    Chronic sclerosing dacryoadenitis - Report of 2 cases

    Chronic sclerosing dacryoadenitis is a rare and under-recognized chronic inflammatory disease of the lacrimal gland. We describe 2 patients with a localized type of chronic sclerosing dacryoadenitis. Both patients presented with a slowly growing painless mass of the eyelid mimicking a tumorous lesion. The morphologic findings of the masses excised under the clinical diagnosis of lymphoma closely recapitulate those of chronic sclerosing sialadenitis (Kuttner tumor). Immunohistochemical staining demonstrated an increased population of IgG4-positive plasma cells confirming that this disease also belongs to the spectrum of a recently described IgG4-related sclerosing diseaseope

    Expression of Glycolysis-Related Proteins in Solid Papillary Carcinoma of the Breast According to Basement Membrane Status

    PURPOSE: The aim of this study was to investigate the differences of expression in glycolysis-related proteins such as Glut-1, carbonic anhydrase (CA) IX, and monocarboxylate transporter (MCT) 4 according to the myoepithelial cell (MEC) and basement membrane (BM) status in solid papillary carcinoma (SPC) of the breast. MATERIALS AND METHODS: Immunohistochemical evaluation of Glut-1, CAIX, and MCT4, as well as p63 and type IV collagen, were performed on 23 SPC cases. RESULTS: Six and nine cases of SPC showed the presence and absence of myoepithelial cells, respectively, and eight cases belonged to the borderline status (p63-positive MEC on some areas of the outer tumor surface but not in others). BM was partially or completely absent in 14 cases and present in nine cases. SPC lacking BM more frequently showed high expression of CAIX than SPC with BM (p=0.037). CONCLUSION: In SPC of the breast, a strong expression of CAIX seems to be associated with an increasing degree of loss of BM, which can be interpreted as BM degradation due to the induction of extracellular acidity with increasing expression of CAIX.ope

    Vimentin and Survivin Expression Rates as PrognosticFactors in Medulloblastoma

    Background : A medulloblastoma is a primitive neuroepithelial tumor of the cerebellum that occurs in children and metastasizes through the cerebrospinal fluid. It is highly malignant and invasive, and the 5-year survival rate is only 60%. Surgical resection techniques, radiation, and chemotherapy have improved the overall survival but the patients suffer life-long cognitive dysfunctions or endocrine abnormalities as the side effects of treatment. Therefore it is essential to identify prognostic markers to determine the appropriate treatment strategy in order to minimize the side effects. Methods : This study evaluated the immunohistochemical differentiation and survival rate with synaptophysin, glial fibrillary acidic protein, epithelial membrane antigen, vimentin and primitive neuroepithelial marker nestin of 55 paraffin-embedded medulloblastomas, using a tissue microarray. The expression of survivin, the apoptotic inhibitor, and the survival rate with regard to the proliferation index of Ki-67 were also investigated. Results : The group testing positive to vimentin, a mesenchymal differentiation marker, had a worse prognosis and there was a strong correlation between vimentin expression and nestin expression. Patients with a survivin expression rate >35% had a significantly poorer clinical course and there was a correlation between the survivin expression rate and Ki-67 expression rate. Conclusion : In conclusion, vimentin and survivin are negative prognostic markers in medulloblastomas.ope

    A case of lung metastasis from unknown primary hepatocellular carcinoma

    Hepatocellular carcinoma (HCC) is a common malignancy and hepatitis B infection is known to be its most common etiologic factor. The most common metastatic site of extrahepatic spread is the lung. We report here on a case of a patient who presented with pulmonary metastasis as the first clinical manifestation of HCC. A 62-year-old-male patient showed an elevated ฮฑ-fetoprotein level during follow up for chronic hepatitis B and liver cirrhosis. The existence of HCC was suspected and an empirical imaging diagnosis was performed. However, repeated computer tomography (CT), magnetic resonance imaging and hepatic angiography failed to detect any HCC. After 12 months, chest CT and PET (positron emission tomography) revealed lung nodules. Histologically, the resected lesion was confirmed as metastatic tumor from HCC. The establishment of the diagnosis of metastatic HCC can occasionally be problematic, particularly when the primary tumor has not been identified.     The risk of developing renal malignancy is increasing in line with increased survival duration of patients with autosomal dominant polycystic kidney disease (ADPKD) on dialysis. However, through previous literature the incidence of renal malignancy in patients with ADPKD is debatable. We experimented a case of renal cell carcinoma in a patient with ADPKD on dialysis whose preoperative radiologic evaluations were not consistent with renal malignancy. We reviewed the association between ADPKD and development of renal malignancies and discussed its management.ope

    A Case of Docetaxel Induced Subacute Cutaneous Lupus Erythematosus

    Drug-induced subacute cutaneous lupus erythematosus (SCLE) is associated with use of the following classes of medications: anti-hypertensives, anti-cholesterolemia, anti-psychotics, and anti-inflammatory drugs. Docetaxel is an anti-neoplastic agent, which is widely used for treatment of non-small cell lung cancer. Few cases of docetaxel-induced SCLE have been reported in the medical literature. Here, we report the case of a 58-year-old female patient who developed drug-induced SCLE after administration of docetaxel. After 4 cycles of chemotherapy with docetaxel and cisplatin, erythematous skin eruptions developed on the patient's face. Skin biopsies of the eruptions were remarkable for interfacing dermatitis with basement membrane thickening. Immunofluorescent study revealed characteristic features of SCLE, including granular deposition of IgM, C3, and apoptotic bodies along the basement membrane. The skin eruptions resolved gradually after cessation of drug and with the use of topical corticosteroids.ope

    Retroperitoneal Extraskeletal Osteosarcoma Misconstrued as a Primary Renal Tumor

    Extraskeletal osteosarcoma is a rare malignant tumor of the soft tissue. In addition, there are only several reported cases of osteosarcoma of the retroperitoneum as a primary site. We report here on a case of extraskeletal osteosarcoma arising in the retroperitoneum. An 80-year-old man visited our hospital because of a known renal mass. Abdominal computed tomography showed a 10cm sized huge renal mass. We then performed radical nephrectomy. The mass revealed a solid and ivory-colored cut surface with multifocally ovoid cystic spaces and hemorrhage. Microscopically, the tumor consisted of polygonal stromal cells, giant cells and varying amounts of neoplastic new bone; mitotic figures were frequently observed. There was no distant metastasis or regional lymph node involvement.ope

    Synchronous Gastric Signet Ring Cell Carcinoma and Mucosa-associated Lymphoid Tissue Lymphoma in a Patient with Helicobacter pylori Infection: A Case Report

    Helicobacter pylori (H. pylori) plays a substantial role in the pathogenesis of primary gastric carcinoma and low grade gastric mucosa-associated lymphoid tissue (MALT) B-cell lymphoma. The synchronous occurrence of both lesions have been reported very rarely. Here we present a case of gastric signet ring cell carcinoma and MALT lymphoma developed simultaneously with H. pylori infection. A 46-year-old man was referred for the evaluation of abnormal endsoscopic findings. Upper endoscopy revealed early gastric cancer in the lower body of the stomach and subsequent biopsy of the lesion proved to be a signet-ring cell carcinoma. After confirmation of signet-ring cell carcinoma, subtotal gastrectomy was performed. Histopathological study of the resected specimens showed collision tumor, which was composed of gastric signet ring cell carcinoma and low grade B-cell MALT lymphoma. H. pylori was detected in the resected specimen and the standard eradication therapy was instituted. We suspect H. pylori infection can be a etiologic factor in the development of both MALT lymphoma and carcinoma of the stomach in this patient.     A rapidly progressing Lung abscess diagnosed as pulmonary mucormycosis associated with Wegener`s granulomatosis

    Pulmonary mucormycosis is an uncommon opportunistic fungal infection associated with diabetes mellitus, leukemia, lymphoma and other debilitating diseases. The diagnosis is made by the pathology demonstration of typical hyphae within the lung tissue, and requires aggressive treatment including surgical and medical methods for reducing the overall mortality. In Korea, the association of pulmonary murcomycosis with Wegener's granulomatosis is rare. However, pulmonary mucormycosis with diabetes has been reported in several cases. Recently, we treated a case of pulmonary murcomycosis in 42-year-old woman with Wegener's granulomatosis, who had recurrent pneumothorax and a rapidly progressive lung abscess. Here we report this case and review the medical literature.ope
