    Prevalence of porcine cysticercosis in the Lake Kyoga Basin, Uganda

    BACKGROUND: Taenia solium is a zoonotic helminth with the potential to cause life threatening epilepsy in people through the aberrant larval infection of the brain called Neurocysticercosis (NCC). The pig is the intermediate host for T. solium where the larval form, cysticercus cellulosae, normally develops after the pig eats eggs of the parasite. Humans are the definitive host where the adult tapeworm develops and are infected through the consumption of poorly cooked, infected meat. T. solium has been acknowledged by the World Health Organization (WHO), Food and Agriculture Organization (FAO) and UK Department for International Development (DFID) as being a neglected zoonotic disease, and was recently included in the WHO roadmap for control of neglected tropical diseases. This neglect encompasses a lack of epidemiological data and a lack of validated, effective control strategies being adopted. Understanding the epidemiology of this parasite in the intermediate host is the first step towards designing suitable intervention strategies for the improvement of public health. This study was undertaken to provide an accurate and up-to-date estimate for the prevalence of porcine cysticercosis in the Lake Kyoga basin. RESULTS: Sera from 378 pigs were analysed with the HP10 Antigen Enzyme Linked Immunosorbant Assay (ELISA) and the prevalence was found to be 25.7% (95% confidence interval 21.0% to 30.0%). Previous sero- surveillance in this region, using the B158/B60 Ag Elisa had indicated a prevalence of 8.6% in 2005 indicating a dramatic increase in prevalence (J. Parasitol Res, Article ID 375493, 2009) within a 6 year period. CONCLUSION: This increasing prevalence in the disease indicates to us that there is currently no effective control of this parasite and that in this region of Uganda at least; cysticercosis remains a neglected zoonotic disease

    The Status of Pig Welfare in Selected Districts of Uganda: Implications for Health and Productivity Interventions

    We conducted a pig welfare survey in four high pig producing districts in central Uganda, namely Masaka, Mukono, Mpigi, and Wakiso in 2021. Data were collected from 270 pig farms, and a total of 3,561 pigs, and consisted of resource-based (housing and water supply), animal-based (pig body condition and physical injuries), and management-based (records of morbidity and mortality) indicators of pig health and welfare. Data on animal and resource-based indicators were obtained from physical assessments of pigs and farm facilities, while management indicators and demographic details were collected by farmer interview. Across all farms, sow mortality in the last 12 months was reported to be 2.5% (95% Confidence Interval: 1.7, 4.8%). Animal-based indicators identified 9% (6.1, 13.7%) of sows with scouring/diarrhea, 7.6% (4.7, 11.5%) with lameness and 92% (85.1, 96.7%) of dry or lactating sows were found to be “skinny” with a body condition score of 1 or 2. In addition, piglet mortality was as high as 10.2% (8.5, 12.9%). A total of 19% of sows were partially or completely restricted from free movement inside pens. Ninety-three per cent of sows had continuous access to water, but only 48.8% of the water supplies were clean. Twenty per cent of farms reported sows experiencing stillbirths , 15% reported ill thrift, 12.5% reported respiratory disease and 10.8% reported gastrointestinal disease. Wakiso district registered the lowest pig mortality which was significantly lower compared to the other three districts. This study reveals that Ugandan pigs are exposed to severe undernutrition, dirty water, high mortality, physical injuries, poor housing, and health challenges (stillbirth, ill thrift, and Gastrointestinal disorders). Meaningful change to farmer livelihoods and pig welfare can be made by designing simple interventions that target improve housing structures, provision of cooling facilities especially during hot periods (heat stress) and bedding materials. The findings also represent a benchmark for the assessment of the effect of such interventions designed to improve farm health and productivity

    Assessment of the Impact of Early Diagnosis and Early Treatment in the Integrated Control of East Coast Fever (ECF) Involving Acquired Immunity Induced by Natural Infection in Ankole Cattle

    The integrated control of East Coast fever (ECF) by early diagnosis and treatment involving acquired immunity induced by natural infection in Ankole cattle was assessed. A longitudinal study was carried out in Kiruhura district, southwestern Uganda for six months on 244 Ankole breed of cattle from 18 herds under natural tick challenge and relaxed tick control measures. Calves aged three to six months old were recruited and monitored daily by farmers for detection of ECF clinical symptoms. The reported sick animals were treated using Buparvaquone and treatment outcome determined. Monthly follow-ups and blood collections were done to monitor ECF status. Blood was analyzed for Theileria parva parasites by microscopy, DNA by polymerase chain reaction (PCR) and antibodies by enzyme-linked immunosorbent assay (ELISA). The overall prevalence of ECF clinical disease within six months period was 30.3% (74). The major symptoms of early clinical ECF disease were fever and enlarged parotid or prescapular lymph nodes. Clinical cases were categorized as mild, 24% (18) or moderate, 76% (56). There was an overall recovery rate of 100% (74) of the ECF cases whereby 94.6% (70) recovered promptly and 5.4% (4) recovered slowly. Based on blood analysis, prevalence of ECF at baseline was 3.7% (9) by microscopy, 31.1% (76) by PCR and 38.1% (93) by ELISA. A significant increase (p T. parva specific antibodies in the sera from 38.1% at baseline to 68.8% after six months. High antibody levels (positive percentage ≥ 50%) were detected in all ECF-treated and recovered calves at the end of six months. The acquired immunity to ECF was high in treated and recovered cattle, indicating that natural exposure to infection, accurate early diagnosis and effective treatment enhance development of immune-protection in indigenous cattle in an endemic area. The prominent early clinical symptoms for ECF could be exploited in the development of decision support tools for chemotherapy and other integrated control measures

    The definitive method for diagnosis of porcine cysticercosis is the detection of cysticerci at necropsy. Cysts are typically located in the striated muscle and brain. Until recently Taenia solium cysticerci have not been definitively identified in other tissue locations, despite several comprehensive investigations having been undertaken which included investigation of body organs other than muscle and brain. Recently a study conducted in Zambia reported 27% infection with T. solium in the liver of pigs with naturally acquired porcine cysticercosis, as well as some T. solium infection in the lungs and spleen of some animals. We investigated the cause of lesions in sites other than the muscle or brain in a total of 157 pigs from T. solium endemic regions of Uganda and Nepal which were subjected to extensive investigations at necropsy. Lesions which had the potential to be caused by T. solium were characterised by macroscopic and microscopic examination, histology as well as DNA characterisation by PCR-RFLP and sequencing. Lesions were confirmed as being caused by Taenia hydatigena (both viable and non-viable), by T. asiatica and Echinococcus granulosus (in Nepal) and nematode infections. No T. solium-related lesions or cysticerci were identified in any tissue other than muscle and brain. It is recommended that future evaluations of porcine cysticercosis in aberrant tissue locations include DNA analyses that take appropriate care to avoid the possibility of contamination of tissue specimens with DNA from a different tissue location or a different animal. The use of appropriate control samples to confirm the absence of cross-sample contamination is also recommended
