The present invention includes receiving (a) downmix signal being generated from plural-channel signal and (b) spatial information indicating attribute of the plural-channel signal in order to upmix the downmix signal and including phase shift flag indicating whether phase of a frame of at least one channel of the plural-channel signal is shifted; obtaining inter-channel phase difference (IPD) coding flag indicating whether IPD value is used to the spatial information from a header of the spatial information; obtaining IPD mode flag indicating whether the IPD value is used to frame of the spatial information from the frame based on the IPD coding flag; obtaining the IPD value of parameter band in the frame, based on the IPD mode flag; upmixing plural-channel signal by applying the IPD value to the downmix signal; and shifting the phase of the frame of the at least one channel of the plural-channel signal based on the phase shift flag

    3-D finite element analysis of the effects of post location and loading location on stress distribution in root canals of the mandibular 1st molar

    Objective The purpose of this study was to evaluate, by using finite element analysis, the influence of post location and occlusal loading location on the stress distribution pattern inside the root canals of the mandibular 1st molar. Material and Methods Three different 3-D models of the mandibular 1st molar were established: no post (NP) – a model of endodontic and prosthodontic treatments; mesiobuccal post (MP) – a model of endodontic and prosthodontic treatments with a post in the mesiobuccal canal; and distal post (DP) – a model of endodontic and prosthodontic treatments with a post in the distal canal. A vertical force of 300 N, perpendicular to the occlusal plane, was applied to one of five 1 mm2 areas on the occlusal surface; mesial marginal ridge, distal marginal ridge, mesiobuccal cusp, distobuccal cusp, and central fossa. Finite element analysis was used to calculate the equivalent von Mises stresses on each root canal. Results The DP model showed similar maximum stress values to the NP model, while the MP model showed markedly greater maximum stress values. The post procedure increased stress concentration inside the canals, although this was significantly affected by the site of the force. Conclusions In the mandibular 1st molar, the distal canal is the better place to insert the post than the mesiobuccal canal. However, if insertion into the mesiobuccal canal is unavoidable, there should be consideration on the occlusal contact, making central fossa and distal marginal ridge the main functioning areas

    Retrospective clinical trial of fusidic acid versus petrolatum in the postprocedure care of clean dermatologic procedures

    Background: Clean dermatologic procedures create wounds with a low risk of infection (usually up to 5%). Whether the use of topical antibiotics is advocated, with regard to its efficacy and safety issues such as antibiotic resistance and sensitizing potential, is controversial. Fusidic acid, a topical antibiotic against gram-positive bacteria, is a rare sensitizer and commonly used in postprocedure care in Korea. Objective: This is a retrospective study aimed at comparing the efficacy and safety between fusidic acid and petrolatum for the postprocedure care of clean dermatologic procedures. Methods: Patients were treated with either fusidic acid or petrolatum ointment, applied on the wound created during clean dermatologic procedures such as biopsy of the punch, incisional, excisional, and shave types. The efficacy, adverse events, and subjective level of satisfaction were retrieved from medical records. Results: A total of 414 patients with a total of 429 wounds were enrolled. The overall rate of adverse events was 0.9%, and the rates of adverse events in the fusidic acid group and the petrolatum group were 1.4% and 0.5%, respectively (p=0.370). There was no wound discharge, pain, tenderness, swelling, induration, or dehiscence in both groups. The patients self-assessment of the wound was not significantly different between the two treatment groups. Conclusion: Our findings support the hypothesis that the routine prophylactic use of topical antibiotics is not indicated for clean dermatologic procedures.
