신시장진출지원자금 사업을 중심으로

학위논문 (석사) -- 서울대학교 대학원 : 행정대학원 행정학과(행정학전공), 2020. 8. 박순애.중소기업을 지원함으로써 이들 기업이 성장하여 중견기업 또는 강소기업이 되어 국가 경제의 주축이 되도록 하고, 중소기업이 창출하는 고용의 질과 양을 개선하여 국가적 고용 문제를 해결할 필요성이 강조되고 있다. 하지만, 한정된 국가 재원으로 보다 효과적인 지원이 이루어질 수 있도록 하려면 기존 지원사업의 효과성을 평가하고 사업구조를 지속적으로 재설계해야 한다. 중소기업 지원사업은 다양한 성격의 개별 사업들로 구성되어 있고 새로운 사업이 도입되기도 하는데, 이에 대한 고려 없이 지원사업 전체의 정책효과를 분석할 경우 연구시기에 따라 효과성에 차이가 발생할 수 있다. 또한, 성격이 다른 각각의 사업이 가지는 정책효과의 차이가 충분히 분석되지 않는 문제점이 있다. 따라서, 중소기업 지원사업에 대한 연구에 있어 개별 사업에 대한 미시적인 분석을 통해 지원사업의 효과성 및 정책효과에 영향을 미치는 조절변수를 정밀하게 분석하고, 이를 통해 사업의 정책효과를 극대화하는데 필요한 정책적 시사점을 도출할 필요가 있다. 특히, 새롭게 도입된 지원사업에 대해서는 사업 도입의 정책 목적이 달성되고 있는지 분석하고, 정책효과 극대화를 위한 사업설계 및 운영방식 개선의 필요성 여부를 검토할 필요가 있다. 본 연구는 이러한 중소기업 지원사업에 대한 미시적 분석 연구의 필요성에 주목하여, 중소벤처기업진흥공단의 자금지원 사업 중 2017년 새롭게 도입된 신시장진출지원자금 사업에 대해 효과성을 평가하고, 지원대상 기업의 업종, 업력, 소재지 등에 따른 정책효과의 차이를 분석하고자 하였다. 이를 위해 자금지원 전후의 매출, 자산, 영업이익, 고용의 통계적으로 유의미한 증가 여부를 STATA 프로그램을 활용하여 회귀분석을 통해 분석하였다. 한편, 신시장진출지원자금 사업은 개발기술사업화와 글로벌진출지원 두 가지 내역사업으로 구성되어 있는데, 이들 내역사업은 내용과 성격이 다른 만큼 미시적인 정책효과 분석을 위해서는 각각의 내역사업의 정책효과를 분석하고 비교할 필요가 있다. 이에 각 내역사업별로 매출, 자산, 영업이익, 고용의 증가 효과를 평가하고, 내역사업 간 정책효과의 차이가 있는지 분석하였다. 분석 결과, 중소벤처기업진흥공단의 2017년 신시장진출지원자금 사업은 매출 및 자산에서 통계적으로 유의미한 증가를 확인할 수 있었고, 영업이익과 고용은 통계적으로 유의미한 증가를 확인할 수 없었다. 그런데, 지원대상 기업 전체를 분석한 결과와 달리, 지원대상 기업 중 건설업에서는 통계적으로 유의미한 영업이익 증가를 확인할 수 있었으며, 업력 5년 미만 기업에서 통계적으로 유의미한 고용 증가를 확인할 수 있었다. 지원대상 기업의 소재지, 업종, 업력을 조절변수로 분석한 결과, 소재지와 관련하여 매출 증가는 서울 소재 기업이 더 큰 반면, 자산 증가는 서울이 아닌 지역에 소재한 기업이 더 큰 것으로 나타났다. 업종과 관련하여, 매출의 경우 제조업과 건설업, 자산의 경우 제조업과 서비스업에서 통계적으로 유의미한 증가를 확인할 수 있었으며, 제조업이 비제조업보다 매출 및 자산 증가가 더 큰 것으로 나타났다. 업력과 관련하여, 5년 미만 기업의 자산 증가가 더 큰 것으로 나타났다. 신시장진출지원자금 사업의 내역사업인 개발기술사업화와 글로벌진출지원에 대해 자금지원의 정책효과를 분석한 결과, 매출 및 자산에서 통계적으로 유의미한 증가를 확인할 수 있었는데, 개발기술사업화의 매출 및 자산 증가가 글로벌진출지원보다 높은 것으로 나타났다. 한편, 영업이익과 고용은 개발기술사업화와 글로벌진출지원 모두 통계적으로 유의미한 증가 효과를 확인할 수 없었는데, 업종별로 분석한 결과 개발기술사업화의 경우 제조업과 건설업에서 통계적으로 유의미한 영업이익 증가를 확인할 수 있었고, 업력별로 분석한 결과 개발기술사업화의 경우 5년 미만 기업에서 통계적으로 유의미한 고용 증가를 확인할 수 있었다. 중소기업의 성장과 고용 창출을 위한 정책수단으로서 중소기업에 대한 자금지원은 중요한 역할을 하고 있다. 다양한 중소기업 지원사업에 대한 미시적인 분석을 통해 기존 사업의 효과성을 평가하고, 문제점과 한계를 분석하여 지원사업을 재설계함으로써 정책효과를 극대화 할 필요가 있다. 중소기업에 대한 자금지원 사업의 효과를 실증적으로 분석하기 위한 시스템을 제도화하고, 개별 지원사업의 정책효과를 지속적으로 분석하면서 보다 효과적인 사업 체계를 설계하고 사업 방식을 개선해 나가야 할 것이다.Supporting SMEs can help these companies grow and improve the quality and quantity of employment. In order to provide more effective support with limited national resources, it is necessary to evaluate the effectiveness of existing support projects and continuously redesign the project structure. SME support projects consist of individual projects of various natures, and new projects are sometimes introduced. When analyzing the policy effects of the entire support project, the research results may vary depending on the research period. In addition, the differences in the policy effectiveness of each support project with different characteristics may not be sufficiently analyzed. Therefore, it is necessary to precisely analyze the variables affecting the effectiveness and policy effectiveness of the support project through microanalysis of individual projects. And it is necessary to derive the policy implications necessary to maximize the policy effectiveness of the project. In particular, it is necessary to analyze whether the policy objectives of the project have been achieved for the newly introduced support projects and to examine whether it is necessary to improve the project to maximize the policy effects. The purpose of this study was to evaluate the effectiveness of support projects introduced in 2017 and to analyze the differences in policy effects depending on the type or location of companies. To this end, the STATA program was used to analyze whether there was a statistically significant increase in sales, assets, operating profit and employment before and after funding. The support projects analyzed in this study consist of two detail projects, which need to analyze and compare the policy effects of each detail project for micro-analysis, as they differ in content and nature. Therefore, it was assessed whether sales, assets, operating profit and employment increased for each detail project and analyzed whether there were differences in policy effects between the detail projects. The analysis showed statistically significant increases in sales and assets, but no statistically significant increases in operating profit and employment. Unlike the analysis of the entire supported entity, a statistically significant increase in operating profit was identified in the construction industry. In addition, a statistically significant increase in employment was seen in companies with less than five years of operation. The increase in sales was larger for Seoul-based companies, while the increase in assets was larger for non-Seoul-based companies. For sales, a statistically significant increase was seen in manufacturing and construction. For assets, a statistically significant increase in manufacturing and service industries could be identified. The increase in sales and assets was greater in manufacturing than in non-manufacturing. The analysis of the detail projects indicated statistically significant increases in sales and assets. Among the detail projects, the increase in sales and assets of technology development projects was higher than that of overseas expansion support projects. Operating profit and employment did not have statistically significant increases in both detail projects. However, in the case of technology development projects, there was a statistically significant increase in operating profit in manufacturing and construction, and a statistically significant increase in employment in entities with less than five years. Funding to small and medium enterprises plays an important role as a policy tool for the growth and employment of SMEs. It is necessary to assess the effectiveness of existing projects through microanalysis of various support projects for small and medium enterprises, and to maximize policy effects by redesigning support projects. Morphologic change of nerve and symptom relief are similar after mini-incision and endoscopic carpal tunnel release: a randomized trial

BACKGROUND: The symptoms in carpal tunnel syndrome (CTS) can be ameliorated by open and endoscopic release of the transverse carpal ligament. It is unknown whether a mini-incision or endoscopic carpal tunnel release more effectively reverses the pathological changes that are observed in the median nerve in patients with CTS and these morphologic changes correlates with the subjective outcomes after carpal tunnel release. We hypothesized that (1) at 24 weeks after surgery, the subjective outcomes of mini-incision release and endoscopic release would not differ in patients with CTS; and (2) the ultrasonographic (US) morphology of the median nerve reverses similarly after mini-incision and endoscopic release; (3) the subjective outcomes correlates with these morphologic changes. METHODS: Between November 2011 and January 2013, 67 patients with CTS in their dominant wrist were randomized to either mini-incision (n = 32) or endoscopic (n = 35) release. Each patient was assessed by both the Boston Carpal Tunnel Questionnaire (BCTQ) and the Disabilities of the Arm, Shoulder, and Hand (DASH) pre-operatively and 24 weeks' post-operation. An US examination was conducted at both time points to measure the cross-sectional area (CSA) at the inlet, middle, and outlet (CSA-I, CSA-M and CSA-O) and the flattening ratio (FR) at the middle and outlet (FR-M and FR-O) of the median nerve. RESULTS: The post-operative mean BCTQ and DASH scores were improved significantly from the pre-operative scores in both groups (p < 0.001). The mean CSA-I decreased and CSA-M and CSA-O increased similarly in both groups (by 3.3, 3.0, and 3.8 mm2 in the mini-incision group and 2.9, 3.1. and 2.7 mm2 in the endoscopic group. The mean FR-M/FR-O decreased similarly from 3.6/4.2 to 3.2/3.0 in the mini-incision group and 3.8/4.3 to 3.2/2.9 in the endoscopic group. There were no significant differences in the subjective outcome scores or median nerve measures between the two groups. Improvement in the BCTQ-S only was significantly correlated with changes in the CSA at the inlet. CONCLUSIONS: Mini-incision and endoscopic release both similarly relieved subjective symptoms and functions along with the pathological changes in the median nerve morphology along the carpal tunnel in patients with idiopathic CTS. Symptom relief after surgical decompression seems to correlate with reduced nerve swelling at carpal inlet and reversed nerve flattening inside carpal tunnel. Reassessment of alkaline phosphatase as serum tumor marker with high specificity in osteosarcoma

The goal of this study was to reassess serum alkaline phosphatase (ALP) as tumor marker in osteosarcoma. We retrospectively examined serum ALP levels at diagnosis, every therapeutic step (neoadjuvant chemotherapy, surgery, and adjuvant chemotherapy), metastasis, and follow-up and analyzed the role of ALP as tumor marker in 210 osteosarcomas. The diagnostic performances of ALP were validated with pathology-proven 899 other primary bone lesions. Elevated ALP at diagnosis was associated with inferior overall survival (OS) (Log Rank P < 0.001) and disease-free survival (Log Rank P = 0.005) and independently significant for OS in multivariate analysis (hazard ratio [HR]=2.12, P = 0.032). During therapy, the ALP level significantly changed according to therapeutic steps (P < 0.001 for patients ≥15 years old, P < 0.001 for patients <15 years old) and survival (P = 0.015 for ≥15 years, P = 0.002 for <15 years), and the response of ALP to therapy and survival were associated (P = 0.042 for ≥15 years, P = 0.036 for <15 years). Initial ALP level was linearly correlated with tumor burden (total tumor volume; P = 0.016 for ≥15 years, bone tumor volume; P = 0.012 for ≥15 years). The sensitivity and specificity of ALP on diagnosis were 53.2% (95% Confidence Interval [CI]: 0.475-0.586) and 90.1% (95% CI: 0.888-0.913). The sensitivity of ALP on metastasis was 53.2% (95% CI: 0.431-0.624), and the specificity was 78.2% (95% CI: 0.720-0.839) at15 months postoperative and 90.0% (95% CI: 0.824-0.952) at 3 years postoperative. Serum ALP was found to be a valuable tumor marker with high specificity in osteosarcoma.ope

Biological Markers for Making the Differential Diagnosis between Intraductal Papillary Mucinous Tumor and Mucinous Cystic Tumor of the Pancreas

PURPOSE: Although intraductal papillary mucinous tumor (IPMT) and mucionus cystic tumor (MCT) share many common features, they are 2 clinically different disease entities. The aims of this study were to compare the clinicopathological characteristics of IPMT and MCT and to find molecular markers for making the differential diagnosis between IPMT and MCT METHODS: Forty-one patients with IPMT and 29 MCT patients who all underwent resection between 1994 and 2003 were enrolled in this study. After one gastrointestinal pathologist reviewed the pathological slides, we compared the clinico-pathological features of the two diseases. Immunohistochemical staining with using 17 biological markers was performed to find useful molecular markers for making the differential diagnosis. RESULTS: Besides the clinical features like gender, the tumor size and location, the patient's age and the radiological images, we found differences of the expressions of PR(p<0.001), ER(p<0.001), MUC2(p=0.038) and MUC5A(p=0.001) between IPMT and MCT. CONCLUSION: PR, ER, MUC2 and MUC5A can be useful in making the final differential diagnosis between IPMT and MCT. Analysis of related factors on middle school student smoking in Korea

지역사회간호학과/석사[한글]본 연구는 날로 증가하는 우리나라 중학생의 흡연실태와 관련요인을 파악하여 흡연예방을 위한 정책적 프로그램 마련을 위한 기초 자료를 제공하기 위한 취지에서 시작되었으며, 연구 자료는 세계보건기구(WHO)와 미국질병관리본부(CDC)가 주관하고 본인이 속한 연세대학교 보건대학원과 연세대학교 국민건강증진연구소가 위임받아 실시한 ‘세계청소년흡연조사(GYTS : Global Youth Tobacco Survey ; 세계 각국의 13～15세 중학생을 대상으로 실시)’ 자료를 활용하였다.표본추출은 2단계 집락추출 방법을 사용하였으며 대상은 제주도를 제외한 서울시 소재 25개교, 6개 광역시 소재 25개교, 7개 지방 소재 25개교를 합하여 총 75개교 6,234명을 대상으로 실시하였다.분석을 통해 전체 대상자의 흡연율, 간접흡연율, 흡연시도율을 알아보았는데 흡연자의 흡연양상을 성, 학년, 지역별로 구분하여 이들 간의 차이를 x2-test를 통해 조사하였다. 흡연 관련요인 분석은 인구사회학적 요인, 흡연예방교육 이수여부, 흡연에 대한 태도와 지식, 대중매체에서의 흡연이나 금연 메시지 접근 정도에 따른 흡연여부를 각각 x2-test와 로지스틱 회귀분석을 통해 실시하였으며 주요 분석결과는 다음과 같다.첫째, 우리나라 중학생의 전체 흡연율은 6.4%이며 이중 남학생 흡연율이 7.4%, 여학생 흡연율이 5.5%로 여학생 흡연율보다 남학생 흡연율이 더 높음을 볼 수 있었다.둘째, 조사대상 전체학생 중 약 26.5%의 학생이 흡연을 시도해 본 경험이 있다고 답했으며 여학생보다는 남학생의 경우, 학년이 올라갈수록 흡연시도율이 증가하였다.셋째, 중학생 흡연자들의 흡연실태를 보면 흡연자의 56.8%가 호기심으로 흡연을 시작하였고 학년이 올라갈수록 1회 흡연량과 흡연일수가 증가하는 것으로 조사되었다. 또한, 흡연학생의 64.0%가 금연을 희망하고 70.8%의 학생이 최근 1년 안에 금연을 시도했다고 응답하였다.넷째, 흡연에 영향을 미치는 인구사회학적 요인은 성, 학년, 부모의 흡연, 친구의 흡연, 용돈 수준 등이었고 이를 통제한 상태에서 관련요인을 분석한 결과 흡연에 대한 지식정도, 흡연예방교육 이수여부, 대중매체 혹은 사회행사를 통한 금연메세지 혹은 담배광고의 접근도에 따른 흡연자와 비흡연자 사이에 통계적으로 유의한 차이가 있었다.이상의 연구 결과를 종합해볼 때 우리나라 중학생의 흡연율은 기존의 연구에서 조사된 것 보다 높았으며 흡연양상과 실태 또한 심각한 수준임을 알 수 있었다. 특히 이들의 흡연은 성, 연령, 부모의 흡연, 친구의 흡연, 용돈수준 등 인구사회학적 특성과 흡연예방교육 이수여부, 흡연자에 대한 태도, 지식수준, 대중매체를 통한 금연 메세지와 담배광고 등과 관련성이 있는 것으로 나타났다. 따라서 초등학교 때부터 조기 흡연예방교육과 흡연자에 대한 금연교육이 정규교육과정을 통해 지속적으로 실시되어져야 할 것으로 사료된다. 아울러 담배가격의 인상과 담배자판기의 성인 인증장치 의무화 등을 통해 청소년들의 담배 접근 가능성을 낮추고, 19세 미만자에게 대한 담배 판매행위나 공공장소에서의 흡연행위에 대한 감시&#8228;규제활동을 보다 구속력 있게 실시해야 할 것이다. 한편 공영방송에서의 간접적인 담배광고와 케이블 방송, 신문, 잡지 및 각종 청소년 대상 사회행사에서도 담배나 흡연관련 광고 금지를 확대 실시해 나갈 필요성이 있다. [영문]The purpose of this study was to serve for a basis for antismoking campaigns and to prevent adolescents from smoking by analyzing the smoking realities and related factors.This study are the secondary data of Global Youth Tobacco Survey (GYTS) developed by the WHO and United State Center for Disease Control(CDC) and Prevention to trace tobacco use among youth in countries across the world. This study was carried out to define the prevalence of youth smoking and passive smoking among middle school students, and described the related factors in these students in focusing the exposure to smoking in the mass media.This study is a cross-sectional school-based survey, which employed a two-stage cluster sample design to produce a nationally representative sample of middle school students aged 13 to 15 years-old. Seventy-five schools were selected. All schools containing grades 1, 2 and 3 that contained 40 or more students were included in the sampling frame.The results of this study were as follows ;1. The proportion of smoking students in middle school was 6.4% and the rate of smoking in male students(7.4%) was higher than it of in female students(5.5%) similarly case of adults and high school.2. 26.5% of all respondent had a experience to try smoke and the results showed that the rate of trying smoke in male students was increased by grade more than female students.3. 56.8% of the smokers answered they have smoked a cigarette prompted by curiosity and 31.7% of the respondents have smoked 2～5 cigarettes in a day and 34.4% of the respondents have smoked 1～2 days in a month.4. According to the test of significance in regression analysis, the independent variables such as sex, school grade, smoking parents, friends distinction, pocket money and others showed statistical significance.It has also been reported that the teenagers with a smoker in the family are more likely to smoke than the ones who don`t.5. There was a statistical significance between smokers and nonsmokers in proportion to the completing the preventive education for no-smoking(p=0.0023) and the acquirement of knowledge on smoking(p<.0001).6. Finally there was a statistical significance between the exposure to anti-smoking campaign(p=0.0053) and tobacco advertisement in the mass media and the smoking(p=0.0036).All things considered, the health education for tobacco control in school need to be developed from an elementary school. We have to pay more attention on discussion why young people start smoking. I suggest that smoking program in school need to be revitalized and be made more accessible for everyone who wish to quit. We were already made a law to ban selling cigarettes to people under 19, however the results of this survey were showed that this regulation is not effective. So, we have to push the implementation of this regulation, and have to punish the retailers who sell cigarette to people under 19. And the government should develop the regulation on total banning promoting cigarette advertising and smoking scene in the movie for young people.ope

A study on factors influencing utilization of Long-term care services for the elderly

보건정보관리학과/석사[한글]본 연구는 노인장기요양보험 인정자를 대상으로 Andersen의 건강서비스 행동모델에 바탕을 두고 노인장기요양급여의 이용여부, 이용형태, 이용량에 영향을 미치는 요인에 대하여 분석하였다. 노인장기요양보험 본 사업 시작부터 2008년 12월 31일까지 등급 내로 판정된 인정자 중에서 등급판정일이 6개월 이상 경과한 자를 대상으로 20%를 16개 지역별로 층화하여 무작위 추출한 41,592명을 대상으로 하였다. 급여이용내역은 각 대상자의 등급판정일 이후부터 2009년 6월 30일까지의 이용내역으로 하였다.본 연구에서는 노인장기요양보험 인정자 중 76.8%가 급여를 이용하였으며, 23.2%는 이용하지 않았고, 급여를 이용한 대상자 중 31.8%는 시설급여를 이용하고, 64.3%는 재가급여, 3.9%는 시설급여와 재가급여를 모두 이용하였다. 노인장기요양급여 이용여부와 시설급여와 재가급여로 결정되는 급여의 이용형태에 영향을 미치는 요인에 대하여 로지스틱 회귀분석을 수행하였다. 급여의 이용에 소인요인으로 연령군, 성별, 자원요인으로 경제수준, 거주지역, 주거상태, 동거여부, 수발자, 욕구욕인으로 노인성 질환 유무, 행동변화와 간호처치 영역의 기능상태가 영향을 미쳤으며, 급여의 이용형태는 소인요인으로 연령군, 자원요인으로 거주지역, 주거상태, 동거여부, 수발자, 욕구요인으로 노인성 질환 유무와 신체기능, 인지기능, 행동변화, 재활 영역의 기능상태가 영향을 미쳤으며 성별과 경제수준은 유의수준을 0.05로 하였을 때 통계적으로 유의하지 않았다.노인장기요양급여의 이용량에 영향을 미치는 요인을 의사결정나무로 분석한 결과 배우자가 수발하는 경우와 요양병원 및 병원에 입원한 상태에서 흡인을 하는 경우에 시설급여의 이용량이 적었다. 의료급여수급권자의 경우 시설급여와 주야간보호 서비스의 이용량이 적고, 기초생활수급권자의 경우 방문요양과 주야간보호 서비스의 이용량이 많았다. 재가서비스 중 방문요양, 방문목욕, 주야간보호 서비스의 이용량은 대도시, 중소도시, 농어촌의 순으로 컸으며, 2개 이상의 재가서비스를 이용하며 행동변화 영역의 증상이 있는 경우에 방문요양 서비스의 이용량은 감소하고, 주야간보호 서비스의 이용량은 증가하였다. 본 연구결과는 노인장기요양급여의 이용에 영향을 미치는 요인에 관한 분석을 통해 향후 노인장기요양급여의 이용량 추계 등 수요를 예측하고 노인장기요양보험의 재정규모 설정 등 서비스 공급을 조절할 수 있는 기초자료로 활용될 수 있을 것이며, 서비스 이용지원 단계에서의 의사결정 보조자료로 의미가 있을 것이다.
