Zespół Brugadów — długa droga do prawidłowego rozpoznania

Brugada syndrome is an important and underrated cause of sudden cardiac death in a relatively young people. Brugada syndrome is diagnosed when specific electrocardiographic abnormalities characterized by incomplete right bundle-brunch block and ST-segment elevations in the anterior precordial leads are reported. They are combined with an absence of structural abnormalities and other clinical criteria defined as documented ventricular fibrillation or polymorphic ventricular tachycardia, a family history of SCD before 45 years, coved type ECGs infamily members, inducibilility of VT during electrophysiological study, syncope or nocturnal agonal respiration. In this article authors present a case of a young men with Brugada syndrome and his long way to correct diagnosis and treatment.Zespół Brugadów jest ważną i niedocenianą przyczyną nagłego zgonu sercowego w młodym wieku. Zespół ten rozpoznaje się na podstawie specyficznych nieprawidłowości w elektrokardiogramie (EKG), niezupełnego bloku prawej odnogi pęczka Hisa i uniesienia odcinka ST w odprowadzeniach przedsercowych. Towarzyszy im brak strukturalnych nieprawidłowości mięśnia sercowego i współistnienie innych kryteriów klinicznych, takich jak: udokumentowany epizod migotania komór lub polimorficzny częstoskurcz komorowy, nagły zgon sercowy poniżej 45. roku życia w wywiadzie rodzinnym, zmiany typu 1 w zapisie EKG u członków rodziny, arytmia komorowa indukowana w trakcie badania elektrofizjologicznego, omdlenia, agonalny oddechnocny. W pracy przedstawiono przypadek młodego mężczyzny z zespołem Brugadów; jego długą drogę do prawidłowego rozpoznania i terapii

Deficiencies in cooperation between researchers and practitioners in solving urban public transport problems

W funkcjonowaniu publicznego transportu miejskiego pojawiły się dwa bardzo niekorzystne procesy: eksploatowanie zużytego taboru i ciągły wzrost kwot dopłat do transportu miejskiego. Jedną z przyczyn tych zjawisk jest nikła współpraca środowisk nauki z praktykami. W artykule przedstawiono niedoskonałości w metodyce badania problemów funkcjonowania transportu publicznego. Ponadto wykazano, że występują niedoskonałości w zarządzaniu transportem publicznym na poziomie władz miejskich. Brak zmian w podejściu do badania procesów w transporcie miejskim przez praktyków i przedstawicieli nauki jest niekorzystny dla wdrażania nowoczesnych rozwiązań w systemie transportu miejskiego.Two very unfavourable processes have emerged in the functioning of urban public transport: the exploitation of used rolling stock and a continuous increase in the amount of urban transport subsidies. One of the reasons for these phenomena is the poor cooperation between the scientific community and practitioners. The article presents imperfections in the methodology of studying problems of the public transport functionning. On the other hand, it has been shown that there are imperfections in public transport management at the level of municipal authorities. The lack of changes in the approach to studying processes in urban transport by practitioners and scientists is unfavorable for the implementation of modern solutions in urban transport system

Conditions for effective implementation of modern urban transport

Celem artykułu jest przedstawienie w oparciu o dorobek teorii zarządzania i ekonomii zasobów oraz procesów, które mogą wpływać na wdrażanie nowoczesnych rozwiązań w miejskich inwestycjach i systemie transportu dla mieszkańców miasta. Skuteczność i szybkość zmian w komunikacji miejskiej zależna jest od zasobów wiedzy, jaką posiadają władze miasta, samorząd i organizator, dostępności zasobów finansowych na wydatki związane z rozwojem nowoczesnego transportu, zasobów w infrastrukturze transportu i taboru oraz stosowanej technologii IT. Potencjał tych zasobów oraz procesy, jakie zachodzą w systemie decyzyjnym i przepływach finansowych wyznaczają zdolność samorządu miejskiego do zrealizowania zadań, jakie należy wdrożyć, aby „lepszy transport wpływał na lepsze miasto”. Innowacyjne podejście i elastyczność procesu zarządzania przez organizatora zależy od formy organizacyjno-prawnej. W artykule przedstawiono trzy formy organizacyjno-prawne, jakie występują w polskiej praktyce. Omówiono ich cechy i zakres swobody w korzystaniu z zasobów, którymi dysponują, aby zwrócić uwagę polityków i władz lokalnych na konieczność poprawy formy organizacyjno-prawnej organizatora przed podjęciem planowanych zmian systemowych w transporcie miejskim. Zwrócono uwagę na znaczenie odpowiedniego kapitału intelektualnego, którego rdzeniem jest wiedza. W podsumowaniu postawiono tezę, że w krajowym transporcie publicznym jednym z ważnych postulatów, które stoją przed osobami kierującymi administracją miejską jest potrzeba kształcenia na studiach podyplomowych oferujących najnowszą wiedzę z zakresu projektowania i wdrażania innowacji w transporcie miejskim.The aim of this article is to present, based on management and economic theory, the resources and processes that can influence the implementation of modern solutions in urban investments and the transport system for city residents. The effectiveness and speed of change in urban transport depends on the knowledge resources of the city authorities, the local government and the organiser, the availability of financial resources for spending on the development of modern transport, the resources in transport infrastructure and rolling stock and the IT technology used. The potential of these resources and the processes that take place in the decision-making system and financial flows determine the ability of the city government to achieve the tasks that need to be implemented in order for “better transport to influence a better city”. The innovative approach and flexibility of the organiser’s management process depends on the organisational and legal form. The article presents three organisational-legal forms that occur in Polish reality. Their characteristics and the scope of their freedom to use the resources at their disposal are discussed in order to draw the attention of politicians and local authorities to the need to improve the organiser’s organisational- legal form before undertaking planned system changes in urban transport. Attention was drawn to the importance of adequate intellectual capital – intangible resources, the core of which is knowledge. It concludes with the thesis that, in national public transport, one of the important demands facing those in charge of urban administration is the need for postgraduate education offering the latest knowledge in the design and implementation of urban transport innovations

Alan Dershowitz to jeden z najwybitniejszych adwokatów amerykańskich, a jednocześnie jeden z bardziej kontrowersyjnych, profesor prawa na Uniwersytecie Harwarda. Napisał bardzo popularną książkę adresowaną do młodego prawnika, w formie listów. Zawiera ona porady oraz wskazówki. Dla Czytelnika z Polski szczególnego waloru nabiera fragment o właściwym dobieraniu sobie autorytetów, w którym wskazuje, kto dla niego – Dershowitza jest niewątpliwym autorytetem. To Jan Karski – polski bohater narodowy, który informował świat o Holokauście i apelował o pomoc dla polskich Żydów, będąc delegatem polskiego rządu w Londynie. Rady Dershowitza mogą czasami zaskakiwać, ale niewątpliwie stanowią powód do refleksji.Alan Dershowitz is one of the most distinguished American advocates and, at the same time, one of the most controversial law professors at Harvard University. He wrote a very popular book addressed to a young lawyer in the form of letters. It contains several advices and tips. For the Polish reader, the part about the proper choice of father figuers will be of special value. Therein, he indicates who has played such a role in his life. It has been Jan Karski, a Polish national hero, who informed the world about the Holocaust and who, as a delegate of the Polish Government in London, appealed for help for the Polish Jews. Some of Dershowitz’s advices can sometimes be surprising to the reader, but undoubtedly they form basis for further reflection
