Spatial and temporal variability of bronchiectasis cases in Silesian voivodeship in 2006–2010
Objectives: Reports on an increasing number of hospitalizations in other European countries and the lack of epidemiological data on the prevalence of bronchiectasis in Poland constituted motivation for the authors to investigate temporal changes of the registered incidence and hospitalization due to bronchiectasis in Silesian voivodeship, and to evaluate spatial variability of the disease in the study region. Material and Methods: The study is a descriptive epidemiological project. Temporal and spatial variability of coefficients describing numbers of newly diagnosed cases and first time hospitalizations due to bronchiectasis (code J47 according to International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10)) were evaluated based on the registered data available from the National Health Found (2006–2010) and the data from MZ/Szp-11 reports (2000–2011). The data concerned adults aged ≥ 19 years, inhabitants of Silesian voivodeship. Maps of incidence or hospitalization rates due to bronchiectasis were constructed by the use of a geographical information system ArcGIS. Results: The obtained results show a stable trend of reported new diseases, whereas the number of first time hospitalizations is increasing. Values of the standardized incidence were 19.9–25.1/100 000 inhabitants, and values of the standardized first-time hospitalization were 1.2–2.9/100 000 inhabitants. The reported rates of bronchiectasis indicate significant spatial differences in epidemiological situation in the study region. Conclusions: The findings showed territorial variability of the incidence and hospitalization of bronchiectasis recorded in Silesian voivodeship. The observed variability might result from regional differences in the availability of specialized medical services
Risk factors and the prevalence of hypertension in young adults
Wstęp Podwyższone wartości ciśnienia tętniczego
(BP) i wydłużony czas ich trwania sprzyjają wzrostowi
ryzyka powikłań i wskazują na konieczność
wczesnej identyfikacji nadciśnienia tętniczego,
zwłaszcza w grupie osób młodych, w odniesieniu do
których istnieją skuteczne działania profilaktyczne.
Szacuje się, że nadciśnienie dotyczy w Polsce blisko
co trzeciej osoby dorosłej (20-30%), odsetek ten jednak
nie obrazuje skali problemu w populacji młodych
osób dorosłych.
Celem pracy było określenie częstości nadciśnienia
w populacji studentów (osób w wieku 18-25 lat)
i poznanie potencjalnych uwarunkowań występowania
nadciśnienia tętniczego w tej grupie wiekowej.
Materiał i metody Dla realizacji celu badawczego
przeprowadzono epidemiologiczne badanie przekrojowe
w grupie 532 studentów. Pomiary BP przeprowadzono
zgodnie z zaleceniami Polskiego Towarzystwa
Nadciśnienia Tętniczego (PTNT) i Europejskiego
Towarzystwa Nadciśnienia Tętniczego
(ESH). W celu określenia potencjalnych uwarunkowań
nadciśnienia tętniczego przeprowadzono badanie
kwestionariuszowe. Analizę statystyczną danych
prowadzono na podstawie procedur dostępnych
w oprogramowaniu EpiInfo 6.0.
Wyniki Prawidłowe optymalne wartości BP stwierdzono
u 39,3% studentów (częściej u kobiet niż mężczyzn,
p < 0,001). Nadciśnienie tętnicze dotyczyło 14,8% badanych
(częściej mężczyzn niż kobiet, p < 0,001),
w tym znamiennie częściej (p < 0,05) występowało
u mężczyzn izolowane nadciśnienie skurczowe - u 3,7%
badanych. Wśród statystycznie znamiennych uwarunkowań
nadciśnienia znalazły się: otyłość i dieta bogatotłuszczowa,
nie wykazano natomiast wpływu nałogu palenia tytoniu i dodatniego wywiadu rodzinnego
w kierunku nadciśnienia tętniczego.
Wnioski Wyniki badania wykazały dość częste występowanie
nadciśnienia tętniczego u młodych osób
dorosłych (blisko 15%), wśród czynników ryzyka
znalazły się otyłość i niewłaściwa dieta. Uzasadnia to
celowość prowadzenia badań przesiewowych w kierunku
nadciśnienia wśród studentów i promowania
zasad zdrowego żywienia w tej grupie populacyjnej.Background Elevated blood pressure value and its elongated
duration increases the risk of associated complications.
It underlines the necessity of detection of hypertension
at the early stage, especially in young adults, when the
prevention is effective. It was estimated that the prevalence
of hypertension in Poland was 20-30%, but this value is
inadeqate in young adults.
The aim of the study was to assess basic risk factors and the
prevalence of hypertension in medical students, aged 18–25.
Material and methods The cross-sectional study involved
a BP measurement, in accordance with Polish Society of
Hypertension and European Society of Hypertension guidelines,
and the questionnaire estimating basic risk factors of
hypertension in 532 students, aged 18–25. The statistical
analysis was performed with EpiInfo 6.0 procedures.
Results Optimal BP was recognized in 39.3% and hypertension
in 14.8% students (both more frequently in men,
p < 0.001). The prevalence of isolated systolic hypertension
was 3.7% (more frequently in men, p < 0.05). Hypertensive
subjects were more frequently overweight than students
with normal BP (p < 0.05). Young adults with hypertension
more frequently used diet rich in fat than
students with normal BP (p < 0.05). Additionally, smoking
habit and family history of hypertension did not increase
the risk of hypertension.
Conclusions The results revealed frequent incidence of hypertension
in young adults (about 15%). The risk factors
were obesity and improper diet. It justifies the necessity of
cross-sectional studies in young adults to detect hypertension
and to promote healthy lifestyle in young people
Lifestyle of young adults with elevated blood pressure values
Wstęp Ryzyko zgonu z przyczyn naczyniowych
wzrasta wprost proporcjonalnie do wartości ciśnienia
tętniczego, a wzrost wartości ciśnienia tętniczego powyżej
115/75 mm Hg istotnie zwiększa ryzyko chorób
układu krążenia. Dlatego istotne jest pytanie
o znaczenie, już w fazie stanu przednadciśnieniowego,
znanych, modyfikowalnych czynników ryzyka
nadciśnienia tętniczego, związanych z nieprawidłowym
stylem życia.
Celem pracy było porównanie stylu życia młodych
osób dorosłych zakwalifikowanych do trzech grup:
grupy z rozpoznanym nadciśnieniem tętniczym, grupy
ze stanem przednadciśnieniowym i grupy z ciśnieniem
prawidłowym optymalnym.
Materiał i metody Ocenę przeprowadzono na podstawie
wyników przygodnego pomiaru skurczowego
ciśnienia tętniczego (SBP) i rozkurczowego ciśnienia
tętniczego (DBP) oraz badania kwestionariuszowego,
oceniającego w modelu epidemiologicznego
badania przekrojowego podstawowe
uwarunkowania nadciśnienia tętniczego. Badanie
przeprowadzono w grupie 532 studentów w wieku
18-25 lat. Statystyczną analizę danych wykonano
na podstawie procedur zawartych w programie
EpiInfo 6,0. Wyniki Prawidłowe optymalne ciśnienie tętnicze występowało
u 39,3% osób, stan przednadciśnieniowy rozpoznano
u 45,9% badanych, a nadciśnienie tętnicze u
14,8% studentów. Nadwaga i otyłość częściej występowały
u osób z nadciśnieniem tętniczym niż u osób ze
stanem przednadciśnieniowym oraz u osób z prawidłowym
optymalnym ciśnieniem tętni-czym (p < 0,05).
Wykazano, iż osoby z nadciśnieniem tętniczym w porównaniu
z badanymi ze stanem przednadciśnieniowym
i osobami z prawidłowym optymalnym ciśnieniem
tętniczym częściej stosowały dietę bogatotłuszczową
oraz rzadziej piły kawę (p < 0,05). Ponadto w
grupie osób z nadciśnieniem tętniczym zaobserwowano
statystycznie znamiennie częstsze rodzinne występowanie
zaburzeń lipidowych, choroby niedokrwiennej
serca i miażdżycy w porównaniu z osobami z prawidłowymi
wartościami ciśnienia tętniczego (p < 0,05).
Wnioski Wyniki badania wykazały zróżnicowanie
w zakresie stylu życia młodych osób dorosłych z nadciśnieniem
tętniczym, stanem przednadciśnieniowym
oraz z prawidłowym optymalnym ciśnieniem
tętniczym w zakresie nadwagi i niewłaściwej diety.
Z tego względu należy prowadzić prospektywne
badania epidemiologiczne oceniające zagrożenie
związane z nieprawidłowym stylem życia u osób
z podwyższonymi wartościami ciśnienia tętniczego
oraz promować zdrowy styl życia u młodych osób.Background The risk of cardiovascular death is associated
with blood pressure value (BP) and its value above
115/75 mm Hg increases the risk in a statistically significantly
way. According to this, the role of lifestyle and risk
factors of hypertension that can be modified is important
also in the stadium of prehypertension.
The aim of the study was to compare lifestyles of young
adults with hypertension, prehypertension and optimal BP.
Material and methods The cross-sectional study ivolred
a casual BP measurement and the questionnaire estimating basic
risk factors of hypertension in 532 students, aged 18–25. The
statistical analysis was performed with EpiInfo 6.0 procedures.
Results Optimal BP was recognized in 39.3%, prehypertension
in 45.9% and hypertension in 14.8% students.
Students with hypertension were more frequently
overweighted/obese than students with prehypertension
and optimal BP (p < 0.05). Young adults with hypertension
more frequently used diet rich in fat and less frequently
drank coffee than students with prehypertension
and optimal BP (p < 0.05). Hypertensive subjects declared
the family history of dyslipidaemia, ischaemic heart disease
and atheroslerosis more frequently than students with
normal BP (p < 0.05).
Conclusion The results revealed different lifestyles of
young adults with hypertension, prehypertension and optimal
BP, and difference in obesity and improper diet. The
study underlines the necessity of prospective epidemiologic
studies to estimate the cardiovascular risk related to abnormal
lifestyle among people with increased BP level and to promote healthy lifestyle of young adults
Development, evaluation and validation of a new instrument for measurement quality of life in the parents of children with chronic disease
<p>Abstract</p> <p>Background</p> <p>Childhood chronic disease may affect patients' and their family's functioning. Particularly parents, who play an important role in cooperation between patient and health care professionals, report impaired health - related quality of life (HRQOL). The aim of this study was development, evaluation and validation of a new instrument: Quality of Life in a Child's Chronic Disease Questionnaire (QLCCDQ). The questionnaire is addressed to parents of children with a chronic disease.</p> <p>Methods</p> <p>Study design included semi structured interview and qualitative study, which allowed to identify most troublesome problems. Following the results the questionnaire was developed, which consists of 15 questions and covers domains - emotions, patients -perceived symptoms, roles limitations. An observational study involving parents of asthma and diabetes children was conducted to assess the psychometric characteristics of the measure. Psychometric testing was based on the reliability of defined subscales, construct validity, reproducibility assessment, as well as comparison between stable/unstable disease stages and parents of healthy children.</p> <p>Results</p> <p>Most troublesome concerns for parents of child with chronic disease included emotional distress and feeling depressed due to child's disease, avoiding social interactions due to child's disease or symptoms. 98 parents of children with asthma or insulin - depended diabetes participated in the psychometric testing of QLCCDQ. Internal consistency reliability for the defined subscales ranged between 0.77 and 0.93. Reproducibility based on the weighted kappa coefficients showed expected level of agreement and was almost perfect in case of 8 questions, substantial for 5 questions and moderate for 2 questions. QLCCDQ demonstrated very good construct validity - all subscales showed statistically significant correlations ranging from 0.4 to 0.9. QLCCDQ scores differed significantly by clinical status - parents of children qualified as stable presented higher scores in most subscales in comparison to parents of children with unstable disease.</p> <p>Conclusions</p> <p>The QLCCDQ shows good internal consistency, test-retest reliability, and construct validity. The questionnaire may be useful in helping to understand the impact of chronic child's disease on parental perception of health outcomes.</p
Socioeconomic determinants of prostate-specific antigen testing and estimation of the prevalence of undiagnosed prostate cancer in an elderly Polish population based on the PolSenior study
Introduction: Socioeconomic determinants of prostate-specific antigen
(PSA) testing and prevalence of undiagnosed prostate cancer (PCa) in the
Polish population are poorly understood. The aim of this study was to identify factors associated with PSA testing in elderly Polish men, and estimate
the size of the population at risk of PCa related to PSA non-testing.
Material and methods: We analyzed questionnaire-derived data concerning PSA testing, obtained in 2567 elderly and 332 younger (age: 55-59)
participants of the population-based PolSenior study. Additionally, PSA was
measured in 2414 subjects.
Results: The PSA had previously been tested in 41.2% of elderly and in 24.8%
of younger participants. Non-smoking status (OR = 2.06, p < 0.001), higher personal income (OR = 1.56, p < 0.001), better education (OR = 1.49, p = 0.001),
previous white-collar work (OR = 1.37, p = 0.005), alcohol abstinence (OR =
1.28, p = 0.02), married status (OR = 1.24, p = 0.04), dependence in Instrumental Activities of Daily Living (IADL) but not in Activities of Daily Living
(ADL) (OR = 0.65, p < 0.001), and dependence in ADL (OR = 0.55, p < 0.001)
were independent predictors of previous PSA testing in elderly participants.
There were 31 elderly previously treated for PCa (calculated standardized
prevalence: 935 per 100,000 elderly population). The PSA levels > 4 ng/ml
were found in 12.8% of 65-74-year-old and 4.5% of 55–59-year-old previously non-tested participants. We calculated the standardized prevalence
rate of undiagnosed PCa as approximately 1370 and 2352 cases per 100,000
population aged 55-59 and 65-74 years, respectively.
Conclusions: In Poland, 58.8% of elderly men have never had PSA tested.
These were less likely to be functionally independent, married, better educated, non-smokers or to have previous office employment or higher than
average personal income. Our data suggest substantial underdiagnosis of
prostate cancer among Polish men
Regression towards the mean in studies of arterial blood pressure
Regresja w kierunku wartości średniej (RTM, regression
towards the mean) jest zjawiskiem statystycznym,
które występuje w badaniach ciśnienia
tętniczego niezależnie od analizowanego problemu
badawczego, wieku badanych, metody pomiaru
lub czasu prowadzonej obserwacji. Problem
ten pojawia się zawsze, gdy dobór uczestników
do grupy badanej nie jest randomizowany. Regresja
w kierunku wartości średniej jest tym większa,
im słabsza jest korelacja między ocenianymi
zmiennymi. Spostrzeżenie to ma istotne implikacje
w badaniach populacyjnych wykorzystujących
pomiar przygodnego ciśnienia tętniczego,
gdyż w największym stopniu może skutkować
błędną interpretacją otrzymanych rezultatów.
Wartość RTM stanowi problem zwłaszcza w ocenie
skuteczności terapii hipotensyjnej oraz w ocenie
rozpowszechnienia nadciśnienia tętniczego.
Mimo że eliminacja efektu RTM nie jest często
możliwa, warto uwzględnić ten problem w metodologii
i odnieść się do niego w dyskusji uzyskanych
wyników.Regression towards the mean (RTM) is a statistical
phemonenon that occurs in blood pressure studies independently
of the problem or duration of the research, age
of the studied population, or the method being used. RTM
is observed whenever the study group is not randomized.
It is assumed that the less correlated the two variables are
the greater the RTM is. The infuence of RTM should be
considered mostly in casual blood pressure measurements
because of imprecise assessment of the results. Consequently,
RTM may be mistakenly attributed to a treatment
effect and the prevalence of hypertension. Despite the entire
elimination of RTM can be impossible, it is suggested
to remind this problem to the reader either in the section of
„materials and methods” or „discussion”
Socio-demographic factors related to under-diagnosis of childhood asthma in Upper Silesia, Poland
Introduction
The presented study of 4,535 children aged 7–17 years in the Upper Silesian region of Poland yielded 186 cases of previously known asthma, and 44 children with newly diagnosed asthma. The aim of the presented study was to identify non-medical factors that could explain why children with a newly established diagnosis (‘undiagnosed asthma’) had not been diagnosed in the past.
Material and Methods
The study was performed according to a case-control design. Parents of the children answered questionnaires on socio-economic status and family-related factors. Statistical determinants of undiagnosed asthma were explored using raw (OR) and logistic odds ratios with their 95% confidence intervals (logOR, 95%CI).
Results
Children with undiagnosed asthma were younger compared to the group with previously known asthma (11.3±2.1 vs. 12.6±2.5 years; p=0.0008). Newly diagnosed cases were more frequent in children who had less parental attention (less than 1 hour/day spent by parent with child – OR=4.36; 95%CI: 1.76–10.81) and who were not registered with specialized health care (OR=2.20; 95%CI: 0.95–5.06). Results of logistic regression analysis suggest that under-diagnosis of asthma is related to age below 12 years – logOR = 3.59 (95%CI: 1.28–10.36), distance to a health centre > 5 km – logOR = 3.45 (95%CI: 1.05–11.36), time spent with child < 1 hour/day – logOR = 6.28 (95%CI: 1.98–19.91).
Conclusions
Among non-medical determinants of undiagnosed asthma the age of a child plays a major role. Another factors of importance is the large distance between residence and health centre, and low parental attention at home
