    Study of the role of erythrocyte and leukocyte intoxication indexes in the pathogenesis of experimental peritonitis

    The article presents the results of studying the role of erythrocyte and leukocyte indices of intoxication in the pathogenesis of experimental peritonitis. When studying the role of erythrocyte and leukocyte indices, a unidirectional trend of their change was revealed. An increase in the erythrocyte index of intoxication in the pathogenesis of experimental fecal peritonitis was established, most pronounced on the 10th day of the experiment (the level of EII increased by 107% compared to intact animals). The results of the study of the leukocyte intoxication index indicate a unidirectional trend, as in the case of the erythrocyte intoxication index. On the first day, the leukocyte index of intoxication increased on 225% (Ρ€<0.05), on the 4th day – on 256.7% (Ρ€<0.05), and on the 10th day – on 260% (Ρ€ <0.05) compared to intact animals. It has been proven that the leukocyte index of intoxication is a more sensitive marker compared to the erythrocyte index

To investigate hematological indices on the first, third and seventh day of the experimental peritonitis development. Маterials and methods. The investigation was conducted on 180 Wistar line rats with a body mass 180 – 200 g. The animals were divided into four Groups: Group I – 20 intact animals; Group II – 80 animals, in whom a biliary peritonitis was simulated without further correction; Group III – 40 animals, in whom the biliary peritonitis simulated was corrected, using abdominal cavity sanation with chlorhexidine solution and further application of standard antibioticotherapy; Group IV – 40 animals, in whom a simulated biliary peritonitis was corrected in accordance to combined scheme of detoxication. Results. While investigation of hematological markers in animals of Group IV, to whom complex treatment was applied, a positive dynamics of indices of the erythrocytes quantity, hemoglobin concentration, hematocrit and ERS were observed on the third day and preserved up to the seventh day. The indices of the leucocytes quantity more significantly had normalized on the first and third days. The leucocytic index of intoxication had lowered on the third day. Most pronounced positive dynamics of the AST activity was observed on the seventh day, and of the ALT – on the third. On the first and third day of experiment the least pronounced lowering of the general protein concentration was noted, and on the seventh day – its raise. Concentration of general bilirubin maximally approached the indices of intact animals on the third day. Conclusion. The most likely mechanism that is damaged in the endothelium during peritonitis is the activation of the synthesis of inducible NO-synthase by neutrophils/macrophages in response to infection. It is possible that hyperproduction of nitric oxide (NO), on the one hand, is aimed at destroying microflora and oxidizing toxins, and on the other hand, at suppressing the expression of tissue factor and cell adhesion molecules. platelet aggregation and cascade disorders in the hemostasis system. All this indicates that the hyperproduction of NO not only reflects the processes that occur in the focus of damage to the vascular endothelium, but also affects the severity of the inflammatory process and the outcome of the disease. In animals with experimental peritonitis on the background of OS, an increase in the number of circulating desquamated endothelial cells in the blood, which is a highly specific marker of endothelial dysfunction, was noted. The level of the Willebrand factor also increased, which can serve as a marker of increased risk of thrombus formation and indicate the pathogenetic dependence of the factors that damage the vascular wall endothelium on the concentration of the Willebrand factor, which contributes to the reduction of vascular permeability by adhesion of platelets to the endothelium. Confirmation of the development of endothelial dysfunction in peritonitis is an increase in the concentration of endothelin-1, which is a regulator of the process of vascular neoangiogenesis in response to endothelial damage.     Dynamics of blood indicators as a marker of the grain of the grease peritonitis

    Introduction. Bile peritonitis is one of the most serious diseases of the abdominal cavity. The severity of its course and the percentage of mortality is primarily depends on endogenous intoxication. Therefore, effective sanitation of the abdominal cavity is an important element of the complex treatment of this pathology. Goal. Study of the dynamics of hematological parameters in the development of experimental bile peritonitis. Materials and methods of research. The investigation was carried out on 180 rats of the Wistar line with a mass 180-200 grams. Animals were divided into 4 groups: 1 group - intact (20 animals); 2 group - control - rats, that were modeled bile peritonitis without further correction (80 animals); Group 3 - animals whose modeled bile peritonitis was corrected by ablation of the abdominal cavity with a solution of furacilin and standard antibiotic therapy (40 animals); 4 group - rats, whose modeled bile peritonitis was corrected according to the combined scheme of detoxification. The task of this work is to investigate the dynamics of the indicators on the 1st, 3rd and 7th days. Results. The choice of erythrocyte, hemoglobin, hematocrit, ESR, leukocyte and leukocyte index of intoxication is optimal for the analysis of pathophysiological aspects of experimental bile peritonitis. Very high differences (p <0.001) between 1 and 3 days were revealed in the analysis of all markers in the 2nd group. Rats of this group who did not receive any therapy did not survive until the 7th day. Differences between the indices on the 1st, 3rd and 7th days at the level of statistical significance p<0.001 were revealed: in the analysis of hemoglobin in the 3rd group; hematocrit in the 3rd and 4th groups; leukocytes in the 4th group; LII in the 3 rd and 4 th groups; ESR in the 3rd and 4th groups. In the study of hemoglobin in the 4th group and hematocrit in the 1st group it was revealed that the differences between the 1st and 3rd, and also the 1st and 7th days were very high (p <0.001). Between the 3rd and the 7th day the indicators were at the level of significance p <0.01. Also, there were different group indicators (p <0.001): between 1st and 3rd, 1st and 7th days. However, there are no differences between the 3 rd and 7 th days. This indicates that the change in the indicator, which was formed on the 3rd day, is preserved without statistically significant dynamics until the 7th day. Such changes are characteristic for the dynamics of the level of erythrocytes in the 4th group, and for changes in the level of leukocytes in the 1 st group. In the group of intact animals, during the studying of hemoglobin and ESR values, differences were found at the level of p <0.01 between the indices obtained on the 1st and 7th day of the experiment. The absence of changes throughout the experiment is typical for the following indicators in the groups: in the 1st group during the analyzing of number of red blood cells and LII; in the second group - in the analysis of hemoglobin, LII and the number of leukocytes. Conclusions. Conducting blood sampling on the 1st, 3rd and 7th day with the conditions of our experiment allows us to fully reveal the features of the flow of experimental bile peritonitis, as well as the influence of the methods of its correction in dynamics
