    Does von Willebrand factor improve the predictive ability of current risk stratification scores in patients with atrial fibrillation?

    Von Willebrand factor (vWF) is a biomarker of endothelial dysfunction. We investigated its role on prognosis in anticoagulated atrial fibrillation (AF) patients and determined whether its addition to clinical risk stratification schemes improved event-risk prediction. Consecutive outpatients with non-valvular AF were recruited and rates of thrombotic/cardiovascular events, major bleeding and mortality were recorded. The effect of vWF on prognosis was calculated using a Cox regression model. Improvements in predictive accuracy over current scores were determined by calculating the integrated discrimination improvement (IDI), net reclassification improvement (NRI), comparison of receiver-operator characteristic (ROC) curves and Decision Curve Analysis (DCA). 1215 patients (49% males, age 76 (71–81) years) were included. Follow-up was almost 7 years. Significant associations were found between vWF and cardiovascular events, stroke, mortality and bleeding. Based on IDI and NRI, addition of vWF to CHA(2)DS(2)-VASc statistically improved its predictive value, but c-indexes were not significantly different. For major bleeding, the addition of vWF to HAS-BLED improved the c-index but not IDI or NRI. DCA showed minimal net benefit. vWF acts as a simple prognostic biomarker in AF and, whilst its addition to current scores statistically improves prediction for some endpoints, absolute changes and impact on clinical decision-making are marginal

    Gestión de un observatorio turístico de un pueblo mágico mediante inteligencia de negocios

    The purpose of this article is to explain the development of a project whose objective is to manage a Tourist Observatory, applying Business Intelligence, in order to measure the indicators of tourism behavior in a Magical Town of Mexico. As part of a program of the Ministry of Tourism that began in 2001, the title of Magical Town is granted to municipalities that meet certain characteristics and requirements, highlighting the cultural richness of the country through its history, uses and customs, with the purpose of activating national tourism in those areas. The participating municipalities are evaluated year after year from their incorporation, one of the requirements is the creation of a Tourist Observatory that can provide information on the development of the towns from their appointment. For the creation of the Tourist Observatory, the methodology proposed by the World Tourism Organization is used, where it is established from the delimitation of the destination, the participatory processes of the different interested parties to the presentation of the results. Another applied methodology is Business Intelligence that helps with the collection, organization and analysis of information, to obtain the final results.El presente artículo tiene como finalidad explicar el desarrollo de un proyecto cuyo objetivo es gestionar un Observatorio turístico, aplicando Inteligencia de Negocios, con la finalidad de medir los indicadores del comportamiento del turismo en un Pueblo Mágico de México. Como parte de un programa de la Secretaría de Turismo que inició en 2001, se otorga el título de Pueblo Mágico a los municipios que cumplen con ciertas características y requisitos, destacando la riqueza cultural del país por medio de su historia, usos y costumbres, con el propósito de activar el turismo nacional en esas zonas. Los municipios participantes son evaluados año con año a partir de su incorporación, uno de los requisitos es la creación de un Observatorio Turístico que pueda dar información sobre el desarrollo de los pueblos a partir de su nombramiento. Para la creación del Observatorio Turístico se utiliza la metodología propuesta por la Organización Mundial de Turismo, donde se establece desde la delimitación del destino, los procesos participativos de los diferentes interesados hasta la presentación de los resultados. Otra metodología aplicada es la Inteligencia de Negocios que ayuda con la recolección, organización y análisis de la información, para la obtención de los resultados finales

    Diez años de innovación docente en Filología Clásica en la Comunidad Autónoma de Madrid (I)

    Los últimos años han sido muy fructíferos en iniciativas oficiales de innovación y mejora de la calidad docente en el ámbito de la Comunidad de Madrid. Tanto en la Universidad, a través de los PIMCD (o análogos), como en los TMFP del ICE (o análogos), como en enseñanza secundaria, se han puesto en marcha diferentes iniciativas y proyectos con el objetivo de mejorar la docencia y la recepción por parte de los estudiantes de las materias relacionadas con el riquísimo legado cultural de Grecia y Roma. La Sociedad Española de Estudios Clásicos ha sido también una institución promotora a través de la convocatoria de premios anuales que reconocen el trabajo de los docentes y estudiantes de estas materias. Sin embargo, a pesar de lo mucho y bueno realizado, se echa en falta una labor que recabe toda la información dispersa, la estructure y organice y, sobre todo, la ponga a disposición, cuando ello sea posible, de los docentes y estudiantes. Por otra parte, los filólogos clásicos se han incorporado de lleno a la profunda renovación metodológica que ha supuesto la irrupción de las TICS en el ámbito docente, sustituyendo las antiguas metodologías basadas en la repetición por otras mucho más dinámicas y polivalentes. Nuestra tarea se restringe a los últimos diez años, los más prolíficos desde el punto de vista de la innovación en Clásicas, sin que ello sea obstáculo para que en convocatorias posteriores ampliemos nuestro abanico cronológico y geográfico

    Cambios en la personalidad y depresión en adultos mayores con enfermedad de Alzheimer

    The literature refers that personality change begins in the premorbid phase of mild neurocognitive disorder (TNC) and that depression can increase these changes, on the other hand, depression is treatable and as such its influence on personality is reversible. In this sense it is urgent to understand what the effective influence of depression in the TNC is, in patients with Alzheimer’s. Objective: Explore level of depression and changes in personality in elderly with TNC due to Alzheimer’s disease. Methodology: A total of 437 older adults participated. Participants were evaluated through the application of a Personality Inventory NEO-FFI; Mini-Mental State Examination (MMSE), and the Beck Depression Inventory (BDI-II). Results: There was an increase in behavioural changes due to the influence of the responsibility trait, associated with the increase in cognitive symptoms, both due to the influence of depression.La literatura refiere que el cambio de personalidad se inicia en la fase premórbida del trastorno neurocognitivo leve (TNC) y que la depresión puede incrementar esos cambios, por otro lado, la depresión es tratable y como tal su influencia en la personalidad es reversible. En este sentido urge entender cuál es la influencia efectiva de la depresión en el TNC, en pacientes con Alzheimer. Objetivo: Explorar nivel de depresión y cambios en la personalidad en pacientes adultos mayores con TNC debido a Enfermedad de Alzheimer. Metodología: Participaron 437 adultos mayores, evaluados a través de la aplicación del Inventario de Personalidad NEO-FFI; Examen del Estado Mental (MME), y el Inventario de Depresión de Beck (BDI- II). Resultados: Se verificó un incremento de cambios comportamentales por influencia del rasgo de responsabilidad, asociado al aumento de la sintomatología cognitiva, ambos por influencia de la depresión

    Heart Histopathology and Mitochondrial Ultrastructure in Aged Rats Fed for 24 Months on Different Unsaturated Fats (Virgin Olive Oil, Sunflower Oil or Fish Oil) and Affected by Different Longevity

    Diet plays a decisive role in heart physiology, with lipids having especial importance in pathology prevention and development. This study aimed to investigate how dietary lipids varying in lipid profile (virgin olive oil, sunflower oil or fish oil) affected the heart of rats during aging. Heart histopathology, mitochondrial morphometry, and oxidative status were assessed. Typical histopathological features associated with aging, such as valvular lesions, endomyocardical hyperplasia, or papillary muscle calcification, were found at a low extent in all the experimental groups. The most relevant finding was that inflammation registered by fish oil group was lower compared to the other treatments. At the ultrastructural level, heart mitochondrial area, perimeter, and aspect ratio were higher in fish oil-fed rats than in those fed on sunflower oil. Concerning oxidative stress markers, there were differences only in coenzyme Q levels and catalase activity, lower in sunflower oil-fed animals compared with those fed on fish oil. In summary, dietary intake for a long period on dietary fats with different fatty acids profile led to differences in some aspects associated with the aging process at the heart.     Effectiveness of new tools to define an up-to-date patient safety risk map: A primary care study protocol

    Background: Reducing incidents related to health care interventions to improve patient safety is a health policy priority. To strengthen a culture of safety, reporting incidents is essential. This study aims to define a patient safety risk map using the description and analysis of incidents within a primary care region with a prior patient safety improvement strategy organisationally developed and promoted. Methods: The study will be conducted in two phases: (1) a cross-sectional descriptive observational study to describe reported incidents; and (2) a quasi-experimental study to compare reported incidents. The study will take place in the Camp de Tarragona Primary Care Management (Catalan Institute of Health). In Phase 1, all reactive notifications collected within one year (2018) will be analysed; during Phase 2, all proactive notifications of the second and third weeks of June 2019 will be analysed. Adverse events will also be assessed. Phases 1 and 2 will use a digital platform and the proactive tool proSP to notify and analyse incidents related to patient safety. Expected Results: To obtain an up-to-date, primary care patient safety risk map to prioritise strategies that result in safer practices. © 2021 by the authors. Licensee MDPI, Basel, Switzerland

    Amyloid β-but not Tau-induced neurotoxicity is suppressed by Manuka honey via HSP-16.2 and SKN-1/Nrf2 pathways in an in vivo model of Alzheimer’s disease

    Alzheimer’s is a chronic degenerative disease of the central nervous system considered the leading cause of dementia in the world. It is characterized by two etiopathological events related to oxidative stress: the aggregation of β-amyloid peptide and the formation of neurofibrillary tangles of hyperphosphorylated Tau protein in the brain. The incidence of this disease increases with age and has been associated with inadequate lifestyles. Some natural compounds have been shown to improve the hallmarks of the disease. However, despite its potential, there is no scientific evidence about Manuka honey (MH) in this regard. In the present work we evaluated the effect of MH on the toxicity induced by Aβ aggregation and Tau in a Caenorhabditis elegans model. Our results demonstrated that MH was able to improve indicators of oxidative stress and delayed Aβ-induced paralysis in the AD model CL4176 through HSP-16.2 and SKN-1/ NRF2 pathways.
