    Kinematic Basis of Emergent Energetics of Complex Dynamics

    Stochastic kinematic description of a complex dynamics is shown to dictate an energetic and thermodynamic structure. An energy function φ(x)\varphi(x) emerges as the limit of the generalized, nonequilibrium free energy of a Markovian dynamics with vanishing fluctuations. In terms of the φ\nabla\varphi and its orthogonal field γ(x)φ\gamma(x)\perp\nabla\varphi, a general vector field b(x)b(x) can be decomposed into D(x)φ+γ-D(x)\nabla\varphi+\gamma, where (ω(x)γ(x))=\nabla\cdot\big(\omega(x)\gamma(x)\big)= ωD(x)φ-\nabla\omega D(x)\nabla\varphi. The matrix D(x)D(x) and scalar ω(x)\omega(x), two additional characteristics to the b(x)b(x) alone, represent the local geometry and density of states intrinsic to the statistical motion in the state space at xx. φ(x)\varphi(x) and ω(x)\omega(x) are interpreted as the emergent energy and degeneracy of the motion, with an energy balance equation dφ(x(t))/dt=γD1γbD1bd\varphi(x(t))/dt=\gamma D^{-1}\gamma-bD^{-1}b, reflecting the geometrical Dφ2+γ2=b2\|D\nabla\varphi\|^2+\|\gamma\|^2=\|b\|^2. The partition function employed in statistical mechanics and J. W. Gibbs' method of ensemble change naturally arise; a fluctuation-dissipation theorem is established via the two leading-order asymptotics of entropy production as ϵ0\epsilon\to 0.     Noninvasive prediction of Blood Lactate through a machine learning-based approach.

    We hypothesized that blood lactate concentration([Lac]blood) is a function of cardiopulmonary variables, exercise intensity and some anthropometric elements during aerobic exercise. This investigation aimed to establish a mathematical model to estimate [Lac]blood noninvasively during constant work rate (CWR) exercise of various intensities. 31 healthy participants were recruited and each underwent 4 cardiopulmonary exercise tests: one incremental and three CWR tests (low: 35% of peak work rate for 15 min, moderate: 60% 10 min and high: 90% 4 min). At the end of each CWR test, venous blood was sampled to determine [Lac]blood. 31 trios of CWR tests were employed to construct the mathematical model, which utilized exponential regression combined with Taylor expansion. Good fitting was achieved when the conditions of low and moderate intensity were put in one model; high-intensity in another. Standard deviation of fitting error in the former condition is 0.52; in the latter is 1.82 mmol/liter. Weighting analysis demonstrated that, besides heart rate, respiratory variables are required in the estimation of [Lac]blood in the model of low/moderate intensity. In conclusion, by measuring noninvasive cardio-respiratory parameters, [Lac]blood during CWR exercise can be determined with good accuracy.     Major Complications and Associated Risk Factors of Transrectal Ultrasound Guided Prostate Needle Biopsy: A Retrospective Study of 1875 Cases in Taiwan

    Background/PurposeComplications from transrectal ultrasound (TRUS) guided prostate needle biopsy are occasionally encountered in the daily practice of urologists. We tried to determine the associated risk factors of patients who suffered from major complications that required hospitalization after TRUS guided prostate needle biopsies.MethodsWe did a retrospective review of 1875 TRUS guided prostate biopsies performed between January 2002 and December 2005. We defined major complications as patients with complications that needed hospitalization. We analyzed the association between biopsy complications and suspected factors, including age, prostate volume, patient's underlying disease, selection of prophylactic antibiotics, biopsy core numbers (6, 12, and 15 cores), and antiplatelet/anticoagulant usage.ResultsThere were 124 patients (6.6%) with major complication. These major complications were categorized as acute prostatitis (3.8%), acute urinary retention (2.1%), hematuria (1.9%), rectal bleeding (0.2%), epididymitis (0.2%), sepsis (0.05%), and vasovagal syncope (0.05%). Patients with larger prostate size were noted to have higher risk of developing transient acute prostatitis and acute urinary retention after prostate biopsy. In contrast, age, prophylactic antibiotics (levofloxacin and pipemidic acid), underlying diseases (diabetic mellitus, hypertension, hyperlipidemia, cerebrovascular accident, coronary artery disease), increased biopsy core numbers, and antiplatelet/anticoagulant usage were not associated with major complications after prostate biopsy.ConclusionTRUS guided prostate needle biopsy is a safe diagnostic tool in most elderly males with or without systemic underlying disease

    Emotion Detection in Textual Information by Semantic Role Labeling and Web Mining Techniques

    Automatic emotion detection in textual information is critical for the development of intelligent interfaces in many interactive multimedia applications. In the literature, existing approaches based on keyword spotting or statistic natural language process techniques, have limited success rate in free text emotion sensing applications. In this paper, we describe a system, developed in the framework of the National ChiNan University and LORIA collaboration, that associates semantic labeling and web mining techniques, to detect several basic emotions. A common sense knowledgebase – ConceptNet – is also used in order to retrieve some additional contextual information that can be used to retrieve appropriate background images for the presentation.     Automatic event-level textual emotion sensing using mutual action histogram between entities

    International audienceAutomatic emotion sensing in textual data is crucial for the development of intelligent interfaces in many interactive computer applications. This paper describes a high-precision, knowledgebase-independent approach for automatic emotion sensing for the subjects of events embedded within sentences. The proposed approach is based on the probability distribution of common mutual actions between the subject and the object of an event. We have incorporated web-based text mining and semantic role labeling techniques, together with a number of reference entity pairs and hand-crafted emotion generation rules to realize an event emotion detection system. The evaluation outcome reveals a satisfactory result with about 85% accuracy for detecting the positive, negative and neutral emotions

    Malignant pleural mesothelioma (MPM), a rare malignancy with a poor prognosis, is mainly caused by exposure to asbestos or other organic fibers, but the underlying genetic mechanism is not fully understood. Genetic alterations and causes for multiple primary cancer development including MPM are unknown. We used whole exome sequencing to identify somatic mutations in a patient with MPM and two additional primary cancers who had no evidence of venous, arterial, lymphovascular, or perineural invasion indicating dissemination of a primary lung cancer to the pleura. We found that the MPM had R282W, a key TP53 mutation, and genome-wide allelic loss or loss of heterozygosity, a distinct genomic alteration not previously described in MPM. We identified frequent inactivating SETDB1 mutations in this patient and in 68 additional MPM patients (mutation frequency: 10%, 7/69) by targeted deep sequencing. Our observations suggest the possibility of a new genetic mechanism in the development of either MPM or multiple primary cancers. The frequent SETDB1 inactivating mutations suggest there could be new diagnostic or therapeutic options for MPM

    The influence of serotonin transporter polymorphisms on cortical activity: A resting EEG study

    <p>Abstract</p> <p>Background</p> <p>The serotonin transporter gene (<it>5-HTT</it>) is a key regulator of serotonergic neurotransmission and has been linked to various psychiatric disorders. Among the genetic variants, polymorphisms in the <it>5-HTT </it>gene-linked polymorphic region (<it>5-HTTLPR</it>) and variable-number-of-tandem-repeat in the second intron (<it>5-HTTVNTR</it>) have functional consequences. However, their genetic impact on cortical oscillation remains unclear. This study examined the modulatory effects of <it>5-HTTLPR </it>(L-allele carriers vs. non-carriers) and <it>5-HTTVNTR </it>(10-repeat allele carriers vs. non-carriers) polymorphism on regional neural activity in a young female population.</p> <p>Methods</p> <p>Blood samples and resting state eyes-closed electroencephalography (EEG) signals were collected from 195 healthy women and stratified into 2 sets of comparisons of 2 groups each: L-allele carriers (<it>N </it>= 91) vs. non-carriers for <it>5-HTTLPR </it>and 10-repeat allele carriers (<it>N </it>= 25) vs. non-carriers for <it>5-HTTVNTR</it>. The mean power of 18 electrodes across theta, alpha, beta, gamma, gamma1, and gamma2 frequencies was analyzed. Between-group statistics were performed by an independent t-test, and global trends of regional power were quantified by non-parametric analyses.</p> <p>Results</p> <p>Among <it>5-HTTVNTR </it>genotypes, 10-repeat allele carriers showed significantly low regional power at gamma frequencies across the brain. We noticed a consistent global trend that carriers with low transcription efficiency of 5-HTT possessed low regional powers, regardless of frequency bands. The non-parametric analyses confirmed this observation, with <it>P </it>values of 3.071 × 10<sup>-8 </sup>and 1.459 × 10<sup>-12 </sup>for <it>5-HTTLPR </it>and <it>5-HTTVNTR</it>, respectively.</p> <p>Conclusions and Limitations</p> <p>Our analyses showed that genotypes with low 5-HTT activity are associated with less local neural synchronization during relaxation. The implication with respect to genetic vulnerability of 5-HTT across a broad range of psychiatric disorders is discussed. Given the low frequency of 10-repeat allele of <it>5-HTTVNTR </it>in our research sample, the possibility of false positive findings should also be considered.</p

    Prognosis of Nasopharyngeal Carcinoma in the Elderly is Worse than in Younger Individuals–Experience of a Medical Institute

    SummaryPurposeWe aimed to evaluate outcomes of the elderly (>65 years) by comparing with younger (<40 years) patients after treatments for nasopharyngeal carcinoma (NPC).Materials and methodsWe retrospectively obtained clinical data from charts for 23 older and 21 younger patients in whom NPC was diagnosed and who underwent curative managements during 2007 and 2011. Occurrence of local recurrence, distant metastasis, and death from any cause were recorded as endpoints. Cox proportional hazards regression was applied to determine age effects on survival risks after adjusting for the potential confounders.ResultsOlder patients more commonly received a diagnosis of chronic diseases than the younger patients (56.5% versus 23.8%, p = 0.036), whereas they were less likely to have received intensive treatments for NPC. After adjusting for medical history and neoadjuvant chemotherapy, older age was the only significant predictor in the study cohort for overall survival and progression-free survival. The adjusted hazard ratio (HR) for death from all causes in older patients was 6.3 (95% confidence interval [CI] = 1.3–30.2), and the adjusted HR for disease progression in older patients was 10.9 (95% CI = 2.3–50.6).ConclusionAging was the only independent prognostic risk factor in this study cohort. Medical history and treatment variations could not fully explain the difference in prognosis. Our results strengthen the need to ameliorate toxicities and improve supportive care for older patients with a diagnosis of NPC
