Syncope in Elderly People: A Threatening Presentation of Pulmonary Embolism: A Case Report
age. Its prognosis is largely unpredictable, pending the origin of the sudden loss of
consciousness. We report a case of an old woman affected by severe chronic heart
failure, who died soon after the development of an episode of syncope, which was
eventually attributed to pulmonary embolism. Anticoagulant therapy, promptly
instituted, was ineffective. In the differential diagnoses of syncope, pulmonary
embolism should always be considered, especially in old patients with risk factors
for venous thromboembolism such as a severe heart failure. In patients with high
risk of death according to the widely adopted risk stratifications score, aggressive
therapy may be considered also in elderly people to prevent unfavourable
outcomes
Bone computed tomography mineral content evaluation in chickens: Effects of substances in homeopathic concentration
Ninety-six cobb race chickens were equally divided in 4 groups and randomly
assigned to receive a standard treatment feed + homeopathic concentrations of Symphytum
(S.) officinalis (9CH), or standard treatment feed + homeopathic concentrations
of Tricalcarea (4CH), or standard treatment feed + homeopathic concentrations of Calcarea
(C.) carbonica (30CH) or a placebo (the same feed but without any homeopathic
compound) in order to assess the ability of the homeopathic compounds to increase
the concentration of calcium hydroxyapatite in the sternal spongy bone tissue. We
measured the concentration of calciumhydroxyapatite in the sternal spongy bone tissue
of all chickens by means of a computed tomography (CT).
Results: 30%, 36% and 63% increase of sternum spongy-bone mineralization was
observed after a 2 years period in the treatment groups with S. officinalis (9CH), Tricalcarea
(4CH) (*P < 0.05) and C. carbonica (30CH) (***P < 0.001) respectively.
Conclusion: Bone mineralization is usually low in battery chickens reared in commercial
poultry-sheds, creating a weakness of the whole animal supporting apparatus. Homeopathic

CT-based tumour response criteria compared after combined treatment for liver metastases of colorectal cancer
open6noPurpose: The aim of this analysis is to compare different tumour response
criteria (TRC) after chemotherapy combined with bevacizumab in liver
metastases from colorectal cancer (mCRC) to ascertain the best early
prognostic indicator of response.
Methods and Materials: 103 target liver metastases from 65 mCRC patients
treated with chemoterapy plus bevacizumab were examined at the Istituto
Oncologico Veneto IOV-IRCSS (March 2008-January 2013). All patients had
baseline CT and at least one follow-up scan. Tumour response was
retrospectively analyzed by two radiologists using RECIST1.1, modified Choi,
and Chun morphologic criteria. Tumour response, classified as good (complete
or partial response) or poor (stable or progressive disease), was compared
with progression-free survival (PFS) at first follow-up (t1) and time of best
response. Interobserver agreement and concordance between TRC were
measured.
Results: At t1, 32.31% showed a good response according to RECIST1.1
(median PFS 11.1), 84.62% according to Choi (median PFS 10.8). These
percentages rose to 49.23% (median PFS 12.1) and 87.69% (median PFS
10.8), respectively, at the time of best response. According to Chun, 67.69%
showed a good response at the time of best response (median PFS 10.8). The
Choi criteria detected a higher proportion of good responders at t1, showing a
better correlation with PFS; all methods correlated with PFS at the time of best
response.
Conclusion: The Choi criteria proved more consistent in the early detection of
response in mCRC treated with chemotherapy plus bevacizumab,
underscoring the importance of using these criteria in the early assessment of

Conflicting or complementary role of computed tomography (CT) and positron emission tomography (PET)/CT in the assessment of thymic cancer and thymoma: Our experience and literature review
Background: To evaluate the role of computed tomography (CT) and positron
emission tomography (PET)/CT in patients with thymic cancer and thymoma at
initial staging.
Methods: We retrospectively reviewed CT and PET/CT scans of 26 patients with a
thymic cancer (n = 9) or thymoma (n = 17). Chest CT findings documented were
qualitative and quantitative. Both qualitative and semiquantitative data were recovered
by PET/CT. The comparisons among histological entities, outcome, and qualitative
data from CT and PET/CT were made by non-parametric analysis.
Results: PET/CT resulted positive in 15/17 patients with thymoma. CT was available
in 5/9 (56%) patients with thymic cancer and in 3/17 with thymoma. All quantitative
CT parameters were significantly higher in patients with thymic cancer than
thymoma (maximum axial diameter: 45 vs. 20 mm, maximum longitudinal diameter:
69 vs. 21 mm and volume: 77.91 vs. 4.52 mL; all P < 0.05). Conversely, only
metabolic tumor volume (MTV) and total lesion glycolysis were significantly different
in patients with thymic cancer than thymoma (126.53 vs. 6.03 cm3 and 246.05
vs. 20.32, respectively; both P < 0.05). After a median follow-up time of 17.45
months, four recurrences of disease occurred: three in patients with thymic cancer
and one with a type B2 thymoma. CT volume in patients with recurrent disease was
102.19 mL versus a median value of 62.5 mL in six disease-free patients. MTV was
higher in the recurrent than disease-free patient subset (143.3 vs. 81.13 cm3),
although not statistically significant (P = 0.075).
Conclusion: Our preliminary results demonstrated that both morphological and
metabolic volume could be useful from a diagnostic and prognostic point of view in
thymic cancer and thymoma patients. A large multi-center clinical trial experience
for confirming the findings of this study seems mandatory
Metformin: a modulator of bevacizumab activity in cancer? A case report.
Recurrent type I endometrial cancer ((EC)) has poor prognosis and demands novel therapeutic approaches. Bevacizumab, a VEGF-A neutralizing monoclonal antibody, has shown clinical activity in this setting. To our knowledge, however, although some diabetic cancer patients treated with bevacizumab may also take metformin, whether metformin modulates response to anti-VEGF therapy has not yet been investigated. Here, we report the case of a patient with advanced (EC) treated, among other drugs, with bevacizumab in combination with metformin. The patient affected by relapsed (EC) G3 type 1, presented in march 2010 with liver, lungs and mediastinic metastases. After six cycles of paclitaxel and cisplatin she underwent partial response. Later on, she had disease progression notwithstanding administration of multiple lines of chemotherapy. In march 2013, due to brain metastases with coma, she began steroid therapy with development of secondary diabetes. At this time, administration of Bevacizumab plus Metformin improved her performance status. CT scans performed in this time window showed reduced radiologic density of the lung and mediastinic lesions and of liver disease, suggestive of increased tumor necrosis. Strong F-18-FDG uptake by PET imaging along with high levels of monocarboxylate transporter 4 and lack of liver kinase B1 expression in liver metastasis, highlighted metabolic features previously associated with response to anti-VEGF therapy and phenformin in preclinical models. However, clinical benefit was transitory and was followed by rapid and fatal disease progression. These findingsalbeit limited to a single casesuggest that tumors lacking LKB1 expression and/or endowed with an highly glycolytic phenotype might develop large necrotic areas following combined treatment with metformin plus bevacizumab. As metformin is widely used among diabetes patients as well as in ongoing clinical trials in cancer patients, these results deserve further clinical investigation
Autonomic dysfunction in Hodgkin and non-Hodgkin lymphoma. A paraneoplastic syndrome?
We wanted to determine whether autonomic dysfunction in patients with lymphoma is related to chemotherapy or represent a paraneoplastic syndrome. 40 patients with current or cured Hodgkin or non-Hodgkin lymphoma and 40 healthy controls, matched for age, gender, hypertension and diabetes mellitus underwent autonomic evaluation (Deep Breath, Valsalva Maneuver, Hand Grip, Lying to Standing, Tilt Test). Current patients also suffering from diabetes or hypertension, or still on chemotherapy revealed autonomic changes, while cured or healthy subjects did not. Autonomic dysfunction in lymphoma is a transient manifestation of a paraneoplastic syndrome
Ruolo della colon-TC nel paziente anziano con colonscopia controindicata o incompleta
Scopo:
Valutare accuratezza, efficacia diagnostica e sicurezza della colon-TC (CTC) in pazienti anziani con sospetta patologia non
candidabili alla colonscopia o nei quali quest\u2019ultima fosse risultata incompleta e non dirimente per la diagnosi.
Materiali e metodi:
Sono state eseguite 83 CTC in pazienti con et\ue0 >= 75 anni (media 86 \ub1 5,52; range 75-97) per completamento diagnostico
in caso di colonscopia controindicata o incompleta, come da linee guida ESGE/ESGAR 2015. Gli esami si sono svolti previa
preparazione intestinale, fecaltagging, insufflazione rettale di CO2 e iniezione endovenosa di spasmolitico, salvo
controindicazioni. Sono state effettuate scansioni dirette a paziente prono e supino, ricostruzioni multiplanari e navigazione
3D. Ogni studio \ue8 stato esaminato da 2 radiologi.
Risultati:
La preparazione intestinale \ue8 stata valutata soddisfacente nel 80,7% dei casi. La quantit\ue0 media di CO2 insufflata \ue8 stata di
3,6 litri. La valutazione dei segmenti colici dopo distensione, compresa la navigazione 3D, \ue8 stata completa nel 91,6% dei
pazienti, con un picco del 96,1% nei 55 pazienti di et\ue0 compresa tra 75 e 85 anni. 62 esami (74,7%) sono risultati positivi: 19
per polipi di dimensioni > 6 mm (5 sessili e 14 peduncolati), 12 per ispessimenti parietali di tipo neoplastico, 45 per
patologia diverticolare; in 14 casi si \ue8 avuta la concomitante presenza di pi\uf9 reperti. Nessuna complicanza procedurale.
Conclusioni:
La CTC, nei pazienti ultrasettantacinquenni con sufficienti preparazione intestinale ed insufflazione rettale di CO2, completa
in gran parte dei casi la diagnosi di una sospetta patologia colica in modo sicuro, relativamente poco invasivo e ben
tollerato, consentendo una pronta prosecuzione delle cure

The aim of this educational poster is to illustrate the role of upper gastrointestinal series
(UGS) in detecting the most common and some more rare complications in patients who
underwent bariatric surgery procedures as
\u2022 laparoscopic adjustable gastric band (LAGB) placement,
\u2022 laparoscopic sleeve gastrectomy (LSG),
\u2022 Roux-en-Y gastric bypass (RYGB).
Our centre (Radiology Unit - Department of Medicine - University Hospital of
Padova, Italy) excels in the follow-up of patients who underwent bariatic surgery
interventions (in the Week Surgery Unit of the same Hospital).
For this reason, our database is rich in images of early and late complications, directly
from our clinical practice.
In this poster we are going to present, describe and comment the findings to the
benefit of all radiologists, not just for those whose field of study is the gastro-intestinal
radiology, adding some useful tips for conducting a good examination with high diagnostic
relevance
Relationship between gastric pouch and GERD after laparoscopic sleeve gastrectomy

Diagnostica e trattamento nelle complicanze della Laparoscopic Sleeve Gastrectomy: la nostra esperienza
Scopo:
L'intervento di laparoscopic sleeve gastrectomy (LSG) rappresenta attualmente uno dei principali interventi di chirurgia
bariatrica proposti in Europa. Le sue complicanze sono infrequenti, ma tra di esse lo sviluppo di raccolte fluide paragastriche,
sia a breve che a medio-lungo termine, risulta un'eventualit\ue0 nota.
La gestione di questi Pazienti coinvolge il Radiologo tanto nella diagnosi, quanto nel follow-up e nell'eventuale trattamento
interventistico. Si presenta l'esperienza degli ultimi 6 anni in un centro di riferimento.
Materiali e metodi:
E' stato realizzato uno studio retrospettivo nell'intervallo temporale da Aprile 2012 al Febbraio 2018. La popolazione di
studio \ue8 costituita da 21 pazienti giunti all'attenzione dell'Istituto di Radiologia dell'Universit\ue0 degli studi di Padova per
l'insorgenza di complicanze in esiti di sleeve gastrectomy, in particolare per l'insorgenza di raccolta paragastrica.
Sono state prese in esame le informazioni cliniche dei pazienti, le indagini radiologiche e le procedure di radiologia
interventistica eseguite.
Tutti i Pazienti hanno eseguito almeno una radiografia del tubo digerente prime vie (RXTDPV), una tomografia
computerizzata (TC) addome con o senza mezzo di contrasto endovenoso e una procedura di radiologia interventistica.
Risultati:
La popolazione comprende 19 femmine e 2 maschi, di et\ue0 media di 44 anni (range 30-61 anni). Il BMI medio dei pazienti \ue8 di
45.6 kg/m^2 (range 66-34 kg/m^2). In 11 pazienti la LSG ha rappresentato il primo intervento addominale, 5 pazienti
avevano eseguito altri interventi di chirurgia bariatrica (4 bendaggi gastrici, 1 plicatura), 5 pazienti chirurgia addominale di
altro tipo.
La RXTDPV del primo giorno post-operatorio, eseguita in tutti i Pazienti, \ue8 risultata negativa in tutti i casi. Le complicanze
sono state diagnosticate in 20 casi con TC ed in 1 caso con RXTDPV, eseguite per peggioramento clinico in media a 22 gg
dall'intervento (intervallo 1-189 gg).
Tra i segni e sintomi presenti all'esordio i pi\uf9 frequenti sono stati la febbre (14 casi, 67%) e il dolore addominale (13 casi,
62%). Altri dati clinici riportati comprendono alterazioni dell'alvo (dal vomito alla mancata canalizzazione), melena,
ematemesi, alterazioni degli indici di flogosi, aumento dell'output di eventuali drenaggi addominali.
Tutti i pazienti hanno presentato una raccolta addominale paragastrica, 10 dei quali con evidenza di fistola. La diagnosi di
fistola \ue8 stata ottenuta in tutti i casi attraverso RXTDPV; dalle indagini TC non sono emersi segni diretti di fistolizzazione.
Ciascun paziente \ue8 stato sottoposto ad almeno una procedura di Radiologia interventistica per un totale di 59 procedure: 44
TC guidate, 14 in scopia e 1 ecografica, con una media di 2.8 procedure per paziente (range 1- 11). Tra le procedure
eseguite, la grande maggioranza (54, pari al 92%) ha riguardato il drenaggio di raccolte o il posizionamento di drenaggi
addominali; le altre l'embolizzazione della fistola mediante colla cianoacrilica ed il drenaggio di raccolte pleuriche.
In 9 pazienti il solo trattamento interventistico ha portato alla risoluzione clinica. I restanti 12 pazienti hanno necessitato di
una terapia combinata chirurgico/endoscopica e radiologica; in 3 di questi ultimi la procedura radiologica \ue8 stato l\u2019ultimo
trattamento. La mortalit\ue0 e le complicanze periprocedurali sono risultate nulle.
Conclusioni:
Lo sviluppo di raccolte paragastriche nel Paziente con esiti di LSG, anche a mesi di distanza dall'intervento, rappresenta un
motivo di accesso al reparto di Radiologia.
In questi Pazienti la presentazione clinica pu\uf2 essere varia; nella nostra esperienza il dolore addominale e la febbre
rappresentano i sintomi pi\uf9 frequenti.
Aneddoticamente la sintomatologia addominale pu\uf2 anche essere misconosciuta per una sospetta embolia polmonare.
Nella popolazione in esame, la RXTDPV in prima giornata si \ue8 rivelato un esame poco utile nella previsione a medio-lungo
termine dello sviluppo di raccolte paragastriche nella LSG. Non solo, ma anche nel caso di complicanze a breve termine, la
sensibilit\ue0 della metodica per aspetti non francamente fistolosi si \ue8 rivelata nulla.
La metodica pi\uf9 indicata per l'inquadramento iniziale di questi pazienti nella nostra esperienza \ue8 rappresentata dalla TC
addome. In presenza di forte sospetto clinico, in alcuni casi pu\uf2 essere poi possibile condensare in un unico momento
radiologico l'atto diagnostico mediante TC e la prima procedura interventistica.
Una volta identificata la raccolta, la RXTDPV rappresenta un utile esame di secondo livello per identificare una eventuale
origine fistolosa della raccolta stessa. Utile sottolineare che essa pu\uf2 non essere evidente ai primi controlli, ma manifestarsi
successivamente nel prosieguo della storia clinica.
In vista di un possibile risparmio di tempo e di dose, un'alternativa a questo approccio multimetodico potrebbe essere
rappresentato dalla sola TC con mdc orale, ma l'eventuale efficacia diagnostica di questa tecnica nella popolazione
bariatrica dovr\ue0 essere validata attraverso opportuni studi clinici.
Da un punto di vista terapeutico le procedure di radiologia interventistica, ed in particolare il drenaggio delle raccolte,
possono offrire discrete soddisfazioni in questi pazienti: nella nostra esperienza in quasi la met\ue0 dei casi esse si sono rivelate
l'unico intervento necessario per la risoluzione del quadro clinico. La mortalit\ue0 pari a zero e l'assenza di complicanze
periprocedurali nel nostro campione sottolineano la sicurezza di questo tipo di approccio nella risoluzione di raccolte
paragastriche in esiti di LSG
