Ancak, FÜTZ analizi ölçme ve değerlendirme bakımından bir dizi eksiklik taşımaktadır. Yazında, bu eksikliklerin analitik yaklaşımlarla giderilebileceğine ilişkin çalışmalar yapılmıştır. Yapılan çalışmalarda, yalnız FÜTZ grupları ve stratejik faktörler analitik yaklaşımla incelenmiştir. FÜTZ faktörleri temelinde belirlenen alternatif stratejilerin seçimi çalışma konusu edilmemiştir. Yazında görülen bu eksiklik, bu çalışmayla giderilmeye çalışılmıştır.     Epilepsy in vacuolating megalencephalic leukoencephalopathy with subcortical cysts

    AbstractVacuolating megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a disorder characterised by acquired macrocephaly, developmental motor delay of varying degrees, slowly progressive cerebellar and pyramidal signs, and initially preserved intellectual function. More than 60% of the published cases had epileptic seizures.In this study, we analysed the seizures and EEG findings of nine patients with MLC. Six patients (66.6%) with moderate to severe neurological impairment had epilepsy, four with partial and two with generalised seizures. The EEG of five epileptic patients revealed epileptogenic foci over the temporal, frontal and parietal regions with variable predominance during waking and sleep. The facilitation of spike-and-wave paroxysms by eye closure, by intermittent photic stimulation and by hyperventilation were determined in four patients. Four patients also showed abnormalities in the background activity. In conclusion, we think that epilepsy is a significant component of MLC compared to the other leukodystrophies. The elucidation of the underlying molecular defect may explain the unusual pathogenetic relation between this leukoencephalopathy and the associated seizures

    Increased pulse wave velocity and carotid intima-media thickness in patients with ulcerative colitis

    Durakoglugil, Emre/0000-0001-5268-4262WOS: 000322650900024PubMed: 23508984Ulcerative colitis (UC) is characterized with chronic, progressive inflammation of the gastrointestinal tract. the association of UC with cardiovascular disease is still a matter of debate. the aim of this study was to investigate whether carotid intima-media thickness (CIMT) and carotid-femoral pulse wave velocity (cf-PWV) as surrogates of atherosclerosis and arterial stiffness are increased in patients with UC. Our study was cross-sectional and observational in design. Baseline characteristics were recorded during interview with the patient. Patients with previous cardiovascular disease, rheumatoid arthritis, chronic renal failure, and infectious and inflammatory disorders other than UC were excluded. Thirty-seven consecutive patients with UC and 30 control participants underwent cf-PWV assessment and CIMT measurement. the diagnosis of UC was based on clinical, radiologic, endoscopic, and histological findings. CIMT, cf-PWV, and C reactive protein were significantly higher in patients with UC. Although linear regression analyses identified UC as an independent predictor of CIMT (beta +/- A SE, 0.39 +/- A 0.08; p < 0.001), only age independently predicted cf-PWV (beta +/- A SE, 0.08 +/- A 0.03; p = 0.003) in our study population. Moreover, we revealed higher CIMT and PWV values in patients with higher disease activity and more extensive involvement, compared to patients with mild activity and limited disease. We revealed increased pulse wave velocity and CIMT in patients with UC. UC appears to be associated with arterial stiffness and atherosclerotic burden, but the underlying mechanisms require further studies to be identified

    Clinical Study Comparison of Wide Conjunctival Flap and Conjunctival Autografting Techniques in Pterygium Surgery

    Pterygium is an abnormal fibrovascular tissue extending on the cornea which is a degenerative and hyperplastic disorder. A stromal overgrowth of fibroblast and blood vessels is accompanied by an inflammatory cell infiltrate and abnormal extracellular matrix accumulation. The surgical excision is the main treatment method of pterygium, but recurrence is the most common postoperative complication. In the present study, we aimed to compare the wide conjunctival flap and the conjunctival autografting techniques in pterygium surgery according to time of operation, safety, and effectiveness. Results showed that the effect of wide conjunctival flap techniques on primary pterygium surgery was found close to the conjunctival autograft techniques. In addition, the flap technique has a shorter surgical time, the surgery does not require extreme experience, feeding of the flap is provided with own vessels since the vascular structure is protected on the upper temporal conjunctival area, reverse placement of the flap is not seen, it needs fewer sutures, so that suture disturbances may reduce, and it is less traumatic than autograft technique during conjunctival transport. Therefore, this technique may be preferred in suitable cases

    Mean platelet volume in patients with retinal artery occlusion

    ABSTRACT Purpose: The aim of this study was to investigate the mean platelet volume (MPV) of patients with retinal artery occlusion (RAO). Methods: Thirty-seven patients diagnosed with RAO and 32 control subjects were included in this retrospective study. Retinal artery occlusion was diagnosed on the basis of clinical examination and fundus fluorescein angiography. All participants underwent complete ocular examination, and MPV, hematocrit, hemoglobin, and platelet counts were recorded. RAO patient data were compared with those of the control subjects. Results: Patients with RAO had significantly higher MPV values (7.96 ± 1.2 fL) compared with control subjects (7.33 ± 0.7 fL, p<0.001). No significant difference was found with regard to platelet count between the RAO group and the control group (262 ± 70.1 × 109/L and 251 ± 56.6 × 109/L, respectively, p=0.50). MPV was an independent predictor of RAO [odds ratio (OR)=0.50; 95% confidence interval (CI)=0.28-0.89; p=0.019). Conclusions: Our results demonstrated that MPV values were significantly higher in patients with RAO, suggesting that larger platelets may contribute to the pathogenesis of the RAOs

    The Quality of Life Measurements Following Oncoplastic Breast-conserving Surgery

    Objective:Oncoplastic breast-conserving surgery (OBCS) is a widely accepted surgical option among breast cancer patients. We aimed to evaluate the histopathological findings of breast tumors and quality of life scores.Method:The clinical and pathological data of 49 female patients who underwent OBSC in University of Health Sciences Turkey, İstanbul Bağcılar Training and Research Hospital between January 2015 and January 2019 for breast cancer were retrospectively evaluated. According to the location of the tumor, the racket method or J-mammoplasty techniques was selected. Quality of life and patient satisfaction assessment questionnaire was performed.Results:The mean age was 48.1 (26-68) years. The most common incision pattern was the racket pattern (85%). The invasive ductal carcinoma (89.8%) was seen more than ductal carcinoma in situ (6.1%) and papillary carcinoma (4.1%). The distance to the surgical margin was approximately 7.1 mm (1-20). The estrogen receptor positivity was 74.2% (5-100%) and progesterone receptor positivity was 61.8% (5-98%). 61.2% received chemotherapy (100%, radiotherapy). Based on questionnaires, the score of quality of life and satisfaction was 34 (25-47). Especially, the body image function [22 (15-29)] and health functions [10 (7-13)], such as breast & arm symptoms, were better in the patients who had higher scores (p<0.001).Conclusion:The histopathological patterns of the patients following OBSC potentially did not show any impact among the quality of life and satisfaction scores

    Does the Change in the Indications of Endoscopic Sinonasal Surgery Continue? Data between 1994-2018

    Objective: Endoscopic sinonasal surgery (ESS) has changing over the years in parallel with the developments in endoscopy devices, video-imaging techniques, and surgical instruments. In the present study we investigated whether the indications of patients who underwent surgery over a period of 25 years have accommodated to these changes.Methods: We retrospectively evaluated 1173 patients who underwent surgery in our clinic from 1994 through 2007, and 954 patients who underwent surgery from 2008 through 2018. The patients were divided into three groups as follows: chronic rhinosinusitis with polyps (CRSwNP), chronic rhinosinusitis without polyps (CRSsNP), and others. The changes in the indications during the first 14 years and the following 11 years were compared, and the results were statistically evaluated.Results: A significant decrease was observed in the number of patients who underwent surgery following the diagnosis of CRSsNP (p<0.001). In addition, a statistically significant increase was found in CRSwNP (p<0.001) and other (p<0.001) indications.Conclusion: When ESS indications identified in our clinic were reviewed, it was observed that the increasing trend in CRSwNP rate in the first 14 years continued, there was a significant increase in non-CRS indications in the last 11 years, and there has been an increase in patients with fungal sinusitis, especially in this group
