    TENDÊNCIA DE MORTALIDADE POR DIABETES MELLITUS NO MARANHÃO

    Introdução: O diabetes mellitus (DM) é uma doença crônica relacionada com o envelhecimento da população, aumento da prevalênciade obesidade e estilos de vida não saudáveis. Objetivo: Analisar a tendência de mortalidade por DM no Maranhão e no Brasilno período de 2000 a 2015. Métodos: Foi realizada uma pesquisa analítica retrospectiva do tipo série histórica com dados do Sistemade Informações sobre Mortalidade (SIM) quanto a DM como causa de óbito no período de 2000 a 2015. Foram calculadas taxasde mortalidade e com o uso do software Joinpoint regression analysis foi calculada a porcentagem anual de mudança (APC) paradeterminação das tendências. Resultados: A taxa de mortalidade por DM no Brasil passou de 20,34/100.000 habitantes em 2000para 29,17/100.000 habitantes em 2015. No Maranhão, a taxa passou de 8,63/100.000 habitantes em 2000 para 34,2/100.000habitantes em 2015. Tanto no Brasil como no Maranhão houve uma tendência de crescimento, contudo no Brasil um crescimentomenos acelerado, com uma APC para o período de 2,4 (IC95% 1,6 – 3,3), e mais acelerado no Maranhão com uma APC de 9,4 (IC95%6,3 - 12,6). Conclusão: A taxa de mortalidade por DM no Maranhão ultrapassou a taxa nacional nos últimos anos, com uma tendênciaa estabilidade na taxa de mortalidade do Brasil e desaceleração no Maranhão no período de 2007 a 2015.Palavras-chave: Diabetes mellitus.     Early Markers of Cardiovascular Disease Associated with Clinical Data and Autosomal Ancestry in Patients with Type 1 Diabetes: A Cross-Sectional Study in an Admixed Brazilian Population

    Patients with type 1 diabetes (T1D) have a higher risk of developing cardiovascular disease (CVD), which is a major cause of death in this population. This study investigates early markers of CVD associated with clinical data and autosomal ancestry in T1D patients from an admixed Brazilian population. A cross-sectional study was conducted with 99 T1D patients. The mean age of the study sample was 27.6 years and the mean duration of T1D was 14.4 years. The frequencies of abnormalities of the early markers of CVD were 19.6% in the ankle-brachial index (ABI), 4.1% in the coronary artery calcium score (CACS), and 5% in the carotid Doppler. A significant percentage of agreement was observed for the comparison of the frequency of abnormalities between CACS and carotid Doppler (92.2%, p = 0.041). There was no significant association between the level of autosomal ancestry proportions and early markers of CVD. The ABI was useful in the early identification of CVD in asymptomatic young patients with T1D and with a short duration of disease. Although CACS and carotid Doppler are non-invasive tests, carotid Doppler is more cost-effective, and both have limitations in screening for CVD in young patients with a short duration of T1D. We did not find a statistically significant relationship between autosomal ancestry proportions and early CVD markers in an admixed Brazilian population

    Health-related quality of life in patients with type 1 diabetes mellitus in the different geographical regions of Brazil : data from the Brazilian Type 1 Diabetes Study Group

    Background: In type 1 diabetes mellitus (T1DM) management, enhancing health-related quality of life (HRQoL) is as important as good metabolic control and prevention of secondary complications. This study aims to evaluate possible regional differences in HRQoL, demographic features and clinical characteristics of patients with T1DM in Brazil, a country of continental proportions, as well as investigate which variables could influence the HRQoL of these individuals and contribute to these regional disparities. Methods: This was a retrospective, cross-sectional, multicenter study performed by the Brazilian Type 1 Diabetes Study Group (BrazDiab1SG), by analyzing EuroQol scores from 3005 participants with T1DM, in 28 public clinics, among all geographical regions of Brazil. Data on demography, economic status, chronic complications, glycemic control and lipid profile were also collected. Results: We have found that the North-Northeast region presents a higher index in the assessment of the overall health status (EQ-VAS) compared to the Southeast (74.6 ± 30 and 70.4 ± 19, respectively; p < 0.05). In addition, North- Northeast presented a lower frequency of self-reported anxiety-depression compared to all regions of the country (North-Northeast: 1.53 ± 0.6; Southeast: 1.65 ± 0.7; South: 1.72 ± 0.7; Midwest: 1.67 ± 0.7; p < 0.05). These findings could not be entirely explained by the HbA1c levels or the other variables examined. Conclusions: Our study points to the existence of additional factors not yet evaluated that could be determinant in the HRQoL of people with T1DM and contribute to these regional disparities
