Palliative care needs in patients hospitalized with heart failure (PCHF) study: rationale and design
Abstract Aims The primary aim of this study is to provide data to inform the design of a randomized controlled clinical trial (RCT) of a palliative care (PC) intervention in heart failure (HF). We will identify an appropriate study population with a high prevalence of PC needs defined using quantifiable measures. We will also identify which components a specific and targeted PC intervention in HF should include and attempt to define the most relevant trial outcomes. Methods An unselected, prospective, near-consecutive, cohort of patients admitted to hospital with acute decompensated HF will be enrolled over a 2-year period. All potential participants will be screened using B-type natriuretic peptide and echocardiography, and all those enrolled will be extensively characterized in terms of their HF status, comorbidity, and PC needs. Quantitative assessment of PC needs will include evaluation of general and disease-specific quality of life, mood, symptom burden, caregiver burden, and end of life care. Inpatient assessments will be performed and after discharge outpatient assessments will be carried out every 4âmonths for up to 2.5âyears. Participants will be followed up for a minimum of 1âyear for hospital admissions, and place and cause of death. Methods for identifying patients with HF with PC needs will be evaluated, and estimates of healthcare utilisation performed. Conclusion By assessing the prevalence of these needs, describing how these needs change over time, and evaluating how best PC needs can be identified, we will provide the foundation for designing an RCT of a PC intervention in HF
ISO Spectroscopy of Young Stellar Objects
Observations of gas-phase and solid-state species toward
young stellar objects (YSOs) with the spectrometers
on board the Infrared Space Observatory
are reviewed. The excitation and abundances of
the atoms and molecules are sensitive to the changing
physical conditions during star-formation. In
the cold outer envelopes around YSOs, interstellar
ices contain a significant fraction of the heavy element
abundances, in particular oxygen. Different ice
phases can be distinguished, and evidence is found for
heating and segregation of the ices in more evolved
objects. The inner warm envelopes around YSOs are
probed through absorption and emission of gas-phase
molecules, including CO, CO_2, CH_4 and H_2O. An
overview of the wealth of observations on gas-phase
H_2O in star-forming regions is presented. Gas/solid
ratios are determined, which provide information on
the importance of gas-grain chemistry and high temperature
gas-phase reactions. The line ratios of molecules
such as H_2, CO and H_2O are powerful probes
to constrain the physical parameters of the gas. Together
with atomic and ionic lines such as [0 I]
63 ”m, [S I] 25 ”m and (Si II] 35 ”m, they can also
be used to distinguish between photon- and shock-heated
gas. Finally, spectroscopic data on circumstellar
disks around young stars are mentioned. The
results are discussed in the context of the physical
and chemical evolution of YSOs
Quasielastic 12C(e,e'p) Reaction at High Momentum Transfer
We measured the 12C(e,e'p) cross section as a function of missing energy in
parallel kinematics for (q,w) = (970 MeV/c, 330 MeV) and (990 MeV/c, 475 MeV).
At w=475 MeV, at the maximum of the quasielastic peak, there is a large
continuum (E_m > 50 MeV) cross section extending out to the deepest missing
energy measured, amounting to almost 50% of the measured cross section. The
ratio of data to DWIA calculation is 0.4 for both the p- and s-shells. At w=330
MeV, well below the maximum of the quasielastic peak, the continuum cross
section is much smaller and the ratio of data to DWIA calculation is 0.85 for
the p-shell and 1.0 for the s-shell. We infer that one or more mechanisms that
increase with transform some of the single-nucleon-knockout into
multinucleon knockout, decreasing the valence knockout cross section and

Extension of Earth-Moon libration point orbits with solar sail propulsion
This paper presents families of libration point orbits in the Earth-Moon system that originate from complementing the classical circular restricted three-body problem with a solar sail. Through the use of a differential correction scheme in combination with a continuation on the solar sail induced acceleration, families of Lyapunov, halo, vertical Lyapunov, Earth-centred, and distant retrograde orbits are created. As the solar sail circular restricted three-body problem is non-autonomous, a constraint defined within the differential correction scheme ensures that all orbits are periodic with the Sunâs motion around the Earth-Moon system. The continuation method then starts from a classical libration point orbit with a suitable period and increases the solar sail acceleration magnitude to obtain families of orbits that are parametrised by this acceleration. Furthermore, different solar sail steering laws are considered (both in-plane and out-of-plane, and either fixed in the synodic frame or fixed with respect to the direction of sunlight), adding to the wealth of families of solar sail enabled libration point orbits presented. Finally, the linear stability properties of the generated orbits are investigated to assess the need for active orbital control. It is shown that the solar sail induced acceleration can have a positive effect on the stability of some orbit families, especially those at the L2 point, but that it most often (further) destabilises the orbit. Active control will therefore be needed to ensure long-term survivability of these orbits
Negotiating Value: Comparing Human and Animal Fracture Care in Industrial Societies
At the beginning of the twentieth-century, human and veterinary surgeons faced the challenge of a medical marketplace transformed by technology. The socio-economic value ascribed to their patients â people and domestic animals â was changing, reflecting the increasing mechanisation of industry and the decreasing dependence of society upon non-human animals for labour. In human medicine, concern for the economic consequences of fractures âpathologisedâ any significant level of post-therapeutic disability, a productivist perspective contrary to the traditional corpus of medical values. In contrast, veterinarians adapted to the mechanisation of horse-power by shifting their primary professional interest to companion animals; a type of veterinary patient generally valued for the unique emotional attachment of the owner, and not the productive capacity of the animal. The economic rationalisation of human fracture care and the âsentimentalâ transformation of veterinary orthopaedic expertise indicates how these specialists utilised increasingly convergent rhetorical arguments to justify the application of innovative fracture care technologies to their humans and animal patients. An integrated mid-range theory of postpartum family development: a guide for research and practice.
AIM: This paper is a report of a study to identify parents' perceptions of postpartum family experiences. BACKGROUND: There is a growing worldwide emphasis on family support. Government policy in the United Kingdom advocates a family-centred approach in which a core universal postbirth service is offered to all families with additional support for parents of children with complex needs. Health visitors provide family postpartum care without an agreed theory directing or standardizing practice. There is a need to identify parental experiences to define family-centred care. METHOD: A qualitative, exploratory approach was undertaken using a purposive sample of 17 postpartum families. Data were collected in one region of Northern Ireland in 2001-2002. Participants' experiences and views were accessed during two focus groups with a total of seven participants, and six in-depth interviews. Thematic analysis was conducted. FINDINGS: One core theme, 'thriving and surviving', and three main themes, 'baby nurture', 'life changes', 'coping and adapting resources', were identified to describe how parents developed during the first 8-week postpartum. These were influenced by the physical, the psychosocial and the environmental factors. The identified themes were mapped together to form an Integrated Mid-Range Theory of Postpartum Parent Development. CONCLUSION: As parents need to negotiate successfully both present coping and future development during the postpartum period, there is a need for professionals to offer services that are orientated to holistic short- and long-term well-being. The findings, further to additional research, may be used by health visitors and other professionals to direct universal postpartum care
US hegemony and the origins of Japanese nuclear power : the politics of consent
This paper deploys the Gramscian concepts of hegemony and consent in order to explore the process whereby nuclear power was brought to Japan. The core argument is that nuclear power was brought to Japan as a consequence of US hegemony. Rather than a simple manifestation of one state exerting material âpower over' another, bringing nuclear power to Japan involved a series of compromises worked out within and between state and civil society in both Japan and the USA. Ideologies of nationalism, imperialism and modernity underpinned the process, coalescing in post-war debates about the future trajectory of Japanese society, Japan's Cold War alliance with the USA and the role of nuclear power in both. Consent to nuclear power was secured through the generation of a psychological state in the public mind combining the fear of nuclear attack and the hope of unlimited consumption in a nuclear-fuelled post-modern world
ParĂąmetros populacionais da raça ovina Santa InĂȘs no Brasil
O objetivo deste trabalho foi avaliar a estrutura populacional de ovinos da raça Santa InĂȘs criados no Brasil. Foram utilizados dados de pedigree de 13.216 animais, pertencentes a 53 rebanhos de oito estados brasileiros, nascidos no perĂodo de 1976 a 2010. O programa Endog foi utilizado para anĂĄlise do pedigree e estimação dos parĂąmetros populacionais. Do total de animais estudados, 80,86% apresentaram pedigree na primeira ascendĂȘncia, 73,78% na segunda e 67,75% na terceira. O nĂșmero mĂĄximo de geraçÔes conhecidas foi de 19, e a mĂ©dia de geraçÔes equivalentes foi de 4,67. A mĂ©dia do intervalo de geraçÔes foi de 3,22±1,77 anos. O tamanho efetivo da população apresentou mĂ©dia de 172,5 animais. O nĂșmero de animais fundadores foi 829, mas o nĂșmero efetivo de fundadores foi apenas 50. Os 17 principais ancestrais explicaram 50% da variabilidade genĂ©tica total. O coeficiente mĂ©dio de relação foi de 3,87% e o de endogamia, de 6,92%. Apesar do satisfatĂłrio coeficiente mĂ©dio de endogamia nas Ășltimas geraçÔes, este coeficiente requer monitoramento por sua proximidade do limite recomendĂĄvel. O fluxo de genes entre os rebanhos Ă© o principal fator para o aumento do tamanho efetivo e a manutenção da variabilidade genĂ©tica da raça Santa InĂȘs
