    Macrolide Therapy in Respiratory Viral Infections

    Background. Macrolides have received considerable attention for their anti-inflammatory and immunomodulatory actions beyond the antibacterial effect. These two properties may ensure some efficacy in a wide spectrum of respiratory viral infections. We aimed to summarize the properties of macrolides and their efficacy in a range of respiratory viral infection. Methods. A search of electronic journal articles through PubMed was performed using combinations of the following keywords including macrolides and respiratory viral infection. Results. Both in vitro and in vivo studies have provided evidence of their efficacy in respiratory viral infections including rhinovirus (RV), respiratory syncytial virus (RSV), and influenza virus. Much data showed that macrolides reduced viral titers of RV ICAM-1, which is the receptor for RV, and RV infection-induced cytokines including IL-1β, IL-6, IL-8, and TNF-α. Macrolides also reduced the release of proinflammatory cytokines which were induced by RSV infection, viral titers, RNA of RSV replication, and the susceptibility to RSV infection partly through the reduced expression of activated RhoA which is an RSV receptor. Similar effects of macrolides on the influenza virus infection and augmentation of the IL-12 by macrolides which is essential in reducing virus yield were revealed. Conclusion. This paper provides an overview on the properties of macrolides and their efficacy in various respiratory diseases

As expectations surrounding personalized radiotherapy using complex data have increased, studies on outcome predictions using artificial intelligence have also increased. Representative artificial intelligence techniques used to predict the outcomes of cancer patients in the field of radiation oncology include collecting and processing big data, text mining of clinical literature, and machine learning for implementing prediction models. Here, methods of data preparation and model construction to predict rates of survival and toxicity using artificial intelligence are described

    Whole body iodine-131 scan is a well-established imaging method for the detection of metastatic or residual tumor sites in patients with well-differentiated thyroid cancer. Many false-positive iodine-131 scan findings mimicking metastatic thyroid cancer have long been reported. The authors describe a false positive uptake in normal gallbladder on post-ablative iodine-131 scan in a patient with papillary thyroid cancer. This finding should be considered to be another possible false-positive finding on iodine-131 whole body scan

    Efficacy and Safety of Autologous Stromal Vascular Fraction in the Treatment of Empty Nose Syndrome

    Objectives Regenerative treatment using stem cells may serve as treatment option for empty nose syndrome (ENS), which is caused by the lack of turbinate tissue and deranged nervous system in the nasal cavity. We aimed to assess the efficacy and safety of the autologous stromal vascular fraction (SVF) in the treatment of ENS. Methods In this prospective observational clinical study, we enrolled 10 ENS patients who volunteered to undergo treatment of ENS through the injection of autologous SVF. Data, including demographic data, pre- and postoperative Sino-Nasal Outcome Test-25 (SNOT-25) scores, overall patient satisfaction, and postoperative complications, were prospectively collected. Nasal secretion was assessed using the polyurethane foam absorption method, and the levels of biological markers were analyzed in both ENS group and control group using enzyme-linked immunosorbent assay. The SVF extracted from abdominal fat was diluted and injected into both inferior turbinates. Results Among the 10 initial patients, one was excluded from the study. Subjective satisfaction was rated as “much improved” in two and “no change” in seven. Among the improved patients, the mean preinjection SNOT-25 score was 55.0 and the score at 6 months after injection was 19.5. However, the average SNOT-25 score of nine participants at 6 months after injection (mean±standard deviation, 62.4±35.8) did not differ significantly from the baseline SNOT25 score (70.1±24.7, P>0.05, respectively). Among the various inflammatory markers assessed, the levels of interleukin (IL)-1β, IL-8, and calcitonin gene-related peptide were significantly higher in ENS patients. Compared with preinjection secretion level, the nasal secretions from SVF-treated patients showed decreased expressions of IL-1β and IL-8 after injection. Conclusion Although SVF treatment appears to decrease the inflammatory cytokine levels in the nasal mucosa, a single SVF injection was not effective in terms of symptom improvement and patient satisfaction. Further trials are needed to identify a more practical and useful regenerative treatment modality for patients with ENS

    Through the Spherical Looking-Glass: Asymmetry Enables Multicolored Internal Reflection in Cholesteric Liquid Crystal Shells

    Spheres of cholesteric liquid crystal generate dynamic patterns due to selec- tive reflection from a helical structure subject to continuously curved bounda- ries. So far the patterns are investigated exclusively as function of reflections at the sphere exterior. Here it is shown that the cholesteric shells in a microfluidics produced double emulsion enable also a sequence of internal reflections if the shells have sufficiently thin top and thick bottom. While such asymmetry is promoted by buoyancy when the internal droplet has lower density than the liquid crystal, the elasticity of the cholesteric helix prefers a symmetric shell geometry, acting against gravity. This subtle balance can hide the internal reflections for long time. Eventually, however, the asymmetry is established, revealing a new class of photonic patterns characterized by colored sharp concentric rings. With the complete knowledge of the diverse light-reflecting behavior of cholesteric liquid crystal shells, and utilizing the tunability of the structure period by, e.g., temperature, electric field, or expo- sure to various chemical species as well as polymer stabilization for making the shells long-term stable, they may be developed into remarkable new optical elements for photonics, sensing, or security pattern generation
