    A preliminary study of a cloth guiding mechanism for automatic sewing system

    Purpose - In the trend from mass production to mass customization, more flexible production systems are required. In the clothing field, many studies about automatization of sewing processes have been done into producing small amounts of various kinds of products. The purpose of this paper is to propose a versatile guiding mechanism of a cloth for an automatic sewing system. Design/methodology/approach - Real sewing processes were referenced for the mechanism, and curved stitch is formed holding a point on a cloth. This mechanism consists of a solenoid for holding a cloth and a roller to prevent deformation of the cloth. When a cloth is sewn with the mechanism, the trajectory of the stitch is unstable because of anisotropy of a cloth. A precise trajectory was obtained by adding a device to control the pressure of the roller for holding a cloth and keeping a tension properly applied to a cloth. Findings - It was found out that shearing property is the most related to the stability of sewing trajectory: lithe tension for guidance applied to a cloth is constant, deformation of the cloth was observed and it was the cause of unstableness of sewing trajectory. By controlling the tension for guidance applied to a cloth properly according to the direction of the cloth, precise sewing trajectory was obtained. Originality/value - There have been some studies in which sewing conditions were dynamically controlled according to the mechanical properties of a cloth.     Suicidal ideation and burnout among psychiatric trainees in Japan

    AIM: Burnout is a psychological condition that may occur in all workers after being exposed to excessive work-related stresses. We investigated suicidal ideation and burnout among Japanese psychiatric trainees as a part of the Burnout Syndrome Study (BoSS) International.  METHODS: In the Japanese branch, 91 trainees fully completed suicide ideation and behaviour questionnaire (SIBQ) and Maslach Burnout Inventory-General Survey (MBI-GS).  RESULTS: Passive suicidal ideation was reported by 38.5% of Japanese trainees and 22.0% of them had experienced active suicidal ideation. The burnout rate among Japanese subjects was 40.0%. These results were worse compared to the all 1980 trainees who fully completed the main outcome measure in BoSS International, 25.9%, 20.4% and 36.7%, respectively.  CONCLUSIONS: Our results suggest a higher risk of suicide among Japanese residents. Japan has a higher suicide rate than other countries. Early detection of, and appropriate intervention for, suicidal ideation is important in preventing suicide in psychiatry residents

    The K1.8BR spectrometer system at J-PARC

    A new spectrometer system was designed and constructed at the secondary beam line K1.8BR in the hadron hall of J-PARC to investigate KˉN\bar K N interactions and Kˉ\bar K-nuclear bound systems. The spectrometer consists of a high precision beam line spectrometer, a liquid 3^3He/4^4He/D2_2 target system, a Cylindrical Detector System that surrounds the target to detect the decay particles from the target region, and a neutron time-of-flight counter array located \sim15 m downstream from the target position.     Surgical repair for a parahiatal hernia with an esophageal hiatal hernia: A case report and literature review

    Introduction and importance: A parahiatal hernia (PH) is a rare diaphragmatic hernia (DH) adjacent to but separated from the esophageal hiatus. The surgical repair for PH needs primary suture closure or complicated hernioplasty and the addition of an anti-reflux procedure. This report describes a case of PH with a symptomatic esophageal hiatal hernia managed using three-dimensional (3D) laparoscopy. Case presentation: A 65-year-old woman with back pain and breathlessness was referred to our hospital for a DH. Computed tomography showed a diaphragmatic defect on the left side of the esophageal hiatus. Upper gastrointestinal endoscopy and 24-hour esophageal impedance-pH monitoring showed a symptomatic esophageal hiatal hernia. Laparoscopic repair for both hernias was performed using 3D laparoscopy. The DH orifice was located in the left crus of the diaphragm, and it was separated from the esophageal hiatus. These findings showed that this DH was a PH. The PH was repaired with primary suturing, and a hiatoplasty was performed. Toupet fundoplication was performed with a 270 degrees posterior wrap of the gastric fornix. The patient has remained asymptomatic a year after surgery without any complications. Clinical discussion: 3D laparoscopy provides significant advantages in surgeries requiring precise suturing. PH repairs require complex procedures, including mesh repair or suturing. Approximately 44 % of PH cases also necessitate fundoplication. 3D laparoscopy was useful for the present case. Conclusions: A rare PH and a symptomatic type 1 hiatal hernia were repaired with 3D laparoscopy, which is helpful for PH treatment in cases requiring complicated procedures
