    UNIVERSITY INTELLECTUAL CAPITAL FORMATION AND DEVELOPMENT

    Purpose of the study: One of the most important terms to solve the problems of the education system is the educational institution’s intellectual capital, which significantly transforms the role and functions of the modern educator. The purpose of the article is to identify the essence of the University's intellectual capital and to justify the trajectory of its development, due to the needs and capabilities of education stakeholders. Methodology: Based on the methodology of education quality management the article justifies the leading role of quality education as an imperative of the University development. Intellectual capital is considered from the standpoint of organizational resources that determine the cost of the final product – the quality of education and the competitive position of the University; its development is carried out based on the project-target approach. Results: Modern requirements for the intellectual capital of the educational organization are revealed, the role and essence of pedagogical activity of teachers of higher education institutions in its formation are shown. The importance of continuous improvement of hard and soft competencies of University teachers as a way of incrementing intellectual capital is shown. The adaptive model’s design of University teachers’ career strategies based on design-target mechanisms is presented that determines the organizational development of the University. Applications of this study: The results determined the possibility to consider organizational and human knowledge and competence as a special type of investment to improve the functioning of the University. The recommendations for the construction of models of the University intellectual capital management are presented. The article is intended for employees of the education system, educators, researchers, and heads of the University departments. Novelty/Originality of this study: The contribution is made to the theory of the University’s social and cognitive management based on expanding the powers of quality management in the field of intellectual capital management

    Background. Differential diagnosis of exacerbations of allergic diseases in children at pre-hospital and hospital stages of rendering emergency medical care (EMC) remains a relevant issue. One of the reasons is that medical care does not always comply with current clinical guidelines.Objective. Our aim was to analyze the quality of emergency medical care for children with acute allergic pathology.Methods. A retrospective study with a retrospective analysis and extraction of data from medical records (in 2 stages) was conducted.Results. The study analyzed the data of 595 children with acute allergic pathology who applied for emergency medical care (girls — 215; 36.13%). The average age of patients was 43.12 ± 41.09 months, 455 (76.47%) were under 5 years of age. 513 (86.22%) children applied for medical care for the first time, 24 (4.03%) children — for the second time; in 58 (9.75%) emergency team call records, this column was not filled. Based on the complaints and diagnoses indicated in the medical documentation, the children were divided into 3 groups: Croup, Cutaneous Manifestations of Allergy, and Bronchopulmonary Manifestations. We analyzed the reliability and validity of final diagnoses made at pre-hospital and hospital stages and assessed the adequacy of the carried out pharmacotherapy.Conclusion. According to our study, the quality of medical care at pre-hospital and hospital stages does not always correspond to current standards, algorithms, and available clinical guidelines.Дифференциальная диагностика обострений аллергических заболеваний у детей на догоспитальном и госпитальном этапах оказания скорой медицинской помощи (СМП) остается актуальной проблемой. Одна из причин в том, что медицинская помощь не всегда проводится в соответствии с современными клиническими рекомендациями.Цель исследования — проанализировать качество оказания скорой и неотложной медицинской помощи детям с острой аллергической патологией.Методы. Проведено ретроспективное исследование с ретроспективным анализом и выкопировкой данных из медицинской документации (в 2 этапа).Результаты. В исследовании проанализированы данные 595 детей с острой аллергической патологией, обратившихся за скорой медицинской помощью (девочек — 215; 36,13%). Средний возраст пациентов — 43,12±41,09 мес, из них 455 (76,47%) — в возрасте до 5 лет. Первично за медицинской помощью обратились 513 (86,22%) детей, повторно — 24 (4,03%); в 58 (9,75%) картах вызова бригады СМП заполнение данной графы отсутствовало. На основании указанных в медицинской документации жалоб и выставленных диагнозов дети разделены на 3 группы: «Круп», «Кожные проявления аллергии» и «Бронхолегочные проявления». Проанализированы достоверность и обоснованность уточненных диагнозов, выставленных на догоспитальном и госпитальном этапах, оценена адекватность проведенной фармакотерапии.Заключение. По данным нашего исследования, оказание медицинской помощи на догоспитальном и госпитальном этапах не всегда соответствует современным стандартам, алгоритмам и имеющимся клиническим рекомендациям.Конфликт интересовЛ.С.     Clinical Features and Management of the Disease Caused by New Coronaviral Infection (COVID-19) in Children. Version 2

    The Ministry of Health of the Russian Federation jointly with professional association and experts in the field of pediatrics, infectious diseases and resuscitation has revised guidelines “Clinical Features and Management of the Disease Caused by New Coronaviral Infection (COVID-19) in Children” in order to provide the child population with effective medical care during the pandemic of the new coronaviral infection. The practical experience of specialists from various countries was considered during the development of this document. Special attention should be given to the evidence base of the presented data, as well as to the efficiency and safety issues of medications used in treatment of coronaviral infection and its complications. The authors highlight the problems of prevention, diagnostics and management of pathological conditions caused by COVID-19 in the article according to the presented guidelines. Patient’s management is presented depending on the age and severity of the disease itself. The therapy is considered with regard to etiological, pathogenetic and symptom focus

    Amino acid formulas in patients with food allergy

    The article presents modern approaches to the use of balanced formulas for nutrition and diet correction in children with various forms of food allergies. The guidelines are based on all available up to date evidence on the efficacy, safety and utility of using such innovative medical technology as specialized amino acid formulas. This formula is the targeted medical intervention for food allergies and confirmed cow's milk protein allergy, and particularly for patients with reduced physical growth and development (growth rates included). The material is based on methodological guidelines on the amino acid formulas usage previously developed by specialist experts of the Union of pediatricians of Russia in 2020

The authors have conducted the extensive analysis of all available for now evidence on the efficacy, safety and utility of using such innovative medical technology as special elemental formula in gastrointestinal tract pathological conditions.     The Union of Pediatricians of Russia together with the Russian Association of Allergologists and Clinical Immunologists and the Russian Society of Dermatovenerologists and Cosmetologists have developed new clinical guidelines for the urticaria in adults and children. Urticaria is a common disease; its various clinical variants are diagnosed in 15–25% of people in the global population, and a quarter of all cases belongs to chronic urticaria. The prevalence of acute urticaria is 20%, and 2.1–6.7% in child population, whereas acute urticaria is more common in children than in adults. The prevalence of chronic urticaria in adults in the general population is 0.7 and 1.4%, and 1.1% in children under 15 years of age, according to the systematic review and meta-analysis, respectively. This article covers features of epidemiology, etiology, and pathogenesis of the disease with particular focus on differential diagnostic search. Guidelines on treatment and step-by-step therapy scheme (both based on principles of evidencebased medicine) for pediatric patients were presented. Clarification on the analysis of the therapy efficacy and the degree of disease activity was given.Союзом педиатров России совместно с Российской ассоциацией аллергологов и иммунологов и Российским обществом дерматовенерологов и косметологов (РОДВК) разработаны новые клинические рекомендации для диагноза «крапивница» для взрослых и детей. Крапивница является распространенным заболеванием: различные ее клинические варианты диагностируются у 15–25% людей в популяции, при этом четверть случаев приходится на хроническую крапивницу. Распространенность острой крапивницы составляет 20%, среди детского населения — 2,1–6,7%, при этом острая крапивница у детей встречается чаще, чем у взрослых. По данным систематического обзора и мета-анализа, хроническая крапивница у взрослых в общей популяции составляет 0,7 и 1,4% соответственно, у детей до 15 лет — до 1,1%. В статье рассматриваются особенности эпидемиологии, этиологии и патогенеза заболевания, особое внимание уделено вопросам дифференциально-диагностического поиска. Для пациентов детского возраста приведены рекомендации по лечению согласно принципам доказательной медицины и предложена ступенчатая схема терапии. Дано четкое разъяснение к проведению анализа эффективности терапии и оценки степени активности заболевания

При разработке документа был учтен практический опыт не только специалистов нашей страны, но и зарубежных коллег. Особое внимание уделено доказательной базе представленных данных, а также вопросам эффективности и безопасности лекарственных препаратов, применяемых при лечении данной инфекции и ее осложнений. В статье на основании указанных методических рекомендаций авторы освещают вопросы профилактики, диагностики, лечения патологических состояний, обусловленных COVID-19. Тактика ведения пациента представлена в зависимости от возраста и степени тяжести течения болезни, терапия рассмотрена с позиций этиологической, патогенетической и симптоматической направленности
