Background and purpose According to the treatment of Wilm’s tumours, two different therapeutic strategies were established in the second half of the last century. Both National Wilm’s Tumour Study (NWTS) group and the International Society of Paediatric Oncology (SIOP) have helped to improve the clinical management and outcome of patients with Wilm’s tumours. In this study, we compared three groups of patients with Wilm’s tumours from different racial backgrounds and therapeutic strategies.Patients and methods A clinicopathological review was carried out for 40 patients treated at Kyushu University Hospital in Japan from 1960 to 2006 according to NWTS, 79 patients treated at the South Egypt Cancer Institute from 2002 to 2009 according to the SIOP protocol and 33 patients treated at the Pediatric Surgery Department of Assiut University Hospital from 2000 to 2009 according to the NWTS protocol. Results In the Kyushu University group, a favourable histology was diagnosed in 80% of patients and an unfavourable histology in 20% of patients. The distribution of the clinical stage was as follows: I = 40%, II =22.5%, III = 22.5%, IV = 7.5% and V = 7.5%. The 5-year overall survival was 82.5%, with the 5-year stage-related survival as follows: I= 93.7%, II =83.3%, III= 62.5%, IV = 33.3% and V = 100%. In the South Egypt Cancer Institute group, favourable histology was diagnosed in 89.9% of patients, unfavourable histology in 8.8% of patients and benign disease in 1.3% of patients. The distribution of the clinical stage (before and after chemotherapy) was as follows: I = 25.3%-50%, II= 19%-20%, III= 35.4-15%, IV = 12.5%-5% and V = 7.6%-7.6%. No case of intraoperative tumour rupture was recorded. The overall 5-year survival rate of 84% with a 5-year stage-related survival was as follows: I= 92%, II =80%, III= 50%, IV = 31% and V = 30%. In the Assiut University group, a favourable histology was diagnosed in 79% of patients and an unfavourable histology in 9% of patients. The distribution of the clinical stage was as follows: I= 39.4%, II =21.2%, III = 21.2%, IV = 12% and V =6%. The overall 5-year survival rate was 77% and the 5-year stage-related survival was as follows: I= 85%, II= 75%, III = 52%, IV = 33% and V = 33%.Conclusion The Egyptian patients with Wilm’s tumour presented at a younger age and had less incidence of an unfavourable histology compared with Japanese patients. Preoperative chemotherapy increased the rate of stage I disease and decreased the incidence of intraoperative tumour rupture and relapse. However, there was a possibility that chemotherapy had been administered in the case of benign disease. Keywords: intraoperative tumour rupture, NWTS, SIOP, Wilm’s tumou

ACL graft re-rupture after double-bundle reconstruction: factors that influence the intra-articular pattern of injury

    Get PDF
To determine the most common rupture patterns of previously reconstructed DB-ACL cases, seen at the time of revision surgery, and to determine the influence of age, gender, time between the initial ACL reconstruction and re-injury, tunnel angle and etiology of failure. Forty patients who presented for revision surgery after previous double-bundle ACL reconstruction were enrolled. Three orthopedic surgeons independently reviewed the arthroscopic videos and determined the rupture pattern of both the anteromedial and posterolateral grafts. The graft rupture pattern was then correlated with the previously mentioned factors. The most common injury pattern seen at the time of revision ACL surgery was mid-substance AM and PL bundle rupture. Factors that influenced the rupture pattern (proximal vs. mid-substance and distal rupture vs. elongated, but in continuity) were months between ACL reconstruction and re-injury (P = 0.002), the etiology of failure (traumatic vs. atraumatic) (P = 0.025) and the measured graft tunnel angle (P = 0.048). The most common pattern of graft re-rupture was mid-substance AM and mid-substance PL. As the length of time from the initial DB-ACL reconstruction to revision surgery increased, the pattern of injury more closely resembled that of the native ACL. Evaluation of patients who have undergone double-bundle ACL reconstruction, with a particular focus on graft maturity, mechanism of injury and femoral tunnel angles, and graft rupture pattern assists in preoperative planning for revision surger

This study aimed to determine the perceived effects of apps utilization to study habits and academic performance. Further, it also sought to determine the views of the participants and respondents on their engagement in math apps and their relationship with study habits and academic performance. This study made use of explanatory-sequential design. The purposive sampling design was used to determine the respondents in quantitative data while the quota sampling design was also used to determine the participants in qualitative data. The study revealed that the mathematical application that is mostly used by the students is the calculator app as it garnered the highest percentage value of 80.85.%. The participants agreed that math applications serve as an aid for math problems, aid for lack of teaching by the teacher, promote learning of specific lessons and increase, and help in improving their academic performance. Respondents’ perception of their utilization of math apps is due to technical problems. The respondents also agreed that math application also affected their study habits in which they become dependent on it which led to laziness and tiredness. In addition, this study also found that there is a significant relationship between respondents’ utilization of math applications and their study habits. Lastly, there is also a significant relationship between the study habits and academic performance of selected math students of Notre Dame of Midsayap College.  Article visualizations

    Get PDF
NTRODUCTION: COVID-19 formed new challenges to the medical institutions; itresulted in the transition from the usual face-to-face classes and direct clerkship training withinthe hospital to a new remote learning with online lectures and virtual clinical experience. Giventhe new online interactive setting, problems were raised given the limited patient care andinteraction as well as restricted bedside teaching opportunities and its impact on how medicalstudents can acquire and hone their clinical skills. OBJECTIVE: To determine the perceived clinicalreadiness of the medical clerks in the new normal setting in the Philippines. METHODOLOGY:Convenience sampling was used to gather respondents who were asked to answer an onlinesurvey questionnaire. The questions pertained to: academic training profile, clinical skills, patientmanagement, communication, understanding clinical guidelines, and personal development. Afteranalyzing the data, the scales of readiness from these subjects were gathered. RESULTS: Themedical clerks in the Philippines perceived that they were ready with regards to understandingclinical guidelines, communication, personal development, and patient management. They weremoderately ready in the different clinical skills in the departments of Family and CommunityMedicine, Internal Medicine, Pediatrics, Surgery and Obstetrics and Gynecology with somespecific skills in Surgery and Obstetrics and Gynecology being perceived as less ready than therest. CONCLUSION: The impact of the pandemic has disrupted the student’s confidence andreadiness. This shows that online clerkship in this time of pandemic may have provided learningto a certain degree but it is not enough to replace what face-to-face training could offer

    Get PDF
Introduction: The purpose of Occupational Safety and Health (OSH) is to protect and prevent workers from being exposed to risks and hazards that are detrimental to their health and safety. For the past two years, the coronavirus disease 2019 (COVID-19) is one of the most significant occupational health outcomes to physical therapists (PTs) due to the nature of their work. However, minimal studies have been conducted about the experiences and OSH practices of PTs during the pandemic. This study aims to understand the experiences of clinical PTs in Metro Manila, Philippines during the pandemic. The study also seeks to explore the OSH practices and responses of PTs to presenting occupational risks and hazards through a qualitative descriptive study design. Methods: In this qualitative descriptive study, a maximum variation purposeful sampling method will be used in recruiting an estimated total of twelve (12) participants. A preliminary questionnaire would be disseminated via Google Forms to determine the eligibility of potential participants. Data will be gathered through a one-to-one semi-structured online interview, which will be transcribed verbatim and recorded with audio and video. Both manual coding and software-aided coding (NVivo) will be utilized in the data analysis. Emerging themes will be identified using thematic inductive analysis. Discussion: The results of the study may contribute to the formulation of better guidelines in handling infectious diseases even after quarantine restrictions are lifted. Furthermore, the findings will also provide the groundwork for the local body of knowledge in the Philippines and may serve as a future reference for research concerning OSH practices in physical therapy locally

Anisotropic surface morphology of azopolymer lms generated by polarized UV light irradiation

    Get PDF
X-ray re ectivity measurements reveal anisotropy of the vertical surface roughness caused by exposure to linearly polarized UV (LPUV) light in the lms of two azopolymers. The irradiated surface is found to have higher roughness in the direction parallel to the direction of polarization than in the orthogonal direction. The photo-modi cation of the surface morphology is caused by spatial changes induced in polymer lms by LPUV irradiation. The important role of surface roughness anisotropy in determining the alignment of liquid crystals is discussed

    Radical anti-realism and substructural logics

    No full text
We first provide the outline of an argument in favour of a radical form of anti-realism premised on the need to comply with two principles, implicitness and immanence, when trying to frame assertability-conditions. It follows from the first principle that one ought to avoid explicit bounding of the length of computations, as is the case for some strict finitists, and look for structural weakening instead. In order to comply with the principle of immanence, one ought to take into account the difference between being able to recognize a proof when presented with one and being able to produce one and thus avoid the idealization of our cognitive capacities that arise within Hilbert-style calculi. We then explore the possibility of weakening structural rules in order to comply with radical anti-realist strictures
