    Incorporating machine reliability issue and backlogging into the EMQ model - Part I: Random breakdown occurring in backorder filling time

    This study is concerned with determination of the optimal replenishment policy for economic manufacturing quantity (EMQ) model with backlogging and machine reliability issue. Classic EMQ model does not consider nonconforming items generated during a production cycle, nor does it deal with the machine breakdown situation. It is noted that in manufacturing system when back-ordering is permitted, a random machine failure can take place in either backorder filling time or in on-hand inventory piling period. The first phase of this study examines the aforementioned practical issues by incorporating rework process of defective items, scrap and random machine failure taking place specifically in backorder satisfying time into the EMQ model. The objective is to determine the optimal replenishment lot-size that minimizes the overall production-inventory costs. Mathematical modelling and analysis is used and the renewal reward theorem is employed to cope with the variable cycle length. Theorem on conditional convexity of total cost function is proposed and proved. The optimal lot size for such a real-life imperfect manufacturing system is derived. A numerical example is given to demonstrate its practical usage

    Relationship between Obesity-related Hormone Peptides and Quality of Life in Obese Women among Different Traditional Chinese Medicine Syndrome Groups

    AbstractThe aim of this study was to explore the relationship between obesity-related hormone peptides and quality of life in obese women among different traditional Chinese medicine (TCM) syndrome groups (證型 zhèng xíng). 260 obese women met with age between 20 and 65years old and body mass index (BMI) ≧ 27kg/m2, were recruited. The participants filled out a questionnaire on obese TCM syndrome groups, which was designed by professional TCM doctors, and two questionnaires on quality of life (QOL), WHOQOL-BREF Taiwan version and MOS Short Form-12 (SF-12). Data of biochemical characteristics and obesity-related hormone peptides were collected at the same time. According to the responses provided, the obese subjects were classified into spleen deficiency with dampness encumbrance syndrome (脾虛濕阻證 pí xū shī zǔ zhèng; SDD), stomach heat with dampness encumbrance syndrome (胃熱濕阻證 wèi rè shī zǔ zhèng; SHD), liver depression and qi stagnation syndrome (肝鬱氣滯證 gān yù qì zhì zhèng; LDQ), dual spleen-kidney deficiency syndrome (脾腎兩虛證 pí shèn liǎng xū zhèng; SKD), yin deficiency with internal heat syndrome (陰虛內熱證 yīn xū nèi rè zhèng; YDI) and a control group. For physical conditions, SDD group had significantly higher means in weight and BMI compared with the control group. The insulin and leptin levels in SHD group were significantly higher than those in the control group. The LDQ group showed marked decrease in mental condition scores compared with the control group. This study found that obese women in the SDD group were fatter than those in the control group. SHD group might have greater influence on the regulation of obesity-related hormone peptides. The LDQ group had poor QOL than the control group. Analysis of TCM syndrome groups among obese women merits further investigation

    Nanoscale Zero-Valent Iron for Sulfide Removal from Digested Piggery Wastewater

    The removal of dissolved sulfides in water and wastewater by nanoscale zero-valent iron (nZVI) was examined in the study. Both laboratory batch studies and a pilot test in a 50,000-pig farm were conducted. Laboratory studies indicated that the sulfide removal with nZVI was a function of pH where an increase in pH decreased removal rates. The pH effect on the sulfide removal with nZVI is attributed to the formation of FeS through the precipitation of Fe(II) and sulfide. The saturated adsorption capacities determined by the Langmuir model were 821.2, 486.3, and 359.7 mg/g at pH values 4, 7, and 12, respectively, for nZVI, largely higher than conventional adsorbents such as activated carbon and impregnated activated carbon. The surface characterization of sulfide-laden nZVI using XPS and TGA indicated the formation of iron sulfide, disulfide, and polysulfide that may account for the high adsorption capacity of nZVI towards sulfide. The pilot study showed the effectiveness of nZVI for sulfide removal; however, the adsorption capacity is almost 50 times less than that determined in the laboratory studies during the testing period of 30 d. The complexity of digested wastewater constituents may limit the effectiveness of nZVI. Microbial analysis suggested that the impact of nZVI on the change of microbial species distribution was relatively noticeable after the addition of nZVI

Further inquiries can be directed to the corresponding author.To address inborn errors of immunity (IEI) which were underdiagnosed in resource-limited regions, our centre developed and offered free genetic testing for the most common IEI by Sanger sequencing (SS) since 2001. With the establishment of The Asian Primary Immunodeficiency (APID) Network in 2009, the awareness and definitive diagnosis of IEI were further improved with collaboration among centres caring for IEI patients from East and Southeast Asia. We also started to use whole exome sequencing (WES) for undiagnosed cases and further extended our collaboration with centres from South Asia and Africa. With the increased use of Next Generation Sequencing (NGS), we have shifted our diagnostic practice from SS to WES. However, SS was still one of the key diagnostic tools for IEI for the past two decades. Our centre has performed 2,024 IEI SS genetic tests, with in-house protocol designed specifically for 84 genes, in 1,376 patients with 744 identified to have disease-causing mutations (54.1%). The high diagnostic rate after just one round of targeted gene SS for each of the 5 common IEI (X-linked agammaglobulinemia (XLA) 77.4%, Wiskott–Aldrich syndrome (WAS) 69.2%, X-linked chronic granulomatous disease (XCGD) 59.5%, X-linked severe combined immunodeficiency (XSCID) 51.1%, and X-linked hyper-IgM syndrome (HIGM1) 58.1%) demonstrated targeted gene SS should remain the first-tier genetic test for the 5 common X-linked IEI.The Hong Kong Society for Relief of Disabled Children and Jeffrey Modell Foundation.http://www.frontiersin.org/Immunologyam2023Paediatrics and Child Healt

    Acute increase of children's conjunctivitis clinic visits by Asian dust storms exposure - a spatiotemporal study in Taipei, Taiwan.

    Adverse health impacts of Asian dust storms (ADS) have been widely investigated and discussed in respiratory disease, but no study has examined the association between ADS events and their impact on eye diseases, especially in children. The impact of ADS events on the incidence of children's conjunctivitis is examined by analyzing the data from children's clinic visits registered in the 41 districts of Taipei area in Taiwan during the period 2002-2007. The structural additive regression modeling approach was used to assess the association between ADS events and clinic visits for conjunctivitis in children with consideration of day-of-the-week effects, temperature, and air quality levels. This study identifies an acute increase in the relative rate for children's conjunctivitis clinic visits during ADS periods with 1.48% (95% CI = 0.79, 2.17) for preschool children (aged <6 years old) and 9.48% (95% CI = 9.03, 9.93) for schoolchildren (aged ≥6 years old), respectively. The relative rates during post-ADS periods were still statistically significant, but much lower than those during ADS periods. The spatial analysis presents geographic heterogeneity of children's conjunctivitis clinic visits where higher relative rates were more likely observed in the most populated districts Compared to previous ADS studies related to respiratory diseases, our results reveals significantly acute impacts on children's conjunctivitis during ADS periods, and much influence on schoolchildren. Vulnerable areas were also identified in high density population

    Risk Factors of Descending Necrotizing Mediastinitis in Deep Neck Abscesses

    Background and Objectives: Cervical space infection could also extend to the mediastinum due to the anatomical vicinity. The mortality rate of descending necrotizing mediastinitis is 85% if untreated. The aim of this study was to identify risk factors for the progression of deep neck abscesses to descending necrotizing mediastinitis. Materials and Methods: We retrospectively reviewed the medical records of patients undergoing surgical treatment of deep neck abscesses from August 2017 to July 2022. Computed tomography (CT) was performed in all patients. Before surgery, lab data including hemoglobulin (Hb), white blood cell count, neutrophil percentage, C-reactive protein (CRP) level, and blood glucose were recorded. Patients’ characteristics including gender, age, etiology, and presenting symptoms were collected. Hospitalization duration and bacterial cultures from the wound were also analyzed. Results: The C-reactive protein (CRP) level was higher in patients with a mediastinal abscess than in patients without a mediastinal abscess (340.9 ± 33.0 mg/L vs. 190.1 ± 72.7 mg/L) (p = 0.000). The submandibular space was more commonly affected in patients without a mediastinal abscess (p = 0.048). The retropharyngeal (p = 0.003) and anterior visceral (p = 0.006) spaces were more commonly affected in patients with a mediastinal abscess. Conclusions: Descending necrtotizing mediastinitis results in mortality and longer hospitalization times. Early detection of a mediastinal abscess on CT is crucial for treatment. Excluding abscesses of the anterior superior mediastinum for which transcervical drainage is sufficient, other mediastinal abscesses require multimodal treatment including ENT and thoracic surgery to achieve a good outcome
