    Gendering cross-border networks in the Greater Mekong Subregion: drawing invisible routes to Thailand

    "This article discusses human trafficking within the Greater Mekong Subregion (GMS) in relation to the strengthened inter-state economic and infrastructural co-operation and connectivity, taking the life history of sex workers in Thailand into account. Over the last decades, Thailand became known as a hub of entertainment sectors. Traffickers often use socio-economic integration in the GMS to their advantage. A large number of trafficked women ends up in the Thai entertainment industry doing sex work in confined conditions similar to slavery. Poor women are often lured by false promises of well-paid jobs abroad and pay exorbitant fees to agents for such an opportunity. Intermediaries introduce family members to agents who promise to make arrangements for the relevant documentation and transportation across borders. Traffickers use their own marked routes to transport their prey which are more invisible than generally could be imagined." (author's abstract)"Der vorliegende Beitrag betrachtet den Menschenhandel in der Greater Mekong Subregion (GMS) unter Berücksichtigung der Stärkung zwischenstaatlicher wirtschaftlicher und infrastruktureller Kooperation und Vernetzung. Dabei nimmt er Lebensgeschichten Prostituierter in Thailand genauer unter die Lupe. Im Laufe der Jahre wurde Thailand als Drehscheibe des Unterhaltungssektors berühmt. Es ist jedoch weitgehend unbekannt, dass eine Großzahl der Opfer des Menschenhandels in der Sexarbeit und in der Sklaverei ähnlichen Arbeitsverhältnissen landet. Menschenhändler nützen die sozioökonomische Integration in der GMS zu ihrem Vorteil. Diese Studie zeigt, dass von Armut betroffene Frauen in der Regel durch falsche Versprechungen gut bezahlter Arbeitsplätze ins Ausland gelockt werden und für solche Gelegenheiten exorbitante Gebühren an ihre Agenten bezahlen. Vermittler bringen Familienmitglieder und Agenten zusammen, welche die entsprechenden Unterlagen vorbereiten um den Transport über den Grenzübertritt möglich zu machen.     Penelitian ini bertujuan untuk mengetahui tingkat pemahaman siswa kelas VIII di SMP Negeri 3 Kalasan terhadap program adiwiyata yang dilaksanakan disekolah. Jenis penelitian ini adalah penelitian deskriptif kuantitatif dengan metode survei yang menggunakan tes pilihan ganda untuk pengumpulan data. Populasi penelitian ini adalah siswa kelas VIII SMP Negeri 3 Kalasan yang berjumlah 109 siswa. tehnik sampling dalam penelitian ini menggunakan total sampling.dan subjek dalam penelitian ini berjumlah 109 siswa. tehnik analisis data menggunakan deskriptif persentase. Berdasarkan hasil perhitungan diketahui bahwa tingkat pemahaman siswa kelas VIII di SMP Negeri 3 Kalasan terhadap program adiwiyata dengan kategori sangat tinggi sebesar 48,62%, kategori tinggi sebesar 33,94%, kategori cukup sebesar 7,33%, kategori rendah sebesar 3,66%, dan kategori sangat rendah sebesar 6,42%

    This study aims to highlight the impact of promotional advertising on children's buying behavior.Promotions are tools that maximize the sales of any organization. Retailers or manufacturers apply this toolto invite consumers to buy more products. The proposed study aims to focus the advertising impact on childpurchases in the Pakistani capital.     Comparison of Bupivacaine and Dexmedetomidine versus Bupivacaine Alone in Transversus Abdominis Plane Block for Post-Operative Analgesia

    Objective: To study the effects of adding dexmedetomidine to bupivacaine in transverses abdominis plane block in comparison with using bupivacaine alone. Materials and Methods: This was a prospective comparative study that was conducted in the Anaesthesia Department, CMH, Lahore over a period of six months from 1st June 2020 to 30th November 2020. Fifty patients of the American Society of Anesthesiologist Class I and II with an age range between 40 to 60 years were divided into two groups. Group B received 20ml of 0.25% bupivacaine with 2ml of normal saline on each side in the transversus abdominis plane block while group BD was given 20ml of 0.25% bupivacaine with dexmedetomidine 0.5mcg/kg on each side (in a volume of 2ml). Post-operative pain was assessed with a visual analogue scale. Rescue analgesia was given when a score of greater than 3 was observed using this scale. Time to first rescue analgesia was noted. Total opioid consumption in the first 24 hours was also recorded. Patients were observed for postoperative hypotension and bradycardia.Results: The mean-time for the first dose of analgesia for group B and BD was 302.92 ± 24.01 and 419.28 ± 31.97 minutes respectively with a p-value of 0.001. The mean of the total consumption of opioids in 24 hours post-operatively for group B and BD was 14.20±2.36 and 10.40±1.38 mg respectively with a p-value of 0.001. Hypotension was not seen in any patient in either group. Only one patient developed bradycardia and he belonged to group BD. P-value was 0.327.Conclusion: The addition of dexmedetomidine to bupivacaine for transversus abdominis plane block for postoperative analgesia significantly prolongs the duration of analgesia and reduces the postoperative opioid requirements

    Acute Ischemic Stroke and Acute on Chronic Kidney Disease

    Ischemic stroke is due to either local thrombus formation or emboli that occlude a cerebral artery, together with chronic kidney disease represent major mortality and morbidity. Here wer present a case of 53 years old Malay man, admitted to a hospital in Malaysia complaining of sudden onset of weakness on right sided upper and lower limb associated with slurred speech. Patient was also suffering from uncontrolled hypertension, hyperlipidemia, chronic kidney disease stage 4, and diabetes mellitus(un controlled). He was diagnosed with acute ischemic stroke with cranial nerve 7 palsy (with right hemiparesis), acute on chronic kidney disease precipitated by dehydration and ACE inhibitor, and hyperkalemia. Patients with ischemic disease and chronic kidney disaese require constant monitering and carefull selected pharmacotherapy.     Cancer-associated fibroblasts expressing fibroblast activation protein and podoplanin in non-small cell lung cancer predict poor clinical outcome

    BACKGROUND: Cancer-associated fibroblasts (CAFs) are a dominant cell type in the stroma of non-small cell lung cancer (NSCLC). Fibroblast heterogeneity reflects subpopulations of CAFs, which can influence prognosis and treatment efficacy. We describe the subtypes of CAFs in NSCLC.METHODS: Primary human NSCLC resections were assessed by flow cytometry and multiplex immunofluorescence for markers of fibroblast activation which allowed identification of CAF subsets. Survival data were analysed for our NSCLC cohort consisting of 163 patients to understand prognostic significance of CAF subsets.RESULTS: We identified five CAF populations, termed CAF S1-S5. CAF-S5 represents a previously undescribed population, and express FAP and PDPN but lack the myofibroblast marker αSMA, whereas CAF-S1 populations express all three. CAF-S5 are spatially further from tumour regions then CAF-S1 and scRNA data demonstrate an inflammatory phenotype. The presence of CAF-S1 or CAF-S5 is correlated to worse survival outcome in NSCLC, despite curative resection, highlighting the prognostic importance of CAF subtypes in NSCLC. TCGA data suggest the predominance of CAF-S5 has a poor prognosis across several cancer types.CONCLUSION: This study describes the fibroblast heterogeneity in NSCLC and the prognostic importance of the novel CAF-S5 subset where its presence correlates to worse survival outcome.</p
