The WTO Decision on Implementation of Paragraph 6 of the Doha Declaration on the TRIPS Agreement and Public Health: A Solution to the Access to Essential Medicines Problem?

Microcytosis and possible early iron deficiency in paediatric inpatients: a retrospective audit.
BACKGROUND: Iron deficiency anaemia is a common paediatric problem worldwide, with significant neurodevelopmental morbidity if left untreated. A decrease in the mean corpuscular volume (MCV) can be used as a surrogate marker for detecting early iron deficiency prior to definitive investigation and treatment. An audit cycle was therefore undertaken to evaluate and improve the identification, follow-up and treatment of abnormally low MCV results amongst the paediatric inpatients in an English district general hospital. METHODS: The audit cycle was performed retrospectively over two three-month periods (February to April 2006; September to November 2006), amongst patients aged between one month and 16 years that had full blood counts performed whilst admitted on the paediatric ward. Patients with at least one abnormally low MCV result were identified, and their notes reviewed. We looked for any underlying explanation for the result, adequate documentation of the result as abnormal, and instigation of follow-up or treatment. In-between the two audit periods, the results of the first audit period were presented to the medical staff and suggestions were made for improvements in documentation and follow-up of abnormal results. The z-test was used to test for equality of proportions between the two audit samples. RESULTS: Out of 701 inpatients across both audit periods that had full blood counts, 61 (8.7%) had a low MCV result. Only 15% of patients in each audit period had an identifiable explanation for their low MCV values. Amongst the remaining 85% with either potentially explicable or inexplicable results, there was a significant increase in documentation of results as abnormal from 25% to 91% of cases between the first and second audit periods (p = 0.00 using z-test). However, there was no accompanying increase in the proportion of patients who received follow-up or treatment for their abnormal results. CONCLUSION: Abnormal red cell indices that may indicate iron deficiency are frequently missed amongst paediatric inpatients. Medical staff education and the use of appropriate protocols or pathways could further improve detection and treatment rates in this setting
ORK1, a potassium-selective leak channel with two pore domains cloned from Drosophila melanogaster by expression in Saccharomyces cerevisiae.
A K+ channel gene has been cloned from Drosophila melanogaster by complementation in Saccharomyces cerevisiae cells defective for K+ uptake. Naturally expressed in the neuromuscular tissues of adult flies, this gene confers K+ transport capacity on yeast cells when heterologously expressed. In Xenopus laevis oocytes, expression yields an ungated K(+)-selective current whose attributes resemble the "leak" conductance thought to mediate the resting potential of vertebrate myelinated neurons but whose molecular nature has long remained elusive. The predicted protein has two pore (P) domains and four membrane-spanning helices and is a member of a newly recognized K+ channel family. Expression of the channel in flies and yeast cells makes feasible studies of structure and in vivo function using genetic approaches that are not possible in higher animals
The Doha Declaration Ten Years on and Its Impact on Access to Medicines and the Right to Health
Access to medicines is a human right, enshrined in legally binding international human rights treaties, select regional agreements and numerous national constitutions. The realization of access to medicines, including antiretroviral treatment, as part of the human right to health depends heavily on the legal framework for the production and distribution of medicines, including intellectual property rights. The adoption of the Agreement on Trade-Related Aspects of Intellectual Property Rights (TRIPS Agreement) within the framework of the World Trade Organization (WTO) changed dramatically the international landscape with regard to intellectual property, particularly in relation to access to medicines. Although one of the stated goals of the TRI PS Agreement was “to reduce tensions arising from intellectual property protection”, the possible conflict between such protection and essential public health objectives, particularly access to medicines, moved developing-country WTO Member States to request the Council for TRIPS to specifically consider the relationship between the TRI PS Agreement and public health in general, and access to medicines more specifically. After negotiations, in 2001 the 4th WTO Ministerial Conference adopted the Declaration on the TRIPS Agreement and Public Health. This Discussion Paper briefly describes the content of the Doha Declaration and examines its implications for the realization of the right to health. The Paper discusses a number of repercussions of the Doha Declaration with regard to the international discourse on the right to health and access to medicines, and its implications within the WTO system and for national legislation. It presents some examples of use of the flexibilities confirmed by the Doha Declaration, and discusses the issue of compulsory licences with regard to patents relating to products for non-communicable diseases. Finally, a number of conclusions and recommendations are presented
Inflation after Planck and BICEP2
We discuss the inflationary paradigm, how it can be tested, and how various
models of inflation fare in the light of data from Planck and BICEP2. We
introduce inflation and reheating, and discuss temperature and polarisation
anisotropies in the cosmic microwave background radiation due to quantum
fluctuations during inflation. Fitting observations of the anisotropies with
theoretical realisations obtained by varying various parameters of the
curvature power spectrum and cosmological parameters enables one to obtain the
allowed ranges of these parameters. We discuss how to relate these parameters
to inflation models which allows one to rule in or out specific models of
