    综合护理干预提高慢性乙型病毒性肝炎自我保健行为能力

    Objectives: To explore suitable long-term antiviral therapy and comprehensive nursing intervention to patients with chronic hepatitis B system. Methods: To treat 136 cases of chronic hepatitis B patients with comprehensive nursing intervention and to investigate their regular medication situation, bad habits, self-observation and illness review visits before the intervention and after one month, three months and six months. Results: Regular medication situation, bad habits, self-observation and illness review visits of the patients who were treated with comprehensive nursing intervention have improved, and differences were statistically significant (P <0.05).Conclusion: Comprehensive nursing intervention can improve compliance, rehabilitation, knowledge level and life quality of the hepatitis B patients who suffered from long-term medication treatment.目的 探讨一种适合长期抗病毒治疗的慢性乙型病毒性肝炎患者的系统、统合的护理干预措施。方法 对136例慢性乙型病毒性肝炎初治患者进行综合护理干预,观察干预前、干预后1个月、3个月、半年时患者的规律服药情况、改变不良生活习惯、正确疾病自我观察及坚持疾病复查与回访等情况。结果 通过综合护理干预后患者规律服药情况、改变不良生活习惯、正确疾病自我观察及坚持疾病复查与回访等情况比较差异均有明显的统计学意义(P<0.05)。结论 对于需长期服药治疗的慢性乙型病毒性肝炎患者,通过向患者及家属开展系统的综合性的护理干预,可以提高患者治疗依从性,促进患者康复,提高患者治疗慢性乙型病毒性肝炎的知识掌握程度,改善生活质量

    Investigating the topological structure of quenched lattice QCD with overlap fermions by using multi-probing approximation

    The topological charge density and topological susceptibility are determined by multi-probing approximation using overlap fermions in quenched SU(3) gauge theory. Then we investigate the topological structure of the quenched QCD vacuum, and compare it with results from the all-scale topological density, the results are consistent. Random permuted topological charge density is used to check whether these structures represent underlying ordered properties. Pseudoscalar glueball mass is extracted from the two-point correlation function of the topological charge density. We study 33 ensembles of different lattice spacing aa with the same lattice volume 163×3216^{3}\times32, the results are compatible with the results of all-scale topological charge density, and the topological structures revealed by multi-probing are much closer to all-scale topological charge density than that by eigenmode expansion.Comment: 12 pages,34 figure

    Efficacy of pegylated interferon α2a in patients without HBeAg loss after the withdrawal of long-term lamivudine therapy

    BACKGROUND: Improving the HBe seroconversion rate of patients without HBeAg loss after long-term lamivudine therapy has become an urgent clinical problem that we have to face. Unfortunately, there is no consensus on the mananement of these patients. The aim of this study was to evaluate the efficacy of pegylated interferon (PEG-IFN) α2a in patients without HBeAg loss after the withdrawal of long-term lamivudine therapy. METHODS: Fifty patients with chronic hepatitis B without the loss of HBeAg after ≥96 weeks of lamivudine treatment were enrolled to withdraw from treatment to induce a biochemical breakthrough. Patients who achieved a biochemical breakthrough within 24 weeks received 48-weeks of PEG-IFN α2a therapy, and were then assessed during a subsequent 24-week follow-up period. RESULTS: Forty-three (86.0%) patients achieved a biochemical breakthrough within 24 weeks of lamivudine withdrawal. The rates of combined response (both undetectable HBV DNA and HBeAg loss) and HBsAg loss were alone 51.2% and 20.9%, respectively after 48 weeks of PEG-IFN α2a therapy, and 44.2% and 18.6%, respectively, at 24 weeks after treatment cessation. The end-of-treatment combined response rate was 65.4% among patients with a baseline HBsAg <20,000 IU/mL, which was significantly higher than 29.4% of patients with HBsAg ≥20,000 IU/mL (P=0.031). For patients with HBsAg levels <1,500 IU/mL at 12 and 24 weeks therapy, the end-of-treatment combined response rate was 68.2% and 69.0%, which were both significantly higher than patients with HBsAg ≥1,500 IU/mL (33.3% and 14.3%; P=0.048 and 0.001). The end-of-treatment combined response rate was significantly higher among patients with HBV DNA<10(5) copies/mL (76.2%) compared to patients with HBV DNA ≥10(5) copies/mL (27.3%) after 24 weeks of therapy (P=0.004). CONCLUSION: Retreatment with PEG-IFN α2a was effective and safe for patients without HBeAg loss after the withdrawal of long-term lamivudine therapy. HBsAg levels at the baseline, 12 and 24 weeks of therapy, and HBV DNA levels at 24 weeks of therapy, can predict the effect of PEG-IFN α2a after 48 weeks of therapy

The sec-butyl­amino group is disordered over two positions, with refined site-occupancy factors of 0.730 (4) and 0.270 (4). The compound could potentially be ligand stabilized in the solid state in a keto–enamine tautomeric form. The amine functionality is involved in an intra­molecular N—H⋯O hydrogen bond, while weak inter­molecular C—H⋯O and C—H⋯N hydrogen bonds participate in the formation of the crystal structure

    The adsorption of Reactive Orange 16 and Basic Blue 3 from aqueous solution using quaternized blue swimmer crab carapace

    The crab carapace is a waste which cannot be decomposed. This waste was used to remove the Reactive Orange 16 (RO16) and Basic Blue 3 (BB3) from aqueous solution at different operational parameters such as pH, mass load, the concentrations of dye and the temperature. The crab collected was modified to obtain quaternized crab (QC) using (3-chloro-2-hydroxypropyl) trimethylammonium chloride solution (C6H15Cl2NO, 65% w/w in water). The pH of the dyes solution was varied from pH 4 to 10. The highest adsorption percentage was achieved at pH 7 for both dyes. Increasing the QC mass for the adsorption process had granted an increase of dyes removal percentage. The highest adsorption percentage was achieved at 91.00% for RO16 and 29.40% for BB3 dyes with 7.5 g/L QC used. However, the adsorption capacity of QC decreased with higher QC mass because the dye molecules occupied on the surface and prevented other molecules to diffuse into the QC. At higher concentration beyond 20 mg/L and 10 mg/L of RO16 and BB3, respectively, the maximum adsorption was achieved at 2.5362 mg/g and 0.6812 mg/g. The adsorption of both dyes by QC was best fitted using Langmuir isotherm model, explaining the adsorption mainly occurred as a single layer on the surface of QC. Comparison to the results obtained from the kinetic models, the adsorption was chemisorption in nature. According to the thermodynamic studies, the adsorption of RO16 was an exothermic, while BB3 was an endorthermic process

    Luspatercept Enhances Hemoglobin Levels in a Chinese Boy With Congenital Sideroblastic Anemia: A Case Report

    BACKGROUND: Congenital sideroblastic anemia (CSA) is a rare and heterogeneous group of genetic disorders. Conventional treatment include pyridoxine (vitamin B6) and allogeneic hematopoietic stem cell transplantation (allo-HSCT), and can alleviate anemia in the majority of cases. Nevertheless, some CSA cases remain unresponsive to pyridoxine or are unable to undergo allo-HSCT. Novel management approaches is necessary to be developed. To explore the response of luspatercept in treating congenital sideroblastic anemia. CASE SUMMARY: We share our experience in luspatercept in a 4-year-old male patient with CSA. Luspatercept was administered subcutaneously at doses of 1.0 mg/kg/dose to 1.25 mg/kg/dose every 3 wk, three consecutive doses, evaluating the hematological response. Luspatercept leading to a significant improvement in the patient\u27s anemia. The median hemoglobin during the overall treatment with three doses of luspatercept was 90 (75-101) g/L, the median absolute reticulocyte count was 0.0593 (0.0277-0.1030) × 10 CONCLUSION: We believe that luspatercept might emerge as a viable therapeutic option for the maintenance treatment of CSA or as a bridging treatment option before hematopoietic stem cell transplantation

    Dendritic cell subsets dynamics and cytokine production in SIVmac239-infected Chinese rhesus macaques

    BACKGROUND: Several studies have demonstrated that SIV infection progresses more slowly to experimental AIDS in Chinese rhesus macaques (Ch Rhs) than in Indian rhesus macaques (Ind Rhs). Here we investigated the dynamic and functional changes in dendritic cell (DC) subsets in SIVmac239-infected Ch Rhs. RESULTS: The numbers of both mDC and pDC strongly fluctuated but were not significantly changed during the acute and chronic phases of infection. However, the concentration of both poly (I:C)-induced IL-12 and HSV-1-induced IFN-α significantly increased in the acute phase of infection but returned to normal levels at the chronic phase of infection. The peak of IFN-α emerged earlier than that of IL-12, and it had a significantly positive correlation with IL-12, which indicated that IFN-α may initiate the immune activation. We also found that only the concentration of IFN-α was positively correlated with CD4+ T-cell counts, but it was negatively correlated with viral load. CONCLUSION: High levels of IFN-α in the early stage of infection may contribute to effective control of virus replication, and normal levels of IFN-α during chronic infection may help Ch Rhs resist the disease progression. The change in DC subsets dynamics and cytokine production may help further our understanding of why Ch Rhs are able to live longer without progressing to an AIDS-like illness

    Gene Responses to Oxygen Availability in Kluyveromyces lactis: an Insight on the Evolution of the Oxygen-Responding System in Yeast

    The whole-genome duplication (WGD) may provide a basis for the emergence of the very characteristic life style of Saccharomyces cerevisiae—its fermentation-oriented physiology and its capacity of growing in anaerobiosis. Indeed, we found an over-representation of oxygen-responding genes in the ohnologs of S. cerevisiae. Many of these duplicated genes are present as aerobic/hypoxic(anaerobic) pairs and form a specialized system responding to changing oxygen availability. HYP2/ANB1 and COX5A/COX5B are such gene pairs, and their unique orthologs in the ‘non-WGD’ Kluyveromyces lactis genome behaved like the aerobic versions of S. cerevisiae. ROX1 encodes a major oxygen-responding regulator in S. cerevisiae. The synteny, structural features and molecular function of putative KlROX1 were shown to be different from that of ROX1. The transition from the K. lactis-type ROX1 to the S. cerevisiae-type ROX1 could link up with the development of anaerobes in the yeast evolution. Bioinformatics and stochastic analyses of the Rox1p-binding site (YYYATTGTTCTC) in the upstream sequences of the S. cerevisiae Rox1p-mediated genes and of the K. lactis orthologs also indicated that K. lactis lacks the specific gene system responding to oxygen limiting environment, which is present in the ‘post-WGD’ genome of S. cerevisiae. These data suggested that the oxygen-responding system was born for the specialized physiology of S. cerevisiae
