
