    Rethinking Large Language Models in Mental Health Applications

    Large Language Models (LLMs) have become valuable assets in mental health, showing promise in both classification tasks and counseling applications. This paper offers a perspective on using LLMs in mental health applications. It discusses the instability of generative models for prediction and the potential for generating hallucinatory outputs, underscoring the need for ongoing audits and evaluations to maintain their reliability and dependability. The paper also distinguishes between the often interchangeable terms ``explainability'' and ``interpretability'', advocating for developing inherently interpretable methods instead of relying on potentially hallucinated self-explanations generated by LLMs. Despite the advancements in LLMs, human counselors' empathetic understanding, nuanced interpretation, and contextual awareness remain irreplaceable in the sensitive and complex realm of mental health counseling. The use of LLMs should be approached with a judicious and considerate mindset, viewing them as tools that complement human expertise rather than seeking to replace it

    High Apoptotic Index in Urine Cytology Is Associated with High-Grade Urothelial Carcinoma

    BACKGROUND The significance of apoptosis and its association with high-grade urothelial carcinoma (HGUC) in urine cytology has yet to be determined. METHODS A computerized search of the study laboratory information system was performed over a 3-year period for all urine cytology specimens processed using the SurePath liquid-based preparation technique. Only those cases with correlating surgical pathology obtained within 6 months after the urine cytologic samples were included in the current study. Cases from ileal conduit samples were excluded. A semiquantitative numerical scoring system (apoptotic index) was used to assess the amount of pyknosis or karyorrhexis, with 0 indicating none, 1 indicating  30 per 10 high-power fields. Statistical analysis using the Pearson chi-square test was performed. RESULTS A total of 228 cases including 105 benign cases, 79 cases of HGUC, and 44 cases of low-grade urothelial carcinoma (LGUC) diagnosed on follow-up surgical pathology were selected. A score of 0 was observed in 70 benign, 11 HGUC, and 8 LGUC cases; a score of 1 was observed in 31 benign, 21 HGUC, and 23 LGUC cases; a score of 2 was observed in 3 benign, 27 HGUC, and 9 LGUC cases; and a score of 3 was observed in 1 benign, 20 HGUC, and 4 LGUC cases. CONCLUSIONS Excluding ileal conduit urine specimens, the finding of a high apoptotic index (score ≥ 2) with the presence of pyknosis or karyorrhexis in ≥10 per 10 high-power fields in the urine cytology appears to be significantly associated with HGUC (P<.05)

    Percutaneous Biopsy of the Renal Mass: Fine Needle Aspiration or Core Biopsy?

    BACKGROUND In recent years, there have been increasing indications for percutaneous renal biopsy. Fine-needle aspiration (FNA), with or without core needle biopsy (CB), has been used increasingly in the management of renal tumors at the study institution. METHODS A computerized search of laboratory records was conducted to retrieve FNA cases of renal masses as well as the correlating CB and/or nephrectomy specimens. The cases spanned a period of 10 years (2006-2015). The diagnoses were classified into 5 categories: malignant, suspicious for malignancy, neoplastic, atypical, and negative/nondiagnostic. Based on the results of the nephrectomy specimens, the diagnostic rate, sensitivity, and diagnostic accuracy were calculated among 3 groups of specimens: FNA only, CB only, and combined FNA and CB. RESULTS A total of 247 cases of FNA with 123 correlating CB and 101 follow-up nephrectomy specimens were identified. The diagnostic rate, sensitivity, and diagnostic accuracy were 72%, 78%, and 96%, respectively, for FNA; 87%, 92%, and 94%, respectively, for CB; and 92%, 92%, and 94%, respectively, for the combined FNA and CB group. Renal cell carcinoma and its variants were the most common histologic diagnoses (112 of 174 cases; 64%). Significant diagnostic discrepancy was noted in one case: a malignant melanoma that was misdiagnosed as renal cell carcinoma in both the preoperative FNA specimen and in the CB specimen. CONCLUSIONS In the current study, both FNA and CB demonstrated excellent diagnostic accuracy (96% and 94%, respectively). The combination of FNA and CB was found to significantly improve the diagnostic rate when compared with either FNA alone (92% vs 72%; P<.05) or CB alone (92% vs 87%)

    Efficiency of underdamped dc SQUIDs as readout devices for flux qubits

    The flux state quantum bit (qubit) is promising for a solid state implementation of scalable quantum computing. The simplest flux state qubit consists of an rf SQUID with two fluxoid states, which can be readout with a de SQUID-the most sensitive magnetic flux detector. Efficient readout with less back-action is desirable for quantum computing. In this work, we report measurements of the switching flux and switching current distributions of under damped dc SQUIDs. The data show that single shot readout of flux qubit with very high efficiency (>99%) can be realized using underdamped hysteretic de SQUIDs

    Quantitative study of macroscopic quantum tunneling in a dc SQUID: A system with two degrees of freedom

    To test whether the theory of macroscopic quantum tunneling (MQT) is applicable to systems with 2 degrees of freedom, we experimentally investigated the switching current distribution of a dc SQUID. Using sample parameters determined from measurements at T=4.2 K, we are able to make quantitative comparison to the theories from 8 mK to 4.2 K. The excellent agreement between the data and the MQT theory demonstrates that tunneling from the zero-voltage state of the dc SQUID is well described by the quantum mechanics

    The Local Origin of the Tibetan Pig and Additional Insights into the Origin of Asian Pigs

    BACKGROUND: The domestic pig currently indigenous to the Tibetan highlands is supposed to have been introduced during a continuous period of colonization by the ancestors of modern Tibetans. However, there is no direct genetic evidence of either the local origin or exotic migration of the Tibetan pig. METHODS AND FINDINGS: We analyzed mtDNA hypervariable segment I (HVI) variation of 218 individuals from seven Tibetan pig populations and 1,737 reported mtDNA sequences from domestic pigs and wild boars across Asia. The Bayesian consensus tree revealed a main haplogroup M and twelve minor haplogroups, which suggested a large number of small scale in situ domestication episodes. In particular, haplogroups D1 and D6 represented two highly divergent lineages in the Tibetan highlands and Island Southeastern Asia, respectively. Network analysis of haplogroup M further revealed one main subhaplogroup M1 and two minor subhaplogroups M2 and M3. Intriguingly, M2 was mainly distributed in Southeastern Asia, suggesting for a local origin. Similar with haplogroup D6, M3 was mainly restricted in Island Southeastern Asia. This pattern suggested that Island Southeastern Asia, but not Southeastern Asia, might be the center of domestication of the so-called Pacific clade (M3 and D6 here) described in previous studies. Diversity gradient analysis of major subhaplogroup M1 suggested three local origins in Southeastern Asia, the middle and downstream regions of the Yangtze River, and the Tibetan highlands, respectively. CONCLUSIONS: We identified two new origin centers for domestic pigs in the Tibetan highlands and in the Island Southeastern Asian region
