    Physician-modified fenestration or in situ fenestration for preservation of isolated left vertebral artery in thoracic endovascular aortic repair

    ObjectiveTo present our experience of preserving the isolated left vertebral artery (ILVA) with physician-modified fenestration (PM-F) or in situ fenestration (ISF) during thoracic endovascular aortic repair (TEVAR) for aortic pathologies involving aortic arch.MethodsThis is a single-center, retrospective, observational cohort study. Between June 2016 and December 2021, 9 patients (8 men; median age 60.0 years old) underwent TEVAR with ILVA reconstruction (PM-F, n = 6; ISF, n = 3) were identified and analyzed.ResultsThe technical success rate was 100%. No early (<30 days) death occurred. No aortic rupture, major stroke or spinal cord injury was observed. The median follow up was 38.0 (rang: 1.0–66.0) months. One death occurred at 56 months, while the reason cannot be identified. No aortic rupture, major stroke or spinal cord injury was observed during follow up. No patient required reintervention. Out of the 22 successfully revascularized target vessels, 2 ILVAs were found occluded in 2 patients at 6 months and 7 months, respectively. However, these two patients were asymptomatic.ConclusionsOur initial experience reveals that PM-F or ISF for ILVA preservation was feasible, safe, and effective during TEVAR for complex thoracic aortic pathologies. However, the patency of preserved ILVA should be improved

    Optimal Control of Microgrid Operation Based on Fuzzy Sliding Mode Droop Control

    In the application of microgrid systems that include wind power, photovoltaic systems, diesel generators, and battery storage, the cooperative control and optimisation of power distribution between power sources is a major issue. Recently, the droop control has been used widely in microgrids. However, droop control relies mainly on the line parameter model between the grid and the load. Therefore, to improve the performance of the microgrid, the optimal control of microgrid operation based on the fuzzy sliding mode droop control method is considered in this paper. To begin, system parameters were obtained by modeling droop control with self-learning fuzzy control strategy. Then, to improve the accuracy of the power distribution in the multi-micro source system, the nonlinear differential smoothing control method was employed. Finally, by comparing the self-learning fuzzy sliding mode control based on drooping strategy and the traditional droop control method, it was demonstrated that the method proposed can effectively reduce the fluctuation of the bus voltage and improve the output voltage quality of the microgrid system

    Urban–Rural Integration Empowers High-Quality Development of Tourism Economy: Mechanism and Empirical Evidence

    Urban–rural integration has become an effective way to promote the high-quality development of the tourism economy in China’s policy evolution. Based on provincial panel data for China for the last ten years, this paper measured the high-quality-development level of the tourism economy based on multi-dimensional indicators, including by empirically analyzing the influence mechanisms in urban–rural integration. We found that urban–rural integration has a positive empowering effect on the high-quality development of the tourism economy as a whole, with the integration of urban and rural areas in the central and eastern regions playing a considerable role; however, its role in the western region is not obvious. This was shown through an analysis of the mediating effect by which urban–rural integration empowers the high-quality development of the tourism economy by improving residents’ income levels. However, urban–rural integration does not empower the high-quality development of the tourism economy through technological innovation, which instead has a covering effect. This paper makes suggestions on three levels: country, region, and enterprise. Firstly, at the national level, we suggest strengthening the overall planning for the high-quality development of the tourism economy, as well as improving its construction system. Secondly, at the regional level, governments at all levels should emphasize the functional differences between urban and rural areas and regional characteristics. Finally, at the enterprise level, enterprises in the western region should seize the opportunities provided by urban–rural integration

    Based on Systematic Pharmacology: Molecular Mechanism of Siwei Jianbu Decoction in Preventing Oxaliplatin-Induced Peripheral Neuropathy

    Chemotherapy-induced peripheral neuropathy (CIPN) is a dose-limiting side effect caused by chemotherapy drugs, and its existence seriously affects the quality of life of patients. We first established an oxaliplatin-induced peripheral neuropathy (OIPN) model and then measured and evaluated mechanical hyperalgesia, thermal nociception, cold allodynia, and intraepidermal nerve fiber (IENF) density to determine Siwei Jianbu Decoction’s role in preventing OIPN. Then, we conducted a systematic pharmacological study that revealed important roles for the MAPK signaling pathway and proinflammatory immune pathway and confirmed these roles by western blot, immunofluorescence, and qPCR. The data show that Siwei Jianbu Decoction can effectively prevent oxaliplatin-induced neuroinflammation by inhibiting an increase in NF-κB expression via downregulation of p-ERK1/2 and p-p38. The present study showed that SWJB may be beneficial in preventing oxaliplatin-induced peripheral neuropathy

    Background: Paclitaxel, a commonly used chemotherapeutic agent, is usually associated with peripheral neuropathy. Paclitaxel induced peripheral neuropathy (PIPN) can be dose limiting and may have detrimental influence on patients' quality of life. However, the mechanism of PIPN remains unclear. Medicinal herbs and their formulas might offer neuronal protection with their multitarget and integrated benefits in chemotherapy-induced peripheral neuropathy (CIPN). Siwei Jianbu decoction (J12) is a classic formula of traditional Chinese medicine which can promote blood circulation and treat diabetic nephropathy in clinical with the symptoms of weakness and pain. Methods: The effects of J12 were treated in C57BL/6 mice before injected with Paclitaxel.Behaviour studies: Measurement of mechanical hyperalgesia, thermal nociception and cold allodynia. On the last day at the end of week 6, DRGs were obtained from mice for western blot and immunohistochemical analysis containing NF-κB, p-ERK1/2 and p-SAPK/JNK protein expression. Quantitative real-time polymerase chain reaction: mRNA expression of NF-κB, IL-1β and TNF-α was analyzed. Additionally, the blood samples collected from the eye socket of the mouse were prepared to examine the levels of NF-κB, TNF-α, IL-6 and IL-1β using ELISA assay kits. Results: Hypersensitivity tests and pathology analysis have demonstrated that J12 could improve paclitaxel-induced peripheral pain. J12 acts by inhibiting the activation of (C-Jun N-terminal kinases) JNK, (extracellular signal-regulated kinase) ERK1/2 phosphorylation in (Mitogen-activated protein kinases) MAPK signaling pathway and the nuclear factor-κB (NF-κB) in C57BL/6 mice model, J12 also inhibits the production of inflammatory cytokines including tumor necrosis factor α (TNF-α), interleukin 1β (IL-1β) and IL-6. Conclusion: The present study showed that J12 ameliorates paclitaxel-induced peripheral neuropathic pain
