A Phase II Study of Concurrent Docetaxel, Epirubicin and Cyclophosphamide as a Neoadjuvant Chemotherapy Regimen in Patients with Locally Advanced Breast Cancer
Background: Neoadjuvant chemotherapy with concurrent docetaxel, doxorubicin and cyclophosphamide is commonly used for patients with locally advanced breast cancer. Epirubicin is another anthracycline used in breast cancer but the concurrent use of epirubicin and taxane is not well-established. We conducted a single institution, phase II study to assess the efficacy and safety of concurrent docetaxel, epirubicin and cyclophosphamide (TEC) as a neoadjuvant chemotherapy regimen in breast cancer. Methods: Patients with newly diagnosed locally advanced breast cancer defined as T2 >3 cm, T3, T4 with any N, or any T with N1-3 were eligible. A chemotherapy regimen of docetaxel 75mg/m2, epirubicin 75mg/m2 and cyclophosphamide 600mg/m2 was given with filgrastim support every 3 weeks for 6 cycles. The primary end-point was pathologic complete response rate. Results: Twenty patients were enrolled from 2003 to 2006. The median age was 51 (29-70) year-old. Eight patients were premenopausal. Ten patients had positive hormone receptors. Four patients had HER2 positive receptor. Nineteen patients completed six cycles of TEC chemotherapy. The pathologic complete response rate was 25%. Eight of sixteen patients with N1-3 disease had pathological negative lymph nodes. With a median follow up of 57.5 (16-71) months, four patients relapsed including one death from recurrence. The estimated 5 year relapse-free survival was 79.3% and the 5-year overall survival was 94.7%. No patient had cardiac failure or death during treatment. The most common grade 3-4 toxicity was neutropenia (35%). Conclusion: TEC regimen is a well- tolerated and effective neoadjuvant chemotherapy regimen for locally advanced breast cancer that results in a pathologic complete response rate of 25%
Parental Differences in Family Processes in Chinese Families Experiencing Economic Disadvantage
Parental differences in family processes (indexed by parental responsiveness, demandingness, control, and sacrifice for children’s education) were examined in 275 Chinese intact families experiencing economic disadvantage in Hong Kong. Consistent with the previous literature, results indicated that there were parent gender differences in family processes, including parental responsiveness, demandingness, control and sacrifice for children’s education based on the responses of parents and adolescents. Relative to mothers, fathers were less involved in parenting and having less sacrifice for their children’s education. Furthermore, adolescents perceived greater paternal-maternal differences in family processes than did parents. This is the first scientific research that studies parent gender differences in family processes in Chinese families experiencing economic disadvantage. Theoretical and practical implications of the findings are discussed

Patients’ experience in Hong Kong hospitals: A comparison between south Asian and Chinese people
Patient experience in hospital is positively associated with both self-rated and objectively measured health outcomes. In many countries ethnic minority patients have more negative experience and bear a disproportionate burden of disease than their majority counterparts. However, hospital experience of ethnic minority patients in Asia is still unexplored. We aimed to explore the hospital experience of South Asian ethnic minority and compare that with local Chinese patients’ experience in Hong Kong. A cross-sectional study sample comprised of 783 participants (388 South Asian and 395 Chinese). Picker Patient Experience-15 (PPE-15) questionnaire was used for data collection. Simple and multiple regressions were used to compare South Asian and Chinese participants’ in-hospital stays. The regression analyses were done before and after adjusting for demographics and after Propensity Score Weighting (PSW). All estimates were accompanied by a 95% confidence interval. Two-sided tests were conducted with a significance concluded by a p-value.

Population genetic analysis of Plasmodium knowlesi reveals differential selection and exchange events between Borneo and Peninsular sub-populations

MR/M01360X/1, MR/N010469/1, MR/R025576/1, MR/R020973/1, and MR/X005895/1).The zoonotic Plasmodium knowlesi parasite is a growing public health concern in Southeast Asia, especially in Malaysia, where elimination of P. falciparum and P. vivax malaria has been the focus of control efforts. Understanding of the genetic diversity of P. knowlesi parasites can provide insights into its evolution, population structure, diagnostics, transmission dynamics, and the emergence of drug resistance. Previous work has revealed that P. knowlesi fall into three main sub-populations distinguished by a combination of geographical location and macaque host (Macaca fascicularis and M. nemestrina). It has been shown that Malaysian Borneo groups display profound heterogeneity with long regions of high or low divergence resulting in mosaic patterns between sub-populations, with some evidence of chromosomal-segment exchanges. However, the genetic structure of non-Borneo sub-populations is less clear. By gathering one of the largest collections of P. knowlesi whole-genome sequencing data, we studied structural genomic changes across sub-populations, with the analysis revealing differences in Borneo clusters linked to mosquito-related stages of the parasite cycle, in contrast to differences in host-related stages for the Peninsular group. Our work identifies new genetic exchange events, including introgressions between Malaysian Peninsular and M. nemestrina-associated clusters on various chromosomes, including in parasite invasion genes (DBPβ, NBPXα and NBPXβ), and important proteins expressed in the vertebrate parasite stages. Recombination events appear to have occurred between the Peninsular and M. fascicularis-associated groups, including in the DBPβ and DBPγ invasion associated genes. Overall, our work finds that genetic exchange events have occurred among the recognised contemporary groups of P. knowlesi parasites during their evolutionary history, leading to apparent mosaicism between these sub-populations. These findings generate new hypotheses relevant to parasite evolutionary biology and P. knowlesi epidemiology, which can inform malaria control approaches to containing the impact of zoonotic malaria on human communities.Publisher PDFPeer reviewe
Evolutionary divergent spliceosomal snRNAs and a conserved non-coding RNA processing motif in Giardia lamblia

biological functions in all organisms, and in eukaryotes,
two such classes of ncRNAs are the small
nucleolar (sno) and small nuclear (sn) RNAs. In this
study, we have identified and characterized a collection
of sno and snRNAs in Giardia lamblia, by exploiting
our discovery of a conserved 12 nt RNA
processing sequence motif found in the 30 end
regions of a large number of G. lamblia ncRNA
genes. RNA end mapping and other experiments
indicate the motif serves to mediate ncRNA 30 end
formation from mono- and di-cistronic RNA precursor
transcripts. Remarkably, we find the motif is
also utilized in the processing pathway of all four
previously identified trans-spliced G. lamblia
introns, revealing a common RNA processing pathway
for ncRNAs and trans-spliced introns in this
organism. Motif sequence conservation then
allowed for the bioinformatic and experimental identification
of additional G. lamblia ncRNAs, including
new U1 and U6 spliceosomal snRNA candidates.
The U6 snRNA candidate was then used as a tool
to identity novel U2 and U4 snRNAs, based on predicted
phylogenetically conserved snRNA–snRNA
base-pairing interactions, from a set of previously
identified G. lamblia ncRNAs without assigned
function. The Giardia snRNAs retain the core
features of spliceosomal snRNAs but are sufficiently
evolutionarily divergent to explain the difficulties
in their identification. Most intriguingly, all of
these snRNAs show structural features diagnostic of U2-dependent/major and U12-dependent/minor
spliceosomal snRNAs.Ye

Leucine-rich repeat containing 8A (LRRC8A) is essential for T lymphocyte development and function
Lrrc8a is a ubiquitously expressed gene that encodes a leucine-rich repeat (LRR)–containing protein detected at higher levels on the surface of thymocytes than on other immune cells. We generated Lrrc8a−/− mice to investigate the role of LRRC8A in lymphocyte development and function. Lrrc8a−/− mice had increased prenatal and postnatal mortality, growth retardation, and multiple tissue abnormalities. Lrrc8a−/− mice displayed a modest block in B cell development but intact intrinsic B cell function. In contrast, both Lrrc8a−/− mice and Lrrc8a−/−→Rag2−/− bone marrow chimeras exhibited a severe cell-intrinsic block in early thymic development, with decreased proliferation and increased apoptosis of thymocytes, and impaired peripheral T cell function. Thymic epithelial cells expressed an LRRC8A ligand that was critical for double-negative to double-positive thymocyte differentiation and survival in vitro. LRRC8A constitutively associated with the GRB2–GAB2 complex and lymphocyte-specific protein tyrosine kinase (LCK) in thymocytes. LRRC8A ligation activated AKT via the LCK–ZAP–70–GAB2–PI3K pathway, and AKT phosphorylation was markedly reduced in the thymus of Lrrc8a−/− mice. The impacts of using smartphone dating applications on sexual risk behaviours in college students in Hong Kong

Dating applications (apps) on smartphones have become increasingly popular. The aim of this study was to explore the association between the use of dating apps and risky sexual behaviours. Data were collected in four university campuses in Hong Kong. Subjects completed a structured questionnaire asking about the use of dating apps, sexual behaviours, and sociodemographics. Multiple linear and logistics regressions were used to explore factors associated with sexual risk behaviours. Six hundred sixty-six subjects were included in the data analysis. Factors associated with having unprotected sexual intercourse with more lifetime sexual partners included use of dating apps (β = 0.93, p < 0.01), having one's first sexual intercourse before 16 years of age (β = 1.74, p < 0.01), being older (β = 0.4, p < 0.01), currently being in a relationship (= 0.69, p < 0.05), having a monthly income at least HKD $5,000 (β = 1.34, p < 0.01), being a current smoker (β = 1.52, p < 0.01), and being a current drinker (β = 0.7, p < 0.01). The results of a multiple logistic regression analysis found that users of dating apps (adjust odds ratio: 0.52, p < 0.05) and current drinkers (adjust odds ratio: 0.40, p < 0.01) were less likely to have consistent condom use. Users of dating apps (adjust odds ratio: 1.93, p < 0.05), bisexual/homosexual subjects (adjust odds ratio: 2.57, p < 0.01) and female subjects (adjust odds ratio: 2.00, p < 0.05) were more likely not to have used condoms the last time they had sexual intercourse. The present study found a robust association between using dating apps and sexual risk behaviours, suggesting that app users had greater sexual risks. Interventions that can target app users so that they can stay safe when seeking sexual partners through dating apps should be developed.published_or_final_versio
