    Gender differences in dual-chamber pacemaker implantation indications and long-term outcomes

    Objective: Dual-chamber (DDD) pacing is the most commonly used mode of heart stimulation. The data on gender-related differences in the long-term follow-up of DDD pacing mode are still limited. We performed a retrospective single-centre study to determine the effect of gender on the implantation indications and the incidence of adverse events resulting in DDD mode loss. Methods and results: A group of 1,049 consecutive patients with DDD pacemaker implanted between 1984 and 2002 were followed up until 2014. The study group consisted of 995 patients who performed at least one follow-up visit. Follow-up period was 124.2 ± 68.3 months, mean age was 63.5 ± 12.4 years, 56% were male. Adverse events were defined as loss of primary DDD stimulation – lead malfunction, progression to permanent AF, and infective complications. Women were older than men (64.7 vs 62.6 years) at the time of implantation and they remained, on average, 1.5 year longer in follow-up compared with men. Female patients had significantly more SSS, history of paroxysmal AF, and a similar percentage of AVB compared with male patients. The incidence of lead malfunction, device-related infections, and progression to permanent AF did not show significant differences. However, in the group without prior paroxysmal AF, women developed permanent AF more frequently. Conclusions: This patients cohort showed that there is an association between gender and indications to DDD pacing therapy. The rate of adverse events was similar in both genders. Women had a significantly longer duration of follow-up, despite markedly higher age at implantation

    By-products of the combustion of coal (wastes) are often used for various types of construction (dwellings, roads, etc.). The legal regulations (The Ordinance of the Council of Ministers of 2 January 2007 “On the requirements for the content of natural radioactive isotopes of potassium K-40, radium Ra-226 and thorium Th-228 in raws and materials used in buildings for the residence of people and livestock, as well as in the industrial by-products used in the construction, and the control of the content of the aforementioned isotopes” – Law Gazette no. 4/2007 item 29) are in force in Poland. The regulations permit the possibility of utilization of raws and by-products basing upon the level of the natural radioactivity of the examined raws and materials. The article is a survey of the results obtained during the measurements of many types of raws and building materials for almost 30 years by the network of the laboratories in Poland. It is based upon the results stored in the database of the Central Laboratory for Radiological Protection (CLRP), Warsaw. The article tends to outline the radioactivity of the waste materials with respect to other raws and materials used in the construction industry. The article shows the possibilities for the use of by-products originating in the power stations and heat- and power stations (mainly ashes, slag and hinter) in the construction of dwellings and roads

    International audienceThe Paris basin is a polarised geographic system with an important centre-periphery divide. Its diachronic analysis indicates that the decline of the marches, which started in the sixties, continues today in a way revealing geographic as well as historic continuities. Today as in the past, this decline can be attributed to the agriculture and industry economic sectors. Moreover, in the "régions" of the Paris basin, dynamic combinations associatingan economic evolution typology and demographic mutations can be identified using multi-variable analyses.L'analyse diachronique du Bassin parisien, système géographique polarisé et marqué par un fort clivage centre-périphérie, nous révèle que les processus de déclin des marges du système, enregistrés ici depuis le début des années 1960, se poursuivent selon des modalités qui révèlent bien des continuités, tant géographiques qu'historiques. Les secteurs d'activité en cause, l'agriculture et l'industrie, demeurent les mêmes, tandis que selonles régions du Bassin parisien, des combinaisons dynamiques associant types de trajectoires économiques et mutations démographiques peuvent être identifiées en recourant aux méthodes d'analyse multivariée

    Pregnancy and congenital complete atrioventricular block: management during pregnancy and periparturient period (RCD code: VII-V)

    Complete atrioventricular block (AVB) is rare during pregnancy. Congenital atrioventricular block is the most common type of heart block in this group of patients. About one‑third of female patients with complete AVB remain asymptomatic until adulthood and may be first diagnosed during pregnancy. We present a case of a 31‑year‑old pregnant woman with complete AVB who was in her final stage of pregnancy. After reviewing the various advantages and disadvantages of feasible approaches with the patient, we decided to use fluoroscopy‑guided temporary backup pacemaker implantation. Estimated radiation skin dose was small and safe. The patient agreed to this treatment plan. Four days prior to scheduled cesarean delivery (39 weeks of gestation), during a one‑day stay in the hospital, the patient underwent single‑chamber temporary pacemaker implantation (using transvenous active fixation lead and external re‑sterilized pacemaker). The abdominal and pelvic regions were covered with a lead shield. The caesarean delivery was uneventful and the baby was healthy with an Apgar score of 10
