Histopathological studies of 300 cases of non-hodgkin's lymphoma in Korean patients

의학과/석사[한글] 악성 임파종은 비교적 흔히 접하는 악성 종양이며,우리 나라의 통계에서도 전체 악성 종양의 5.2 %를 차지한다. 악성 임파종은 그 분류가 다양하여 많은 혼돈이 초래되어 왔으나 1982년 National Cancer Institute에서 Working Formulation이라는 새로운 분류법을 제시하여 예후나 치료의 결과와 연결짓고자 하였다. 우리 나라의 악성 임파종에 대한 통계적 분석 및 병리 조직학적 관찰은 과거 여러 종류의 분류법을 사용하여 문헌에 보고되어 왔지만 Working Formulation 에 의한 전반적인 통계적 분석은 문헌에 보고된 것이 미 비하다. 따라서 본 연구는 1977년 1월부터 1986년 12월까지만 10년간 연세대학교 의과대학 병리학 교실, 영동 세브란스 병원 및 원주 의과대학부속 원주기독병원 병리과에서 비 호지킨씨 악성 임파종으로 진단된 300예를 Working Fonnulation과 Rappaport 분류법에 의해 조직학적 재분류를 시행하고, 그 발생양상과 특성을 분석하였으며, 더불어 임파종 유사질환 중 혈관 면역아구성 임파선증과 다형성 세망증에 대해 그 전반적인 발생 빈도와 함께 면역 글로불린 Kappa와 Lambda Light Chain, α1-antichy-motrypsin에 대한 면역 과산화 효소염색을 시행하여 다음과 같은 결론을 얻었다. 1. 총 300예의 비 호지킨씨 악성 임파종중 임파절에서 발생한 예는 141예, 위장관계에 67예, 편도에 32예의 순으로 발생하였으며 ,남녀비는 1.4 : 1로 남성에서 다소 많이 발생하였다. 2. Working Formulation에 의하면 총 300예중 중등도가 199예 (66.4%)로서 가장 많았고 이중 "diffuse, large cell " 형이98 (32 7%)로 가장 높은 빈도를 보였다. Rappaport 분류법에 의하면 결절형은 3예(1.0 % )에 불과하였고 나머지가 미만형으로 이중에 " histio cytic "형이 134예( 44.7% )로 가장 많았다. 3. 악성 임파종 300예중 32예에서 경색이 동반되었는데" diffuse, large cell "형에서 가장 빈번하게 동반되었다. 4 .총 7예의 혈관 면역아구성 임파선중에서 면역 글로블린에 대한 면역 과산화 염색을 시행한 결과 7예중 4예에서 kappa흑은 Lambda chain에 양성 반응을 보이는 면역 아구 세포들이 관찰되어 polyclonality를 보였으며 alpha-1-antichymotrypsin 에는 정상 조직구들만이 염색되었다. 총 26예의 다형성 세망증에 대한 면역 과산화 효소 염색에서 이형성 세망세포는 kappa, lambda와 alpha-1-antichymotrypsin에 대해 모두 음성반응을 보였다. [영문] Non-Hodekin's malignant lymphoma is a relatively frequent lymphoreticular malignancy, and has been reported to constitute up to 5.2% of all malignant turners In Korean patients. Various morphologic classifications of non-Hodgkin's lymphoma have been proposed, and among them, the Rappaport's classification has been most widely accepted. In 1982, a National Cancer Institute sponsored study on clasaification led to the creation of the Working Formulation in an attempt to resolve the controversy and debate regarding the various classiftications of non-Hodgkin' s lymphoma. Angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosts is a lymphoreticular ploitferative disorder, which has been reported to transform to malignant lymphoma. The purpose of the present study is to reclassify non-Hodgkin's lymphomas according to the Working Formulation and to investigate the histopathological and immunocytochemical characteristics of angioimmunoblstic lymphadenopathy with dysproteinemia and polymorphic reticulosis. The study reviewed 300 cases of nodal and extranodal non-Hodgkin's lymphoma, 26 cases of polymorphic reticulosis, and 7 cases of angiommunblastic lymphadenopathy with dysproteinemia examined in the Departments of Pathology, Yonsei University College of Medicine, Youngdong Severance Hospital and Yonsei University Wonju College of Medicine from January 1977 to December 1986. In non-Hodgkin's lymphomal each case was classified according to Working Formulation and the Rappaport classification. All angioimmunoblastic lynphadenopathy with dysproteinemia and polymorphic reticulosis cases were subsjected to histopathological analysis and a review of the clinical records. Immunocytochemical studies were done using kappa and lambda chains for B-cell markers and alpha-1-antichymotrypsin for histiocytic markers. The results obtained were as follows : 1) Among 300cases of non-Hodgkin's lymphoma, the primarily involved tumor sites were the lymph nodes (141cases), the gastrointestinal tract (67 cases), and the tonsils (32 cases) in descending order of frequency. 2) Using the Working Formulation, intermediate-grade lymphomas occurred in 66.4% of the patients, and the most comnon subtype was "diffuse, large cell" (32.7%) .By the Rappaport classification, 3 patients had nodular lymphomas, and "diffuse, hitstiocytic" was the most common subtype. 3) Infarction was present in 32 cased in which the "diffuse, large cell" type was most frequently associated. 4) In a review of 7 cases of angioimmunoblastic lymphadenopathy with dysproteinemia, immnunoperoxidase stains were done, and proliferatinsg immunoblasts revealed a polyclonal positivity for kappa or lambda chains. 26 cases of polymorphic reticulosis were reiewed, and atypical reticulocytes revealed a negativity for kappa, lambda and alpha-1-antichymotrypsin with immunoperoxidase stain.restrictio

(The) effect of ethalol on the experimental hepatocarcinogenesis induced by 3'-methyl-4-dimethylamino-azobenzene

의학과/박사[한글] 화학발암제를 이용한 실험적 간암 발생과정에서는 전암단계로 생각되는 여러 병변들이 관찰되지만 아직까지 이들과 최종적으로 발생하는 암종과의 관계는 분명하지 않다. 3'-Methyl-4-dimethylaminoazobenzene(3'-MeDAB)에 의한 간암발생은 간세포의 활면소포체에 존재하는 cytochrome P^^450 산화효소계에 의해 발생하는 중간 대사산물이 최종 발암물질로 작용하기 때문에 ethanol과 같이 cytochrome P^^450 산화효소계에서 산화 과정을 거치는 약물은 3'-MeDAB대사에 영향을 미쳐 간암 발생과정에 변화를 줄 것으로 여겨진다. 따라서 본 실험에서는 3'-MeDAB을 단독투여하거나 여러가지 방법으로 ethanol과 함께 투여하여 간암 발생과정에 나타나는 전암병변들의 상호 관계와 ethanol이 간암 발생과정에 미치는 영향을 알아 보고자하였다. 실험동물로는 체중 180g 내외의 숫컷 횐쥐 156마리를 사용하였고, 이를 3'-MeDAB 투여기간이나 ethanol 투여방법에 따라 다음과 같은 8군으로 나누었으며, 제Ⅰ군은 정상대조군, 제Ⅱ군은 3'-MeDAB 6주 투여군, 제Ⅲ군은 3'-MeDAB 9주 투여군, 제Ⅳ군은 3'-MeDAB과 소량의 ethanol(무수 alcohol 4g/kg, 월, 수, 금) 병행투여군, 제Ⅴ군은 3'-MeDAB과 다량의 ethanol(무수 alcohol 12g/kg, 월, 화) 병행투여군, 제Ⅵ군은 소량의 ethanol(3주) 전처치후 3'-MeOAB과 ethanol 병행 투여군, 제Ⅶ군은 소량의 ethanol 단독투여군, 제Ⅷ군은 다량의 ethanol 단독투여군으로 하였다. 각군은 3, 6, 9, 12, 15주의 소군으로 나누어 희생하였으며, 적출한 간의 무게를 측정하고 육안적 변화를 관찰하였으며 일반적인 조직학적 검색과 bromodeoxyuridine(BrdU)에 대한 면역조직화학적 검색을 시행하여 다음과 같은 결과를 얻었다. 1. 3'-MeOAB 투여시 간의 무게는 시간이 경과함에 따라 점차 증가하는 반면, ethanol투여시에는 점차로 감소하는 경향이 있었다. 2. 3'-MeDAB 단독 혹은 ethanol과 병행투여시 간표면의 과립, 결절, 종괴형성은 투약중단후에도 계속 증가하였으나 ethanol로 전처치한 경우에는 투약중단 후 결절의 수와 크기가 감소하였다. 3. 3'-MeDAB 투여시 ethanol 병행투여 여부와 상관없이 난원형세포 증식, 선종성 증식(Adenomatous hyperplasia, AH), 섬유성 담관증식(cholangiofibrosis, CF) 및 암종이 발생하였으며, 특히 AH와 CF는 투약중지 후에도 계속 관찰되었다. 그러나 ethanol을 전처치하 고 3'-MeDAB을 투여한 경우에는 투약중단 후 AH와 CF의 발생이 적었으며, 이형성 선종성증식(Atypical adenomatous hyperplasia, AAH)과 암종의 발생도 없었다. 4. 난원형세포는 상피세포 표지자인 cytokeratin의 발현을 보이지 않았으며, 투약초기인 3주에는 위선이나 세포삭의 모양을 취하면서 간소엽내로 자라들어간 반면 AH가 나타난 6주 이후에는 난원형세포가 AH 주변에서만 관찰되었다. 5. CF의 정도와 BrdU 표지지수는 투약중단 후에도 매우 높은 상태로 유지되었으나 난원형세포와 AH의 BrdU 표지지수는 투약중단 후 감소하였다. 6. 3'-MeDAB 단독 혹은 ethanol 병행투여시 소수에서 관찰되었던 AAH는 투약중단 후에 주로 발견되었고, 높은 BrdU 표지지수를 보였다. 이상의 결과를 종합해 보면 난원형세포의 증식과 AH는 가역적인 병변인 반면 CF와 AAH는 비가역적인 전암병변으로 생각되었으며, ethanol로 전처치하는 경우에는 발암제 투여중단 후 CF, AH 및 AAH와 같은 전암병변과 최종적인 간암발생이 억제되는 것을 알 수 있었다. [영문] It has been generally accepted that sequential histological changes develop during the course of hepatocarcinogenesis as the precancerous lesions. However, the exact pathogenic mechanism and the relationship between these precancerous lesions and carcinoma remain controversial. In 3'-methyl-4-dimethylaminoazobenzene(3'-MeDAB ) induced hepatocarcinogenesis, the intermediate metabolites Produced by cytochrome P^^450 oxidase system of hepatocytes are thought to be the ultimate carcinogen, and conceivably such drubs as ethanol which is also oxidized by the same hepatic oxidase system may affect the 3'-MeDAB induced hepatocarcinogenesis. The Present study aimed to elucidate that the cytokinetic nature of the hepatic lesions developed by 3'-MeDAB administration and the effect of ethanol on the course of hepatocarcinogenesis. Sprague-Dawley male rats about 180 g were used for the experiment, and divided into 8 groups according to the duration and doses of 3'-MeDAB and ethanol administered: Ⅰ. normal control, Ⅱ. 3'-MeDAB only for 6 weeks, Ⅲ. 3'-MeDAB only for 9 weeks, Ⅳ. 3'-MeDAB with a small dose(4g/kg) of ethanol, Ⅴ. 3'-MeDAB with a large dose(12g/kg) of ethanol, Ⅵ. ethanol pretreatment for 3 weeks followed by 3'-MeDAB plus ethanol for 9 weeks, Ⅶ. ethanol only for 9 weeks in a small dose, Ⅷ. ethanol only for 9 weeks in a large dose. Animals were sacrificed at 3, 6, 9, 12 and 15weeks. The liver weight, the gross and microscopical chanties of the liver were compared between the experimental groups. Cell kinetics of the various hepatic lesions developed in the hepatocarcinogenesis was examined by applying the immunohistochemical method for bromodeoxyuridine(BrdU). The results are as follows: 1. Liver weights tended to increase gradually in the experimental groups treated with 3'-MeDAB with or without ethanol, whereas it decreased in the animals given ethanol alone. 2. The number and size of hepatic granules, nodules and masses continued to increase in the experimental groups given 3'-MeDAB with or without simultaneous ethanol administration, but in the ethanol pretreated animals, nodules or masses larger than 5mm were not found after cessation of 3'-MeDAB. 3. The administration of 3'-MeDAB gave rise to the development of oval cell proliferation, adenomatous hyperplasia(AH), cholangiofibrosis(CF), atypical adenornatous hyperplasia (AAH) and carcinoma. On the other hand, the prior administration of ethanol caused a decreased development of AH, CF, AAH and carcinoma. 4. The oval cells appealed at portal areas as early as 3 weeks when they grow into the hepatic lobule in a pseudoglandular or cord Pattern, but after 6 weeks they were seen extending along the margin of hepatic lobule or AH. The oval cells showed negative staining for both cytokeratin AE1 and AE3. 5. The BrdU labeling index of oval cell and AH were decreased following cessation of 3'-MeDAB administration, but that of CF continued to increase even after the cessation. 6. Most AAH developed after cessation of 3'-MeDAB administration, and its BrdU labeling index was higher than that of AH. In conclusion, the octal cell proliferation and AH appear to be reversible lesions in contrast to the CF and AAH that are thought to gain access to the autonomously proliferative activity, and it is thought that the prior administration of ethanol inhibits the development of AH, CF, AAH and carcinoma following cessation of 3'-MeDAB.restrictio

The Epistemological Structure of Angst : Heideggers Concept of Angst and the Buddhist Vijñapti-matra Concept of Four Aspects of Theory of Cognition

이 글은 불안의 인식 구조를 하이데거의 불안(Angst) 개념과 유식의 사분설(四分說)을 통해 정립하고자 한다. 불안은 무화작용(das Nichten)으로 존재를 드러내는 현상적 토대이다. 이러한 근본기분은 세계 곁에서 머무르던 존재자를 세계 내에서 존재할 수 있도록 밝혀준다. 유식은 대상을 객관적으로 인식할 수 있는가란 물음 하에 식의 전체적인 구조를 드러낸다. 그것은 집착과 고통을 어떻게 바라보아야 하는가에 대한 유식 불교의 반성적 대답이다. 이러한 두 이론은, 존재론적 측면에서 전체 현상을 밝혀내기 위한 구조적인 분석이다. 본론에서는 단계, 과정, 토대로 나누어 불안의 인식 구조를 살펴본다. 첫째, 불안을 지각하는 단계로 유식의 사분설 인식작용을 도입해본다. 도출된 결과는 다음과 같다. 둘째, 불안한 기분을 경험하는 비본래적인 존재자의 양상을 살펴보고 무화작용에서 드러나는 일상의 무의미성과 망상을 알아본다. 이러한 무의미와 망상을 통해 불안의 인식 구조 안에서 발생하는 비본래적인 과정을 밝혀본다. 셋째, 인식 방향을 바꾸어 대상을 지각하는 인식 작용이 아닌 심층의식에서 불안을 형성해가는 작용을 살펴본다.This paper attempts to define and establish the cognitive structure of angst through Heideggers concept of angst and the Buddhist Vijñapti-matra concept of Four Aspects of Theory of Cognition. Angst is a fundamental phenomenon that arises from nihilism (das Nichten). This fundamental feeling reveals to the being that he exists not only next to but also within the world in which he lives. Buddhism manifests the overall structure through raising questions such as Is the subject perceived objectively? This is the Buddhist Vijñapti-matras reflection on its perception of obsession and pain. In an ontological aspect, these two theories are the structural analysis that attempt to reveal the overall phenomenon. First of all, the Buddhist Vijñapti-matra concept of Four Aspects of Theory of Cognition is applied as the initial stage in which angst is perceived by the being. The conclusion drawn is as follows: the feeling of angst (mental phenomena) à perceiving oneself to be anxious (discriminating such phenomena)à questioning why one is feeling angst (the power that discriminates) à perceiving oneself that is questioning the why (the proof or assurance of that power). Second, the paper examines the condition of the inauthentic being that experiences angst and investigates the meaninglessness of the routines of everyday life that emerges from nihilism. Through such exploration of futile delusions, the paper strives to reveal the inauthentic process that occurs within the epistemological structure of angst. Third, the paper examines the effect on the development of angst when there is a change in the method of perception from cognition to in-depth awareness
