    Postural effects on spontaneous retinal venous pulsations in healthy individuals

Published by John Wiley & Sons Ltd Purpose: To assess amplitudes of spontaneous retinal venous pulsations (SVP) in three various postures (sitting, supine and lateral decubitus) in healthy individuals. Methods: Thirty participants (28 ± 8 years, 25 females) were included in the study. Intraocular pressure (IOP), blood pressure (BP) and SVP's were measured at three different postures using a calibrated Tono-Pen applanation tonometer, a digital sphygmomanometer, and a custom-built handheld video ophthalmoscope, respectively. Retinal venous pulsations (SVP) amplitudes were extracted from the retinal videos using a custom written MATLAB algorithm. Mean arterial pressure (MAP = (systolic + 2diastolic)/3) and mean ocular perfusion pressure (MOPP = (2/3 MAP)-IOP) were also calculated at each posture. A one-way ANOVA was applied to each parameter to determine any significant difference for the various postural changes. Results: Mean IOP increased (p < 0.0001) and mean SVP decreased (p < 0.0001) from sitting to supine. The mean IOP (mmHg) and SVP (MU; measuring units) in sitting, supine and lateral decubitus were 16.2 ± 2, 19.4 ± 4, 19.8 ± 2 mmHg and 5.8 ± 2, 4.5 ± 2, and 4.7 ± 2 MU, respectively. Mean arterial pressure (MAP) and MOPP also decreased significantly from sitting to supine (p < 0.001, p < 0.001) and sitting to lateral decubitus (p < 0.05, p < 0.01). There were no significant differences between IOP, SVP, MAP or MOPP during a postural modification from supine to lateral decubitus. Conclusions: In this study, we showed a significant reduction in SVP amplitudes and a significant increase in IOP from sitting to supine position in a healthy young cohort. This supports the rationale to further study such phenomenon in ocular conditions such as glaucoma to determine whether relative SVP change, for a similar postural change, can reveal early signs of vascular dysfunction

    Méthode de conception simplifiée des amortisseurs pour la réhabilitation des ponts avec isolation sismique de la base

    Abstract: Highway bridges have great values in a country because in case of any natural disaster they may serve as lines to save people’s lives. Being vulnerable under significant seismic loads, different methods can be considered to design resistant highway bridges and rehabilitate the existing ones. In this study, base isolation has been considered as one efficient method in this regards which in some cases reduces significantly the seismic load effects on the structure. By reducing the ductility demand on the structure without a notable increase of strength, the structure is designed to remain elastic under seismic loads. The problem associated with the isolated bridges, especially with elastomeric bearings, can be their excessive displacements under service and seismic loads. This can defy the purpose of using elastomeric bearings for small to medium span typical bridges where expansion joints and clearances may result in significant increase of initial and maintenance cost. Thus, supplementing the structure with dampers with some stiffness can serve as a solution which in turn, however, may increase the structure base shear. The main objective of this thesis is to provide a simplified method for the evaluation of optimal parameters for dampers in isolated bridges. Firstly, performing a parametric study, some directions are given for the use of simple isolation devices such as elastomeric bearings to rehabilitate existing bridges with high importance. Parameters like geometry of the bridge, code provisions and the type of soil on which the structure is constructed have been introduced to a typical two span bridge. It is concluded that the stiffness of the substructure, soil type and special provisions in the code can determine the employment of base isolation for retrofitting of bridges. Secondly, based on the elastic response coefficient of isolated bridges, a simplified design method of dampers for seismically isolated regular highway bridges has been presented in this study. By setting objectives for reduction of displacement and base shear variation, the required stiffness and damping of a hysteretic damper can be determined. By modelling a typical two span bridge, numerical analyses have followed to verify the effectiveness of the method. The method has been used to identify equivalent linear parameters and subsequently, nonlinear parameters of hysteretic damper for various designated scenarios of displacement and base shear requirements. Comparison of the results of the nonlinear numerical model without damper and with damper has shown that the method is sufficiently accurate. Finally, an innovative and simple hysteretic steel damper was designed. Five specimens were fabricated from two steel grades and were tested accompanying a real scale elastomeric isolator in the structural laboratory of the Université de Sherbrooke. The test procedure was to characterize the specimens by cyclic displacement controlled tests and subsequently to test them by real-time dynamic substructuring (RTDS) method. The test results were then used to establish a numerical model of the system which went through nonlinear time history analyses under several earthquakes. The outcome of the experimental and numerical showed an acceptable conformity with the simplified method.Résumé: Les ponts routiers ont une grande valeur dans un pays parce qu’en cas de catastrophe naturelle, ils peuvent servir comme des lignes pour sauver des vies. Étant vulnérable sous des charges sismiques importantes, on peut considérer différentes méthodes pour concevoir des ponts routiers résistants et également pour réhabiliter des ponts existants. Dans cette étude, l'isolation de la base a été considérée comme une méthode efficace qui peut réduire significativement les effets des charges sismiques sur la structure. En réduisant la demande en ductilité sur la structure sans une augmentation notable de force, la structure est conçue pour rester élastique sous des charges sismiques. Le problème associé aux ponts isolés, particulièrement avec des appuis en élastomère, peut être leurs déplacements excessifs sous les charges de service et de séisme. Ceci peut défier l’objectif d'utiliser des appuis en élastomère pour les ponts typiques de petite portée où les joints de dilatation et les dégagements peuvent aboutir à une augmentation significative des frais d'exploitation et de maintenance. Ainsi, supplémenter la structure avec des amortisseurs d’une certaine rigidité peut servir de solution, ce qui peut cependant augmenter l’effort tranchant transmis à la sous-structure. Cette étude a pour but de fournir une méthode simplifiée afin d’évaluer les paramètres optimaux des amortisseurs dans les ponts isolés. Dans cette thèse, premièrement, basé sur une étude paramétrique, quelques directions sont données pour l'utilisation de dispositifs d'isolation simples, dont les appuis en élastomère, afin de réhabiliter des ponts existant avec une haute importance. Les paramètres comme la géométrie du pont, les clauses des normes et le type de sol sur lequel la structure est construite ont été appliqués sur un pont typique de deux portées. Il est conclu que les paramètres mentionnés peuvent déterminer l'emploi d'isolement de la base des ponts routiers. À la deuxième phase, basé sur le coefficient de réponse élastique des ponts isolés, une méthode de conception simplifiée d’amortisseur pour des ponts routiers réguliers isolés à la base a été présentée dans cette étude. En sélectionnant des objectifs pour la réduction du déplacement et la variation de l’effort tranchant, la rigidité et l'amortissement exigés d'un amortisseur hystérétique peuvent être déterminés. L’étude s’est poursuivie par une modélisation numérique d’un pont à deux portées pour vérifier l'efficacité de la méthode. Pour un modèle numérique d'un pont isolé typique, la méthode a été utilisée pour identifier des paramètres linéaires équivalents pour un certain déplacement et effort tranchant désigné. Par la suite, assumant un amortisseur de type hystérétique, les paramètres non linéaires de l’amortisseur ont été calculés et utilisés. La comparaison des résultats du modèle numérique sans amortisseur et avec l'amortisseur a démontré que la méthode proposée est suffisamment précise. Par la suite, un nouvel amortisseur hystérétique simple en acier a été conçu. Cinq spécimens ont été fabriqués de deux différents grades d’acier et ont été testés en combinaison avec un isolateur à l’échelle réelle dans le laboratoire de structures de l'Université de Sherbrooke. La procédure comprenait la caractérisation des spécimens par des tests cycliques en contrôle de déplacement et par la suite la réalisation d’essais par la méthode de sous-structuration dynamique en temps réel. Les résultats des essais ont été utilisés pour établir un modèle numérique du système qui a subi des analyses temporelles non linéaires sous plusieurs séismes. Le résultat des essais expérimentaux et numériques montrent une conformité acceptable avec la méthode simplifiée

    STR-929: EXPERIMENTAL TESTING OF A NEW HYSTERETIC DAMPER FOR BASE ISOLATED BRIDGES

    An experimental procedure was developed to implement a simplified design method of supplemental dampers for isolated highway bridges. For seismically base-isolated bridges subjected to an earthquake, the principal modes of the structure are in the isolation level which effectively lessens the seismic base shear conveyed from the superstructure to the substructure. However, this also has the effect of increasing superstructure displacement. To control the deformation of the isolators, supplemental energy dissipating devices can be introduced into the isolation system. This may nevertheless increase the total structure base shear and the merit of adding dampers has to be evaluated properly. In this paper a new type of steel hysteretic damper is proposed and tested in combination with an elastomeric isolator using displacement controlled cyclic tests. This very simple and economic damper was designed using a simplified method presented in earlier works. First, having characterized the response of an elastomeric isolation bearing used in a real structure, a simple and economic hysteretic damper was designed in accordance with the simplified method. In the next step, the designed damper in parallel with an elastomeric isolation bearing was tested by a unidirectional displacement-controlled quasi static method. Results show that this new type of damper could be used in combination with isolators to control superstructure displacement and substructure base shear

    Correlation of retinal nerve fibre layer thickness and spontaneous retinal venous pulsations in glaucoma and normal controls

Purpose: To study the relationship between amplitude of spontaneous retinal venous pulsatility (SRVP) and retinal nerve fibre layer (RNFL) thickness in glaucomatous eyes, and to determine if this parameter may be a potential marker for glaucoma severity. Method: 85 subjects including 50 glaucoma (21 males, 67±10 yrs) and 35 normals (16 males, 62±11 yrs) were studied. SRVP amplitude was measured using the Dynamic Vessel Analyser (DVA, Imedos, Germany) at four regions of the retina simultaneously within one disc diameter from the optic disc - temporal-superior (TS), nasal-superior (NS), temporal-inferior (TI) and nasal-inferior (NI)). This was followed by RNFL thickness measurement using spectral domain optical coherence tomography (Spectralis OCT). The correlation between SRVP amplitude and corresponding sectoral RNFL thickness was assessed by means of non-linear regression (i.e. logarithmic). Linear regression was also applied and slopes were compared using analysis of covariance (ANCOVA). Results: Greater SRVP amplitude was associated with thicker RNFL. Global SRVP amplitude was significantly lower in glaucoma eyes compared with normals (p0.05). Since the slopes are not significantly different, it is possible to calculate one slope for all the data. The pooled slope equals 10.8 (i.e. RNFL = 10.8SRVP+41)

    Implementation of a simplified method in design of hysteretic dampers for isolated highway bridges

    Abstract: Using seismic isolation systems for highway bridges modifies the structure’s principal vibration modes, effectively reducing the seismic base shear conveyed from the superstructure to the substructure. However, for some low damping rubber isolation bearings, large displacements can be a problem. Supplemental hysteretic dampers can be introduced into the base-isolated bridge. This may nevertheless increase the structure base shear and the merit of adding dampers has to be evaluated properly. In this paper, a simplified method was implemented for the design of a low-cost hysteretic damper and the resulting isolator-damper system was tested experimentally. The employed design method is based on equivalent linearization approach. A full-scale elastomeric isolation bearing was characterized and used in the design of a hysteretic damper. Both the isolator and the damper went through cyclic testing and real-time dynamic substructuring (RTDS) methods to verify the capacity of the method to design base isolation-damping systems. The study was further extended to extreme seismic loading by nonlinear time history analysis. Results show that the simplified method is adequate to be used in the performance optimization of isolated-damped bridges

    Impact of COVID-19 pandemic on ophthalmic presentations to an Australian outer metropolitan and rural emergency department: a retrospective comparative study.

    BACKGROUND: To analyse ophthalmic presentations to an outer metropolitan and a rural emergency department (ED) during the first wave of the COVID-19 pandemic in New South Wales (NSW), Australia. METHODS: A retrospective comparative study of ophthalmic emergency presentations to Campbelltown Hospital (fifth busiest NSW metropolitan ED; population 310,000) and Bowral and District Hospital (rural ED; population 48,000) before and during COVID-19 was conducted. Patient demographics, triage category, referral source, diagnosis, length of stay, departure status, and follow-up location were assessed from coding data between March 1st to May 31st in 2019 and 2020, corresponding to the peak case numbers and restrictions during the first wave of the COVID-19 pandemic in NSW. Differences before and during COVID-19 were analysed using chi-squared tests or independent sample t-tests. RESULTS: There was no change in ophthalmic presentations at Campbelltown (n = 228 in 2019 vs. n = 232 in 2020; + 1.75%, p = 0.12) and an increase at Bowral (n = 100 in 2019 vs. n = 111 in 2020; + 11%, p < 0.01) during COVID-19. Urgent ophthalmic presentations (Triage Category 3) decreased at Bowral (p = 0.0075), while non-urgent ophthalmic presentations (Triage Category 5) increased at both hospitals (Campbelltown p < 0.05, Bowral p < 0.01). CONCLUSIONS: There was no change in the total number of ophthalmic presentations to an outer metropolitan and an increase to a rural ED during the first wave of the COVID-19 pandemic in New South Wales, Australia. A change in the type of ophthalmic presentations at these peripheral EDs suggest that a high demand for ophthalmic services remained despite the pandemic and its associated gathering and movement restrictions. A flexible healthcare delivery strategy, such as tele-ophthalmology, may optimise patient care during and after COVID-19

    Th(IV)-hexacyanoferrate modified carbon paste electrode as a new electrocatalytic probe for simultaneous determination of ascorbic acid and dopamine from acidic media

    A stable carbon paste electrode (CPE) was prepared with Th(IV)-hexacyanoferrate (Th-HCF) ion pair and its electrochemical behavior was investigated by cyclic voltammetry. The apparent heterogeneous rate constant, k s, and transfer coefficient, a, for electron transfer between Th-HCF ion-pair and CPE were calculated as 3.1 ± 0.1 s-1 and 0.47, respectively. The surface coverage (&#915;) of the proposed electrode was calculated as 7.06 &#215; 10-10 mol cm-2. The proposed Th-HCF modified carbon paste electrode (THMCPE) showed a good electrocatalytic behavior with a significant shift toward negative potentials in oxidation of ascorbic acid (AA) and dopamine (DA) in acidic media (phosphate buffer solution, pH 3). The THMCPE exhibited excellent characteristics for simultaneous determination of AA and DA. Amperometric curves using the catalytic currents are linear for DA and AA concentrations in the ranges 8 &#215; 10-6 - 2 &#215; 10-3 and 1 &#215; 10-5 - 2 &#215; 10-3 mol L-1 with detection limits 5.6 &#215; 10-6 mol L-1 and 4.7 &#215; 10-6 mol L-1, respectively.     A combined convolutional and recurrent neural network for enhanced glaucoma detection.

    Glaucoma, a leading cause of blindness, is a multifaceted disease with several patho-physiological features manifesting in single fundus images (e.g., optic nerve cupping) as well as fundus videos (e.g., vascular pulsatility index). Current convolutional neural networks (CNNs) developed to detect glaucoma are all based on spatial features embedded in an image. We developed a combined CNN and recurrent neural network (RNN) that not only extracts the spatial features in a fundus image but also the temporal features embedded in a fundus video (i.e., sequential images). A total of 1810 fundus images and 295 fundus videos were used to train a CNN and a combined CNN and Long Short-Term Memory RNN. The combined CNN/RNN model reached an average F-measure of 96.2% in separating glaucoma from healthy eyes. In contrast, the base CNN model reached an average F-measure of only 79.2%. This proof-of-concept study demonstrates that extracting spatial and temporal features from fundus videos using a combined CNN and RNN, can markedly enhance the accuracy of glaucoma detection

    Systematic and Bibliometric Analysis of Magnetite Nanoparticles and Their Applications in (Biomedical) Research

    Abstract Recent reports show air pollutant magnetite nanoparticles (MNPs) in the brains of people with Alzheimer's disease (AD). Considering various field applications of MNPs because of developments in nanotechnology, the aim of this study is to identify major trends and data gaps in research on magnetite to allow for relevant environmental and health risk assessment. Herein, a bibliometric and systematic analysis of the published magnetite literature (n = 31 567) between 1990 to 2020 is completed. Following appraisal, publications (n = 244) are grouped into four time periods with the main research theme identified for each as 1990–1997 “oxides,” 1998–2005 “ferric oxide,” 2006–2013 “pathology,” and 2014–2020 “animal model.” Magnetite formation and catalytic activity dominate the first two time periods, with the last two focusing on the exploitation of nanoparticle engineering. Japan and China have the highest number of citations for articles published. Longitudinal analysis indicates that magnetite research for the past 30 years shifted from environmental and industrial applications, to biomedical and its potential toxic effects. Therefore, whilst this study presents the research profile of different countries, the development in research on MNPs, it also reveals that further studies on the effects of MNPs on human health is much needed
