    Evaluating the effectiveness of public health financing based on financial and non-financial indicators in terms of the knowledge economy

    In conditions of the knowledge economy human capital, human value are the main factors of economic growth and prosperity of the state. Qualitative characteristics of human capital are indicators of public health. Therefore, one of the most important tasks of the state is to assess the effectiveness of financing public health expenditures. The methodological base of the research was the methods of system and economic analysis, mathematical statistics and decision optimization, which resulted in the identification of factors influencing the resource supply, availability and quality of public health. The formation of a comprehensive indicator of the effectiveness of the public health system based on the system of financial and non-financial indicators will allow to form numerically an assessment of the effectiveness of investments in public health and draw conclusions about the resources' provision, level of development, accessibility and quality of public health. The obtained results can be used in making effective public financial decisions, which allow to achieve an increase of accessibility and quality of health care, focusing on the human capital in conditions of knowledge economy.peer-reviewe

    Worked out by the Rospotrebnadzor Administration in cooperation with the Amur Region Government and the head office of the Central Bank of Russia was a plan of operation aimed at the provision of financial loan consumers’ rights protection through the implementation of simplified repayment procedure. Rospotrebnadzor Administration charged itself with the responsibility to inform and provide counseling to affected borrowers and to gather the data on their numbers and the volume of the credits allowed. The measures undertaken made it possible to delay the deadlines and decision making on the financial problems emerged, which concerned the repayment of debts, all in order to use the available sums for elimination of consequences and aftermaths of the disaster

    Molecular Epidemiology of Pathogenic Hantaviruses in the Far East of Russia, 2015-2018

    Human pathogenic hantaviruses, belonging to the family Hantaviridae, genus Orthohantavirus, are disseminated worldwide and cause hemorrhagic fever with renal syndrome (HFRS) in Europe and Asia. In the Far East of Russia Hantaan (HTNV), Amur (AMRV) and Seoul (SEOV) viruses are etiologic agent of HFRS, while the human pathogenic potential of other seven hantaviruses, circulating in this region, has not been researched adequately yet. Objective of the study was genetic identification of hantaviruses, associated with HFRS in the Far East of Russia during 2015-2018. Materials and methods. Blood samples of 64 HFRS patients from Jewish Autonomous Region, Khabarovsk and Primorsky Territories were analyzed for hantavirus RNA using reverse transcription polymerase chain reaction. Results and discussion. A total of 19 viral RNA isolates from HFRS patients were genetically typed. Etiologic agents of HFRS were three pathogenic hantaviruses: HTNV (13 isolates), AMRV (3 isolates), SEOV (3 isolates). Three genetic lineages were identified among HTNV, two lineages among AMRV One genetic variant of SEOV virus was identified among HFRS patients, which was more close to the strains from South-Eastern Asia than to those from the neighboring countries

    Asymmetry and structural system analysis of the proximal femur meta-epiphysis: osteoarticular anatomical pathology

    <p>Abstract</p> <p>Background</p> <p>The human femur is commonly considered as a subsystem of the locomotor apparatus with four conspicuous levels of organization. This phenomenon is the result of the evolution of the locomotor apparatus, which encompasses both constitutional and individual variability. The work therein reported, therefore, underlies the significance of observing anatomical system analysis of the proximal femur meta-epiphysis in normal conditions, according to the anatomic positioning with respect to the right or left side of the body, and the presence of system asymmetry in the meta-epiphysis structure, thus indicating structural and functional asymmetry.</p> <p>Methods</p> <p>A total of 160 femur bones of both sexes were compiled and a morphological study of 15 linear and angulated parameters of proximal femur epiphysis was produced, thus defining the linear/angulated size of tubular bones. The parameters were divided into linear and angulated groups, while maintaining the motion of the hip joint and transmission of stress to the unwanted parts of the limb. Furthermore, the straight and vertical diameters of the femoral head and the length of the femoral neck were also studied. The angle between the neck and diaphysis, the neck antiversion and angle of rotation of the femoral neck were subsequently measured. Finally, the condylo-diaphyseal angle with respect to the axis of extremity was determined. To visualize the force of intersystem ties, we have used the method of correlation galaxy construction.</p> <p>Results</p> <p>The absolute numeral values of each linear parameter were transformed to relative values. The values of superfluity coefficient for each parameter in the right and left femoral bone groups were estimated and Pearson's correlation coefficient has been calculated (> 0.60). Retrospectively, the observed results have confirmed the presence of functional asymmetry in the proximal femur meta-epiphysis. On the basis of compliance or insignificant difference in the confidence interval of the linear parameters, we have revealed, therefore, a discrepancy in values between the neck and the diaphysis angle and the angle of femoral neck rotation (range displacement of confident interval to a greater degree to the right).</p> <p>Conclusion</p> <p>This study assessed the observations of a systemic anatomical study encompassing the proximal femur meta-epiphysis behavior in normal condition. This work has significance in medical practice as the theoretical basis is also required in knowing the decreased frequency and degree of severity of osteoarthritic pathologies in the dominant lower extremity.</p

    Detection of highly conductive surface electron states in topological crystalline insulators Pb1−xSnxSe using laser terahertz radiation

    We suggest a method for detection of highly conductive surface electron states including topological ones. The method is based on measurements of the photoelectromagnetic effect using terahertz laser pulses. In contrast to conventional transport measurements, the method is not sensitive to the bulk conductivity. The method is demonstrated on an example of topological crystalline insulators Pb1−xSnxSe. It is shown that highly conductive surface electron states are present in Pb1−xSnxSe both in the inverse and direct electron energy spectrum

    Hantavirus infection in patients with fever of unknown origin and in small mammals in the Baikal region

    Background. Hantavirus infections are one of the most important zoonotic diseases in the Russian Federation. However, the pathogenicity of Hantaviruses circulating in Siberia is still unknown. The aim of the study was to assess the incidence of Hantavirus infection in patients with fever of unknown origin from Irkutsk region and adjacent territories. Materials and methods. Serum samples collected in 2016 from individuals with symptoms of health deterioration were screened by ELISA to investigate possible Hantavirus infection, in addition to tick-borne infections. Depending on date of tick bite specific immunoglobulin G (IgG) and immunoglobulin M (IgM) antibodies to tick-borne encephalitis, borreliosis and Hantavirus were tested by ELISA or screened by PCR for tick-borne encephalitis, borreliosis, human granulocytic anaplasmosis and monocytic erlychiosis RNA/DNA. Results. Out of 92 serum samples, 6.5 % were positive for hantavirus-specific antibodies, which is at least twice higher than the seroprevalence among healthy people in the Irkutsk region. IgM and IgM+IgG antibodies were found in 5.4 % of cases, while IgG antibodies - in 1.1 % of cases. The clinical symptoms among sero-positive patients were fever, skin lesion, fatigue syndrome, headache and lymphadenitis. IgM antibodies to borreliosis were found in two hantavirus sero-positive cases. Lung tissues from small mammals captured in 2016 were screened by ELISA for the presence of Hantavirus antigen. Hantaviral antigen was detected in 6.3 % of 48 samples. Conclusions. Thus, present and previous studies demonstrate existence of natural foci of Hantavirus infections in addition to tick-born infections in the territory of the Irkutsk region

    Hantavirus Associated with Hemorrhagic Fever with Renal Syndrome Outbreak in the Saratov Region in 2019

    Pathogenic hantaviruses, belonging to the family Hantaviridae, genus Orthohantavirus, are widely spread in many regions of the world and cause hemorrhagic fever with renal syndrome (HFRS) in Europe and Asia. In the European Russia, the most active HFRS foci are located in the optimum habitat area of natural reservoir of the Puumala virus (PUUV), bank voles (Myodes glareolus), – in the Middle Volga and Cis-Urals. The largest number of cases of HFRS was registered in the Volga Federal District. In 2019, an outbreak of HFRS was registered among the residents of Saratov and the Saratov Region, the number of cases was 2702. Objective of the study was genetic identification of hantaviruses from HFRS patients and rodent carriers and phylogenetic analysis of full-size genomes from natural hosts during HFRS outbreak in the Saratov Region, 2019. Materials and methods. Blood samples of 8 HFRS patients from Saratov and 3 lung samples of bank voles captured in the suburb of Saratov were analyzed using reverse transcription polymerase chain reaction, followed by sequencing and phylogenetic analysis. Results and discussion. A total of 6 viral RNA isolates from HFRS patients were genetically typed, full-length RNA-isolate genomes were obtained for 3 natural carriers. Our data indicate that PUUV virus was associated with HFRS outbreak in Saratov. Genetic analysis revealed that the virus belonged to RUS lineage of PUUV, related most closely to strains from Udmurtia and Tatarstan and differed from strains circulating in the territory of Bashkortostan and Samara Region

    TO HANTAVIRUS INFECTION IN PEOPLE AND SMALL MAMMALS IN BAIKAL AREA

    The article describes the results of recognaissance surveys of blood serums of humans in Baikal area for hantavirus antibodies and of tissue specimens (lungs) of small mammal for hantavirus antigens. This investigation demonstrated that Baikal area inhabitants contact such dangerous pathogens as hantaviruses. It's proved by the presence of hantavirus antibodies in human serum samples. IgG or IgM antibodies to hantaviruses were found in 3,7 % of 379 tested serum samples. IgM were found in 1,6 % of cases, IgG - in 2,1 % of cases. 2 patients with IgG and IgM with suspected traces of hantavirus infection had complaints to hyperthermia (up to 40,5 °C) and wasting. Blood serums with positive results of hantavirus antibodies were found in residents of Irkutsk region. Hantavirus antigens weren't found in researched lung samples of mouse-like rodent and insectivores from Angarsk, Irkutsk, Zalari and Ust-Kut districts of Irkutsk region and Tunkinsky district of Republic of Buryatia
