Concomitant vancomycin and piperacillin/tazobactam treatment is associated with an increased risk of acute kidney injury in Japanese patients
Introduction
Recent studies have corroborated that the co-administration of vancomycin (VCM) and piperacillin/tazobactam (PT) is correlated with an increased incidence of acute kidney injury (AKI). However, evidence directed at the Japanese population is scarce. Therefore, we conducted a retrospective study to compare the occurrence of AKI among Japanese patients who received VCM with PT (VP therapy) and VCM with another β-lactams (VA therapy).
Methods
The present study, performed at Tsuyama Chuo Hospital between June 2012 and December 2018, included adult patients who received VCM and β-lactam antibiotics for ≥48 h. We defined the primary outcome as the incidence of AKI based on the risk, injury, failure, loss, and end-stage kidney disease criteria. Patients' clinical characteristics and outcomes were reviewed and compared between the two groups with univariate and multivariate logistic regression analyses. Subgroup analysis was conducted by stratifying the patients’ baseline hospital admittance status, as intensive care unit or general wards.
Results
We analyzed 272 patients (92 V P therapy and 180 VA therapy). Univariate analysis revealed a significant difference in AKI development between VP and VA therapy (25.0% vs 12.2%; p

Why Guided Dialog Policy Learning performs well? Understanding the role of adversarial learning and its alternative
Dialog policies, which determine a system's action based on the current state
at each dialog turn, are crucial to the success of the dialog. In recent years,
reinforcement learning (RL) has emerged as a promising option for dialog policy
learning (DPL). In RL-based DPL, dialog policies are updated according to
rewards. The manual construction of fine-grained rewards, such as
state-action-based ones, to effectively guide the dialog policy is challenging
in multi-domain task-oriented dialog scenarios with numerous state-action pair
combinations. One way to estimate rewards from collected data is to train the
reward estimator and dialog policy simultaneously using adversarial learning
(AL). Although this method has demonstrated superior performance
experimentally, it is fraught with the inherent problems of AL, such as mode
collapse. This paper first identifies the role of AL in DPL through detailed
analyses of the objective functions of dialog policy and reward estimator.
Next, based on these analyses, we propose a method that eliminates AL from
reward estimation and DPL while retaining its advantages. We evaluate our
method using MultiWOZ, a multi-domain task-oriented dialog corpus
Integrating Preprocessing Operations into Deep Learning Model: Case Study of Posttreatment Visual Acuity Prediction
Designing a deep neural network model that integrates clinical images with other electronic medical records entails various preprocessing operations. Preprocessing of clinical images often requires trimming of parts of the lesions shown in the images, whereas preprocessing of other electronic medical records requires vectorization of these records; for example, patient age is often converted into a categorical vector of 10-year intervals. Although these preprocessing operations are critical to the performance of the classification model, there is no guarantee that the preprocessing step chosen is appropriate for model training. The ability to integrate these preprocessing operations into a deep neural network model and to train the model, including the preprocessing operations, can help design a multi-modal medical classification model. This study proposes integration layers of preprocessing, both for clinical images and electronic medical records, in deep neural network models. Preprocessing of clinical images is realized by a vision transformer layer that selectively adopts the parts of the images requiring attention. The preprocessing of other medical electrical records is performed by adopting full-connection layers and normalizing these layers. These proposed preprocessing-integrated layers were verified using a posttreatment visual acuity prediction task in ophthalmology as a case study. This prediction task requires clinical images as well as patient profile data corresponding to each patient's posttreatment logMAR visual acuity. The performance of a heuristically designed prediction model was compared with the performance of the prediction model that includes the proposed preprocessing integration layers. The mean square errors between predicted and correct results were 0.051 for the heuristic model and 0.054 for the proposed model. Experimental results showed that the proposed model utilizing preprocessing integration layers achieved nearly the same performance as the heuristically designed model
Search for time-dependent B0s - B0s-bar oscillations using a vertex charge dipole technique
We report a search for B0s - B0s-bar oscillations using a sample of 400,000
hadronic Z0 decays collected by the SLD experiment. The analysis takes
advantage of the electron beam polarization as well as information from the
hemisphere opposite that of the reconstructed B decay to tag the B production
flavor. The excellent resolution provided by the pixel CCD vertex detector is
exploited to cleanly reconstruct both B and cascade D decay vertices, and tag
the B decay flavor from the charge difference between them. We exclude the
following values of the B0s - B0s-bar oscillation frequency: Delta m_s < 4.9
ps-1 and 7.9 < Delta m_s < 10.3 ps-1 at the 95% confidence level.Comment: 18 pages, 3 figures, replaced by version accepted for publication in

The Japanese space gravitational wave antenna; DECIGO
DECi-hertz Interferometer Gravitational wave Observatory (DECIGO) is the future
Japanese space gravitational wave antenna. DECIGO is expected to open a new window of
observation for gravitational wave astronomy especially between 0.1 Hz and 10 Hz, revealing
various mysteries of the universe such as dark energy, formation mechanism of supermassive
black holes, and inflation of the universe. The pre-conceptual design of DECIGO consists of
three drag-free spacecraft, whose relative displacements are measured by a differential Fabry–
Perot Michelson interferometer. We plan to launch two missions, DECIGO pathfinder and pre-
DECIGO first and finally DECIGO in 2024
Measurement of the branching ratios of the Z0 into heavy quarks
We measure the hadronic branching ratios of the Z0 boson into heavy quarks:
Rb=Gamma(Z0->bb)/Gamma(Z0->hadrons) and Rc=Gamma(Z0->cc/Gamma(Z0->hadrons)
using a multi-tag technique. The measurement was performed using about 400,000
hadronic Z0 events recorded in the SLD experiment at SLAC between 1996 and
1998. The small and stable SLC beam spot and the CCD-based vertex detector were
used to reconstruct bottom and charm hadron decay vertices with high efficiency
and purity, which enables us to measure most efficiencies from data. We obtain,

The effect of 5-aminolevulinic acid on cytochrome c oxidase activity in mouse liver
<p>Abstract</p> <p>Background</p> <p>5-Aminolevulinic acid (ALA) is a precursor of heme that is fundamentally important in aerobic energy metabolism. Among the enzymes involved in aerobic energy metabolism, cytochrome <it>c </it>oxidase (COX) is crucial. In this study, the effect of ALA on cytochrome <it>c </it>oxidase activity was measured.</p> <p>Findings</p> <p>c57BL/6N species of mice were administered ALA orally for 15 weeks. After ALA administration, mice were sacrificed and livers were obtained. COX activity in mitochondria from ALA-administered mouse livers was 1.5-fold higher than that in mitochondria from PBS-administered mouse livers (P < 0.05). Furthermore, ATP levels in ALA-administered mouse livers were much higher than those in PBS-administered mouse livers. These data suggest that oral administration of ALA promotes aerobic energy metabolism, especially COX activity.</p> <p>Conclusions</p> <p>This is the first report of a drug that functions in aerobic energy metabolism directly. Since COX activity is decreased in various diseases and aging, the pharmacological effects of ALA will be expanding.</p
Direct Measurements of A_b and A_c using Vertex/Kaon Charge Tags at SLD
Exploiting the manipulation of the SLC electron-beam polarization, we present
precise direct measurements of the parity violation parameters A_c and A_b in
the Z boson - c quark and Z boson - b quark coupling. Quark/antiquark
discrimination is accomplished via a unique algorithm that takes advantage of
the precise SLD CCD vertex detector, employing the net charge of displaced
vertices as well as the charge of kaons that emanate from those vertices. From
the 1996-98 sample of 400,000 Z decays, produced with an average beam
polarization of 73.4%, we find A_c = 0.673 +/- 0.029 (stat.) +/- 0.023 (syst.)

Measurement of the Branching Fraction for B->eta' K and Search for B->eta'pi+
We report measurements for two-body charmless B decays with an eta' meson in
the final state. Using 11.1X10^6 BBbar pairs collected with the Belle detector,
we find BF(B^+ ->eta'K^+)=(79^+12_-11 +-9)x10^-6 and BF(B^0 ->
eta'K^0)=(55^+19_-16 +-8)x10^-6, where the first and second errors are
statistical and systematic, respectively. No signal is observed in the mode B^+
-> eta' pi^+, and we set a 90% confidence level upper limit of BF(B^+->
eta'pi^+) eta'K^+- decays is
