AIM: To compare release parameters of various betahistine drugs in vitro using a comparative dissolution kinetics test. MATERIAL AND METHODS: Objects of research are solid dosage forms (tablets) containing betahistine in a dose of 24 mg permitted for medical use in the Russian Federation. A method of comparative dissolution kinetics test was carried out as follows. The study was performed on a paddle stirrer at a speed of 50 rpm in three different pH dissolution media (pH 1.2, 4.5, 6.8), simulating the main sections of the digestive tract in which the active ingredient was decomposed, released and absorbed. This was performed in a quality controlled environment using a citrate buffer solution with pH 6.8. The time points for sampling the medium were 10 min, 15 min, 20 min and 30 min. RESULTS: The results of betahistine release were significant (RSD<10%) for all time points, except the first time point (RSD<20%). Regardless of pH, there was a complete release (β₯85% over 15 minutes, <10%) of betahistine from betaserc, 24 mg, tablets (manufactured by Mylan Laboratories SAS). The dissolution profiles of betahistine in other investigational drugs did not show complete drug release (parameter <85% in 15 minutes, <10%) in different pH media. Therefore, dissolution profiles of the studied drugs were not comparable to the reference profile. CONCLUSION: Starting from 10 minutes, the reference drug of betahistine (betaserc, 24 mg) has a consistently higher release at different pH levels (representing the various stages of gastric digestion), vs. other studied generic analogues showing significantly lower levels of betahistine release. Modern masks of infectious endocarditis
The article gives the definition of infectious endocarditis, raises the issues of epidemiology, etiology, classification, and diagnostic criteria of this disease. It also presents the results of the treatment regimens analysis of infectious endocarditis cases for 2014-2017 year in MCH No. 24, Department of Health, addresses the perspective implementation of new drugs. Clinical case of successful treatment of infectious endocarditis, characterized by a variety of clinical manifestations, and, consequently, extended diagnostic search is discussed. In addition, the case of diagnosing endocardial tuberculosis is of scientific interest, because tuberoculous nature is atypical for infectious endocarditis, however, it should be borne in mind that nowadays, due to the increase in the incidence of tuberculosis, such etiology may occur. Paraproctitis is one of the most common nosologies in coloproctologistβs practice. The main treatment of this pathology is surgical, including dissection, debridement and drainage of the suppurative focus. However, in spite of surgical debridement, adequate perioperative antibiotic prophylaxis and, in rare and difficult cases, antibiotic therapy, are very important. Microbial spectrum obtained from the wound can be diverse, and, considering modern increase in the resistance of microorganisms, the decision on the prescription of effective antibiotic prophylaxis and therapy is not an easy task. In our study, that was carried out at the Municipal Clinical Hospital No. 24 of the Moscow Healthcare Department, we study the effectiveness of using the inhibitor-protected cephalosporin Ceftriaxone/Sulbactam as a drug for the perioperative prophylaxis and possible antibiotic therapy based on the results of microbiological culture. This article is a literature review on the comparison of international and Russian recommendations for the treatment of intra-abdominal infections. Despite today's modern possibilities of their early diagnosis, as well as effective therapy, intra-abdominal, infections are extremely serious conditions. However, there are many questions in relation to individual cases of Β«specific patientΒ» infection, some inconsistency of various recommendations and isolation of individual Β«advancedΒ» techniques from the realities within the country or region. There are also many problems associated with multi-resistant bacteria - with producers of extended spectrum beta lactamases resistant to vancomycin enterococci and with other infection agents. On March 18, 2021, an online council of experts in the field of epilepsy treatment was held, dedicated to the use of perampanel in the additional therapy of focal seizures (FS) and primary generalized tonic-clonic seizures (PGTCS) in children 4β12 years old. During the event, the features of the use of perampanel in this group of patients were discussed, considering the specifics of everyday clinical practice, current possibilities of PGTCS and FS therapy in children, its goals, as well as unsolved problems in the treatment of pediatric patients with PGTCS and FS.
