    Background & aims: Many studies have reported the effects of preoperative clear fluid carbohydrate supplements; however, few studies have reported the effects of preoperative jelly-type carbohydrate supplements. This study aimed to assess the effect of a jelly-type oral nutritional supplement (ONS) on metabolism, redox balance by using various surrogate markers and to evaluate its excretion from the stomach. Methods: This study was conducted according to a crossover design. Participants underwent a control experiment whereby they fasted after dinner and only ingested water until the experiment. The remaining participants underwent an ONS experiment whereby they ingested 400 g of ONS before bed and another 400 g at 7:00 am. Blood samples were collected at 9:00 am. After a break of at least 24 h, participants underwent the alternate experiment. Results: Thirty minutes after intake of jelly, the gastric antrum appeared flat (the same result as that at baseline) on ultrasonography. The ONS group showed significantly lower serum free fatty acid levels (100 μEq/L, p = 0.027, vs. 327 μEq/L, n = 6), total ketone bodies levels, 3-MH/creatinine levels, and oxidative stress surrogate markers. Serum insulin levels were significantly higher and participant's satisfaction was improved in the ONS group. Conclusions: We have the limitations of our methodologies as surrogate markers, compared with direct measurement of lipolysis, proteolysis and redox balance regulation. But Jelly-type ONS suppresses the catabolism of adipose tissue and muscle protein, decreases oxidative stress and improves patient satisfaction in healthy participants, without any increased risk of aspiration

    Five-antituberculosis Drug-resistance Genes Detection Using Array System

    Detection of resistance to drugs for Mycobacterium tuberculosis takes about two months from the sample collection using culture-based methods. To test a rapid method for detection of resistance for five antituberculosis drugs using DNA microarray and to examine its potential for clinical use, we employed a DNA microarray for detection of seven mutations genes related to resistance of five kinds of antituberculous drugs using Mycobacterium tuberculosis DNA isolated from sputum. The results of microarray analysis were compared with the results of a standard culture method of Lowenstein-jensen drug sensitivity testing system. DNA microarray analysis showed a high sensitivity (>90%) for all five drugs. Specificity of rifampicin and ethambutol were nearly 90%, however specificity of isoniazid (60%) and kanamycin (67%) were not enough. The amount of Mycobacterium tuberculosis DNA required for microarray analysis corresponded to at least 1–9 Acid-Fast Bacilli per 10 fields by carbolfuchsin staining. DNA microarray analysis appears to be useful for estimation of drug resistances, nevertheless its limitations. To minimize misunderstanding, it is necessary to confirm the number of bacilli in the sputum, and culture method is needed for comparison when use the PCR-based array system

    Rapid Effect of nCPAP Therapy on Circulating Plasma Leptin in OSAS Patients

    Obstructive sleep apnea syndrome (OSAS) is a problem of obese patients. Leptin prevents fat deposition in adipose tissue and insulin reduces plasma glucose, but levels of these hormones are sometimes high in obese patients with OSAS. Thus, leptin and insulin do not function properly in OSAS, and increase resistance to leptin and insulin may cause paradoxical state, that is, leptin and insulin resistance. The present study investigates the rapid effect of nasal continuous positive airway pressure (nCPAP) ventilation on serum leptin and insulin levels in OSAS. Levels of plasma leptin and insulin at four sampling points a day decreased in OSAS patients (*p<0.05, n=10) by nCPAP. Nocturnal mean-nadir arterial oxygen desaturation (nmSaO2) correlated with plasma leptin levels in OSAS patients (*p<0.05, n=10). Nasal CPAP did not affect circulating soluble leptin receptor levels in OSAS patients. These findings indicate that leptin and insulin are related to nocturnal hypoxia in OSAS patients, and that nCPAP have a possibility of improving leptin and insulin resistance
