    Observational study: handgrip strength, body composition and diabetes mellitus.

    ObjectivePrevious studies have shown that relative handgrip strength, handgrip measure divided by body mass index (BMI), affects the future onset of diabetes and prediabetes. However, fat free mass (FFM) has been suggested to adjust for this effect better than BMI. In this study, we examined applicability of models that adjusted handgrip-diabetes relationship with either BMI or FFM.ResultsOf 1940 participants (56.2% male, average (SD) age, 57.2 [11.2] years), 267 (13.8%) had diabetes (DM) and 912 (47.0%) had prediabetes (pre-DM). The average handgrip measure for men was 40.0 kg (tertile measures, 37.4 kg and 42.5 kg) and for women 24.2 kg (tertile measures, 22.6 kg and 25.7 kg). Among both sexes, the percentage of people unaffected by DM or pre-DM was highest in the strong handgrip group and lowest in the weak handgrip group. Analysis using binary logistic models showed that an increase in handgrip measure was associated with a decrease in the chance of having either pre-DM or DM. This effect was detected by both BMI models and FFM models, even after adjustment for medical and lifestyle factors. Either or both should be used depending on the research aims, setting and methods

    Application of RFLP-PCR-Based Identification for Sand Fly Surveillance in an Area Endemic for Kala-Azar in Mymensingh, Bangladesh

    Mymensingh is the most endemic district for kala-azar in Bangladesh. Phlebotomus argentipes remains the only known vector although a number of sand fly species are prevalent in this area. Genotyping of sand flies distributed in a VL endemic area was developed by a PCR and restriction-fragment-length polymorphism (RFLP) of 18S rRNA gene of sand fly species. Using the RFLP-PCR analysis with AfaI and HinfI restriction enzymes, P. argentipes, P. papatasi, and Sergentomyia species could be identified. Among 1,055 female sand flies successfully analyzed for the species identification individually, 64.4% flies was classified as Sergentomyia species, whereas 35.6% was identified as P. argentipes and no P. papatasi was found. Although infection of Leishmania within the sand flies was individually examined targeting leishmanial minicircle DNA, none of the 1,055 sand flies examined were positive for Leishmania infection. The RFLP-PCR could be useful tools for taxonomic identification and Leishmania infection monitoring in endemic areas of Bangladesh

    Resolution and Resurgence of Schistosoma haematobium—induced Pathology After Community-based Chemotherapy in Ghana, as Detected by Ultrasound

    Community-based treatment is recommended for endemic populations with urinary schistosomiasis; however, the optimal target group for treatment and retreatment interval have not been established. Using ultrasound, this study identified subpopulations whose lesions were most likely to respond to treatment and characterized resurgence of pathology. Ultrasound examination of 1202 infected patients was followed by chemotherapy with praziquantel. A sample of 698 patients was followed for 18 months after treatment. Nearly all types of bladder pathologies resolved after treatment, regardless of patient's age or intensity of initial infection. However, many patients' upper urinary tract pathologies (62.5%) did not resolve. During the 18-month follow-up period, reappearance of severe bladder pathologies was rare, and < 10% of persons had resurgence of mild bladder pathologies. For this population, re-treatment is not needed annually but might be cost effective if given several years later. Confirmation from other areas is required before general policies can be forme

    Geographical distribution for malignant neoplasm of the pancreas in relation to selected climatic factors in Japan

    <p>Abstract</p> <p>Background</p> <p>Malignant neoplasm of the pancreas has become one of the leading causes of death from malignant neoplasm in Japan (the 5th in 2003). Although smoking is believed to be a risk factor, other risk factors remain unclear. Mortality from malignant neoplasm of the pancreas tends to be higher in northern Japan and in northern European countries. A recent study reported that standardized mortality ratios (SMRs) for malignant neoplasm of the pancreas were negatively correlated to global solar radiation level. People residing in regions with lower solar radiation and lower temperatures may be at higher risk of development of malignant neoplasm of the pancreas. Therefore, this study aimed to examine the relationship between SMRs for malignant neoplasm of the pancreas and climatic factors, such as the amount of global solar radiation and the daily maximum temperature in Japan.</p> <p>Results</p> <p>The study used multiple linear regression models. Number of deaths and demographic data from 1998 to 2002 were used for the calculation of SMR. We employed mesh climatic data for Japan published in 2006 by the Japan Meteorological Agency. Regression coefficients for the amount of global solar radiation and the daily maximum temperature in males were -4.35 (p = 0.00034) and -2.81 (p < 0.0001) respectively, and those in females were -5.02 (p < 0.0001) and -1.87 (p < 0.0001) respectively. Increased amount of global solar radiation or daily maximum temperature was significantly related to the decreased SMRs for malignant neoplasm of the pancreas in both males and females.</p> <p>Conclusion</p> <p>This study suggested that low solar radiation and low temperature might relate to the increasing risk of malignant neoplasm of the pancreas. Use of group data has a limitation in the case of evaluating environmental factors affecting health, since the impact of climatic factors on the human body varies according to individual lifestyles and occupations. Use of geographical mesh climatic data, however, proved useful as an index of risk or beneficial factors in a small study area. Further research using individual data is necessary to elucidate the relationship between climatic factors and the development of malignant neoplasm of the pancreas.</p

    Postoperative daily living activities of geriatric patients administered general or spinal anesthesia for hip fracture surgery: A retrospective cohort study

    Purpose:Maintaining independence after hip fracture repair is important for geriatric patients and general welfare. We investigated the effects of anesthetic methods on postoperative activities of daily living (ADLs) following hip fracture surgery in elderly patients.Methods:The medical records of 12,342 patients aged ≥65 years who underwent typical surgeries for hip fracture using either general anesthesia or spinal anesthesia were reviewed. To adjust for baseline differences and minimize selection bias for the chosen method of anesthesia, patients were matched by propensity scores. Factors affecting the deterioration in ADLs during hospital stay were also investigated in all subjects using a multivariate logistic regression analysis. Eating, grooming, toileting, bathing, and walking were selected as the ADL parameters, as they are considered important for an independent life.Results:Of the 12,342 patients, 6918 (56.1%) received general anesthesia and 5424 (43.9%) received spinal anesthesia. After the propensity score matching, the anesthesia types were not associated with ADL scores except toileting at discharge. Results from the multivariate logistic regression analysis showed that the types of anesthesia were not associated with deterioration in ADL scores. Advanced age, male sex, high Charlson Comorbidity Index scores, psychiatric disease, no administration of nonsteroidal anti-inflammatory drugs, and short length of hospital stay were associated with deterioration in ADL scores.Conclusion:The anesthesia types were not associated with ADL dependency except toileting at discharge. Spinal anesthesia adversely affected toilet use at hospital discharge. However, anesthesia types were not factors that affected deterioration in ADL during hospital stay in elderly patients who underwent hip fracture surgery

    Nutritional status and childhood wheezing in rural Bangladesh.

    OBJECTIVE: To investigate the association between current childhood nutritional status and current wheezing among pre-school children in rural Bangladesh. DESIGN: Cross-sectional study. SETTING: Matlab region, rural Bangladesh. SUBJECTS: Children (n 912) aged 4·5 years. Anthropometric measurements of the mothers and their children were taken during a 1-year period from December 2007 to November 2008. Current wheezing was identified using the International Study of Asthma and Allergies in Childhood questionnaire. Serum total IgE was measured by human IgE quantitative ELISA. IgE specific antibody to dust mites (Dermatophagoides pteronyssinus) was measured by the CAP-FEIA system (Phadia AB, Uppsala, Sweden). RESULTS: Wheezing at 4·5 years old was significantly associated with stunting (OR = 1·58; 95 % CI 1·13, 2·22) and underweight (OR = 1·39; 95 % CI 1·00, 1·94). The association with stunting remained significant after adjustment for sex, birth weight, birth length, gestational age at birth, mother's parity, maternal BMI, family history of asthma, socio-economic status, season of birth and intervention trial arm (OR = 1·74; 95 % CI 1·19, 2·56). CONCLUSIONS: Stunting was a significant risk factor for wheezing among rural Bangladeshi children. Further studies will be required to confirm the relationship between nutritional status and allergic illnesses in developing countries

    Cholera in Bangladesh: Climatic Components of Seasonal Variation

    Background: The mechanisms underlying the seasonality of cholera are still not fully understood, despite long-standing recognition of clear bimodal seasonality in Bangladesh. We aimed to quantify the contribution of climatic factors to seasonal variations in cholera incidence. Methods: We investigated the association of seasonal and weather factors with the weekly number of cholera patients in Dhaka, Bangladesh, using Poisson regression models. The contribution of each weather factor (temperature and high and low rainfall) to seasonal variation was estimated as the mean over the study period (1983-2008) for each week of the year of each weather term. Fractions of the number of cholera patients attributed to each weather factor, assuming all values were constant at their minimum risk levels throughout the year, were estimated for spring and monsoon seasons separately. Results: Lower temperature predicted a lower incidence of cholera in the first 15 weeks of the year. Low rainfall predicted a peak in spring, and high rainfall predicted a peak at the end of the monsoon. The risk predicted from all the weather factors combined showed a broadly bi-modal pattern, as observed in the raw data. Low rainfall explained 18% of the spring peak, and high rainfall explained 25% of the peak at the end of the monsoon. Conclusions: Seasonal variation in temperature and rainfall contribute to cholera incidence in complex ways, presumably in interaction with unmeasured environmental or behavioral factors

    Temperature during pregnancy influences the fetal growth and birth size

    BackgroundBirth weight and length have seasonal fluctuations. However, it is uncertain which meteorological element has an effect on birth outcomes and which timing of pregnancy would explain such effect. Therefore, the purpose of this study was to examine temperature effects during pregnancy and which timing of pregnancy has effects on size at birth.MethodsA large, randomized, controlled trial of food and micronutrient supplementation for pregnant women was conducted in Matlab, Bangladesh (MINIMat Study), where women were enrolled from November 2001 to October 2003. The fetal growth data which included the size at birth and information of their mothers were obtained (n = 3267). Meteorological data such as temperature, precipitation, relative humidity, and daily sunshine hours during pregnancy were observed at the nearest observatory site of Bangladesh Meteorological Department.ResultsInfants born in colder months (November–January) were shorter than those born in hot and dry, and monsoon months (mean (SD) of birth length was 47.5 cm (2.2) vs. 47.8 cm (2.1) vs. 47.9 cm (2.1) respectively; P < 0.001). Increased temperature during the last month of pregnancy was significantly related with increased birth length with adjustment for gestational weeks and the season at birth, and remained significant with further adjustments for precipitation, sex of infants, maternal early-pregnancy BMI, parity, and education status of the mother (P < 0.01). On the other hand, increased temperature at mid-gestation was associated with increased birth weight (P < 0.05).ConclusionsThese findings suggest that temperature affects both birth weight and length. The more temperature increased at the last month of pregnancy, birth length became longer. For birth weight, the temperature at mid-pregnancy affected in a positive way
