With the population aging a significant percentage of patients qualified for invasive procedures have a significant heart valve defect and/or multilevel atherosclerosis. The problem of underestimating of peripheral vascular disease is multithreaded. The optimal selection of vascular access and the concept of access to the heart is the key point. This is crucial to avoid possible complications and the proper access to the heart to ensure the effectiveness of the procedure. This applies to two clinical situations in terms of urgency - planned and ad hoc procedures. The knowledge of the legal and formal status is also important.Wraz z tendencją starzenia się populacji znaczny odsetek pacjentów kwalifikowanych do procedur kardiologicznych ma istotną wadę serca oraz miażdżycę wielopoziomową. Problem niedoszacowania choroby naczyń obwodowych jest wielowątkowy. W kontekście interwencji przezskórnej optymalny wybór dostępu naczyniowego oraz koncepcji dojścia do serca jest kluczowy dla uniknięcia możliwych powikłań oraz właściwego utorowania dostępu do serca w celu zapewnienia skuteczności procedury. The Wnt signalling pathway is a complex, conservative network of molecular interactions which is most active in tissue formation and regeneration processes. A growing number of studies suggest that its suboptimal function can be crucial in cardiovascular diseases. Acute coronary syndromes are the most frequent cause of heart failure. The pharmacological agents used up to date are mainly renin–angiotensin–aldosterone and sympathetic signalling antagonists. Studies show that interference with the Wnt pathway in the post myocardial infarction period may hinder left ventricular remodelling and left ventricle function impairment, preventing early complications, as well as the development of chronic heart failure.Szlak sygnalizacyjny Wnt jest złożonym, konserwatywnym systemem oddziaływań molekularnych aktywnym przede wszystkim w procesach powstawania i regeneracji tkanek. Rosnąca liczba badań wskazuje, że jego nieoptymalne działanie może mieć znaczenie w chorobach układu krążenia. Ostre zespoły wieńcowe są najczęstszą przyczyną rozwoju niewydolności serca. Dotychczas stosowane w leczeniu niewydolności serca leki to głównie antagoniści szlaku renina–angiotensyna–aldosteron oraz części sympatycznej autonomicznego układu nerwowego. Wyniki badań pokazują, że ingerencja w szlak Wnt po zawale serca może mieć istotne znaczenie dla hamowania niekorzystnej przebudowy oraz upośledzenia funkcji lewej komory, zapobiegając zarówno wczesnym powikłaniom, jak i rozwojowi przewlekłej niewydolności serca
