    Aircraft observations on a continuous haze pollution event in Shijiazhuang area

    A continuous haze event was recorded on November 14th∼17th, 2020 over Shijiazhuang. Two flights of King-air 350 meteorological research aircraft were performed on November 14th and 16th for the retrieval and observations of meteorological elements, aerosols, and black carbon. In this study, we combined airborne data with air pollution data (PM2.5), ground meteorological data, and ERA-5 reanalysis data to describe the vertical distribution of aerosols (namely 0.1–3.0 μm) and black carbon. We further explicated the formation of this haze event. PM2.5 pollution dominated this haze event, and the highest concentration of PM2.5 was 209 μg/m3. The intensity and height of thermal layers highly linked with the vertical transport of pollution. The highest number concentration of aerosols and black carbon was found below the thermal layers on both airborne sounding days. On the 14th, both BC and aerosol concentrations showed unimodal distribution, and the highest concentrations of BC and aerosols were 12683 ng/m3 and 6965.125#/L at 250 m within layer Ⅰ. The intensity of the thermal layer near-ground was weaker on the 16th that the number concentrations of BC and aerosols also remained at high levels in layer Ⅱ. Backward trajectories of air mass indicated the long-range transport of pollution contributed to the high level of pollution on the 16th. Vapor conditions were more favorable for aerosols growth through moisture absorption. The maximum concentration of 943.58#/L was recorded at particles with a diameter of 0.4 μm on the 16th, while 749.26#/L was reached at 0.14 μm on the 14th. The corresponding height was consistent with the height of maximum concentration in the vertical distribution

    CryoEM structure of the human SLC4A4 sodium-coupled acid-base transporter NBCe1.

    Na+-coupled acid-base transporters play essential roles in human biology. Their dysfunction has been linked to cancer, heart, and brain disease. High-resolution structures of mammalian Na+-coupled acid-base transporters are not available. The sodium-bicarbonate cotransporter NBCe1 functions in multiple organs and its mutations cause blindness, abnormal growth and blood chemistry, migraines, and impaired cognitive function. Here, we have determined the structure of the membrane domain dimer of human NBCe1 at 3.9 Å resolution by cryo electron microscopy. Our atomic model and functional mutagenesis revealed the ion accessibility pathway and the ion coordination site, the latter containing residues involved in human disease-causing mutations. We identified a small number of residues within the ion coordination site whose modification transformed NBCe1 into an anion exchanger. Our data suggest that symporters and exchangers utilize comparable transport machinery and that subtle differences in their substrate-binding regions have very significant effects on their transport mode

    Sex Differences in Quality of Life and Clinical Outcomes in Patients with Heart Failure

    Background: Heart failure (HF) is generally associated with poor quality of life (QoL). Limited data are available characterizing health-related QoL (HRQL) in Chinese patients with HF. Methods: We used the Minnesota Living with Heart Failure Questionnaire (MLHFQ) to record QoL in 4082 patients with HF from China who were followed up over 12 months in the Heart Failure Registry of Patient Outcomes (HERO) study. Baseline HRQL and differences in QoL between women and men with heart failure were compared. We used multivariable Cox regression with adjustment for variables to assess the association between MLHFQ summary scores and a composite of all-cause mortality and HF hospitalization. Result: At baseline, the mean MLHFQ in the overall population was 42.9 ± 19.57; the scores for physical and emotional domains were 22.0 ± 8.69 and 8.66 ± 6.08, respectively. Women had a higher (poorer) MLHFQ summary score (44.27 ± 19.13) than men (41.63 ± 19.90) (P<0.001). Female patients also had higher MLHFQ physical and emotional scores than male patients (P<0.001). The specific scores of the questionnaire were higher in women than men. NYHA class was the strongest independent predictor of MLHFQ score (β=6.12 unit increment; P<0.001). Sex was not independently associated with higher MLHFQ scores after multivariable adjustments. The 12-month mortality in the overall cohort was 19.6%, the hospitalization rate was 24.4%, and the composite endpoint was 40.15%. A 10-point increase in MLHFQ score was associated with higher risk of mortality (female and male HRs=1.19 [95% CI 1.12–1.26]; P<0.001 and 1.18 [95% CI 1.12–1.24]; P<0.001, respectively) and composite outcomes (HRs=1.08 [95% CI 1.04–1.13]; P<0.001 and 1.11 [95% CI 1.07–1.14]; P<0.001, respectively). Females did not show a significant association between HRQL and hospitalization (HR=1.04 [95% CI 0.99–1.09]; P=0.107). Conclusion: Quality of life was largely poorer in women than men, but was similar between sexes in terms of physical burden and emotional limitation. HRQL is an independent predictor of all-cause death and HF hospitalization in patients with HF

    What Makes a Top-Performing Precision Medicine Search Engine? Tracing Main System Features in a Systematic Way

    From 2017 to 2019 the Text REtrieval Conference (TREC) held a challenge task on precision medicine using documents from medical publications (PubMed) and clinical trials. Despite lots of performance measurements carried out in these evaluation campaigns, the scientific community is still pretty unsure about the impact individual system features and their weights have on the overall system performance. In order to overcome this explanatory gap, we first determined optimal feature configurations using the Sequential Model-based Algorithm Configuration (SMAC) program and applied its output to a BM25-based search engine. We then ran an ablation study to systematically assess the individual contributions of relevant system features: BM25 parameters, query type and weighting schema, query expansion, stop word filtering, and keyword boosting. For evaluation, we employed the gold standard data from the three TREC-PM installments to evaluate the effectiveness of different features using the commonly shared infNDCG metric.Comment: Accepted for SIGIR2020, 10 page

    An approach for medical event detection in Chinese clinical notes of electronic health records

    Abstract Background Medical event detection in narrative clinical notes of electronic health records (EHRs) is a task designed for reading text and extracting information. Most of the previous work of medical event detection treats the task as extracting concepts at word granularity, which omits the overall structural information of the clinical notes. In this work, we treat each clinical note as a sequence of short sentences and propose an end-to-end deep neural network framework. Methods We redefined the task as a sequence labelling task at short sentence granularity, and proposed a novel tag system correspondingly. The dataset were derived from a third-level grade-A hospital, consisting of 2000 annotated clinical notes according to our proposed tag system. The proposed end-to-end deep neural network framework consists of a feature extractor and a sequence labeller, and we explored different implementations respectively. We additionally proposed a smoothed Viterbi decoder as sequence labeller without additional parameter training, which can be a good alternative to conditional random field (CRF) when computing resources are limited. Results Our sequence labelling models were compared to four baselines which treat the task as text classification of short sentences. Experimental results showed that our approach significantly outperforms the baselines. The best result was obtained by using the convolutional neural networks (CNNs) feature extractor and the sequential CRF sequence labeller, achieving an accuracy of 92.6%. Our proposed smoothed Viterbi decoder achieved a comparable accuracy of 90.07% with reduced training parameters, and brought more balanced performance across all categories, which means better generalization ability. Conclusions Evaluated on our annotated dataset, the comparison results demonstrated the effectiveness of our approach for medical event detection in Chinese clinical notes of EHRs. The best feature extractor is the CNNs feature extractor, and the best sequence labeller is the sequential CRF decoder. And it was empirically verified that our proposed smoothed Viterbi decoder could bring better generalization ability while achieving comparable performance to the sequential CRF decoder

    Poly(lactic-co-glycolic) Acid/Nanohydroxyapatite Scaffold Containing Chitosan Microspheres with Adrenomedullin Delivery for Modulation Activity of Osteoblasts and Vascular Endothelial Cells

    Adrenomedullin (ADM) is a bioactive regulatory peptide that affects migration and proliferation of diverse cell types, including endothelial cells, smooth muscle cells, and osteoblast-like cells. This study investigated the effects of sustained release of ADM on the modulation activity of osteoblasts and vascular endothelial cells in vitro. Chitosan microspheres (CMs) were developed for ADM delivery. Poly(lactic-co-glycolic) acid and nano-hydroxyapatite were used to prepare scaffolds containing microspheres with ADM. The CMs showed rough surface morphology and high porosity, and they were well-distributed. The scaffolds exhibited relatively uniform pore sizes with interconnected pores. The addition of CMs improved the mechanical properties of the scaffolds without affecting their high porosity. In vitro degradation tests indicated that the addition of CMs increased the water absorption of the scaffolds and inhibited pH decline of phosphate-buffered saline medium. The expression levels of osteogenic-related and angiogenic-related genes were determined in MG63 cells and in human umbilical vein endothelial cells cultured on the scaffolds, respectively. The expression levels of osteogenic-related and angiogenic-related proteins were also detected by western blot analysis. Their expression levels in cells were improved on the ADM delivery scaffolds at a certain time point. The in vitro evaluation suggests that the microsphere-scaffold system is suitable as a model for bone tissue engineering

    Unique Fractional Crystallization of Poly(l‑lactide)/Poly(l‑2-hydroxyl-3-methylbutanoic acid) Blend

    The crystallization behaviors and microstructures of poly­(l-2-hydroxyl-3-methyl­butanoic acid)/poly­(l-lactide) blends [P­(L-2H3MB)/PLLA] were investigated by OM, DSC, SAXS, in situ temperature-dependent WAXD, and in situ synchrotron WAXS. The blends exhibited a homogeneous state at 250 °C. In the cooling process, P­(L-2H3MB), with higher melting temperature, crystallized first at 166.7 °C and drove the formation of the phase separation and microstructure. And the amorphous P­(L-2H3MB) and PLLA were excluded into the interlamellar and interfibrillar regions of the former P­(L-2H3MB) crystallites. For P­(L-2H3MB)/PLLA (5/5), the amorphous P­(L-2H3MB) in the interfibrillar regions continued to form crystallites sequentially at 134.6 °C, which clearly confirmed the fractional crystallization of P­(L-2H3MB). In our knowledge, this unique fractional crystallization has not been reported yet, of the component in miscible blend, which crystallized first under little confinement.
