Can Voluntary Health Insurance for Non-reimbursed Expensive New Treatments Be Just?
Public healthcare systems are increasingly refusing (temporarily) to reimburse newly approved medical treatments of insufficient or uncertain cost-effectiveness. As both patient demand for these treatments and their list prices increase, a market might arise for voluntary additional health insurance (VHI) that covers effective but (very) expensive medical treatments. In this paper, we evaluate such potential future practices of VHI in public healthcare systems from a justice perspective. We find that direct (telic) egalitarian objections to unequal access to expensive treatments based on different ability to afford VHI do not stand up to scrutiny. However, such unequal access might lead to loss of self-respect among individuals, or loss of fraternity within society, rendering it more difficult for citizens to interact on equal moral footing. This would be problematic from a relational egalitarian perspective. Moreover, the introduction of VHI might turn out to have negative consequences for the comprehensiveness and/or the quality of the public healthcare services that are offered to all patients equally through basic health insurance. These consequences must be weighed against potential health gains and the value of liberty. We conclude that governments should be careful when considering the introduction of VHI in public healthcare systems.</p
Immigrant Parents' experiences of co-operation with the day care centers in Helsinki
Tavoitteet. Tutkimuksen tarkoitus oli tutkia maahanmuuttajavanhempien kokemuksia yhteistyöstä
helsinkiläisten päiväkotien kanssa. Tästä aihealueesta on Suomessa tehty hyvin vähän tutkimusta,
joten asettamani tutkimusongelmat kohdistuvat melko laajalle aihealueelle. Taustateorian luvuissa on
käsitelty maahanmuuttajaperheiden osuutta suomalaisissa päiväkodeissa, kuinka varhaiskasvatuslaki
ja valtakunnallinen varhaiskasvatussuunnitelma ohjeistavat ja velvoittavat vanhempien kanssa
tehtävään yhteistyöhön, sekä miksi pitäisi tavoitella avointa ja luottamuksellista yhteistyötä
maahanmuuttajien kanssa. Tämän lisäksi olen esitellyt sosiaalipsykologian teorioita, kuten kategoriset
skeemat ja stereotypiat, ennakkoluuloiset asenteet, ryhmien väliset suhteet, sosiaalinen identiteetti
sekä etninen identiteetti.
Menetelmät. Tämän laadullisen tutkimuksen aineisto koostuu viidestä teemahaastattelusta.
Haastattelin viittä vanhempaa, kahta isää ja kolmea äitiä. Heistä neljä olivat asuneet Suomessa alle
viisi vuotta. Tavoitin heidät kolmen eri päiväkodin kautta. Heillä oli kokemuksia yhteensä yhdeksästä
eri helsinkiläisestä päiväkodista. Aineisto on analysoitu teema–analyysin metodilla. Analysoin
aineistoni kertaalleen aineistolähtöisesti, jotta pystyin vastaamaan ensimmäiseen
tutkimuskysymykseen, ja toisen kerran analysoin aineistoni teorialähtöisesti, jotta pystyin vastaamaan
toiseen tutkimuskysymykseen. Teorialähtöisen analyysin lähtökohtana olivat sosiaalipsykologian
teoriat.
Tulokset ja johtopäätökset. Tässä kuvataan päätulokset, johtopäätökset ja tulosten käytännön
sovellusmahdollisuudet. Tulokset kirjoitetaan imperfektissä (esim. "Temperamentiltaan pelokkailla oli
turvattomampi kiintymyssuhde kuin muilla lapsilla") ja johtopäätökset preesensissä (esim. ”Samalla
kehitystasolla olevat toverit vaikuttavat lasten välittömään hyvinvointiin ja sosiaaliseen kehittymiseen”).
Tulokset ja johtopäätökset. Vanhempien kokemusten mukaan päiväkodeissa ei oltu erityisemmin otettu
huomioon heidän erilaisia tarpeita yhteistyössä kasvattajien kanssa. Kuitenkin vuosittain pidettäviin
varhaiskasvatuskeskusteluihin ja aloituskeskusteluihin oli tilattu tulkki. Silloin kun vanhemmat eivät
pystyneet vielä puhumaan suomea, heillä ei käytännössä ollut mahdollisuutta keskustella lapsensa
kuulumisista päivittäisissä lapsen hakutilanteissa. Vaikuttaa siltä, että suomen kielen osaamisesta on
tullut ehto, joka mahdollistaa ja rohkaisee vanhempien eriarvoistamista ja eristämistä. Suomen kielen
osaaminen on itsessään tekninen ongelma, mutta sitä käytetään verukkeena syrjinnälle. Tämän
varhaiskasvatuksen asiakasryhmän tarpeiden kohtaaminen on hyvin ajankohtainen ongelma. Käsitän,
että tarvittavien järjestelyjen ja uusien yhteistyörakenteiden aikaansaaminen on varhaiskasvatusta
tarjoavan tahon vastuulla.The purpose. The purpose of the reseach was to study the experiences of immigrant parents about cooperation
with kindergartens in Helsinki. Very little research has been done on this topic in Finland, so
my research questions are focused on a fairly wide research field. The background literature focuses on
the prevalence of immigrant families in Finnish kindergartens, how the Early Childhood Education Act
and the national Early Childhood Education Plan guide and enforce co-operation with parents, and why
kindergartens should seek open and confidential co-operation with immigrants. In addition, I have
presented the social psychology theories, such as the cognitive categorical schemas, prejudiced
attitudes, stereotypes, intergroup relations, social identity and ethnic identity.
Methods. The data corpus of this qualitative research consists of five thematic interviews. I interviewed
five parents, two fathers and three mothers. Four of them had lived in Finland for less than five years. I
found them from three different kindergartens. They had an experience in total of nine different
kindergartens in Helsinki. The data corpus has been analyzed using the thematic-analysis method. I
analyzed data corpus first time with a data-based orientation, to answer the first research question;
second time I analyzed my data corpus with a theory-based orientation, to answer the second research
question. Theory-based analysis was premised by the social psychology theories, which were presented
in the third chapter.
Results and conclusions. According to the parents' experiences, kindergartens had not taken into account
their different needs in collaboration with them. However, an interpreter was ordered for annual discussion
sessions and start-up discussions. The time while parents were not able to speak in Finnish, they
practically did not have the opportunity to communicate with the kindergartens' educators on daily bases.
It seems that being able to communicate in Finnish language has become a condition that allows and
encourages the inequalities and isolation of immigrant parents. Finnish speaking skills should be a
technical problem in itself, but it is used as a pretext for exclusion. Meeting the needs of this early age
education customer group is a very timely problem. I think that the necessity for new arrangements and
new cooperation structures are the responsibilities of the party who provides the services
Employers have a Duty of Beneficence to Design for Meaningful Work:A General Argument and Logistics Warehouses as a Case Study
Artificial intelligence-driven technology increasingly shapes work practices and, accordingly, employees’ opportunities for meaningful work (MW). In our paper, we identify five dimensions of MW: pursuing a purpose, social relationships, exercising skills and self-development, autonomy, self-esteem and recognition. Because MW is an important good, lacking opportunities for MW is a serious disadvantage. Therefore, we need to know to what extent employers have a duty to provide this good to their employees. We hold that employers have a duty of beneficence to design for opportunities for MW when implementing AI-technology in the workplace. We argue that this duty of beneficence is supported by the three major ethical theories, namely, Kantian ethics, consequentialism, and virtue ethics. We defend this duty against two objections, including the view that it is incompatible with the shareholder theory of the firm. We then employ the five dimensions of MW as our analytical lens to investigate how AI-based technological innovation in logistic warehouses has an impact, both positively and negatively, on MW, and illustrate that design for MW is feasible. We further support this practical feasibility with the help of insights from organizational psychology. We end by discussing how AI-based technology has an impact both on meaningful work (often seen as an aspirational goal) and decent work (generally seen as a matter of justice). Accordingly, ethical reflection on meaningful and decent work should become more integrated to do justice to how AI-technology inevitably shapes both simultaneously.</p
