This case report illustrates the recovery of dizziness of a young healthy patient after root canal therapy of upper second molar. The patient developed dizziness and unbalanced walking four months ago. After cardiac, ENT, neurological, physiotherapy and medical investigations, his entire checkup showed no abnormalities. The patient visited a dental clinic for a routine checkup; after dental clinical and radiographical examination, a chronic abscess infection in an upper second molar region close to the sinus was diagnosed. Root canal therapy was performed that resulted in a disappearance of the dizziness and full recovery was achieved. Conclusion. Infected upper teeth with periapical lesion are associated with dizziness as a complication of odontogenic-related sinusitis. Dental and medical cooperation contributes to a better management diagnosis of the dizziness. The authors wish to express their gratitude to Dr. Emad Qirresh at Al Quds University

is a descriptive study that was conducted on 50 Palestinian elderly women whose ages were above 65 years. Open- ended, face-to-face, tape recorded interviews were used as a method for data collection. The sample was selected conveniently through personal contacts and health centers at various living localities of the Occupied Palestinian Territories. The interviews were transcribed verbatim then analyzed using appropriate themes and categories, then simple calculations with percentages and count of responses were done for each of the questions of the interview. Results indicated that (42%) of the participants reported having limitation of movement and mobility caused by some musculoskeletal problem; 60% had cardiovascular problems, 34% had diabetes, and 28% had respiratory illnesses. The data indicated that the majority of the participants (92%) have some kind of social connectedness with family members and the local community. The main role the participants is routine work at home such as cleaning, cooking, and other house chores, as reported by 28 (56%) of the participants. Thirty-one (62%) participants said that they spend quite a lot of time in praying and sometimes reading the Quran. The main feelings of the participants are located under “a depressive” symptom, anxiety-related, or loss of control and autonomy, however; there was a variation in the severity of these symptoms. While 44% of the respondents were thankful to God and appreciative that their lives are satisfying and they were content for whatever they get from life and family, 20% of the participants were waiting for death to come, had death ideation or wished to die

presentation, leading to challenges in its diagnosis and management. Objectives: The main objectives of this study is to highlight the musculoskeletal etiology and manifestations for Temporomandibular joint disorders (TMDs) in patients from Bethlehem and Jerusalem. Materials and Methods: A case control study was conducted in on a convenient sample of 96participants (48 case with diagnosed TMDs and 48 control without TMDs). Data was collected by giving patients with TMDs and their matched (for age and gender) control group personal data sheet about the medical history and a check list of TMDs clinicalsymptoms and risk factors to be answered on a scale of 5 where 1 meant strongly disagree, and 5 strongly agree, in addition to a functional examination, which included ROM, MMT, VAS scale and palpation for tenderness. These together helped to determine musculoskeletal etiology and manifestations of Temporomandibular joint disorders (TMDs). Results: The result showed that 75% of sample that had TMDs were female, while 25% male. Para functional habit of resting head on the hand was the most common risk associated with TMDs (mean =3.87). TMJ pain was exacerbated by eating and yawing (mean= 3.44), and 37% of participants had pain in both sides of the joint. Along with this, Participants mainly complained of pain in the TM joint(mean = 3.85), forehead pain (3.56), other symptoms were clicking in the joint, shoulder pain, pressing on teeth, pain when yawing and chewing, and pain in neck. Furthermore, there was an association between missing teeth and pain in joint (P<0.05), and there was no correlation between age of participant and limitation in the ROM of TMJ (p> 0.05). Conclusion: TMDs are associated with female gender, parafunctions like resting head on hand, main clinical symptoms appeared in eating and yawing, TMDs were associated with regional referred pain

The incidence of cerebral palsy is significantly growing in children, especially among specific spasticity C.P children at south west bank (Hebron and Bethlehem) areas in Palestine. This serious health concern disrupts the active daily living (ADL) among C.P children and affects the lives of their families, both socially as well as economically. The aim of this study was to investigate the factors associated with mother’s adherence towards cerebral palsy home exercise program among children at Hebron and Bethlehem city. We did a clinical trial among the cerebral palsy children at Al- Caritas hospital and Al- Ihsan rehabilitation centre. The study participants were children with cerebral palsy (mild to moderate spasticity) followed up at Al-Ihsan rehabilitation centre in Hebron city and Al-Caritas hospital in Bethlehem city. The calculated sample size was48 participants. Data were collected at two different time periods, one at baseline right before implementing the rehabilitation program and another at six-month’ time interval after the intervention was carried out. Our results indicated that there was no significant association between mother’s age and adherence towards home exercise program (p=0.946). Furthermore, there were no significant differences among mothers’ adherence to rehabilitation program in relation to socioeconomic status (p= 0.415), work (p= 0.704) and their level of education (p= 0.265). This study deduced that all mothers with C.P children were strongly committed towards the implementation f home exercise program regardless of their age, work, education level and financial background

Objective: Parkinson’s disease (PD) is associated with non-motor complications such as sleep disturbance and depression. Transcranial direct current stimulation (tDCS) showed therapeutic effects on the motor dysfunctions. However, the potential effects of tDCS therapy on melatonin hormone, sleep dysfunctions, and depression in patients with PD still unclear. This feasibility study aimed to identify any potential changes in melatonin serum level, sleep functions and depression after the bilateral anodal tDCS in patients with PD. Material and Methods: Tensessions of bilateral anodal tDCS stimulation applied over left and right prefrontal and motor areas were given to twenty-five patients with PD. Melatonin serum level, Pittsburgh sleep quality index, and geriatric depression scale examined before and after tDCS stimulation. Results: After bilateral anodal tDCS, there was a significant reduction in melatonin serum level, improvement in depression, improvements in overall sleep quality, and sleep latency. Correlations test showed significant associations between melatonin serum level reduction and changes in subjective sleep quality, and sleep duration, as well as between improvements in depression and overall sleep quality, sleep latency, and sleep disturbance. Conclusion: Bilateral anodal tDCS therapy was a feasible and safe tool that showed potential therapeutic effects on melatonin serum level, sleep quality, and depression level in patients with PD. Although the further large scale and randomized-control trial studies are crucially needed, there is still a need for such a feasibility study to be established before such trials can be implemented as is recommended in the new medical research council guidelines

Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

Get PDF
Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures. Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge. Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not. Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely
