    EFFECT OF FLUOXETINE ON SLEEP ARCHITECTURE IN PATIENTS WITH METABOLIC SYNDROME AND OBSTRUCTIVE SLEEP APNEA SYNDROME

    Aim. To study effect of fluoxetine on sleep architecture in patients with metabolic syndrome and obstructive sleep apnea syndrome.Material and methods. 98 patients with obstructive sleep apnea syndrome and metabolic syndrome (aged 54.3±8.7 y.o.) were included into the study. All patients received fluoxetine 20 mg once daily during 6 months. Influence of fluoxetine on sleep architecture was evaluated with special questionnaire and by polysomnography, including electroencephalogram, electrooculogram, mentalis electromyogram and pulseoxymetry.Results. Decrease in wake time after sleep onset (Δ33%; р<0.05) was found at the end of treatment. It resulted in improvement of sleep efficacy index. Decrease in respiratory sleep disorders index (Δ20%) and rising of blood oxygen saturation (Δ12%; р<0.05) was also found. Improvement of the sleep architecture (reduction in the 2nd phase of slow wave sleep by 15%, increase in delta-sleep (Δ71%) and rapid eye movement sleep (Δ25%; р<0.05) was also observed. Besides reduction in body mass index after fluoxetine therapy (Δ12%; р<0.05) was found. Serious adverse effects were not registered.Conclusion. Fluoxetine use in patients with metabolic syndrome and obstructive sleep apnea syndrome shown positive effect on objective indices of sleep architecture and respiratory sleep disorders. It improved adaptive function of the sleep and contributed to reduction in sleep disorders

    Clinical value of oral fluid chemokines in chronic periodontitis

    Activation of cytokine network is one of the key mechanisms in immunopathogenesis of chronic periodontitis (CP), a common dental disease. Therefore, the study of the proteomic (including cytokine) profile of saliva is not coincidentally considered among global research areas in periodontology. However, the existing data on the contents of cytokines / chemokines in oral fluid (OF) in CP are contradictory. This ambiguity of the results can be associated both with a variety of methodological approaches to the cytokine determination, and with different severity of CP in the studied cohorts of patients. Moreover, recent systematic reviews and meta-analyses show that clinical value of chemokines in CP is also not yet determined thus indicating a need for future research in this area.The aim of this study was to assess clinical and diagnostic value of some chemokines from oral fluid in chronic mild periodontitis. The work was based on a study of 18 patients diagnosed with mild chronic periodontitis (MCD-10 K05.3) and 12 practically healthy volunteers with relatively intact periodontal tissue. The diagnosis was based on standard clinical and radiological criteria. In all cases the levels of 8 chemokines were determined using multiparametric fluorescence analysis with magnetic microspheres (xMAP technology, Luminex 200, USA). Statistical analysis was carried out by methods of non-parametric statistics. To determine the predictive value of the test, ROC analysis was performed.It has been shown that CP is accompanied by increased levels of CXCL1 (5.5-fold), CXCL8 (8.1-fold), CXCL12 (3.5-fold), CCL2 (2.7-fold). At the same time, the level of other chemokines did not change significantly. There was a lack of correlations between individual parameters in the group of patients with CP, in contrast to the control group, thus, probably reflecting disturbed mechanisms of “balance” regulation of chemokines. By means of ROC analysis, highly sensitive biomarkers of chronic mild periodontitis were identified. The diagnostic sensitivity and diagnostic specificity were for CXCL1 (91 and 75%, respectively), CXCL8 and CXCL12 (95 and 75%), CCL2 (82 and 75%). The data obtained indicate that the salivary chemokines CXCL1, CXCL8, CXCL10, CXCL12, CCL2 can be considered potential biomarkers of mild chronic periodontitis

    Properties of resistive hydrogen sensors as a function of additives of 3d-metals introduced in the volume of thin nanocrystalline SnO2 films

    Analysis of the results of studying electrical and gas sensitive characteristics of the molecular hydrogen sensors based on thin nanocrystalline SnO2 films coated with dispersed Au layers and containing Au+Ni and Au+Co impurities in the bulk showed that the characteristics of these sensors are more stable under the prolonged exposure to hydrogen in comparison with Au/SnO2:Sb, Au films modified only with gold. It has been found that introduction of the nickel and cobalt additives increases the band bending at the grain boundaries of tin dioxide already in freshly prepared samples, which indicates an increase in the density Ni of the chemisorbed oxygen. It is important that during testing, the band bending eφs at the grain boundaries of tin dioxide additionally slightly increases. It can be assumed that during crystallization of films under thermal annealing, the 3d-metal atoms in the SnO2 volume partially segregate on the surface of microcrystals and form bonds with lattice oxygen, the superstoichiometric tin atoms are formed, and the density Ni increases. If the bonds of oxygen with nickel and cobalt are stronger than those with tin, then, under the prolonged tests, atomic hydrogen will be oxidized not by lattice oxygen, but mainly by the chemisorbed one. In this case, stability of the sensors’ characteristics increases

    Prospects for cooling of marine organisms with the ice using chitosan and its polyelectrolyte complexes

    A new cooling medium for storage of fish and seafood is proposed - frozen water solution of polysaccharides of natural origin, as the natural biopolymer chitosan. Rational concentration of its solution for producing the chitosan ice is determined experimentally. Organoleptic, physical, and microbiological parameters of the ice prepared from solution of polyelectrolyte complexes of chitosan and its co-polymers are examined. Better results for cooling of marine raw materials are shown for the solution of polyelectrolyte complexes based on chitosan with sodium alginate and carboxymethyl cellulose. The chitosan-alginate ice is distinguished by higher density and homogeneous consistency within the temperature range from -5 to +5оC. Antiseptic microbiological activity of the chitosan-alginate ice is in 2.3 times higher as compared with the water ice and in 1.8 times higher than for the chitosan ice. High technological properties of the ice of polyelectrolyte complexes of chitosan with sodium alginate provide good prospects for its using in technology of cooling in fishery industry

    Evaluating the Efficacy of Synbiotic Mouthwash in Patients with Dyscirculatory Encephalopathy: Observational Clinical Study

    Background. At present, the priority direction of medical science and practice is the prevention of dental diseases, especially in patients with somatic pathology. Patients with dyscirculatory encephalopathy develop a complex of neurological disorders, accompanied by impaired motor functions of hands, lips and chewing muscles, which leads to ineffective cleansing of the oral cavity from food residues and formation of soft and mineralized dental deposits on the dental surface. Therefore, these patients should use additional personal hygiene products.Objective. To evaluate the efficacy of hygienic care in patients with dyscirculatory encephalopathy using mouthwash with synbiotic complex.Methods. An observational clinical study was conducted on the basis of Neurology Unit No. 2 of Research Center of Neurology, Department of Cariology and Endodontics and Department of Biological Chemistry, A.I. Yevdokimov Moscow State Medical and Dental University. The study enrolled 82 patients aged from 45 to 74 years with cerebrovascular disease, dyscirculatory encephalopathy, decompensation (I68.7, according to ICD-10). Patients were divided into two study groups: group 1 (n = 44) — patients who used mouthwash with synbiotic complex; group 2 (n = 38) — patients who did not use mouthwash. The efficacy of mouthwash with synbiotic complex was assessed by using an index score of a simplified Greene Vermillion Oral Hygiene Index (OHI-S), Russell Periodontal Index (PI), Papillary Bleeding Index (PBI), and physicochemical parameters of the oral fluid. Statistical processing of the results was carried out using Stat Plus 7 (Analyst Soft Inc., USA).Results. The comparative analysis proved the efficacy of mouthwash with synbiotic complex. The hygienic status improved to (1.50 ± 0.12) points, the state of periodontal tissues — to (PI = (1.94 ± 1.45) points; PBI = (1.12 ± 0.11) points). The hydrogen index (pH) of the oral fluid decreased to weakly alkaline values — (7.92 ± 0.22) points; the activity of alkaline phosphatase decreased by a factor of 1.4, lactate dehydrogenase — by half, aminotransferases — by a factor of 1.3, and D-dimer — by a factor of 1.2.Conclusion. The obtained results demonstrate the efficacy of mouthwash as an additional means for personal hygiene. Mouthwash with synbiotic complex should be included in the daily oral hygiene care for patients with dyscirculatory encephalopathy

    Serum nesfatin in overweight and obese children

    BACKGROUND: Obesity is one of the most common children diseases. In present time it is being actively studied the natural hormones role in the pathogenesis of obesity. AIMS: To determine the relationship between serum nesfatin level with impaired lipid metabolism in school age children with overweight and obesity. MATERIALS AND METHODS: The study included children with overweight and obesity («case») and healthy children with normal body mass («control») aged 9 to 15 years. All children were determined by concentration in cholesterol blood and its factions, apoA-1, apoB. Enzyme-linked immunosorbent assay (Bio Human Nesfatin Enzyme Immunoassay Kit). RESULTS: Groups of patients with overweight and obesity (n = 53) and healthy children (n = 31) were comparable by age and sex. In obese children, a significant decrease in the level of nesfatin-1 was revealed compared with the group with normal body weight. In cases of children obesity there is a marked decrease in the concentration of nesfatin-1 in serum, especially expressed in the dyslipidemia presence. The nesfatin-1 level are negatively correlated to BMI and body fat content. CONCLUSIONS: The obtained data suggest that along with the known key role of nesfatin in the regulation of eating behavior and appetite, it also affects anthropometric indicators, primarily associated with the accumulation of excess fat mass. Based on the data obtained, nesfatin-1 should be considered as a possible marker for the development of obesity in children

    Сhange in pletelet parameters in patients with liver cirrhosis

    The research is devoted to the study of the platelets parameters in patients with liver cirrhosis (LC). 208 patients was examined. Platelet counts (PLT), mean platelet volume (MPV), platelet distribution width (PDW), plateletcrit (PCT) were assessed in these people. It was established that thrombocytopenia at the initial stage had hyperdestructive type. This pathology in late stages had hypoproductive type. We identified differences in platelet parameters in patients with LC, depending on their severity, may play a crucial role in accurate diagnosis verification, laboratory monitoring and selection of optimal conservative treatment strategies.Данное исследование посвящено изучению тромбоцитарных показателей пациентов при циррозе печени (ЦП) различной степени тяжести. Обследовано 208 человек, у которых оценивали количество тромбоцитов(PLT), средний объем тромбоцитов (MPV), анизоцитоз (PDW) и тромбокрит (PCT). В результате исследования установили, что на начальной стадии цирроза печени тромбоцитопения имела характер гипердеструктивного типа, а уже на поздних стадиях приобрела гипопродуктивный генез. Выявленные различия в тромбоцитарных параметрах у пациентов с ЦП в зависимости от степени тяжести могут играть решающую роль в точной верификации диагноза, в лабораторном мониторинге и подборе оптимальных консервативных стратегий лечения
