Effect of Information on mood state and support behavior of mothers in emergency room for their children

지역사회간호학과/석사[한글] 응급실을 내원하는 아동과 어머니는 낯선 응급상황에서 설명되지 않은 검사와 침습적 처치에 대해 많은 스트레스를 경험하게 되고 준비 없는 상황에서 불확실한 아동의 상태에 대해 어떻게 대처하고, 아동을 어떤 방법으로 도와주어야 하는지 대해 고통과 불안을 느끼게 된다. 그러므로 아동의 어머니들에게 응급실 상황과 관련한 정보와 대처방안을 제공하여 아동에 대한 부모 역할을 확립하고 아동의 돌봄을 지지하고 증진시킬 수 있는 간호중재가 필요하다.이에 본 연구에서는 아동이 응급실에서 공통적으로 경험해야 하는 검사와 치료과정에 대한 구체적이고 객관화된 간호정보를 개발하고, 어머니의 기분상태와 아동 지지행위에 미치는 효과를 평가하고자 본 연구를 시행하였다. 연구 설계는 비동등성 대조군 사전 사후 설계에 의한 유사 실험 연구로 연구 대상은 E 의료원 응급실을 내원한 유아 및 학령전기 아동 어머니를 대상으로, 그 중 대상자 선정 기준에 맞는 대조군 30명, 실험군 30명으로 하였다. 자료수집기간은 2005년 10월 21일부터 11월 6일까지 17일간 이었으며, 자료수집방법은 실험군과 대조군 각각 30명에게 설문조사 및 관찰을 통하여 수행하였다. 독립변인은 연구자가 제작 한 간호정보 팜플렛이며, 종속변인은 어머니의 기분상태와 아동에 대한 어머니 지지행위이다.연구도구로 어머니의 아동 지지행위는 Melnyk(1994)에 의해 개발하고 김선구 등(1999)이 한국 부모의 정서에 맞게 수정 보완하여 사용한 것을, 기분상태척도는 McNair, Lorr 와 Droppleman (1992)이 개발한 기분상태 측정 도구(Profile of Mood States, POMS)를 신윤희(1996)가 한국 문화에 맞도록 수정하여 타당화시킨 도구를 어머니의 정서에 맞게 연구자와 아동간호학 교수 1인이 합의하에 수정․보완하여 사용하였으며 동일한 도구로 대조군도 조사하였다. 자료분석은 수집된 자료는 SPSS/PC+WIN을 이용하였고, 일반적 특성과 동질성 검증은 χ²-test와 t-test, Fisher''s exact test를, 어머니의 기분상태와 지지행위의 변화는 paired t-test로, 평균비교는 t-test로, 관련성 비교는 Fisher''s exact test로, 영역별 비교는 ANOVA test로 분석하였다.연구결과는 다음과 같다.첫째, “정보제공을 받은 어머니의 기분상태는 정보제공을 받기 전보다 정보제공을 받은 후에 좋아질 것이다”라는 제 1 가설은 통계적으로 유의하여 지지되었다(t=3.543, P=0.001).둘째, “정보제공을 받은 어머니의 지지행위는 정보제공을 받기 전보다 후에 좋아질 것이다”라는 제 2 가설은 통계적으로 유의하여 지지되었다 (t= -10.934, p=0.000).이상의 연구결과를 종합해 볼 때, 응급실에서의 간호정보제공은 어머니의 기분상태와 아동 지지행위에 긍정적인 효과가 있었다. 따라서 응급실에서 어머니에 대한 간호정보제공은 응급실 간호에 대한 구체적인 지침이 될 수 있을 뿐 아니라 타 질환자의 응급실 방문 시 교육자료 개발의 초석이 될 수 있으리라 사료된다. [영문]The purpose of this study was to evaluate the effect of information on the mothers'' mood and support behaviors when invasive procedure for children who visit emergency room. Research design was non-equivalent Quasi-experiment. The dependent variable was information giving and the dependent variable was mood state and support behavior of the mothers.The study subjects were mothers whose child were toddlers and preschoolers in emergency room. Total subjects were 30 mothers in the treatment group and 30 mothers in the control group.The data collection was carried out at the emergency room, E university hospital from Oct. 21, 2005 to Nov. 6, 2005 for 17 years.The instruments used in this study were; The Profile of Mood States(POMS) of McNair, Lorr and Droppleman(1992) and Index of mother''s support Behavior during invasive procedure was developed by Melnyk(1994).The data analysis was carried out using the χ²-test and t-test and Fisher''s exact test for evaluating homogeneity of the subject, the paired t-test for evaluating mothers'' mood status and changes of support behaviors, the t-test for evaluating average comparison and component comparison, the Fisher''s exact test for analysis of relevance.The results from this study were following.The hypothesis I. "Mothers'' mood status be better after they received the information was supported and it was statistically significant (t=3.543, P=0.001).The hypothesis II. "Mothers'' support behavior who received the information be better after they received the information" was supported and it was statistically significant(t= -10.934, p=0.000).In conclusion, the nursing information to the mothers for their children in emergency room had positive effect on the mother''s mood status and support behavior for the children. Based on the result of this study recommend that nursing information should be included in the routine nursing care activities in emergency room.ope
