    Effect of indomethacin and adrenocorticotrophic hormone on renal function in man: An experimental model of inappropriate antidiuresis

    Effect of indomethacin and adrenocorticotrophic hormone on renal function in man: An experimental model of inappropriate antidiuresis. The effect of prostaglandin synthesis inhibition on basal and ACTH-stimulated adrenal and renal function was investigated in normai volunteers. Data were collected during control and experimental study periods (13 days each). Adrenocorticotrophic hormone (Cosyntropin, 80 U/day) was administered i.v. on days 8 and 9 of each period. Indomethacin (150 mg/day) was given on days 5 through 13 of the experimental period. The subjects ate a constant diet containing 9 mEq of sodium, 100 mEq of potassium, and 2,500 ml of fluid daily. Indomethacin markedly inhibited urinary PGE excretion and plasma PGE concentration. The effect of ACTH alone as compared to the effect of ACTH and indomethacin showed: plasma sodium concentration, 139 ± 1 vs. 131 ± 3 mEg/liter (P < 0.01, mean ± SEM); plasma osmolality, 287 ± 3 vs. 270 ± 3 mOsm/liter (P < 0.01); free water clearance, 97 ± 66 vs. -1100 ± 380 ml/24hr (P < 0.01); urine volume, 2,000 ± 60 vs. 950 ± 200 ml/day (P < 0.01); and urine osmolality 282 ± 12 vs. 720 ± 144 mOsm/liter (P < 0.01). We conclude that the effects of ACTH and prostaglandin synthesis inhibition interact to result in inappropriate antidiuresis.Effet de l'indométhacine et de l'hormone adrénocorticotrope sur la fonction rénale chez l'homme: Un modÚle expérimental d'antidiurÚse inappropriée. L'effet de l'inhibition de la synthÚse de prostaglandine sur la fonction surrénalienne basale et stimulée par l'ACTH et sur la fonction rénale a été étudié chez des sujets normaux volontaires. Les résultats ont été obtenus au cours de périodes contrÎles et expérimentales de 13 jours chacune. De l'hormone adrénocorticotrope (Cosyntropin) a été administrée par voie i.v. les 8Úme et 9Úme jours de chaque période à raison de 80 U par jour. L'indométhacine, 150 mg/jour, a été donnée du 8Úme au 13Úme jour de la période expérimentale. Les sujets ont été soumis à un régime constant contenant 9 mEq de sodium, 100 mEq de potassium, et 2,500 ml de liquide par jour. L'indométhacine a fortement inhibé l'excrétion urinaire de PGE et abaissé la concentration plasmatique de PGE. L'effet de l'ACTH seul comparé à l'effet de l'ACTH associé à l'indométhacine a montré: une concentration plasmatique de sodium de 139 ± 1 vs. 131 ± 3 mEq/litre (P < 0,01) moyenne et SEM); une osmolalité plasmatique de 287 ± 3 vs. 270 ± 3 mOsm/litre (P < 0,01); une clearance de l'eau libre de 97 ± 66 vs. - 1100 ± 380 ml/24 hr (P < 0,01); un débit urinaire de 2,000 ± 60 vs. 950 ± 200 ml/jour (P < 0,01); et une osmolalité urinaire de 282 ± 12 vs. 720 ± 144 mOsm/litre (P < 0,01).     Sustainable wastewater management in Indonesia's fish processing industry: Bringing governance into scenario analysis

    The government of Indonesia has pledged to meet ambitious greenhouse gas mitigation goals in its Nationally Determined Contribution as well as reduce water pollution through its water management policies. A set of technologies could conceivably help achieving these goals simultaneously. However, the installation and widespread application of these technologies will require knowledge on how governance affects the implementation of existing policies as well as cooperation across sectors, administrative levels, and stakeholders. This paper integrates key governance variables--involving enforcement capacity, institutional coordination and multi-actor networks--into an analysis of the potential impacts on greenhouse gases and chemical oxygen demand in seven wastewater treatment scenarios for the fish processing industry in Indonesia. The analysis demonstrates that there is an increase of 24% in both CH4 and CO2 emissions between 2015 and 2030 in the business-as-usual scenario due to growth in production volumes. Interestingly, in scenarios focusing only on strengthening capacities to enforce national water policies, expected total greenhouse gas emissions are about five times higher than in the business-as-usual in 2030; this is due to growth in CH4 emissions during the handling and landfilling of sludge, as well as in CO2 generated from the electricity required for wastewater treatment. In the scenarios where there is significant cooperation across sectors, administrative levels, and stakeholders to integrate climate and water goals, both estimated chemical oxygen demand and CH4 emissions are considerably lower than in the business-as-usual and the national water policy scenarios

    An atypical receiver domain controls the dynamic polar localization of the Myxococcus xanthus social motility protein FrzS

    The Myxococcus xanthus FrzS protein transits from pole-to-pole within the cell, accumulating at the pole that defines the direction of movement in social (S) motility. Here we show using atomic-resolution crystallography and NMR that the FrzS receiver domain (RD) displays the conserved switch Tyr102 in an unusual conformation, lacks the conserved Asp phosphorylation site, and fails to bind Mg2+ or the phosphoryl analogue, Mg2+·BeF3. Mutation of Asp55, closest to the canonical site of RD phosphorylation, showed no motility phenotype in vivo, demonstrating that phosphorylation at this site is not necessary for domain function. In contrast, the Tyr102Ala and His92Phe substitutions on the canonical output face of the FrzS RD abolished S-motility in vivo. Single-cell fluorescence microscopy measurements revealed a striking mislocalization of these mutant FrzS proteins to the trailing cell pole in vivo. The crystal structures of the mutants suggested that the observed conformation of Tyr102 in the wild-type FrzS RD is not sufficient for function. These results support the model that FrzS contains a novel ‘pseudo-receiver domain’ whose function requires recognition of the RD output face but not Asp phosphorylation

    Localized states in strong magnetic field: resonant scattering and the Dicke effect

    We study the energy spectrum of a system of localized states coupled to a 2D electron gas in strong magnetic field. If the energy levels of localized states are close to the electron energy in the plane, the system exhibits a kind of collective behavior analogous to the Dicke effect in optics. The latter manifests itself in ``trapping'' of electronic states by localized states. At the same time, the electronic density of states develops a gap near the resonance. The gap and the trapping of states appear to be complementary and reflect an intimate relation between the resonant scattering and the Dicke effect. We reveal this relation by presenting the exact solution of the problem for the lowest Landau level. In particular, we show that in the absence of disorder the system undergoes a phase transition at some critical concentration of localized states.Comment: 28 pages + 9 fig

    Oral Health Status of Patients with Mental Disorders in Southwest Ethiopia

    BACKGROUND: Psychiatric disorders are known to be a risk factor for the development of different oral health problems especially for dental caries and periodontal diseases. In spite of this fact, no study has been conducted to reveal its magnitude in Ethiopia. Hence, this study was conducted to determine the oral health status of psychiatric patients at Jimma University Specialized Hospital (JUSH), Psychiatric Clinic. METHODS: A hospital based cross- sectional study was used from January to May 2011. A total of 240 participants were included in the study. Dental examination was done to measure indices of oral health: decayed, missing, and filled teeth (DMFT) index and community periodontal index (CPI). Oral examination was performed using mirror, probe and explorer by experienced dental doctors. A simple random sampling technique was implemented to collect data. ANOVA test, binary logistic and multinomial logistic regression analyses were done using SPSS 16.0 statistical software. RESULTS: The mean DMFT score among the psychiatric patients was 1.94 ± 2.12 (mean ± SD) with 1.28 ± 1.69, 0.51 ± 1.19 and 0.14 ± 0.48 (mean ± SD) for decayed, missed and filled teeth respectively. Only about 24% of the psychiatric patients had a healthy CPI score. Incorrect tooth brushing technique was significantly associated with a DMFT score greater than 2 (AOR = 3.58; 95% CI: 1.65, 7.79). The habit of sweet intake was also associated with dental caries (AOR = 2.91; 95% CI: 1.43, 5.95). Similarly, patients with a smoking habit also demonstrated statistically significant association with dental caries (AOR = 18.98; 95% CI: 5.06, 71.24). CONCLUSION: The oral health status of the psychiatric patients was poor. Thus, health education about oral hygiene should be given for psychiatric patients so they can avoid the frequent intake of sweets, smoking and learn correct tooth brushing technique

    R-PEP-27, a potent renin inhibitor, decreases plasma angiotensin II and blood pressure in normal volunteers

    The hemodynamic and humoral effects of the specific human renin inhibitor R-PEP-27 were studied in six normal human subjects on low and high sodium intake diets. An intravenous infusion of R-PEP-27 (0.5 to 16 micrograms/min/kg body wt) reduced blood pressure in a dose-dependent fashion; the mean arterial blood pressure at the end of the infusion fell from 128 +/- 4/83 +/- 4 to 119 +/- 3/71 +/- 3 mm Hg (mean +/- SEM) (P &lt; .01) during the low sodium intake diet. R-PEP-27 had no effect on blood pressure during the high sodium intake diet. R-PEP-27 significantly reduced plasma angiotensin II and aldosterone concentrations. The temporal response to R-PEP-27 suggests that it is a short-lived although highly potent competitive inhibitor of renin; this peptide is a valuable and specific physiologic probe of the renin-angiotensin system

    Maternal and Child Oral Health Interventions in Middle East and North Africa Regions:a rapid review

    Objectives: To conduct a rapid review to identify any maternal and/or child oral health interventions implemented and/or tested in Middle East and North Africa (MENA) countries generally, and Lebanon, Palestine and Syria specifically, and to compile information on the relative effectiveness of these interventions. Methods: A systematic search was conducted for primary and secondary literature indexed in five online databases, and the websites of the World Health Organisation (WHO), the International Union for Health Promotion and Education (IUHPE), the United Nations Children’s Fund (UNICEF), United Nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA), and the United Nations Refugee Agency (UNHCR). Results: Two independent researchers reviewed 1,180 records from the online databases, and 3,120 reports from the WHO, UNRWA, UNICEF and UNHCR. Four unique studies were included and conducted in Syria, Saudi Arabia and Iran. No systematic reviews were found for targeted interventions in MENA regions. However, interventions using fluoridated toothpaste (Syria), preventive treatment and fluoridated chewing gum (Saudi Arabia), and oral health education with oral health reminders (Iran) were significantly effective in reducing early child caries (ECC) experience. In Syria and Iran, mother and child oral health promotions integrated into ongoing vaccination programmes were effective in reducing ECC. These interventions formed part of WHO and Ministry of Health programmes. Conclusion: Further investigation is essential to verify the effectiveness of incorporating multi-disciplinary, theory-driven oral health interventions into ongoing WHO maternal and child health programmes in MENA countries to assist in promoting oral health and wellbeing

    Insights into the oral health beliefs and practices of mothers from a north London Orthodox Jewish community

    <p>Abstract</p> <p>Background</p> <p>The objective of this study was to explore oral health knowledge and beliefs and access to dental care in a culturally distinct Orthodox Jewish community in North London, with a view to informing local health policy.</p> <p>Methods</p> <p>A dual method qualitative approach to data collection was adopted in this study utilising semi-structured face to face interviews and focus groups with women from this North London orthodox Jewish community. In total nine interviews and four focus groups were conducted with a purposive sample of thirty three mothers from the community aged 21-58 years. The data were transcribed and analysed using Framework Methodology</p> <p>Results</p> <p>Cultural influences, competing pressures and perceptions of hereditary influences, together with a lack of contemporary oral health knowledge are the main factors affecting oral health knowledge and beliefs. This supported an overall perspective of disempowerment or a perceived lack of control over oral health behaviours, both for mothers and their children. Community signposting pointed mothers to dental services, whilst family pressures together with inadequate capacity and capability and generic barriers such as fear and cost acted as barriers. Mothers from this community welcomed community development initiatives from the NHS.</p> <p>Conclusions</p> <p>The results of this study provide insight into the challenges of a culturally isolated community who would welcome community support through schools and expanded culturally appropriate opening hours to improve access to dental care.</p

    Quantum dynamics in strong fluctuating fields

    A large number of multifaceted quantum transport processes in molecular systems and physical nanosystems can be treated in terms of quantum relaxation processes which couple to one or several fluctuating environments. A thermal equilibrium environment can conveniently be modelled by a thermal bath of harmonic oscillators. An archetype situation provides a two-state dissipative quantum dynamics, commonly known under the label of a spin-boson dynamics. An interesting and nontrivial physical situation emerges, however, when the quantum dynamics evolves far away from thermal equilibrium. This occurs, for example, when a charge transferring medium possesses nonequilibrium degrees of freedom, or when a strong time-dependent control field is applied externally. Accordingly, certain parameters of underlying quantum subsystem acquire stochastic character. Herein, we review the general theoretical framework which is based on the method of projector operators, yielding the quantum master equations for systems that are exposed to strong external fields. This allows one to investigate on a common basis the influence of nonequilibrium fluctuations and periodic electrical fields on quantum transport processes. Most importantly, such strong fluctuating fields induce a whole variety of nonlinear and nonequilibrium phenomena.     Active Brownian Particles. From Individual to Collective Stochastic Dynamics

    We review theoretical models of individual motility as well as collective dynamics and pattern formation of active particles. We focus on simple models of active dynamics with a particular emphasis on nonlinear and stochastic dynamics of such self-propelled entities in the framework of statistical mechanics. Examples of such active units in complex physico-chemical and biological systems are chemically powered nano-rods, localized patterns in reaction-diffusion system, motile cells or macroscopic animals. Based on the description of individual motion of point-like active particles by stochastic differential equations, we discuss different velocity-dependent friction functions, the impact of various types of fluctuations and calculate characteristic observables such as stationary velocity distributions or diffusion coefficients. Finally, we consider not only the free and confined individual active dynamics but also different types of interaction between active particles. The resulting collective dynamical behavior of large assemblies and aggregates of active units is discussed and an overview over some recent results on spatiotemporal pattern formation in such systems is given.Comment: 161 pages, Review, Eur Phys J Special-Topics, accepte
