    \u27Phenomenal Woman\u27: A Phenomenological Exploration of the \u27Voices\u27 and Experiences of My Sistas\u27

    This research study provided a descriptive picture of educational experiences of three sets of Black sisters, including the researcher, who grew up in inner-city Knoxville. This study was not designed to be the “voice” of all Black women. It was designed to show the universal essence of the experience of Black inner-city women living in a similar socioeconomic and cultural context. A qualitative existential-phenomenological research study design was used to derive common themes that represented the universal essence of the participants’ experiences. Individual interviews were conducted for all participants and an existential-phenomenological analysis provided the common meaning underlying the stories we shared about our educational experiences. The findings derived from the data analysis are presented in themes that provided a descriptive picture of lived-experiences of the participants. This study illuminated the formal and informal learning experiences of Black women and the profound role cultural experiences of family and community play in shaping their learning experiences through adulthood. The common ground of the educational experiences shared by women in the study was “Learning As You Go” which served as a backdrop for the experiences (themes). The three themes are as follows: I Seemed Like a Stupid Little Girl, Again/I Was This Smart Intelligent Woman; Staying in the Box/Stepping Out of the Box; They Taught Us the Values/and You Can’t Let People Keep You Down. Findings suggest that the educational experiences of Black women influenced our development and our sense of voice, self-esteem, and self-defined achievement and success. These findings dispute adult development theories that suggest learning or development must precede or trail the other. Recommendations and implications are provided to educators, specifically to adult educators, that might benefit from a deeper understanding about how to provide positive educational experiences for Black females. Since minimal research on the relationship between adult development and educational experiences of Black women exists with regards to race, culture, gender and class, this study should also help create new integrated theoretical approaches to adult development that go beyond traditional theories, as well as contribute to the practice of adult education

    Molecular Gas and Star Formation in the Host Galaxy of I Zw 1

    A recent analysis of high angular resolution NIR imaging and spectroscopic data in conjunction with Plateau de Bure interferometric mm-line observations indicate the presence of a circum-nuclear starburst ring of about 1.5 arcsec (1.5 kpc) diameter in I Zw 1. High angular resolution NIR imaging, using the MPE SHARP camera at the ESO NTT, HST V-band images, as well as NIR spectroscopy with MPE 3D provide an improved analysis of the star formation activity in the disk and nucleus of I Zw 1.     Functional recovery after bilateral extended autologous latissimus dorsi breast reconstruction : a prospective observational study

    Background: The impact of unilateral extended autologous latissimus dorsi (EALD) flap harvest and axillary surgery on shoulder function has been well described, but the impact of bilateral EALD flap harvest has not been clearly defined nor is it clear whether reconstructions should be synchronous or staged. Methods: In this prospective observational study, patients undergoing bilateral EALD breast reconstruction (February 2003–December 2009) completed the disability, arm, shoulder and hand (DASH) questionnaire preoperatively and at five post-operative timepoints. Intensive shoulder physiotherapy was offered to those whose DASH score was >30 at 6 weeks or >20 at 12 weeks post-operatively. Results: Sixty patients underwent bilateral EALD flap breast reconstruction (51 synchronous, 9 metachronous). Patients with pre-existing shoulder pathology (n = 1) and those who failed to return any post-operative DASH questionnaire (n = 10) were excluded from initial DASH analysis. However, these eleven patients were included in a separate analysis as an intention-to-treat analysis. Statistical analysis was performed using non-parametric, Friedman test and multiple comparison model. Forty-nine patients’ DASH scores were analysed. DASH score initially increased after surgery and then returned to functionally normal within 3–6 months (median DASH: preoperative = 1 vs 6 weeks post-operation = 26, p = <0.001; vs 3 months = 19, p = <0.001; vs 6 months = 13, p = <0.001); thereafter, the scores remained less than 12 (p = <0.001). Median DASH score after synchronous reconstruction was not higher than that after metachronous reconstructions, although the metachronous sample size was small. Conclusion: With appropriate patient selection and intensive physiotherapy, bilateral EALD breast reconstruction does not appear to cause significant long-term impairment of shoulder function, and patients can now be counselled about the likely timecourse of shoulder recovery. There seems no reason to stage bilateral reconstruction to reduce shoulder morbidity

    Immunogenicity and safety of three consecutive production lots of the non replicating smallpox vaccine MVA: A randomised, double blind, placebo controlled phase III trial

    <div><p>Background</p><p>Modified Vaccinia Ankara (MVA) is a live, viral vaccine under advanced development as a non-replicating smallpox vaccine. A randomised, double-blind, placebo-controlled phase III clinical trial was conducted to demonstrate the humoral immunogenic equivalence of three consecutively manufactured MVA production lots, and to confirm the safety and tolerability of MVA focusing on cardiac readouts.</p><p>Methods</p><p>The trial was conducted at 34 sites in the US. Vaccinia-naïve adults aged 18-40 years were randomly allocated to one of four groups using a 1:1:1:1 randomization scheme. Subjects received either two MVA injections from three consecutive lots (Groups 1-3), or two placebo injections (Group 4), four weeks apart. Everyone except personnel involved in vaccine handling and administration was blinded to treatment. Safety assessment focused on cardiac monitoring throughout the trial. Vaccinia-specific antibody titers were measured using a Plaque Reduction Neutralization Test (PRNT) and an Enzyme-Linked Immunosorbent Assay (ELISA). The primary immunogenicity endpoint was Geometric Mean Titers (GMTs) after two MVA vaccinations measured by PRNT at trial visit 4. This trial is registered with ClinicalTrials.gov, number NCT01144637.</p><p>Results</p><p>Between March 2013 and May 2014, 4005 subjects were enrolled and received at least one injection of MVA (n = 3003) or placebo (n = 1002). The three MVA lots induced equivalent antibody titers two weeks after the second vaccination, with seroconversion rates of 99·8% (PRNT) and 99·7% (ELISA). Overall, 180 (6·0%) subjects receiving MVA and 29 (2·9%) subjects in the placebo group reported at least one unsolicited Adverse Event (AE) that was considered trial-related. Vaccination was well tolerated without significant safety concerns, particularly regarding cardiac assessment.</p><p>Conclusions</p><p>The neutralizing and total antibody titers induced by each of the three lots were equivalent. No significant safety concerns emerged in this healthy trial population, especially regarding cardiac safety, thus confirming the excellent safety and tolerability profile of MVA.</p><p>Trial registration</p><p>ClinicalTrials.gov <a href="https://clinicaltrials.gov/ct2/show/NCT01144637" target="_blank">NCT01144637</a></p></div
