    Flourishing, mental health professionals and the role of normative dialogue

    This paper explores the dilemma faced by mental healthcare professionals in balancing treatment of mental disorders with promoting patient well-being and flourishing. With growing calls for a more explicit focus on patient flourishing in mental healthcare, we address two inter-related challenges: the lack of consensus on defining positive mental health and flourishing, and how professionals should respond to patients with controversial views on what is good for them. We discuss the relationship dynamics between healthcare providers and patients, proposing that ‘liberal’ approaches can provide a pragmatic framework to address disagreements about well-being in the context of flourishing-oriented mental healthcare. We acknowledge the criticisms of these approaches, including the potential for unintended paternalism and distrust. To mitigate these risks, we conclude by suggesting a mechanism to minimize the likelihood of unintended paternalism and foster patient trust

    Enhance Safety Performance through Quantitative and Qualitative Analysis of Accidents in Egyptian National Railway

    Rail transport plays an important role in creating a sustainable future for transport. Safety can be defined as qualitatively or quantitatively to prevent unacceptable risks. Improving safety is the main goal of each railway company as safe operation helps in increasing the level of service offered to the users. This paper deals with quantitative and qualitative analysis of accidents in Egyptian National Railway for annual accidents during the period of 2011 till 2017 for the six zones classified according to causes as absolute values and relative values to determine the worst zones and the most popular causes to improve the safety level and put the corrective plan to minimize the effective of accidents on both economic and safety of the society, to realize this goal a data collections were obtained and traffic fluction were calculated by the use of the official time table in train.km/year for the six zones. The study proposes effective solution for the decision maker to enface this very dangerous problem and improve the economic efficiency of the system, and preserve the environment surrounding the system

    Prognostic significance of cytochrome C release and tissue lactate dehydrogenase in patients with breast cancer

    The aim of this study was to evaluate the prognostic significance of cytochrome c release and tissue lactate dehyrogenase (LDH) in patients with breast cancer. This study included 90 patients with breast cancer and 35 patients with benign breast lesions. In cytosolic fraction, release of cytochrome c from mitochondria to cytosol was detected by Western blot, while total LDH activity was measured spectrophotometrically. Tissue LDHA mRNA was assessed by RTPCR. We observed a significant increase in cytochrome c release, LDH activity and LDHA mRNA in patients with breast cancer compared to patients with benign breast lesions. Cytochrome c release was significantly declined in poorly differentiated tumors (grade 3)compared to well-differentiated and moderately differentiated tumors (grades 1, 2); and in stages III and IV compared to stages I and II ofbreast cancer. Levels of LDH activity and LDHA mRNA were remarkably elevated in breast cancer patients with grade 3 compared to those with grades 1, 2; and in stages III and IV compared to stages I and II of breast cancer. High levels of LDH activity and LDHA mRNA were detected in breast cancer patients with positive lymph node compared to those with negative lymph node. These data indicate that cytochrome c release, cytosolic LDH activity and tissue LDHA mRNA can be considered as useful prognostic markers in patients with breast cancer.Keywords: Breast cancer, cytochrome c release, LDH, prognosticmarker
