(A) study evaluating the effect of microthread on the maintenance of marginal bone level : a 3-year prospective

치의학과/석사[한글] 골유착성 임프란트의 장기간 성공에 있어 임프란트 변연골의 보존은 필수적이다. 또한 변연골의 양과 형태에 따라 임프란트 주위 점막의 형태가 좌우되며 이는 임프란트의 심미성과 관련이 있다. 따라서 임프란트 식립 후 변연골의 변화에 대한 연구는 중요한 의미를 갖는다. 최근 많은 연구에서 치관부 임프란트 design에 변화를 주어 골 흡수를 감소시키려는 노력이 진행되어 왔다. 이에 본 연구에서는 치관부 유지요소인 미세나사선이 변연골 수준의 유지에 미치는 장기적 영향을 알아보려고 하였다.본 연구에서는 연세대학교 영동세브란스병원 치주과에 내원하여 만성 치주염 진단하에 치주치료와 발치를 시행하고 자가 치태조절 교육을 받은 17명의 환자의 부분 무치악 부위에 치관부 미세나사선을 갖는 Astra Tech Single Tooth Implant(Astra Tech AB, Molndal, Sweden; ATST)와 미세나사선을 갖지 않는 Astra Tech TiOblast Implant(ATTB)를 인접하여 매식하고 고정성 보철물을 장착한 후 3년 동안 주기적으로 방사선 사진을 촬영하여 변연골 흡수량을 비교하였다.1. 연간 평균 변연골 흡수량은 ATST가 ATTB보다 통계학적으로 유의하게 작았다(p<0.05).2. ATST는 보철물 장착 1년 후부터, ATTB는 2년 후부터 변연골 흡수량이 안정화 되는 경향을 보였다. 위의 결과를 종합해 볼 때 미세나사선은 임프란트 변연골의 흡수를 감소시키고, 안정화하여 이를 장기간 유지시켜 준다고 볼 수 있었다. [영문]The success of dental implant therapy in esthetic aspect, relies mainly upon the presence and maintenance of the bone adjacent to the implant since the preservation of bone support is essential for the pink esthetics. Therefore, there have been many efforts to reduce the marginal bone loss by modifying the implant design, and microthread is one of them. The purpose of this study is to evaulate the long-term effect of microthread on the maintenance of marginal bone level. In this study, 17 patients were selected and two types of Astra Tech implants(Astra Tech AB, Molndal, Sweden) were used, one with microthread and one without it. In these patients, Astra Tech Single Tooth Implant(ATST) with microthread and Astra Tech TiOblast Implant(ATTB) without it were installed adjacent to each other within the same partially edentulous sites and marginal bone loss was evaluated by radiographic image. Using the level of marginal bone around the implant at the time of prosthesis insertion as reference, the change of bone level at 1-year, 2-year, and 3-year follow-ups were analyzed. 1. There was a statistically significant difference in the amount of annual marginal bone loss between ATST and ATTB(p<0.05).2. Marginal bone loss of ATST was stabilized after the first year, but ATTB reached a steady state after the second year. In couclusion, the microthread might be effective on maintaining and stabilizing the marginal bone loss against loading.ope
