    Executive Summary of the Korean Society of Nephrology 2021 Clinical Practice Guideline for Optimal Hemodialysis Treatment

    The Korean Society of Nephrology (KSN) has published a clinical practice guideline (CPG) document for maintenance hemodialysis (HD). The document, 2021 Clinical Practice Guideline on Optimal HD Treatment, is based on an extensive evidence-oriented review of the benefits of preparation, initiation, and maintenance therapy for HD, with the participation of representative experts from the KSN under the methodologists’ support for guideline development. It was intended to help clinicians participating in HD treatment make safer and more effective clinical decisions by providing user-friendly guidelines. We hope that this CPG will be meaningful as a recommendation in practice, but not on a regulatory rule basis, as different approaches and treatments may be used by health care providers depending on the individual patient’s condition. This CPG consists of eight sections and 15 key questions. Each begins with statements that are graded by the strength of recommendations and quality of the evidence. Each statement is followed by a summary of the evidence supporting the recommendations. There is also a link to full-text documents and lists of the most important reports so that the readers can read further (most of this is available online)

    Korean Society of Nephrology 2022 Recommendations on controversial issues in diagnosis and management of hyponatremia

    The Korean Society for Electrolyte and Blood Pressure Research, in collaboration with the Korean Society of Nephrology, has published a clinical practice guideline (CPG) document for hyponatremia treatment. The document is based on an extensive evidence-based review of the diagnosis, evaluation, and treatment of hyponatremia with the multidisciplinary participation of representative experts in hyponatremia with methodologist support for guideline development. This CPG consists of 12 recommendations (two for diagnosis, eight for treatment, and two for special situations) based on eight detailed topics and nine key questions. Each recommendation begins with statements graded by the strength of the recommendations and the quality of the evidence. Each statement is followed by rationale supporting the recommendations. The committee issued conditional recommendations in favor of rapid intermittent bolus administration of hypertonic saline in severe hyponatremia, the use of vasopressin receptor antagonists in heart failure with hypervolemic hyponatremia, and syndrome of inappropriate antidiuresis with moderate to severe hyponatremia, the individualization of desmopressin use, and strong recommendation on the administration of isotonic fluids as maintenance fluid therapy in hospitalized pediatric patients. We hope that this CPG will provide useful recommendations in practice, with the aim of providing clinical support for shared decision-making to improve patient outcomes

    Efficacy of Itraconazole Prophylaxis for Autologous Stem Cell Transplantation in Children with High-Risk Solid Tumors: A Prospective Double-Blind Randomized Study

Purpose: The risk of invasive fungal infection is greater for allogeneic hematopoietic stem cell transplantation (HSCT) than for autologous transplantation. Therefore, many transplantation centers use antifungal prophylaxis for allogeneic HSCT, however, there exists no standard guidelines or consensus regarding autologous HSCT. Materials and Methods: A prospective double-blind randomized study was conducted in autologous HSCT recipients who were divided into prophylaxis and empirical treatment groups, and we investigated the efficacy of itraconazole prophylaxis in pediatric autologous HSCT. Results: Total 87 autologous HSCT episodes in 55 children with high-risk solid tumors were studied. No invasive fungal infections occurred in either group. However, patients in the prophylaxis group had a significantly shorter duration of fever (p < 0.05) and received antibacterial treatment of shorter duration (p < 0.05) with fewer numbers of antibiotics (p < 0.05 for the use of second line antibiotics) than those in the empirica

    Random Lasing with a High Degree of Circular Dichroism by Chiral Plasmonic Gold Nanoparticles

    Random lasers have distinct advantages to be the next-generation light sources owing to their simple fabrication process, high flexibility in shape and size, and unique optical characteristics, such as low spatial coherence, high intensity, and multi-directionality. In this paper, we discuss how to realize random lasing with a high degree of circular dichroism with the aid of chiral plasmonic gold nanoparticles. The extinction dissymmetry factor of the chemically synthesized chiral plasmonic gold nanoparticles is measured to be -0.11 at its peak wavelength of 575 nm. The lasing properties and luminescence dissymmetry factor of the emission of the random laser are measured and characterized. An optimal inclusion of the chiral plasmonic gold nanoparticles to an ethylene glycol solution of rhodamine 6G laser dye molecules mixed with dielectric titanium dioxide nanoparticles eventually results in the laser emission having a considerably high level of asymmetry between the right- and left-handed circularly polarized light, yielding a luminescence dissymmetry factor of 0.20-0.23. This study paves the way for the development of a random laser of a high degree of circular dichroism in a highly flexible compact form through a simple, mass-productive fabrication process, inviting numerous potential applications in nano-photonics.N

    Morphological Evolution Trajectory of Multifaceted Palladium Nanoparticles

    Precise control over the morphology and facets of Pd nanomaterials has great importance in catalytic and sensing applications. In this study, we synthesized Pd nanoparticles with multiple types of low-Miller-index-faceted morphologies by systematically defining the synthesis conditions of the seed-mediated colloidal preparation method. We discovered the morphological evolution of Pd nanoparticles by following the trajectory of the surface Miller indices, which were determined by the cooperative effects of cetyltrimethylammonium bromide and ascorbic acid. By precise control of the morphological trajectory, Pd nanoparticles with a new cuborhombicube morphology, composed of 36 facets and concave edges, were discovered. This study provides important insight into the design of the surface Miller indices and morphologies of functional nanomaterials.N
