Number-conserving cellular automata with a von Neumann neighborhood of range one
We present necessary and sufficient conditions for a cellular automaton with
a von Neumann neighborhood of range one to be number-conserving. The conditions
are formulated for any dimension and for any set of states containing zero. The
use of the geometric structure of the von Neumann neighborhood allows for
computationally tractable conditions even in higher dimensions.Comment: 15 pages, 3 figure
A split-and-perturb decomposition of number-conserving cellular automata
This paper concerns -dimensional cellular automata with the von Neumann
neighborhood that conserve the sum of the states of all their cells. These
automata, called number-conserving or density-conserving cellular automata, are
of particular interest to mathematicians, computer scientists and physicists,
as they can serve as models of physical phenomena obeying some conservation
law. We propose a new approach to study such cellular automata that works in
any dimension and for any set of states . Essentially, the local rule of
a cellular automaton is decomposed into two parts: a split function and a
perturbation. This decomposition is unique and, moreover, the set of all
possible split functions has a very simple structure, while the set of all
perturbations forms a linear space and is therefore very easy to describe in
terms of its basis. We show how this approach allows to find all
number-conserving cellular automata in many cases of and . In
particular, we find all three-dimensional number-conserving CAs with three
states, which until now was beyond the capabilities of computers

A binary number-conserving cellular automaton is a discrete dynamical system that models the movement of particles in a d-dimensional grid. Each cell of the grid is either empty or contains a particle. In subsequent time steps the particles move between the cells, but in one cell there can be at most one particle at a time. In this paper, the von Neumann neighborhood is considered, which means that in each time step a particle can move to an adjacent cell only. It is proven that regardless of the dimension d, all of these cellular automata are trivial, as they are intrinsically one-dimensional. Thus, for given d, there are only 4d+1 binary number-conserving cellular automata with the von Neumann neighborhood: the identity rule and the shift and traffic rules in each of the 2d possible directions
Physiotherapeutic approach in managing a patient after below-knee amputation due to diabetic foot syndrome
Diabetic foot syndrome (DFS) is the most common complication in the course of diabetes and can lead to loss of the lower limb. In case of amputation an important role is played by physiotherapeutic procedures and secondary prevention. The article presents the case of a 75-year-old man with type 2 diabetes, after below- knee amputation of the left lower limb in the course of DFS. The patient underwent a physiotherapeutic treatment consisting of a number of procedures aimed at re-education of the gait function and learning to use the prosthesis. It played an important role in restoring social and professional activities of the patient and thus enabled him to return to his usual life. Badanie IMPROVETM - międzynarodowe badanie obserwacyjne w cukrzycy typu 2: wyniki dotyczące populacji Polski
Introduction: Observational studies are valuable tools for assessing the applicability of results from randomised controlled trials to broader
patient populations. They are especially important in chronic diseases such as diabetes, as they can provide a comprehensive picture
of the safety and effectiveness of a particular therapy across cultures and phenotypes.
Material and methods: Patients with type 2 diabetes who required insulin and whose physician had decided to initiate biphasic insulin
aspart 30 (BIAsp 30) were eligible. A total of 4117 type 2 diabetic patients were recruited to the study in Poland, and 809 primary and
secondary care physicians were involved. The aim of this study was to assess the safety and effectiveness of BIAsp 30 treatment in type 2
diabetes in routine clinical practice.
Results: Baseline glycaemic control was poor in the Polish cohort enrolled in the IMPROVETM study, with a mean HbA1c value of 9.0 ± 1.7%. A very high proportion of patients were thus at risk of macrovascular and microvascular complications. A twice-daily regimen for
the start of BIAsp 30 therapy was the most common choice, including 72.2% of patients at baseline. HbA1c was significantly reduced by
1.66% for the total cohort and by 3.07% and 1.55% in the pre-study no-therapy or oral antidiabetic drug group respectively (p < 0.001). The
rates (episodes per subject year) of overall major hypoglycaemia were 0.012 and 0.12 at follow-up and final visits respectively. For minor
hypoglycaemia rates of 5.12 per subject per year at follow-up visit and 4.54 episodes per subject per year at final visit were recorded.
Conclusions: BIAsp 30 appears to be an effective and flexible treatment approach and can be safely intensified to achieve glycaemic
control in a majority of patients with type 2 diabetes.Wstęp: Badania obserwacyjne stanowią wartościowe narzędzie uzupełniające informacje uzyskane z badań RCT (randomised controlled
trials). Odgrywają one szczególnie ważną rolę w przypadku schorzeń przewlekłych, takich jak cukrzyca, w przypadku których, w odniesieniu
do dużych populacji, dostarczają istotnych danych dotyczących skuteczności i bezpieczeństwa stosowanych terapii.
Materiał i metody: Grupa badana obejmowała chorych na cukrzycę typu 2, którzy w opinii lekarza prowadzącego wymagali rozpoczęcia
insulinoterapii i kwalifikowali się do włączenia dwufazowej insuliny aspart 30 (BIAsp 30) w celu poprawy wyrównania metabolicznego.
Do badania, którego celem była ocena bezpieczeństwa i skuteczności leczenia cukrzycy typu 2 przy użyciu BIAsp 30 zakwalifikowano
w Polsce łącznie 4117 chorych.
Wyniki: Wyrównanie metaboliczne polskiej grupy wchodzącej w skład globalnego badania IMPROVETM, w momencie rozpoczęcia obserwacji,
było wysoce niezadowalające - średnia wartość HbA1c wynosiła 9,0 ± 1,7%. W konsekwencji istotny odsetek pacjentów prezentował
powikłania naczyniowe o typie mikro- i makroangiopatii. Najczęściej stosowanym schematem rozpoczęcia terapii BIAsp 30,
w 72,2% przypadków, było zastosowanie dwóch wstrzyknięć na dobę. W trakcie 26-tygodniowej obserwacji redukcja HbA1c wynosiła
1,66% dla całej populacji oraz odpowiednio 3,07% i 1,55% dla osób wyjściowo nie leczonych farmakologicznie oraz otrzymujących doustne
leki przeciwcukrzycowe. Ryzyko epizodów ciężkiej hipoglikemii w trakcie całego badania (epizody/pacjenta/rok) wynosiło 0,012, natomiast
lekkiej hipoglikemii odpowiednio 5,12/pacjenta/rok w trakcie wizyty kontrolnej oraz 4,54/pacjenta/rok na wizycie końcowej.
Wnioski: Dwufazowa insulina aspart 30 stosowana w codziennej praktyce jest bezpieczną i skuteczną insuliną w terapii cukrzycy typu 2
Postępowanie fizjoterapeutyczne u pacjenta z zespołem stopy cukrzycowej po amputacji kończyny dolnej na poziomie podudzia
Zespół stopy cukrzycowej (ZSC) jest najczęstszym powikłaniem w przebiegu cukrzycy, które może prowadzić do utraty kończyny dolnej. Po amputacji bardzo ważny aspekt stanowią rehabilitacja i prewencja wtórna. W niniejszym artykule przedstawiono przypadek 75-letniego chorego na cukrzycę typu 2 po amputacji kończyny dolnej lewej na poziomie podudzia w przebiegu ZSC. Pacjent został poddany postępowaniu fizjoterapeutycznemu składającemu się z licznych procedur mających na celu reedukację chodu i naukę korzystania z protezy. Odegrało to istotną rolę w przywróceniu sprawności możliwie najbardziej zbliżonej do tej sprzed zabiegu amputacji, a w związku z tym umożliwiło choremu powrót do życia społecznego i zawodowego
