    Muscle Glycogen Depletion Following 75-km of Cycling Is Not Linked to Increased Muscle IL-6, IL-8, and MCP-1 mRNA Expression and Protein Content

    The cytokine response to heavy exertion varies widely for unknown reasons, and this study evaluated the relative importance of glycogen depletion, muscle damage, and stress hormone changes on blood and muscle cytokine measures. Cyclists (N=20) participated in a 75-km cycling time trial (168±26.0 min), with blood and vastus lateralis muscle samples collected before and after. Muscle glycogen decreased 77.2±17.4%, muscle IL-6, IL-8, and MCP-1 mRNA increased 18.5±2.8-, 45.3±7.8-, and 8.25±1.75-fold, and muscle IL-6, IL-8, and MCP-1 protein increased 70.5±14.1%, 347±68.1%, and 148±21.3%, respectively (all, P<0.001). Serum myoglobin and cortisol increased 32.1±3.3 to 242±48.3 mg/mL, and 295±27.6 to 784±63.5 nmol/L, respectively (both P<0.001). Plasma IL-6, IL-8, and MCP-1 increased 0.42±0.07 to 18.5±3.8, 4.07±0.37 to 17.0±1.8, and 96.5±3.7 to 240±21.6 pg/mL, respectively (all P<0.001). Increases in muscle IL-6, IL-8, and MCP-1 mRNA were unrelated to any of the outcome measures. Muscle glycogen depletion was related to change in plasma IL-6 (r=0.462, P=0.040), with change in myoglobin related to plasma IL-8 (r=0.582, P=0.007) and plasma MCP-1 (r=0.457, P=0.043), and muscle MCP-1 protein (r=0.588, P=0.017); cortisol was related to plasma IL-8 (r=0.613, P=0.004), muscle IL-8 protein (r=0.681, P=0.004), and plasma MCP-1 (r=0.442, P=0.050). In summary, this study showed that muscle IL-6, IL-8, and MCP-1 mRNA expression after 75-km cycling was unrelated to glycogen depletion and muscle damage, with change in muscle glycogen related to plasma IL-6, and changes in serum myoglobin and cortisol related to the chemotactic cytokines IL-8 and MCP-1

    Independent Ion Migration in Suspensions of Strongly Interacting Charged Colloidal Spheres

    We report on sytematic measurements of the low frequency conductivity in aequous supensions of highly charged colloidal spheres. System preparation in a closed tubing system results in precisely controlled number densities between 1E16/m3 and 1E19/m^3 (packing fractions between 1E-7 and 1E-2) and electrolyte concentrations between 1E-7 and 1E-3 mol/l. Due to long ranged Coulomb repulsion some of the systems show a pronounced fluid or crystalline order. Under deionized conditions we find s to depend linearily on the packing fraction with no detectable influence of the phase transitions. Further at constant packing fraction s increases sublinearily with increasing number of dissociable surface groups N. As a function of c the conductivity shows pronounced differences depending on the kind of electrolyte used. We propose a simple yet powerful model based on independent migration of all species present and additivity of the respective conductivity contributions. It takes account of small ion macro-ion interactions in terms of an effectivly transported charge. The model successfully describes our qualitatively complex experimental observations. It further facilitates quantitative estimates of conductivity over a wide range of particle and experimental parameters.Comment: 32 pages, 17 figures, 2 tables, Accepted by Physical Review

    “Emotional Exhaustion and Perceived Corporate Social Responsibility: A Case Study of a Port Logistics Organization”

    In an era of economic crisis, and at the shadow of major ethical scandals in organizations, Corporate Social Responsibility (CSR) strategy has emerged as a crucial element to reestablish the bond between corporations and all other stakeholders such as the local community, society and labor force. Crisis makes employees more stressful, since they work on unwarranted jobs causing them emotional exhaustion. This study aims to examine the association between employee emotional exhaustion and perceived corporate social responsibility (CSR). For this purpose, this study conducted a survey which examines if CSR (ethical, social, environmental dimensions) is negatively related to emotional exhaustion of employees on a sample of 93 employees of a port logistics management services organization. A structured questionnaire was developed in order to measure emotional exhaustion and employee perceptions about CSR activities. Building on the claim that employee perceptions of CSR activities may significantly related to emotional state, this paper examines three CSR dimensions (social, ethical and environmental) and emotional exhaustion. The results of this study indicate that environmental CSR exerts a negative significant effect on Emotional exhaustion. These finding will be of great value as they can contribute on understanding the impact of environmental CSR on emotional exhaustion with detrimental effects on employees’ productivity, job performance, and creativity. The importance of CSR environmental aspects and the relative strategies guiding CSR impact on emotional exhaustion affecting job-related outcomes are also discussed

    Second intravenous immunoglobulin dose in patients with Guillain-Barre syndrome with poor prognosis (SID-GBS):a double-blind, randomised, placebo-controlled trial

    Background Treatment with one standard dose (2 g/kg) of intravenous immunoglobulin is insufficient in a proportion of patients with severe Guillain-Barre syndrome. Worldwide, around 25% of patients severely affected with the syndrome are given a second intravenous immunoglobulin dose (SID), although it has not been proven effective. We aimed to investigate whether a SID is effective in patients with Guillain-Barre syndrome with a predicted poor outcome. Methods In this randomised, double-blind, placebo-controlled trial (SID-GBS), we included patients (>= 12 years) with Guillain-Barre syndrome admitted to one of 59 participating hospitals in the Netherlands. Patients were included on the first day of standard intravenous immunoglobulin treatment (2 g/kg over 5 days). Only patients with a poor prognosis (score of >= 6) according to the modified Erasmus Guillain-Barre syndrome Outcome Score were randomly assigned, via block randomisation stratified by centre, to SID (2 g/kg over 5 days) or to placebo, 7-9 days after inclusion. Patients, outcome adjudicators, monitors, and the steering committee were masked to treatment allocation. The primary outcome measure was the Guillain-Barre syndrome disability score 4 weeks after inclusion. All patients in whom allocated trial medication was started were included in the modified intention-to-treat analysis. Findings Between Feb 16, 2010, and June 5, 2018, 327 of 339 patients assessed for eligibility were included. 112 had a poor prognosis. Of those, 93 patients with a poor prognosis were included in the modified intention-to-treat analysis: 49 (53%) received SID and 44 (47%) received placebo. The adjusted common odds ratio for improvement on the Guillain-Barre syndrome disability score at 4 weeks was 1.4 (95% CI 0.6-3.3; p=0.45). Patients given SID had more serious adverse events (35% vs 16% in the first 30 days), including thromboembolic events, than those in the placebo group. Four patients died in the intervention group (13-24 weeks after randomisation). Interpretation Our study does not provide evidence that patients with Guillain-Barre syndrome with a poor prognosis benefit from a second intravenous immunoglobulin course; moreover, it entails a risk of serious adverse events. Therefore, a second intravenous immunoglobulin course should not be considered for treatment of Guillain-Barre syndrome because of a poor prognosis. The results indicate the need for treatment trials with other immune modulators in patients severely affected by Guillain-Barre syndrome.     Association between i.v. thrombolysis volume and door-to-needle times in acute ischemic stroke

    Centralization of intravenous thrombolysis (IVT) for acute ischemic stroke in high-volume centers is believed to improve the door-to-needle times (DNT), but limited data support this assumption. We examined the association between DNT and IVT volume in a large Dutch province. We identified consecutive patients treated with IVT between January 2009 and 2013. Based on annualized IVT volume, hospitals were categorized as low-volume (≤ 24), medium-volume (25-49) or high-volume (≥ 50). In logistic regression analysis, low-volume hospitals were used as reference category. Of 17,332 stroke patients from 11 participating hospitals, 1962 received IVT (11.3 %). We excluded 140 patients because of unknown DNT (n = 86) or in-hospital stroke (n = 54). There were two low-volume (total 101 patients), five medium-volume (747 patients) and four high-volume hospitals (974 patients). Median DNT was shorter in high-volume hospitals (30 min) than in medium-volume (42 min, p < 0.001) and low-volume hospitals (38 min, p < 0.001). Patients admitted to high-volume hospitals had a higher chance of DNT < 30 min (adjusted OR 3.13, 95 % CI 1.70-5.75), lower risk of symptomatic intracerebral hemorrhage (adjusted OR 0.39, 95 % CI 0.16-0.92), and a lower mortality risk (adjusted OR 0.45, 95 % CI 0.21-1.01), compared to low-volume centers. There was no difference in DNT between low- and medium-volume hospitals. Onset-to-needle times (ONT) did not differ between the groups. Hospitals in this Dutch province generally achieved short DNTs. Despite this overall good performance, higher IVT volumes were associated with shorter DNTs and lower complication risks. The ONT was not associated with IVT volum

    Pilot ondernemend werknemerschap

    Goede, ondernemende zorgprofessionals in de ouderenzorg vertrekken regelmatig uit organisaties om te starten als ZZP’er. Om deze ondernemende zorgprofessionals te behouden is geëxperimenteerd met het concept Ondernemend Werknemerschap. Ondernemend Werknemerschap is de arbeidsrelatie die de Ondernemende Werknemer heeft met de werkgever, waarbij de werkgever ruimte biedt aan de Ondernemende Werknemer om zelf invulling te geven aan kansen die deze ziet in de externe omgeving en autonoom mag handelen op het terrein van arbeidsinhoud, binnen de kaders van de overkoepelende organisatiedoelen. De afspraken over arbeidsinhoud, arbeidsvoorwaarden en arbeidsomstandigheden komen tot stand via onderhandeling en dialoog tussen de Ondernemende Werknemers en de werkgever. Succesvol Ondernemend Werknemerschap is gebaseerd op wederzijds vertrouwen, gezamenlijke verantwoordelijkheid en het ervaren van wederkerigheid door zowel de werkgever als de Ondernemende Werknemer. Het onderzoek bestaat uit een interviewstudie onder 17 experts naar Ondernemend Werknemerschap en twee uitgebreide casestudies in de Ouderenzorg. In deze organisaties is Ondernemend Werknemerschap geïntroduceerd en deze pilots zijn een jaar gevolgd, waarbij data zijn verzameld via interviews, focusgroepen, documentanalyse en vragenlijsten. Respondenten waren management, HR, direct leidinggevenden, medewerkers, OR en cliënten. Het aantal respondenten voor de interviews was in totaal 20. Het aantal respondenten voor de kwantitatieve meting was respectievelijk 85, 17 en 75. De algemene conclusie die getrokken kan worden is dat Ondernemend Werknemerschap voor medewerkers en cliënten leidt tot een betere kwaliteit van leven en werken. Ondernemend Werknemerschap levert ook een win op voor de organisatie, in de zin van nieuwe modellen van zorg- en dienstverlening

    Doorontwikkeling Integraal Gezondheids Management : evaluatie pilotprojecten

    In de periode 2002-2003 is door TNO Arbeid en NIGZ-Werk & Gezondheid een ontwikkelingsproject Integraal Gezondheids Management (IGM) uitgevoerd. Dit rapport doet verslag van de definitiestudie, methodiekontwikkeling en een aantal pilotprojecten. Aan de hand daarvan zijn concept en methodiek in de praktijk getest en is op systematische wijze materiaal verzameld die vijf vragen beantwoorden: Spreekt de methodiek het management en ook de medewerkers aan? Is de methodiek zelfstandig te gebruiken binnen het bedrijf? In hoeverre voldoet de huidige IGM Methodiek voor het strategisch verankeren van gezondheidsmanagement? Op welke punten behoeft de IGM Methodiek aanpassing en/of aanvulling? Op welke punten behoeft het IGM concept aanpassing en/of aanvulling? De pilots vonden plaats bij de ministeries van VWS en OCW, Arbo Unie en het Waterland Ziekenhuis (WLZ). Een belangrijke hoofdconclusie van de IGM pilots is dat het steeds om een ontwikkelingsproces gaat. In plaats van één (standaard) methodiek is er daarom veeleer behoefte aan een range van methodieken en instrumenten die kunnen worden toegepast. Voor de toekomst is het stimuleren van IGM, en van het feitelijk gebruik van de (aanvullende)IGM methodiek en instrumenten van belang (mainstreaming). Daarbij is ook een herkenbaar kenniscentrum (met website) wenselijk. Voordat de fase van “mainstreaming” kan worden bereikt, is er echter meer kennisontwikkeling nodig. Op termijn zijn dit vooral de effectstudies en evaluaties

    The UV-induced unscheduled DNA synthesis (UDS) in cultured human fibroblasts of repair-deficient xeroderma pigmentosum complementation groups A and C was assayed after injection of identical activities of either Uvr excinuclease (UvrA, B, C and D) from Escherichia coli or endonuclease V from phage T4. Under conditions where the T4 enzyme was able to induce repair synthesis in both XP complementation groups in agreement with earlier observations (de Jonge et al., 1985), no effect of the UvrABCD excinuclease could be observed either when the enzymatic complex was injected into the cytoplasm, or when it was delivered directly into the nucleus. In additon, no effect of the E. coli excinuclease was found on the repair ability of normal repair-proficient human fibroblasts. We conclude that the UvrABCD excinuclease may not work on DNA lesions in human chromatin.</p
