Luxación acromioclavicular: resultados del tratamiento con cerclaje alámbrico sobre agujas de Kirschner
Hemos revisado retrospectivamente 44 pacientes (38 años de edad media) de un
total de 57 con luxación acromioclavicular tipo III de Allman intervenidos con la técnica de Phemister
modificada desde 1975 hasta 1992. Han sido evaluados desde el punto de vista funcional
(escala de Imatani), radiológico, resultado estético, valoración subjetiva del paciente, así como sus
complicaciones. La media de seguimiento es de 6 años y medio (rango: 2-17 años). El resultado ha
sido excelente en el 89%, bueno en el 9 y malo en el 2. La reducción ha sido anatómica en el 75%.
El 91% no presentaron bultoma residual. Subjetivamente el 75% estaban muy contentos o contentos.
Las complicaciones más frecuentes han sido la infección superficial y las derivadas del material.
Consideramos esta técnica una buena alternativa cuando decidamos realizar tratamiento
quirúrgico de estas lesiones. Se debe restringir la movilidad del hombro hasta la retirada del material
de osteosíntesis.A series of 44 out of 57 patients with Allman grade III acromioclavicular dislocation
operated on from 1975 to 1992 by a modified Phemister technique was studied. Mean age was
38 years. Functional Imatani scale, radiological and aesthetic results, and own patients opinion were
considered to evaluate this procedure. Average follow-up was 6 years (range: 2-17 years). Results
were considered excellent in 89%, good in 9% and poor in 2%. Anatomical reduction was achieved
in 75%. In 91% of cases no clavicular protrusion was observed. A total of 75% were satisfied or
very satisfied with their result. Superficial infection and implant failure were found to be the main
complications. This technique can be a good procedure when a surgical treatment is considered.
Shoulder function must be restricted until the implants have been removed
Fractura por estrés del cuello femoral sobre clavo centromedular en cerrojo
Se presenta un caso de fractura de estrés del cuello femoral en una paciente de
81 años de edad, a los 7 meses del tratamiento de una fractura de diáfisis femoral mediante
clavo intramedular en cerrojo estático. El aumento de solicitaciones mecánicas en cuello femoral
producidas por la rigidez femoral que provoca el clavo centromedular, junto a la pobre masa
ósea por la edad, parecen ser las causas de la fractura de estrés de cuello femoral. Esta
posible complicación plantea la necesidad de retirar los clavos centromedulares en pacientes
ancianos que lleven una vida activa.A case of femoral neck stress fracture is reported in a 81 years old patient, seven
months after diaphyseal femoral fracture treated with static intramedullar interlocking nail.
The increase of mechanical solicitations on the femoral neck caused for the femoral stiffness
induced by the intramedullar nail together with the poor bone mass related with the age seemed
to be the ethiology of the stress fracture of the femoral neck. This complication support
the need for the ablation of the intramedullar nails in aged patients with an active life
Influencia de los cerrojos distales en el tratamiento de las fracturas trocantéreas estables mediante clavo gamma
Hemos tratado consecutivamente mediante clavo gamma 100 pacientes mayores de
60 años con fracturas trocantéreas estables colocando en 50 de ellas los cerrojos distales y no en
las otras 50, habiéndose realizado un estudio comparativo quirúrgico y evolutivo entre ambas series
con un seguimiento mínimo de 3 meses. Se han producido complicaciones operatorias en el
18% de la serie con cerrojos, todas ellas en la colocación de los mismos, y en 1 caso en la serie sin
cerrojos, que obligó a la colocación de éstos. La consolidación se ha producido en todos los casos,
no habiendo diferencias finales en la deambulación. No se han producido complicaciones evolutivas
en la serie con cerrojos y sí en la de sin cerrojos, donde se han producido 2 fracturas en la
zona externa proximal del fémur que no precisaron tratamiento adicional. Los resultados apoyan
que la colocación de los cerrojos distales no es necesaria en el tratamiento con clavo gamma de
las fracturas estables en la región trocantérea.One hundred consecutive patients, older than 60 years, with stable trochanteric
fractures were treated with gamma nail: 50 of them with distal locking screws and the other 50 without
them. We performed a prospective surgical and clinical comparative study between both
groups, with a minimum follow-up of three months. Opcrative complications were found in 18% of
cases in the group with distal locking, all of them during the implantation of the distal screws; and
in one case in the group without distal screws, which forced its placing. Healing was always achieved,
without diferences for walking ability. Complications during follow-up were not observed in
the group treated with distal locking, but in the other group two proximal femoral fractures of the
external cortical ocurred, although they do not need additional treatment. Our results support the
hypothesis that distal locking screws are not necessary for the treatment of stable trochanteric
fractures with the gamma nail
Toward a decision support system for the clinical pathways assessment
This paper presents a decision support system to be used in hospital management taskswhich is based on the clinical pathways. We propose a very simple graphical modeling lan-guage based on a small number of primitive elements through which the medical doctorscould introduce a clinical pathway for a specific disease. Three essential aspects relatedto a clinical pathway can be specified in this language: (1) patient flow; (2) resource uti-lization; and (3) information interchange. This high-level language is a domain specificmodeling language calledHealthcare System Specification (HSS), and it is defined as anUnified Modeling Language (UML) profile. A model to model transformation is also pro-posed in order to obtain, from the pathways HSS specification, a Stochastic Well-formedNet (SWN) model that enables a formal analysis of the modeled system and, if needed, toapply synthesis methods enforcing specified requirements. Influencia de la marcha en la biomecánica del clavo de Grosse-Kempf
El objetivo de este estudio ha sido conocer la influencia que tiene la deambulación
en carga en la posibilidad de rotura del clavo centromedular encerrojado de fémur tipo Grosse-
Kempf. El estudio ha consistido en 2 partes: en primer lugar se realiza un estudio estático-lineal
del clavo mediante un programa de elementos finitos aplicándole las diferentes fuerzas que el clavo
debe de soportar durante la marcha para conocer qué tipo de fuerzas son las que más afectan
a su estructura; en segundo lugar hemos llevado a cabo un estudio cinemático-tridimensional de
la marcha observando en qué fases de la misma se producen este tipo de fuerzas. Las fuerzas que
más afectan al clavo son las de torsión, y el momento en que mayor fuerzas de este tipo se transmiten
al clavo durante la marcha es al realizar el apoyo de la extremidad fracturada mientras la
extremidad sana se encuentra en el aire. Como conclusión, no debe permitirse la deambulación
total en carga en pacientes con el clavo colocado en estático ya que se corre un riesgo innecesario
de rotura de material.This study was aimed at knowing the influence of gait with total weight bearing on
the possibility of breakage of the Grosse-Kempf's locked femoral nail. The study consisted of two
parts: firstly, we carry out a static linear study of the nail by means of a finite element method
applying the different forces that the nail support during gait in order to know what type of forces
affect more the structure.; secondly a kinematic-tridimensional study of gait was performed
showing the phases in which the implant was more affected for different forces. Torsion forces
specially at the single lines support phase during gait are the forces inducing the major stress at the
implant. As conclusion, total weight bearing showed not be permited in patients with insufficient
cortical support or static locking since there is an increase in the risk of breakage of the nail
Fagocitosis del polietileno Estudio con microscopía electrónica
La fagocitosis de las partículas de polietileno (PE) liberadas de los implantes protésicos
es efectuada por células macrofágicas. En este trabajo estudiamos con microscopía electrónica
de transmisión las alteraciones intracelulares inducidas por las partículas de PE. Se han
tomado muestras de la interfase situada entre el cotilo de PE y su lecho óseo en 8 casos de recambio
de cotilo que habían sido tratados mediante artroplastia total de cadera con cotilo de polietileno
roscado de Endler y que habían desarrollado migración por osteólisis. Se han identificado
como componentes del tejido granulomatoso reactivo al PE los siguientes componentes: macrófagos
polinucleados, macrófagos mononucleados, linfocitos, fibroblastos y células endoteliales
capilares. Hemos observado depósitos de PE extracelulares en gran cantidad alternando con fibras
de colágeno. La microscopía electrónica de transmisión permite identificar macrófagos activados
con gran producción de vesículas lisosómicas. El PE intracelular acumulado en fagolisosomas
presenta imágenes poligonales, prismáticas y rectangulares.High density polyethylene particles released from the articular surfaces of joint
prostheses are phagocytosed by macrophaged. We study the intracelular changes provoked by polyethylene
particles using transmitted-light electronic microscopy. Samples from the bone-implant
interface were taken from 8 socket replacements that had been operated with total hip arthroplasty
using Endler polyethylene threaded socket that failed with migration. As components of the reactived
fibrous tissue, the following components were identified: polinuclear macrophages, mononuclear
macrophages, lymphocytes, fibroblasts, and capilar endotelial cells. We have found many
aggregates of extracellular polyethylene mixed with collagen libres. Transmitted-light electronic
microscopy helps to identify actived macrophages with an increased lisosomes production. The aggregated
intracellular polyethylene has polygonal, prismatic and rectangular shapes in fagolisosomes
Aloinjerto como material de relleno de defecto óseo metafisario en las fracturas de platillo tibial
El propósito de este estudio fue valorar la eficacia del aloinjerto en las fracturas
de platillo tibial como material de relleno para los defectos metafisarios, así como una alternativa
que disminuya la morbilidad (dolor en el lugar de la extracción, infección del mismo,
alargamiento del tiempo de intervención quirúrgica...) sin que disminuya la capacidad de integración
ósea y se comprometa la realización de una movilización precoz. Para ello se han revisado
trece casos clínicos en los que se ha practicado una reducción anatómica, fijación interna
y relleno con aloinjerto procedente del Banco de Huesos, apreciándose una integración ósea
del mismo en el plazo aproximado de 10 semanas. Valorando los resultados con arreglo a los
criterios de Honkonen hemos obtenido 7 casos con excelente resultado, 4 casos con bueno
y 2 casos con mal resultado. Consideramos que el aloinjerto óseo es una eficaz alternativa en
el relleno de los defectos metafisarios óseos asociados a las fracturas de platillo tibial.The purpose of this study was to assess the efficacy of allograft bone as filling
agent of metaphyseal defects in tibial plateau fractures, trying to find an effective alternative
to decrease the morbility (donor site complications: painful, infection, increased surgical time...),
without affecting bone ingrowth and early motion. A total of 13 cases treated by anatomic open
reduction, rigid internal fixation and filling with frozen allograft bone have been reviewed. When
evaluating results according to Honkonen, excellent results were found in 7 cases, good results
in 4 and bad results in 2. Allograft bone seems to be an effective alternative for repairing
metaphyseal defects associated with tibial plateau fractures
A decision support system for elective surgery scheduling under uncertain durations
The operation room (OR) is one of the most expensive material resources in hospitals. Additionally, the demand for surgical service is increasing due to the aging population, while the number of surgical interventions performed is stagnated because of budget reasons. In this context, the importance of improving the efficiency of the surgical service is accentuated. The main objective of this work is to propose and to evaluate a Decision Support System (DSS) for helping medical staff in the automatic scheduling of elective patients, improving the efficiency of medical teams'' work. First, the scheduling criteria are fixed and then the scheduling problem of elective patients is approached by a mathematical programming model. A heuristic algorithm is proposed and included in the DSS. Moreover, other different features are implemented in a software tool with a friendly user interface, called CIPLAN. Considering realistic data, a simulation comparison of the scheduling obtained using the approach presented in this paper and other similar approaches in the bibliography is shown and analyzed. On the other hand, a case study considering real data provided by the Orthopedic Surgical Department (OSD) of the "Lozano Blesa" hospital in Zaragoza (HCU) is proposed. The simulation results show that the approach presented here obtains similar occupation rates and similar confidence levels of not exceeding the available time than approaches in the bibliography. However, from the point of view of respecting the order of the patients in the waiting list, the approach in this paper obtains scheduling much more ordered. In the case of the Orthopedic Surgical Department of the "Lozano Blesa" hospital in Zaragoza, the occupation rate may be increased by 2.83%, which represents a saving of 110, 000 euros per year. Moreover, medical doctors (who use this tool) consider CIPLAN as an intuitive, rapid and efficient software solution that can make easier the corresponding task
Estudio epidemiológico de las fracturas trocantéreas en pacientes mayores de 65 años: área de Salud III, Zaragoza
Se presenta un estudio epidemiológico de las fracturas trocantéreas en pacientes
mayores de 65 años tratadas en nuestro Servicio y de Cirugía Ortopédica y Traumatología del Hospital
Clínico Universitario de Zaragoza entre los años 1980-1993, haciendo hincapié en los últimos
4 años. Como parámetros de referencia hemos utilizado la edad, sexo, lado afectado, mecanismo
de producción, clasificación, estancia hospitalaria, demora en la intervención, tratamiento efectuado,
mortalidad y morbilidad. Observamos la elevada incidencia de trauma menor en el mecanismo
de producción, la inversión del índice cervicotrocantéreo y en general la elevada incidencia
en la población a riesgo, lo que han hecho denominar a este tipo de fractura la epidemia del futuro.We have performed an epidemiologic study on the incidence of trocanteric fractures
in patients older than 65 years treated at the traumatology unit of the University Clinic Hospital
of Zaragoza from 1980 to 1993, insisting on last four years. We have valued age, sex, injurycausing,
clasification. length of hospital stay, delay in treatment, in-hospital mortality and morhility.
We have observed that injury causing hip fractures were minimal. Gradual inversion in the
neck/trochanteric ratio was also detected. In general, we confirmed the high incidence in the population
with risk of hip fracture, considering this type of fracture as the epidemy of future
