    Role of gut microbiome in COVID-19: An insight into pathogenesis and therapeutic potential

    Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), resulted in an unprecedented global crisis. Although primarily a respiratory illness, dysregulated immune responses may lead to multi-organ dysfunction. Prior data showed that the resident microbial communities of gastrointestinal and respiratory tracts act as modulators of local and systemic inflammatory activity (the gut-lung axis). Evolving evidence now signals an alteration in the gut microbiome, brought upon either by cytokines from the infected respiratory tract or from direct infection of the gut, or both. Dysbiosis leads to a leaky gut . The intestinal permeability then allows access to bacterial products and toxins into the circulatory system and further exacerbates the systemic inflammatory response. In this review, we discuss the available data related to the role of the gut microbiome in the development and progression of COVID-19. We provide mechanistic insights into early data with a focus on immunological crosstalk and the microbiome\u27s potential as a biomarker and therapeutic target

    Political Communication and TV Comedy: Examining the Descriptive Patterns of Political Entertainment on Mainstream TV Channels of Pakistan

    Political entertainment shows may have political implications whileentertaining. Political issues, political parties and politicalleadership are discussed in these shows employing differentcomedy frames including satire, humor and comedy. Identificationand classification of entertainment content of these shows, on thebasis of comedy formats, frames, types of issues, different politicalparties and their leadership, could help to understand the patternsof political comedy. Thus, this research examines how politicspresented utilizing different patterns of comedy. Data for the study,using content analysis of 1118 representative segments, come fromthe political comedy shows broadcast on mainstream TV channelsof Pakistan. Findings of the study show that there are more satiricalframes used in these shows as compared to humor and parody.Political issues have been given more coverage as compared tosocial and economic issues; and political parties including PPP,PML-N, PTI remain the target of these shows predominantly

    Itopride for gastric volume, gastric emptying and drinking capacity in functional dyspepsia

    AIM: To study the effect of itopride on gastric accommodation, gastric emptying and drinking capacity in functional dyspepsia (FD). Methods: Randomized controlled trial was conducted to check the effect of itopride on gastric accommodation, gastric emptying, capacityof tolerating nutrient liquid and symptoms of FD. We recruited a total of 31 patients having FD on the basis of ROME III criteria. After randomization, itopride was received by 15 patients while 16 patients received placebo. Gastric accommodation was determined using Gastric Scintigraphy. 13C labeled octanoic breadth test was performed to assess gastric emptying. Capacity of tolerating nutrient liquid drink was checked using satiety drinking capacity test. The intervention group comprised of 150 mg itopride. Patients in both arms were followed for 4 wk. Results: Mean age of the recruited participant 33 years (SD = 7.6) and most of the recruited individuals, i.e., 21 (67.7%) were males. We found that there was no effect of itopride on gastric accommodation as measured at different in volumes in the itopride and control group with the empty stomach (P = 0.14), at 20 min (P = 0.38), 30 min (P = 0.30), 40 min (P = 0.43), 50 min (P = 0.50), 60 min (P = 0.81), 90 min (P =0.25) and 120 min (P = 0.67). Gastric emptying done on a sub sample (n = 11) showed no significant difference (P = 0.58) between itoprideand placebo group. There was no significant improvement in the capacity to tolerate liquid in the itopride group as compared to placebo (P = 0.51). Similarly there was no significant improvement of symptoms as assessed through a composite symptom score (P = 0.74). The change in QT interval in itopride group was not significantly different from placebo (0.10). Conclusion: Our study found no effect of itopride on gastric accommodation, gastric emptying and maximum tolerated volume in patients with FD

    Infective endocarditis in patients with congenitally malformed hearts: characterization of the syndrome in a developing country

    Cardiac surgery for correction or palliation of congenital cardiac disease in infancy and childhood remains a privilege that is rarely accessible to two-thirds of the world’s population. This imbalance has created a unique spectrum of illness in patients with underlying congenital cardiac disease and complicating infective endocarditis in developing countries, including Pakistan. In this study, we characterize endocarditis as seen in such patients presenting in Karachi. We reviewed retrospectively patients admitted to Aga Khan University with underlying congenitally malformed hearts and endocarditis between 1991 and 2004. We identified 48 patients with endocarditis according to the modified Duke Criterions, with just over half the cases (54%) classified as definite endocarditis. Of the patients, 23 (49%) patients were more than 16 years old. Uncorrected left-to-right-shunts, tetralogy of Fallot, and congenital mitral valvar disease were the most common underlying defects. Patients with cyanotic defects, particularly of the complex type, were underrepresented (4%). Only 11 (22.9%) of the patients had a previous palliative or corrective surgery. In one-third of the patients (16), streptococcal species were identified as the microbiologic cause of endocarditis, and 22 (45.8%) had culture-negative endocarditis. In contrast, Staphylococcus aureus and enterococci caused endocarditis in only one patient each. There were no differences in mortality or complications between cyanotic and acyanotic congenital defects. Surgery was performed in nine (18.7%) patients with endocarditis, and of these, 13 (27.1%) died. In contrast to the developed world, endocarditis in the developing countries, such as Pakistan, complicates uncorrected left-to-right shunts and tetralogy of Fallot, probably because patients with complex cyanotic defects fail to survive long after birth due to the lack of available surgery. Almost half of patients had culture-negative endocarditis, likely related to several factors

    Pancreatic Involvement in Hermansky–Pudlak Syndrome- A Case Report

    Hermansky-Pudlak Syndrome (HPS) is a rare autosomal recessive disorder that presents with oculocutaneous albinism, bleeding disorders, and immunodeficiency. Granulomatous colitis and pulmonary fibrosis are two major complications of this syndrome. On rare instances, HPS can involve the heart and lungs. This report discusses a 32-year-old man who presented with oculocutaneous albinism and immunodeficiency along with renal, pulmonary, and pancreatic complications. Pancreatic atrophy is a unique finding in our patient, which has not been reported in the literature. The purpose of our case report is to bring into light unusual complications of HPS so that timely action could be taken to avoid the progression of complications.&nbsp

    Cricket, a famous game played and watched at its peak in Asia. The game is governed by International Cricket Council (ICC). It is a team sport of eleven players in each side with different roles, of which one is bowler. The bowlers have followed different styles of bowling sinces game has started, of which some declared illegal initially. In our study, we have focused on the cases of illegality reported form South Asian region in post 1990 period a new era of biomechanics based bowling laws started. We have also investigated the updated ICC process for bowlers review and remedy along with biomechanics based labs established worldwide. Finally, we have proposed a new information technology based bowling action evaluation solution, lower in cost and affordable at domestic level. Such study will help bowlers, their coaches and viewers to understand the biomechanics based bowling laws, standings of South Asian bowling after these laws and alternative solutions to rescue it

    Performance Evaluation of Flexible Pavement Using Carbon Nanotubes and Plastic Waste as Admixtures

    This paper investigates the properties of waste plastic film and carbon nanotubes modified asphalt mixes. The composed asphalt mixes have been evaluated for fatigue cracking and rutting. In this research, a total of 30 numbers of samples were prepared with different percentages of waste plastic films and carbon nanotubes from 0.1 to 0.4% and evaluated for dynamic stability and rutting using the wheel tracking machine. Results show that the addition of carbon nanotubes significantly increases the dynamic stability of asphalt and decreases the rutting up to a sufficient level as compared to plastic and conventional bitumen

    Psychiatry as a career: A survey of factors affecting students’ interest in Psychiatry as a career

    The objectives of this study were to determine the characteristics of medical students and graduates interested in choosing psychiatry as a career and the obstacles in choosing this field of medicine. Two private and two public medical institutes were surveyed from June 2007 to August 2007. A self-administered questionnaire was distributed to third, fourth and final year students and to medical graduates doing their internship in these four medical institutes. A total of 909 medical students and graduates participated in the study. Seventeen percent of participants responded positively regarding their interest in psychiatry as a career. Significantly higher proportion belonged to private medical institutes (14% vs. 24%, P-value =0.001). There was no significant difference in reporting interest for psychiatry in regard to age, sex, year in medical school and whether or not the participant had done a psychiatry ward rotation. However significantly higher proportion of participants (22%, n=43) were reporting their interest in the field of psychiatry who had done more than a month long psychiatry ward rotation as compared to those participants (14%, n=54) with less than a month or no psychiatry rotation (P-value=0.01). More students were reporting their interest in psychiatry with a family history of psychiatric illness as compared to without family history (24% vs 16%, P-value=0.03). In conclusion, students and graduates with more than a month long rotation in psychiatry, studying in private medical colleges and with a family history of psychiatric illness were more interested in choosing psychiatry as a career
