Ileal Intussusception Caused by Vanek's Tumor: A Case Report

    Get PDF
Inflammatory fibroid polyps (Vanek's tumor) are rare benign localized lesions originating in the submucosa of the gastrointestinal tract. Intussusceptions due to inflammatory fibroid polyps are uncommon; moreover, ileo-ileal intussusception with small bowel necrosis and perforation has rarely been reported. We report a 56-year-old woman who was admitted two days after complaints of nausea and vomiting. Abdominal examination revealed distension, signs of gastrointestinal perforation and clanging intestinal sounds. The patient underwent a emergency laparotomy which found a 17-cm invaginated mid-ileal segment with necrosis, perforation and fecal peritonitis. The ileal segment was resected and single-layer end-to-end anastomosis was performed. Histopathological analysis showed an ulcerative lesion with variable cellularity, formed by spindle cells with small number of mitosis and an abundant inflammatory infiltrate comprising mainly eosinophils. Immunohistochemistry confirmed the diagnosis of ileal Vanek's tumor. Although inflammatory fibroid polyps are seen very rarely in adults, they are among the probable diagnoses that should be considered in obstructive tumors of the small bowel causing intussusception with intestinal necrosis and perforation

Abstract

Introduction

The initial diagnosis of intussusception in adults very often can be missed and cause delayed treatment and possible serious complications. We report the case of an adult patient with complicated double ileoileal and ileocecocolic intussusception.

Case presentation

A 46-year-old Caucasian man was transferred from the gastroenterology service to the abdominal surgery service with severe abdominal pain, nausea, and vomiting. An abdominal ultrasound, barium enema, and abdominal computed tomography scan revealed an intraluminal obstruction of his ascending colon. Plain abdominal X-rays showed diffuse air-fluid levels in his small intestine. A double ileoileal and ileocecocolic intussusception was found during an emergent laparotomy. A right hemicolectomy, including resection of a long segment of his ileum, was performed. The postoperative period was complicated by acute renal failure, shock liver, and pulmonary thromboembolism. Our patient was discharged from the hospital after 30 days. An anatomical pathology examination revealed a lipoma of his ileum.

Conclusions

Intussusception in adults requires early surgical resection regardless of the nature of the initial cause. Delayed treatment can cause very serious complications.</p

A game, sometimes known as a video game, is a form of modern technology-based game. Gaming is currently one of the major industries on the planet. Because gaming aficionados from all over the world number in the hundreds of millions, if not billions, this sector is very substantial. The game may be played on a PC, an Android device, or a game machine both offline and online. Artificial intelligence is also used in video games. Artificial Intelligence (AI) is artificial intelligence implemented on a computer system that allows players to compete against the computer in a game similar to that of other players. Artificial intelligence, often known as machine intelligence, is a mimic of human intellect that has been trained to think like humans. Artificial intelligence is a technology that uses data as knowledge in order for the intelligence created to improve and learn from prior failures. When employed by humans, artificial intelligence can be activated by human orders or by itself based on AI experience. The goal of this research was to figure out how artificial intelligence was progressing in the gaming business

The protective effects of hypericum perforatum and nigella sativa on testicular tissue after testicular torsion and detorsion

The aim of this study was to investigate the antioxadant effects of Hypericum perforatum (HP) and Nigella sativa (NS) in protecting against biochemical and histopathological changes in rat testis injured by experimental ischemia/reperfusion (I/R). This research study was conducted on 40 adult male Wistar albino rats. Rats were randomly separated into four groups of 10 animals each: the sham (control) group in which only the scrotal area opened and closed without torsion and detorsion (T/D); the I/R group, which was T/D applied but not treated by any agent; the HP+I/R, and NS+I/R groups, HP and NS were given intraperitoneally 30 min before T / D 25 mg / kg groups. In the I/R group with HP+I/R and NS+I/R treatment groups, torsion was created by rotating only left testis 720 degrees clockwise for 2 hours. At the end of the 2 hours torsion, the left testis was detorsioned and replaced in the scrotum for 4 hours. Along with the completion of the experiment, the left testis was rapidly removed to measure oxidative stress markers and to examine it histopathologically. In the HP+I/R and NS+I/R groups, the malondialdeyhde (MDA) level was significantly lower than in the I/R group only, and the superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), catalaze (Cat) activities were higher than the I/R group. Also our histopathological results supported these biochemical findings. In conclusion, our results suggest that both HP and NS protect against injury of left testis induced by I/R, however the protective effects of NS were more pronounced than those the HP. © 2018, Veterinary Solutions LLC. A rare cause of intestinal hemorrhage: Stromal tumor of duodenum

    No full text
Stromal tumor of the duodenum is rarely seen among the gastrointestinal stromal tumors. In this paper, a patient presented with obscure gastrointestinal bleeding and the small bowel endoscopy revealed a bleeding ulcer at the second part of duodenum. The patient underwent segmental duodenectomy. The histopathology of the mass confirmed the diagnosis of duodenal stromal tumor

Gossypiboma mimicking a uistal pancreatic mass: Report of a case

    No full text
Gossypiboma (retained surgical sponge) is a pseudotumor within the body that is composed of non-absorbable surgical material with a cotton matrix. Because the symptoms of gossypiboma usually are nonspecific and may appear years after surgery, the diagnosis of gossypiboma may be difficult because the condition may mimic a benign or malignant soft-tissue tumour in the abdomen and pelvis. A 61-year-old woman with a one-year history of left upper-quadrant pain and weight loss was referred to our center. She had undergone peptic ulcer perforation 23 year ago. Physical examination revealed dullness and palpable mass In the left upper abdomen. On examination by computed tomography (CT), a hypodense mass of 12 cm in diameter between the greater curvature of the stomach, pancreas, and splenic hilus was detected. Upon exploration, a mass lesion of 10 cm in diameter was detected between the greater curvature of the stomach and splenic hilus, which caused dense adhesions not In communication with the pancreas. It was excised and a splenectomy was performed. After a macroscopic examination, the mass lesion was diagnosed as gossypiboma. Although ultrasonography (US), CT, angiography, and magnetic resonance imaging (MRI) may be used to diagnose gossypiboma, definitive diagnosis is possibile only upon surgery or histopathological examination. As a result, when an abdominal mass Is observed, surgeons should carefully investigate the patient's past surgical history while taking the possibility of gossypiboma into consideration. © Versita Warsaw and Springer-Verlag Berlin Heidelberg
